1.Expression and clinical significance of P16INK4a in human primary mesangial proliferative glomerulo-nephritis

Objective The abnormal proliferation of renal cells underlies the pathology of progressive glomerulosclerosis.Cell proliferation is regulated by cell regulatory proteins.This study aims to investigate the expression of cyclin-dependent kinase inhibitor P16INK4a in renal tissue of patients with primary mesangial proliferative glomemlonephritis(MsPGN)and its clinical significance.Methods Paraffinembedded renal biopsy renal tissue sections from 36 patients with MsPGN were detected by immunohistochemical staining.Normal renal ti8.Sue sections from 6 nephrectomized patients with trauma were used as controls.Possible correlation between p16 positive area and sclerotic and crescentic glomeruli,blood pressure,serum creatinine,endogenous creatinine clearance rate,and total proteinuria were evaluated.Resuits There were very few expression of P16INK4a in normal glomemli and renal tubular-interstitial.Compared to the normal comrol group,the expression of P16INK4a in 36 renal biopsy specimens with MsPG'N was significantly higher(P<0.01),especially in sclerotic and crescentic glomeruli,and it had wild expression of the same components in no sclerotic and crescentic glomeruli.There was a statistic difference(P<0.01).No statistic difference was found between the expression of P16 INK4a in renal tubular-interstitial of two groups(P>0.05).Therewag positive correlation among P16 INK4a of glomeruli without use of glucocorticoid/immunoinlfibitors and ACEI/ARB(r=0.774,0.497,P<0.01),also blood pressure and 8ul~m creatinine(r=0.64,0.473,P<0.01).It was negstively correlated with endogenous creatinine clearance rate and total proteinuria(r=-0.487,-0.694,P<0.01).There is no correlation between P16INK4s and clinical data in renal tubular-interstitial.Conclusion This study demonstrated that overexpression of P16INK4a in renal tissue with MsPGN may promote the regression ofabnormal proliferation and senescence cells,and induce sclerotic and crescentic glomeruli.The expression level of P16 INK4a not only reflected the degree of renal function,but also hypertension and proteinuria.In brief,expression of P16INK4a may be an important marker in renal sclerotic or crescentic glomeruli of MsPGN.Some drugs may prevent the expression of P16INKa.
2.The effect of transcranial electrical stimulation on functional recovery and the expression of microtubule-associated protein-2 and growth-associated protein-43 after cerebral focal ischemia

Objective To assess the influence of transcranial electric stimulation (TES) on the recovery of motor function after cerebral focal ischemia and reperfusion and to explore the mechanisms in terms of neural plasticity.Methods An acute focal ischemia-reperfusion model was established by transient occlusion of the right middle cerebral artery (MCAO).Seventy-two male Sprague-Dawley rats were randomly divided into a TES group,a model group,a sham-operation group and a normal group.The TES group was given TES 24 h after MCAO;the model group received the operation without any treatment.Forelimb placing (FPT) and beam walking (BWT) were mea-sured at the 3rd,7th,14th and 28th day after reperfusion.Microtubule-associated protein-2 (MAP-2) and growth-associated protein-43 (GAP-43) and grey levels of reaction products in the peri-infarct region were examined by immunohistochemical techniques.Results The TES group rats had markedly better FPT and BWT performance at the 7th,14th and 28th day after MCAO,compared with the model group.Expression of MAP-2 had increased significantly more at the 14th and 28th day in the peri-infarct region in the TES group compared with the model group.Expression of GAP-43 was significantly elevated in the peri-infarct region in the TES group compared with the model group at all time points.Conclusions TES can improve motor function and neural plasticity following cerebral ischemia and reperfusion damage.The functional enhancement may be partly due to up-regulation of the expression of GAP-43 and MAP-2 in the peri-infarct region.
3.Interactions of Helicobacter pylori and its L-forms with surfactant protein D

Objective To explore the affinify of combination of Helicobacter pylori (Hp) and its L-form with surfactant protein D(SP-D),and to find function of SP-D in Hp infection and immunity. Methods SP-D was extracted from the lung of rats by Tris-HCl-EDTA and maltose-agarose affinity column. The combination of Hp and SP-D was detected by slide and tube agglutination tests,and the agglutination inhibition with SP-D immune serum was carried out. The L-form of Hp was induced by Ceftriaxon sodrum and identified by PCR and sequencing of the 16S rDNA fragment.The combination of SP-D and the L-form was tested by slide agglutination test. Results Agglutination of Hp NCTC11637 and SP-D was observed on slide,but NCTC11639,SS1 and the L-form failed to agglutinate. Agglutination was found 45 minutes after reaction and was inhibited by SP-D immune sera. Conclusion The ability of combination with SP-D was different between strains of Hp.The combination could be weakened or vanished when Hp lost the cell wall.
4.Transcranial Electrical Stimulation and Rehabilitation Training on Functional Recovery and Growth Associated Protein-43 Expression in Rats after Focal Ischemic Infarction

ObjectiveTo assess how transcranial electrical stimulation (TES) concurrent with rehabilitation training influences brain plasticity and behavioral functional performance in rats following a focal ischemic infarction.MethodsAfter an acute focal ischemic infarction by transient occlusion of right middle cerebral artery (MCAO), the electric stimulation concurrent with rehabilitation group was given TES, balancing and rotating and walking exercise everyday; the rehabilitation group was given only balancing and rotating and walking exercise everyday; the control group received no treatment. Growth associated protein 43(GAP-43) was examined by immunohistochemical techniques, and density of reaction product and forelimb placing test (FPT) were measured on the 3rd, 7th, 14th and 28th day after infarction respectively.ResultsThe electrical stimulation concurrent with rehabilitation group provided marked improvement in FPT on the 7th, 14th and 28th day compared with the rehabilitation group and the control group (P<0.01~0.05). The GAP-43 demonstrated statistically significant increase on the 3rd, 7th, and 14th day in the peri-infarct region in the electrical stimulation concurrent with rehabilitation group compared with other two groups(P<<0.01~0.05).ConclusionThe efficacy of transcranial electrical stimulation combining with rehabilitation training can improve functional outcome and neuronal plasticity following ischemic cerebral damage. The mechanism may be partly due to the upregulation of the expression of GAP-43 in the peri-infarct region. Meanwhile, the efficacy is superior to rehabilitation training only.

Objective To investigate the clinical effect of maternal pat with Chinese medicine in episiotomy wound care. Methods One thousand one hundred cases of maternal with full-term,vaginal delivery, episiotomy in the Beijing Tongren Hospital Affiliated to Capital Medical University from Jun. 2013 to Jun. 2014 were randomly divided into test group and control group. Patients in test group were given routine nursing care combined with Chinese medicine maternal pat to nurse perineal wound,while in the control group were given routine nursing combined with common maternal pat. The data of perineal wound swelling,pain and healing were recorded and compared in two groups. Results The incidences of mild,moderated swelling within 24 h,48 h in test group were 35. 5%( 195/550 )and 6. 0%( 33/550 ),13. 4%( 74/550 )and 1. 6%( 9/550 ),significantly lower than that of the control group( 24 h:51. 5%( 283/550 ),24. 5%( 135/550 );48 h:34. 2%( 188/550 ), 2. 7%(15/550);Z=12. 555,8. 144;P﹤0. 01). The wound healing rate in test group was 92. 36%,significantly higher than that of control group( 80. 73%,P ﹤0. 01 ). The wound infection rate in test group was 1. 82%, significant lower than that of control group(3. 82%,P﹤0. 01). The incidence of comfort and satisfaction in test group was 90. 18%,significantly higher than that of control group(73. 09%;Z= -5. 614,-7. 345;P ﹤0. 05). Conclusion Nursing lateral episiotomy wound using Chinese medicine maternal pat can reduce the swelling, pain and promote wound healing with convenient and comfortable. Therefore,it is worth of the clinical popularization and application.
6.Effect of bacille calmette-guerin polysaccharide nucleic acid (BCG-PSN) on the expression of T cell-derived cytokines in patients with atopic dermatitis

Objective To assess the changes in frequency of peripheral T lymphocytes expressing different cytokines in patients with atopic dermatitis (AD) before and after treatment with BCG-PSN and their relationship with disease severity. Methods A randomized, double blinded and placebo cross-over control study was conducted. A total of 8 patients with AD were recruited in this study. Intramuscular BCG-PSN or placebo was given to patients every other day for 36 days. Flow cytometry was performed to measure the frequency of IL4-, IL5-, IFN-γ- and TNFα-expressing peripheral CD4+ T cells and CD8+ T cells before and after the therapy. Disease severity was evaluated by atopic dermatitis area and severity index score (ADASIS). Results The difference value in IFN-γ+CD8+ T cell frequency before and after therapy was significantly higher in patients treated with BCG-PSN than in those with placebo (8.056 ± 13.962 vs -6.549 ± 10.491, U = 2.26, P< 0.05). There was no statistical difference in the frequency of IL4-, IL5-, TNFa-expressing CD8+ T cells between BCG-PSN- and placebo-treated patients (all P > 0.05). The decrease in ADASIS was 1.56 ± 1.49 in patients treated with BCG-PSN, which was statistically higher than that in placebo-treated patients (-0.05 ± 1.54, U = 2.00, P< 0.05). Conclusion As an immunomodulator, BCG-PSN may control AD by restoring the balance of T-cell subsets.
7.Selection of different surgical methods and curative effect analysis of lumbosacral tuberculosis

Objective To evaluate the indications and clinical efficacy of the different surgical methods for lumbosacral tuberculosis.Methods 112 patients suffering from lumbosacral tuberculosis from January 1998 to October 2011 were reviewed retrospectively.Based on American Spinal Injury Association (ASIA) grading system,31 case was classified as grade C,32 as grade D and 49 as grade E.The affected locations were L5,S1 in 66 cases,L4-S1 in 32,and L5-S2in 14 cases.41 cases (posterior group) underwent posterior pedicle screw instrumentation,debridement and allograft; and 38 cases (anterior group) underwent one-stage anterolateral debridement plus allograft and internal fixation.33 (posterior-anterior group) cases experienced posterior pedicle screw instrumentation and anterior debridement and allograft.All cases underwent routine support and anti-tuberculosis treatment before and after operation.The change of erythrocyte sedimentation rate (ESR),signs of tuberculosis activity,graft fusion,neurological recovery and correction of deformity were evaluated in follow-up and compared among three groups.Results The average operating time in posterior,anterior and posterior-anterior group was (150±22) m in,(140± 18) min,(180± 1 8) min respectively.ESR and C-reactive protein (CRP) were recovered to normal 3 months post surgery.The quality of life total score of lumbosacral tuberculosis patients was very low (55.54 ± 9.23).After surgery and drug treatment,the total score and each dimension scores of the SF-36 were significantly improved.Postoperative ASIA classification and 1umbosacral angle were significantly improved.Tuberculosis recurrence occurred in three cases,fixation loosening in two cases,fixation fracture in one case,and all above cases were cured after revision surgery.The others all were cured and bone graft fusion was determined.Conclusion The quality of life of lumbosacral tuberculosis patients were impacted seriously,and which could be significantly improved with surgical intervention.According to the patient MRI,CT characteristics,surgical approach selection was based on lesion location.Effective internal fixation was based on the extent of damage in bone structure and anatomical characteristics.Following above principles,satisfactory clinical results could be achieved.
8.The posterior parietal cortex in visuospatial attention:study with continuous theta burst stimulation

Objective To seek more direct evidence of the role of the posterior parietal cortex (PPC) in controlling visuospatial attention.Methods Forty healthy subjects took the Attention Network Test following continuous theta burst stimulation (cTBS) applied over the left or right PPC or sham stimulation.The Attention Network Test measures the alerting,orienting and executive control components of visual attention separately.Results Subjects responded to spatial cues significantly slower after cTBS.Alerting and orienting showed deficits after cTBS over the right PPC.cTBS over the left PPC resulted in significant improvements in alerting,but not in the orienting.Furthermore,there were significant differences in the alerting and orienting indices between cTBS over the left and right PPC,but not in the executive control index.Conclusions The results suggest that the right PPC is associated with spatial orienting and the alerting function.The findings supported the theory of inter-hemispheric competition for visuospatial attention.Visuospatial attention bias might be selectively modulated through cTBS.
9.Culture conditions optimization and high cell density fermentation of recombinant bacteria producing heparinase II from Flavobacterium heparinum.

Heparinase II (Hep II) from Flavobacterium heparinum is an enzyme that could specifically cleave certain sequence of heparin and heparan sulfate. In this work, fermentation conditions of recombinant heparinase II (His-Hep II) producing bacteria were optimized, including initial induction time, inducer (IPTG) concentration, induction temperature and induction time. The optimum conditions were as follows: cultivating recombinant bacteria to exponential prophase under 37 degrees C, then adding IPTG to a final concentration of 0.3 g/L, finally cultivating recombinant bacteria under 20 degrees C for 10 h. The total crude enzyme activity reached 570 U/L. Based on these results, high cell density fermentation of recombinant bacteria was studied. The final OD600 could reach 98 and the total crude enzyme activity of His-Hep II increased to 9 436 U/L.

10.Questionnaire investigation on occurrence post-traumatic stress disorder and sleep quality of nurses in emergency department

Objective To investigate the relationship between post-traumatic stress disorder (PTSD) and sleep quality among emergency nurses, as well as the regulatory effects of gender on this relationship. Methods Two hundred and forty eight nurses in departments emergency of 8 tertiary general hospitals in Zhejiang province from October 2017 to February 2018 were enrolled as the respondents including 62 males and 186 females. The differences in sleep quality and PTSD among nurses of different genders were analyzed by questionnaires; the PTSD checklist and Pittsburgh sleep quality index (PSQI) were used to evaluate PTSD and sleep quality situations of different gender nurses; the correlations of PTSD, PSQI total score and each dimension score were analyzed by Pearson correlation analysis; and the regulatory role of gender on PTSD and PSQI score were analyzed by multiple linear regression method. Results The survey showed that the positive rate of sleep disorder was 46.77% (116/248) and the positive rate of PTSD was 43.95% (109/249) in emergency nurses. The total scores of PTSD and each score of following dimensions:re-experience symptoms, avoidance symptoms, high alert symptoms, and the total scores of PSQI and each score of dimensions of sleep quality, sleep entering time, sleep time, sleep efficiency, sleep disorders and using hypnotics of male nurses were significantly lower than those of the female nurses (total scores of PTSD: 33.94±9.36 vs. 38.05±5.69, re-experience symptoms: 9.43±2.62 vs. 10.40±2.39, avoidance symptoms: 14.85±4.43 vs. 16.54±3.69, high alertness symptoms: 9.66±2.35 vs. 11.04±2.49; total scores of PSQI: 7.39±1.42 vs. 8.32±3.52, sleep quality:0.91±0.42 vs. 1.08±0.49, sleep time: 1.82±0.77 vs. 1.69±0.69, sleep efficiency: 0.85±0.44 vs. 1.07±0.45, sleep disorders: 1.08±0.29 vs. 1.33±0.56, hypnotics: 0.23±0.08 vs. 0.46±0.12, all P < 0.05). Pearson correlation analysis showed that re-experience symptoms of female nurses had no significant correlations with sleep time and sleep efficiency (all P >0.05), but had significant positive correlations with other dimensions of PSQI (all P < 0.05), and the correlation coefficient was 0.22 - 0.64; in the aspect of male nurses, re-experience symptoms and hypnotic drugs, as well as high alert symptoms and sleep disorders, time function were of no correlations (all P > 0.05), but with other PSQI dimensions showed significant positive correlations (all P < 0.05), and the correlation coefficient was 0.11 - 0.43. The multiple linear regression analysis showed that the re-experiencing symptoms, avoidance symptoms and high alertness symptoms were positive predictive PSQI scores (β= 0.840, 0.970, 0.500, P = 0.026, 0.012, 0.000); the relationship between re-experiencing symptoms, high alertness symptoms and scores of PSQI could be adjusted by gender (β= 0.950, 0.290, P = 0.003, 0.032), the re-experiencing symptoms and high alertness symptoms could positively predict the PSQI scores of female nurses (β= 0.440, 0.570, P = 0.017, 0.001), and were slightly worse in predicting the PSQI scores of male nurses (β= 0.390, 0.110, P = 0.004, 0.048). Conclusion The predictive effect of PTSD different dimensions on PSQI score varies with difference in gender, and compared with male emergency nurses, the stress state of female emergency nurses is more easily to reduce sleep quality.
