1.Influencing factors on injury severity of expressway traffic accidents

Objective To study factors influencing injury severity of expressway traffic acci- dents. Methods Data of expressway traffic accidents involving casualties in areas around Hangzhou during 2005-2006 were collected and analyzed by non-conditional simple and multiple logistic regression analysis. Results (1) During 2005-2006, there were 220 traffic crashes involving 327 eases, of which there were 64 deaths (19.6%), 121 severe injuries (37.0%, ISS≥16 points) and 142 slight and mod- erate injuries (43.4%, 1 point≤ ISS≤15 points). (2) Non-conditional simple logistic regression analy- sis showed that risk factors significantly influencing the injury outcomes of traffic accidents included gen- der, weekday, weather, type of collision, vehicle type, driver category, purpose of vehicle use, purpose of travel and crash responsibility. (3) Non-conditional multiple logistic regression analysis manifested that influential factors were gender, type of collision, driver category and crash respensibihty. Conclu- sion Analysis of expressway traffic accidents data helps identify factors influencing injury severity of ex- pressway traffic accidents may provide suggestions for prevention or decrease of expressway traffic acci- dents.
2.Serum Procalcitonin Levels for Differentiating Gram-positive and-negative Bacteria Infection in Patients of ICU

OBJECTIVE To evaluate the value of serum procalcitonin(PCT) levels for differentiating Gram-positive and-negative bacteria infected critically ill patients in intensive care unit(ICU). METHODS Serum PCT levels were measured in 80 patients with sepsis and severe sepsis in ICU.Of the 80 patients,40 were infected with Gram-positive bacteria and 40 were infected with Gram-negative bacteria.Another 40 patients with fungi or viruses infection or non-infection from ICU were measured in parallel as control. RESULTS Serum levels of PCT in patients with Gram-negative bacteria were significantly higher than in those with Gram-positive bacteria and control ones(18.5?5.8ng/ml vs 3.9?2.1ng/ml and 0.3?0.2 ng/ml,respectively,P
3.Nosocomial Infection and Antimicrobial Resistance of Pseudomonas aeruginosa among Geriatrics Patients

OBJECTIVE To investigate nosocomial infection and antibiotic resistance of Pseudomonas aeruginosa among geriatrics patients and to instruct clinical monitoring and control infection.METHODS P.aeruginosa isolates from geriatrics patients were identified and the resistance to antibiotics was determined by K-B method.RESULTS The risk factors of nosocomial infection were underlying diseases,low immunological function,long time hospitalization,invasive treatment,and prolonged antibiotic therapy.The resistant rates to trimethoprim/sulfamethoxazole and cefotaxime in PA were 74.8% and 60.2%,respectively.The resistance rate to imipenem was 3.9%.CONCLUSIONS It is suggested to carry out proper measurement to control the spread of multiresistant strains in order to improve the treatment of PAE infection in geriatrics patients.
4.Role of standard trauma system in improving care of severe trauma patients

Objective To study the positive influence of a standard trauma system on the care of severely injured patients.Methods The severely injured patients (NISS≥16 points)were divided into study group ( in 2006-2008,after the establishment of trauma center in our hospital) and control group (in 2001-2005,before the establishment of trauma center),which was bound by the establishment of the trauma center in our hospital in January 2006.The injury severity,care and outcomes were recorded by using China Trauma Database and changes in efficiency and quality of injury care were compared.Results The study group (66 patients) and control group (260 patients) with NISS of (20.59 ±4.63)points and (20.57 ± 5.38 ) points respectively,were similar in the distribution of severity ( P > 0.05 ).The emergency care time was (0.33 ± 0.03) hour in the study group,which was significantly shortened compared with (0.57 ±0.35 ) hours in the control group (P < 0.01 ).The length of hospital stay was (27.64 ±29.01 ) days in the study group,which was shorter than (30.84 ± 32.87 ) days in the control group (P > 0.05 ),while the length of ICU stay was (2.98 ± 5.77 ) days in the study group,longer than (2.65 ± 7.00) days in the control group (P > 0.05 ).The recovery rate was significantly increased from 76.5% to 87.9% (P <0.05) and mortality was significantly decreased from 20.8% to 9.1% (P <0.05).Conclusion The study indicates that the standard first aid model can notably improve the trauma care in our hospital.
5.Clinical observation of ureteroscopy and minimally invasive percutaneous nephrolithotomy in the treatment of ;124 urinary calculi patients with acute renal dysfunction

Objective To explore the clinical value of ureteroscopy and minimally invasive percutaneous nephrolithotomy ( MPCNL) in the treatment of ureteral calculi patients with acute renal dysfunction .Methods Clini-cal data of 124 ureteral calculi patients with acute renal dysfunction were retrospectively analyzed .86 cases were trea-ted with holmium-laser lithotripsy under ureteroscope .38 cases were treated with MPCNL under the guide of ultra-sound.Results Three months after operation ,the stone clearance rate was 100%,and no severe complications were observed.The renal function decreased to normal levels in 102 cases(82.3%).Conclusion The holmium laser lith-otripsy under ureteroscope and MPCNL can deal with double sites of ureteral calculi ,and offer advantages with less in-vasion,safety and efficiency ,which can be the first choice for the ureteral calculi combined with acute renal dysfunc -tion.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
