1.Comparison of the Efficacy of Transurethral Holmium Laser and Electric Incision in the Treatment of Cystitis Glandularis:Randomized Controlled Trial

Objective To compare the clinical efficacy of two minimal invasive surgeries,transurethral Holmium laser and electric incision,in the treatment of cystitis glandularis(CG).Methods From January 2004 to June 2006,63 cases of CG were confirmed pathologically by using cystoscopy.Among the patients,35 were treated by transurethral Holmium laser and 28 by transurethral electric incision.All the cases received postoperative chemotherapy with mitomycin bladder irrigation and were followed up for 6 to 18 months.The efficacy of the two surgical procedures was evaluated during the follow-up.Results Compared with the electric incision group,the mean operation time and hospital stay was significantly shorter [(15.3?5.1) min vs(20.8?6.3) min,t=-3.831,P=0.000;and(2.4?1.7) d vs(4.0?1.5) d,t=-3.909,P=0.000;respectively],and the rate of complications was significantly lower [0%(0/35) vs 14.3%(4/28),?2=5.339,P=0.021] in the Holmium laser group.Six months after the operation,the cure rate of the Holmium laser group was significantly higher and the recurrence rate was significantly lower than those in the electric incision group [82.9%(29/35) vs 60.7%(17/28),?2=3.871,P=0.049;and 5.7%(2/35) vs 25.0%(7/28),?2=4.725,P=0.030].Conclusions The transurethral Holmium laser is a safe,effective,and convenient method for the treatment of CG.Since the method can not only achieve a high cure rate and low recurrence rate,but also avoid lower urinary obstruction,we recommend it as the first choice for CG.

In order to compare the effects of several experimental renal calcium oxalate stones formation models in rats and to find a simple and convenient model with significant effect of calcium oxalate crystals deposition in the kidney, several rat models of renal calcium oxalate stones formation were induced by some crystal-inducing drugs (CID) including ethylene glycol (EG), ammonium chloride (AC), vitamin D(3)[1alpha(OH)VitD(3), alfacalcidol], calcium gluconate, ammonium oxalate, gentamicin sulfate, L-hydroxyproline. The rats were fed with drugs given singly or unitedly. At the end of experiment, 24-h urines were collected and the serum creatinine (Cr), blood urea nitrogen (BUN), the extents of calcium oxalate crystal deposition in the renal tissue, urinary calcium and oxalate excretion were measured. The serum Cr levels in the stone-forming groups were significantly higher than those in the control group except for the group EG+L-hydroxyproline, group calcium gluconate and group oxalate. Blood BUN concentration was significantly higher in rats fed with CID than that in control group except for group EG+L-hydroxyproline and group ammonium oxalate plus calcium gluconate. In the group of rats administered with EG plus Vitamin D(3), the deposition of calcium oxalate crystal in the renal tissue and urinary calcium excretion were significantly greater than other model groups. The effect of the model induced by EG plus AC was similar to that in the group induced by EG plus Vitamin D(3). EG plus Vitamin D(3) or EG plus AC could stably and significantly induced the rat model of renal calcium oxalate stones formation.
3.Serum Procalcitonin Levels for Differentiating Gram-positive and-negative Bacteria Infection in Patients of ICU

OBJECTIVE To evaluate the value of serum procalcitonin(PCT) levels for differentiating Gram-positive and-negative bacteria infected critically ill patients in intensive care unit(ICU). METHODS Serum PCT levels were measured in 80 patients with sepsis and severe sepsis in ICU.Of the 80 patients,40 were infected with Gram-positive bacteria and 40 were infected with Gram-negative bacteria.Another 40 patients with fungi or viruses infection or non-infection from ICU were measured in parallel as control. RESULTS Serum levels of PCT in patients with Gram-negative bacteria were significantly higher than in those with Gram-positive bacteria and control ones(18.5?5.8ng/ml vs 3.9?2.1ng/ml and 0.3?0.2 ng/ml,respectively,P

Objective To definite the status of tone of autonomic nervous system during water immersion process by analyzing the nonlinear characteristic of ECG signals;To explore the relation of approximate entropy(ApEn),scaling exponent(?) changes and modulation function status of autonomic nervous system.Methods The ECG signals of rats during the process of restraint stress(RS),water-immersion restraint stress(WRS) and Vagotomy and Water-immersion restraint stress(vagotomy-WRS) were recorded.The ApEn and ? of RR intervals were calculated.Results After the stress,the differences of the value of ApEn and ? of RR intervals between RS and vagotomy-WRS groups and between different periods in vagotomy-WRS group were not significant.The value of ApEn increased in WRS group and decreased in RS group significantly.Compared with pre-stress,? showed decreased trend,all of the values smaller than 0.5 in WRS group and increased obviously,most values larger than 0.5 in RS group.Conclusion The increased ApEn and decreased ? indicates a higher intensity of parasympathetic activity.The parasympathetic activity is the predominant response of autonomic nervous system during water immersion process.
5.Effects and mechanism of EGCG on human prostate cancer xenografted tumor growth and connexin43 expression in nude mice

Objective To observe the effects of ( - )-epigallocatechin-3-gallate (EGCG) on human prostate cancer xenografted tumor growth and connexin43 expression in nude mice,and explore the mechanism of the EGCG on prevention for prostate cancer.Methods The methyl thiazolyl tetrazolium and annexin-V/PI double-labeled flow cytometry methods were used to observe the growth inhibiting rate (IR)and apoptosis rate (AR) of human prostate cancer cell line PC-3 which was treated by EGCG at different concentration (10,20 and 40 mg/L,respectively).The scrape-loading fluorescence dye transfer method was applied to assess the gap junction intercellular communication (GJIC) through fluorescence microscope.PC-3 cells were subcutaneously transplanted to establish tumor-bearing nude mice model.A total of 32 mice were randomly divided into four groups,both control group and three treatment groups were treated with different doses of EGCG ( 10,20 and 40 mg/kg,respectively).After two weeks,the mice prostate tumor tissues were taken out.The tumor wet weight was measured and tumor growth inhibiting rate was calculated.The tumor microvascular density (MVD) and apoptosis index (AI) were detected by the immunohistochemical techniques and terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling techniques,respectively.Semi-quantitative reverse transcription polymerase chain reaction was used to examine the expression level of the Cx43 mRNA.Results EGCG at concentration ( 10 and 20 mg/L) could significantly inhibit the proliferation[(22.33 ±4.62)%,(38.67 ±5.67)% vs (3.47 ±0.31 )%,P <0.01],induce the apoptosis [(7.84 ± 1.37 ) %,( 24.53 ± 2.28 ) % vs ( 2.17 ± 0.70 ) %,P < 0.01] and enhance the GJIC of PC-3 cells.EGCG of different doses could inhibit prostate cancer xenografted tumor growth,induce tumor cells apoptosis and inhibit angiogenesis.EGCG ( 20 and 40 mg/kg) could effectively up-regulate Cx43 mRNA expression in xenografted tumor (0.58 ± 0.08,0.80 ± 0.07 vs 0.42 ± 0.04,P < 0.0 ).The effects had significant correlation with the dose-dependent of EGCG ( P < 0.05 ).Conclusions EGCG could up-regulate the Cx43 expression and enhance the gap junction intercellular communication mediated by Cx43 in the prostate tumor,which provide the experimental evidence for the mechanism of its effectively inhibiting the prostate cancer growth.
6.Study of the expression and function of PIWIL2 in the bladder urothelial carcinoma

Objective To investigate the gene expression of PIWIL2 in the bladder urothelial carcinoma (BTCC) and siRNA interact on PIWIL2 gene expression in human bladder cancer cell line BIU-87.Methods Semi-quantitative reverse transcription polymerase chain reaction (qRT-PCR) was applied to detect the PIWIL2 expressions in tissues of BTCC (46 cases),cystitis glandularis(21 cases),adjacent non-cancerous tissues (17 cases) and normal bladder tissues (7 cases). 3 specific siRNA targeted PIWIL2 gene were synthesized after designed and transferred. After siRNA was transferred into BIU-87 cells, MTI and TUNEL methods were applied to detect the proliferation inhibitory rate (IR) and apoptosis index (AI) in BIU-87 cells,qRT-PCR and Western blot were used to examine effects of siRNA on the expressions of the PIWIL2 gene and protein,respectively.Results The expression rate of PIWIL2 mRNA in BTCC tissues was 76.08 %(35/46) and significantly higher than those in the cystitis glandularis tissues (42.86 %,9/21),adjacent non-cancerous tissues (41.17 %,7/17) and normal tissues (7.14 %,1/14) (P =0.008,P =0.010,P =0.000).The IR [(37.52±8.84) %,(64.36±9.64)%] and (62.94±8.43) %] and AI [(26.18±5.42) %,(38.75±6.19) % and (40.02±5.64) %] of BIU-87 cells in the siRNA 1~3 groups were respectively significantly higher than those [(1.97±0.02) % and (3.35±0.47) %] in the control group(P=0.000),and expressions of PIWIL2 mRNA and protein in the siRNA groups were both lower than those in the control group. Moreover, the effects of siRNA 2 group and siRNA 3 group on inhibiting PIWIL2 expression, IR and AI of BIU-87 cells were stronger than siRNA 1 group. Conclusion The over-expression of PIWIL2 suggested that it played an important role in the mechanism of development and malignant progression of BTCC. The siRNA of transcription can significantly inhibit its expression, induce cell apoptosis and inhibit the growth of BIU-87 cells which might provide the experimental evidence for the gene targeting therapy of bladder tumor.
7.Algorithm of real-time QRS complex detection for the GPRS mobile ECG telemonitoring system

Objective To develop a real-time QRS complex detection algorithm of dynamic ECG signals for the GPRS mobile telemonitoring system. Methods Before the first and second derivatives of ambulatory ECG signals were processed by moving average method, the signals sampled from CM5 monitoring lead were filtered with the average of continuous four ECG sample signal points. The R waves could be detected precisely by local minima of second derivatives and Q & S waves were located correctly by cross-zero points of first derivatives of ambulatory ECG signals in a short-time searching windows. The QRS recognition thresholds, which could revise themselves according to the detected values and vary with the analyzing signals, were designed in this paper. Results With a polynomial computation complexity, the novel algorithm insensitive to baseline draft and noise caused by mobile communication filtered power-line interference and most of muscle noise and reduced the search time below 0.02 s during detecting each Q wave, R wave and S wave. For the normal and clinical patients, this algorithm correctly detected up to 99.8% of the QRS complex of ambulatory ECG signals. Conclusion The algorithm can meet the need of real-time QRS complex detection and analysis for the GRRS mobile ECG telemonitoring system.
8.Clinical observation of ureteroscopy and minimally invasive percutaneous nephrolithotomy in the treatment of ;124 urinary calculi patients with acute renal dysfunction

Objective To explore the clinical value of ureteroscopy and minimally invasive percutaneous nephrolithotomy ( MPCNL) in the treatment of ureteral calculi patients with acute renal dysfunction .Methods Clini-cal data of 124 ureteral calculi patients with acute renal dysfunction were retrospectively analyzed .86 cases were trea-ted with holmium-laser lithotripsy under ureteroscope .38 cases were treated with MPCNL under the guide of ultra-sound.Results Three months after operation ,the stone clearance rate was 100%,and no severe complications were observed.The renal function decreased to normal levels in 102 cases(82.3%).Conclusion The holmium laser lith-otripsy under ureteroscope and MPCNL can deal with double sites of ureteral calculi ,and offer advantages with less in-vasion,safety and efficiency ,which can be the first choice for the ureteral calculi combined with acute renal dysfunc -tion.
9.Diagnosis and Treatment of Early Anuria or Oliguric after Kidney Transplantation (Report of 66 Cases)

Objective:To investigate the diagnosis and treatment of early anuria or oliguric after kidney trans-plantation. Methods: Retrospective analysis of cause, dignosis and different immunodepressant treatments (FK506/CsA + MMF +Pred) of early anuria or oliguria in the 66 patients after kidney transplantation. Results: The primary cause of early anuria or oliguric after kidney transplantation in 66 patients was acute tubular necrosis (77.27%), the second was acute rejection (10.61%). 2 patients with primary grafts nonfunction, and the other 2 patients with rupture and thrombosis of renal artery respectively, were removed grafts after transplantation. The renal function of 34 patients treated with FK506 became normal in 5-35 d after transplantation. The renal function of 24 patients treated with CsA recovered in 7-48 d after transplantation except 1 patient was dead because of acute rejection combined with serve pulmonary infection. And the other 3 patients' serum creatinine ranged from 142 μtmol/L to 215 μtmol/L. Conclusions: The cause of early anuria or oliguric after kidney transplantation should be promptly analyzed and correct treatment should be applied. The combination immunodepressant treatments (FK506 + MMF+Pred) contributed to the early recovery of the renal function after transplantation.
10.Porphyromonas gingivalis persisters induce the immuno-inflammatory responses in macrophages by upregulating the forkhead box1 signaling pathway

Objective:To investigate the effects of Porphyromonas gingivalis (Pg) persisters (Ps) on immuno-inflammatory responses in macrophages, and to explore the underlying mechanisms. Methods:Pg cells were cultured to the stationary phase (72 h), and subsequently treated by high concentration of metronidazole at 100 mg/L, amoxicillin at 100 mg/L and the combination of them for different time period, named as metronidazole group, amoxicillin group and (metronidazole+amoxicillin) group. Pg cells without treatment were used as Blank control. The survival profile of PgPs cells was measured by colony-forming unit assay. The living state of PgPs was observed by Live/Dead staining. Then, Pg and metronidazole-treated PgPs (M-PgPs) were used to treat macrophages, named as Pg group and M-PgPs group. Transmission electron microscopy (TEM) was used to observe the bacteria in the macrophages. The expression levels of proinflammatory cytokines in macrophages were determined by real-time fluorescence quantitative PCR and enzyme-linked immunosorbent assay. The location of forkhead box transcription factor 1 (FOXO1) was detected by confocal immunofluorescence microscopy. After inhibiting or enhancing the FOXO1 expressions using inhibitors (Fi) or activators (Fa) respectively, the macrophages were treated with Pg and M-PgPs, divided as Blank group, Pg group, M-PgPs group, Fi group, (Fi+Pg) group, (Fi+M-PgPs) group, Fa group, (Fa+Pg) group and (Fa+M-PgPs) group. Then, the expression pattens of proinflammatory cytokines were assessed.Results:Remarkable number of lived PgPs was observed, both in planktonic culture and Pg biofilms either treated with metronidazole, amoxicillin or both, and those persisters could form new colonies. Pg and M-PgPs were able to enter into the macrophages and the protein expression levels of interleukin (IL)-1β, IL-6, IL-8 and tumor necrosis factor-α (TNF-α) [Pg group: (2 392±188), (162±29), (5 558±661), (789±155) μg/L; M-PgPs group: (2 415±420), (155±3), (5 732±782), (821±176) μg/L] were significantly upregulated than those in Blank group [(485±140), (21±9), (2 332±87), (77±7) μg/L] ( P<0.01). Moreover, Pg and M-PgPs could facilitate the nuclear translocation and accumulation of FOXO1. In addition, the relative mRNA expression levels of FOXO1, B-cell lymphoma 6 and Krüppel-like factor 2 were upregulated when compared to Blank group ( P<0.05). Furthermore, the protein expression levels of IL-1β, IL-6, IL-8 and TNF-α in Fi+Pg group [(1 081±168), (70±8), (1 976±544), (420±47) μg/L] were remarkably lower than Pg group [(4 411±137), (179±6), (5 161±929), (934±24) μg/L] ( P<0.05). Similarly, the protein expression levels of IL-1β, IL-6, IL-8 and TNF-α in Fi+M-PgPs group [(1 032±237), (74±10), (1 861±614), (405±32) μg/L] were remarkably lower than M-PgPs group [(4 342±314), (164±17), (4 438±1 374), (957±25) μg/L] ( P<0.05). On the contrary, the protein expression levels of IL-1β, IL-6, IL-8 and TNF-α in Fa+Pg group [(8 198±1 825), (431±28), (8 919±650), (2 186±301) μg/L] and Fa+M-PgPs group [(8 159±2 627), (475±26), (8 995±653), (2 255±387) μg/L] were significantly higher than Pg group and M-PgPs group, respectively ( P<0.05). Conclusions:PgPs are highly tolerant to metronidazole and amoxicillin. The M-PgPs could enhance the immuno-inflammatory responses in macrophages by upregulating the FOXO1 signaling pathway, while this effect exhibits no significant difference with Pg.
