1.Distribution of intravenously transplanted bone mesenchymal stem cells in ischemic brain and their effects on learning and memory in rats

Objective To investigate the distribution and migration of bone mesenchymal stem cells (BMSCs) in the brain after intravenous injection into brain-ischemic rats and to explore the effects on the learning and memory in rats. Methods Bone mesenchymal stem cells from adult rats were cultured in vitro. Then the cells were labeled by Hoechst 33342 and infused into the adult rats on day 3 after the ischemia/reperfusion model was established by cerebral artery occlusion. Morris water maze test was used to explore the effects on learning and memory function of the rats at 3 weeks after transplantation. Fluorescence microscope was used to identify BMSCs in rat brains at 4 weeks after transplantation. 2.Effect of early treatment with clopidogrel in patients with non-ST-elevation acute coronary syndrome undergoing early percutaneous coronary intervention

Objective To evaluate the short-term effect of early treatment with clopidogrel before elective coronary stenting in patients with NSTE-ACS,compared with treatment with clopidogrel at the time of the procedure. Methods Patients with NSTE-ACS undergoing elective coronary stenting were randomly divided into two groups: early treatment group (n=272) and control group (n=265). Patients in the early treatment group were given clopidogrel immediately since hospitalization (300 mg loading dose, then 75 mg once daily); while in the control group, patients were given clopidogrel (300 mg) after coronary angiography followed by 75 mg per day after PCI. The composite endpoints were death, recurrent angina pectoris, instent thrombosis, myocardial infarction and urgent target vessel revascularization within 30 days after coronary stenting. Results Differences in baseline characteristics among patients between the two groups were negligible. There were no significant difference in the target vessel lesions requiring stenting between the 2 groups. Compared with the control group, early administration of clopidogrel showed a lower rate of death, myocardial infarction, refractory ischaemia, instent thrombosis and revascularization within 30 days of PCI. Conclusion Compared with administration of clopidogrel at the time of coronary stenting, pre-treatment with clopidogrel in patients with NSTE-ACS could significantly reduce the odds of adverse cardiovascular event and improve clinical outcomes within 30 days of PCI.
3.Differentiation of neural stem cells from the anterior subventricular zone into dopaminergic neurons induced by astrocyte-conditioned medium in vitro

BACKGROUND: Effective proliferation in vitro of neural stem cells and directional induction and differentiation of dopaminergic neurons play a key role in neural stem cell transplantation for treatment of degeneration of the nervous system and repair of injured nervous system. OBJECTIVE: To explore the differentiation of neural stem cells from anterior subventricular zone into dopaminergic neurons in astrocyte-conditioned medium. DESIGN, TIME AND SETTING: The in vitro controlled cytology experiment was performed at the Experimental Center of Xinqiao Hospital of Third Military Medical University of Chinese PLA from December 2006 to August 2007. MATERIALS: A total of 15 neonatal KM mice aged 2 days and 40 pregnant 16 days Wistar rats were provided by the Experiment Animal Center of Third Military Medical University of Chinese PLA. METHODS: Astrocytcs of KM mice were isolated and purified by a standard shaking method and differential adhesion. Astrocyte-conditioned medium was collected when astrocytcs at passage 3 were cultured for 5 days maintained at –20 ℃ for use. Neural stem cells from the anterior subventricular zone of Wistar rats were isolated and cultured in serum-free DMEM/F12, supplemented with B27 and basic fibroblast growth factor for primary culture. Neural stem cells at passage 3 were collected and incubated at 0.5?108/L. Cells in the control group were incubated in the DMEM/F12 medium containing fetal bovine serum of 0.1 volume fraction. Cells in the experimental group were incubated in the astrocyte-conditioned medium. MAIN OUTCOME MEASURES: Neural stem cells in the anterior subventricular zone were determined by the immunocytochemical technique. Positive rate of the neural stem cell differentiation into dopaminergic neurons in the anterior subventricular zone was detected by the flow cytometry. RESULTS: Cells in neurospheres expressed nestin and differentiated into neuro-specific enolase and glial fibrillary acidic protein. Tyroxine hydroxylase positive cells were found in the experimental group, showing round or ellipse. Tyroxine hydroxylase was seen in cytoplasm and cluster-shape processes. Tyroxine hydroxylase positive cells in control group were different from that in the experimental group in amount and appearance. 4.Change of cornea posterior elevation after laser in situ keratomileusis

Background Keratectasia after laser in situ keratomileusis (LASIK) is one of the serious complications which affect the vision.The measurement of cornea posterior elevation is an important way helping to find this change.Objective The aim of this study was to investigate the change of cornea posterior elevation after LASIK in myopic eyes with astigmatism and affecting factors.Methods A series case-observational study was adopted.One hundred and twenty-seven myopic eyes with astigmatism of 66 patients who received LASIK in 2008 May through 2010 January in Beijing Tongren Hospital were retrospectively analyzed to evaluate the change of cornea posterior elevation following the LASIK.The parameters related to cornea posterior elevation were measured and compared before and 3 months,6 months and 1 year after surgery,respectively,with Oculyzer anterial segment analysis system.Results The height values from posterior cornea highest point,the lowest point and central vertex central elevation zone were (12.20±3.39),(-19.02±7.38) and (1.05 ±3.25) μm respectively before LASIK and were (14.38±3.80),(-18.55±7.11),(2.83±4.81)μm in3 months and (13.99±3.38),(-17.57±6.54),(2.45±4.61) μm in 6 months after LASIK.They were (14.40±3.85),(-17.76±6.00),(2.16±5.00) μm in 1 year after surgery.Significant increases were found in the vertex height and central elevation after LASIK compared with before surgery(highest pointq=6.813,5.594,6.875,all at P<0.001.central vertex:q=4.488,P=0.002;q =3.530,P=0.013 ;q =2.799,P =0.047).However,no significant difference was seen in various time points after LASIK (P>0.05).A positive correlation was obtained between the height value of the posterior cornea central vertex with the spherical equivalent (SE),maximum cutting depth or cutting proportion 1 year after LASIK (r =0.295,0.297,0.295,all at P=0.001),and there was a negative correlation between it with residual stroma (r=-0.208,P=0.019).The intraocular pressure (IOP) measured with noncontact tonometry (NCT) was (14.24±3.33) mmHg before LASIK,and those of 3 months,6 months and 1 year were (8.42± 1.90),(8.61 ± 1.64) and (8.76± 1.64)mmHg after LASIK,showing a significant lowing in IOP after LASIK in comparison with before LASIK (q =29.851,28.317,26.337,all at P<0.001).But no significant change was found in the different time points after LASIK (P>0.05).There was significant difference in the IOP corrected by Ehlers after LASIK (P>0.05).The IOP before LASIK had positive correlation with the central elevation of posterior cornea surface 1 year after LASIK(r=0.258,P =0.003).Conclusions The cornea posterior elevation increases slightly early stage after LASIK but retains stable level with lapse of time.The refraction diopter before LASIK is the main factor affecting the posterior corneal shape.To reduce the risk of corneal ectasia,more attention should be paid to the reserving of appropriate corneal residual stroma and smallest degree of eccentricity during the ablation procedure.
5.Immunohistochemical technology of labeling skin of inner mongolia cashmere goats with BrdU

Objective:The purpose of this study is to investigate the feasibility of labeling skin of inner mongolia cashmere goats with BrdU,and to optimize its immunohistochemical conditions.Methods:We injected different dose of BrdU into the neck vein of cashmere goats,then we took samples from the back skin of the goats,fixed in 4% paraformaldehyde.Next the samples were embedded with paraffin and sectioned.We examined the samples with immunohistochemical technology to test the effect of the influencing factors to labeling results as the BrdU injection,such as dosage,the concentration of second antibody and streptavidin-peroxidase,the way of antigen pretreatment,and DNA denature temperature.Results:We observed red BrdU positive cells in labeled goats whereas not in the control.Meanwhile,in the samples of the unlabeled goats or those incubated with PBS,no nuclear staining is observed.Conclusion:BrdU can be used for labeling of inner mongolia cashmere goats in vivo,which have no obvious dosage effect.The best condition of labeling is that puttig the section in 2 mol/L HCl 30 min at 37℃,in trypsin 30 min at 37℃.The most appropriate concentration of biotin-conjugated second antibody and streptavidin-peroxidase is 1∶100,under which the results of immunohistochemical are the best.
8.PBL curriculum in the University of Manchester in the UK and some relevant thoughts about its current practice

University of Manchester who first introduced Problem based learning(PBL)to the UK medical education,and adopted this new curriculum in 1994.Notably,Cambridge and Oxford have always had a high proportion of student-centred and self-directed learning,and have therefore not introduced PBL.Other medical schools have followed the example of Manchester,now PBL has become the mainstream curriculum in British medical edacation.First,this article is going to introduce how PBL curriculum is structured at Manchester medical school,and then reflect and give some thoughts on its current practice.

10.IL-21 level in Chinese HIV infected individuals and its dynamics undergoing HAART

To investigate the dynamics of interleukin-21 (IL-21) cytokine in the Chinese HIV patients undergoing highly active antiretroviral therapy (HAAPT).Methods A total of 25 adults with chronic HIV infections,responding to combined highly active antiretroviral therapy (HAART) guideline criteria were enrolled for a 1-year follow-up.After signing an informed consent,20 mL blood was collected from each patient at the base line,6 month and 12 month,respectively.CD4 and CD8 cell count was quantified by flux cytometry,serum HIV RNA quantified by real time PCR and IL-21 concentrations by ELISA.Results IL-21 levels increased gradually during the follow-up but did not reach the healthy levels.IL-21 correlated positively with the CD4 cells but not with CD8 T cells.HIV RNA correlated negatively with CD4 cell count but did not show any relationship with the CD8 cells.Conclusion IL-21 has potential role in the immunopathogenesis of HIV,and might be an important factor in immune construction during HAART.
