1.Cultivation of clinical pathological quality of non-pathological-specialty medical undergraduates

Clinical medical workers should understand the pathology,and the cultivation of the clinical pathological quality needs to start from the undergraduate course of medical students.In the past 6 years,based on the platform of undergraduate pathology science and technology innovation,and histology research platform of Department of Pathology,and through the creation of the clinical pathology elective courses and other forms,the researchers have given medical undergraduates the systematical and standardized training of the concept,knowledge,skills and application of pathology to develop their clinical and pathological quality during basic courses,professional courses and internship stage of a medical undergraduate.Practice shows that the relevant teaching measures enable students to fully understand the importance of clinical pathology,and improve their ability to apply clinical and pathological knowledge to solve clinical problems.
2.Antiendothelial cell antibodies in systemic vasculitis:prevalence and clinical significance

Objective To investigate the clinical significance of antiendothelial cell antibodies (AECA) in systemic vasculitis. Method With Human umbilical vein endothelial cell (HUVEC) as substrate cell, sera from 129 systemic vasculitis patients [including 59 Behcet′s disease(BD), 28 Takayasu arteritis (TA), 20 Wegener′s granulomatosis (WG), 8 polyarteritis nodosa (PAN), 9 microscopic polyangiitis (MPA), 5 Churg-Strauss syndrome (CSS)] were screened for the presence of AECA by ELISA. Sera from SLE, RA and healthy donors were examined as controls. The association of AECA to clinical disease activity was analyzed. Result The prevalence of AECA by HUVEC cell-ELISA was 59% in systemic vasculitis [48% in BD,79% in TA, 65% in WG, 63% in PAN, 44% in MPA, 80% in CSS], 46% in SLE, 4% in RA, and 2.4% in control group. 3.Therapeutic Effect of He-Mu-Shu Point Combination Acupuncture for Patients with Functional Diarrhea and Its Influence of Anxiety

Objective To observe the efficacy of electroacupuncture(EA) on the acupoint combination of He-(sea) points, front-mu points, and shu-points(abbrev. He-Mu-Shu) for the treatment of functional diarrhea, and to explore the effect of He-Mu-Shu point combination EA on the anxiety of patients. Methods A randomized controlled clinical trial was performed in 62 functional diarrhea patients. All of the patients were randomly divided into He-Mu-Shu group(32 cases) and medicine group(30 cases). He-Mu-Shu group was treated with EA on unilateral Quchi, Shangjuxu, Tianshu, Dachangshu point alternatively, and medicine group was treated with oral use of Loperamide hydrochloride capsules. Both groups were treated for 4 continuous weeks. Defecation frequency, stool property and anxiety scores were observed before treatment and 2, 4 weeks after treatment. Results After treatment, weekly defecation frequency, stool property and anxiety scores were improved in both groups, and the differences were statistically significant compared with those before treatment(P < 0.05). He-Mu-Shu point combination EA had better effect on improving weekly defecation frequency, stool property and anxiety scores of functional diarrhea patients than Loperamide hydrochloride capsules, and the differences were statistically significant (P < 0.05). Conclusion The clinical efficacy and anxiety-relief action of He-Mu-Shu point combination EA on functional diarrhea are superior to oral use of Loperamide hydrochloride capsules.

Medical knowledge and knowledge of medical ethics are the basic ethical qualities that a good medical editor should possess. Editor should strictly control the manuscript process, abide by the professional ethics, and contribute to the fast and correct spread of medical information.

6.Von Willebrand factor research on related factor of coagulation abnormalities in type 2 diabetic nephropathy

Objective To study the early changes of the coagulation system in type 2 diabetic nephropathy.Methods Sixty-two cases of patients with type 2 diabetic nephropathy were divided into two groups:normal albuminuria group ( N-UAlb group,UACR < 30 mg/g,32 cases ),microalbuminuria group ( MUAlb group,UACR:30~300 mg/g,30 cases).Thirty healthy persons constituted a control group (NC group).Fibrinogen( FIB ),antithrombin Ⅲ ( AT-Ⅲ ),protein C ( PC ),protein S ( PS ) were measured by coagulation analyzer,while yon willebrand factor (vWF) and platelet granule membrane protein 140 (GMP-140) were detected by ELISA assay,platelet count (PLT),mean platelet volume(MPV),platelet hematocrit (PCT),platelet distribution width(PDW) by hematology analyzer.Results The level of fibrinogen,GMP-140 and vWF in the M-UAlb group were (4.20 ± 1.53 ) g/L,( 30.03 ± 7.77 ) μg/L,and ( 315.53 ± 47.24 ) % respectively,vwhich were significantly higher than those in the N-UAlb group [ ( 3.21 ± 0.89 ) g/L,( 18.22 ± 5.08 ) μg,/L and ( 191.88 ± 57.25 ) % respectively ] and the NC group [ ( 2.75 ± 0.53 ) g/L,( 14.26 ± 2.29 ) μg/L and ( 138.12 ± 61.27 ) % respectively ] ( F =5.42,10.42,30.44,P < 0.05 or 0.01 ).The fibrinogen,vWF,GMP-140 were positively correlated with UACR ( r =0.313,P < 0.05 ; r =0.620,P < 0.01 ; r =0.680,P < 0.01 ) and PC was negatively correlated with UACR ( r =-0.255,P < 0.05 ).Conclusion Hypercoagulable state in diabetic nephropathy is associated with the high fibrinogen,endothelial dysfunction and platelet activation,and these changes have already emerged in patients without albuminuria.This might mind us that we should strengthen anticoagulant therapy on patients when they are not progressing to albuminuria.

Objective: To observe the precess of evoking high risk complications when using ATRA to induce differentiation therapy acute promyelocytic leukemia(APL),to provide criteria which could be useful for earlier diagnosis and therapy. Methods: Ten patients with primary APL and clinical data was assessed with unification centification. Results:In the induction period the main complications which would be dangerous for the life ,were hyperleucocytosis symptom and RAS,and their incidence were 90% and 10% separately. Conclusion: In the period of therapy using ATRA,we should pay close attention to the clinical situation and leucocyte number,and medicating on time,in order to save the patients’ life.

Objectives To assess the performance of first trimester ultrasound screening for fetal structural and chromosomal anomalies based on a detailed anomaly and nuchal translucency (NT) scan at 11-13+6 weeks' gestation.Methods A prospective cohort study was conducted at Nanjing Drum Tower Hospital.Fetuses with a crown-rump length (CRL) between 45 mm and 84 mm scanned during December 2015 to March 2016 were enrolled in this study.After a detailed first-trimester anomaly scan followed the protocol of systematic standardized scan plans,fetuses with congenital abnormalities were screened out.Second trimester ultrasound screening and postnatal examination were performed for further examination of fetal anomalies.Cytogenetic analysis was performed on the fetuses with informed consent.Results (1) A total of 1 154 fetuses were enrolled in this study and among them,36 (3.1%) cases of fetal abnormalities were diagnosed through prenatal examination (35 cases) and postnatal examination (one case).(2) Twenty-one (58.3%) out of the 36 cases with structural and chromosomal anomalies were screened out by using the first-trimester scan,including eight cases of congenital cardiac defect (two cases of atrioventricular septal defect,one case of tricuspid atresia,one case of tetralogy of tetralogy,one case of right ventricle aneurysms and one cases of hypoplastic left heart syndrome combined with cystic hygroma with one case combined with polydactyly),four cases of central nervous system anomaly (three cases of exencephaly and one case of anencephaly combined with double outlet right ventricle),two cases of cleft palate/lip with one case combined with double outlet right ventricle,two cases of exomphalos,one case of amniotic band syndrome,one case of spinal bifida combined with megacystis,one case of umbilical cyst,one case of polydactyly and one case of cystic hygroma.One case of twin pregnancy chose selective fetocide to the fetus with exencephaly and 16 cases terminated pregnancy.The other four cases were confirmed by second trimester ultrasound screening and postnatal examination.Fourteen (38.9%,14/36) new cases of structural and chromosomal anomalies were detected by the second-trimester scan,six of which terminated the pregnancies and the rest were confirmed at term.One (2.8%,1/36) case of polydactyly was detected postnatally.(3) Chromosomal microarray analysis was performed on 28 cases,seven of which were identified as having chromosomal abnormalities including five cases detected in the first trimester and two cases detected in the second trimester.(4) Out of the 20 fetuses with abnormal NT in early trimester,which accounted for 1.7% of all enrolled fetuses,nine were indentified with major structural or chromosomal abnormalies,a quarter of all abnormal fetus.Conclusions Detailed anomaly scan and NT scan in the first-trimester can increase the detection rate of fetal structural and chromosomal anomalies as compared with the traditional NT scan and provide earlier detection of severe fetal abnormalities as compared with second trimester anomaly scan.
