1.Intraperitoneal transplantation of human umbilical cord mesenchymal stem cells in the treatment of acute liver injury in rats

BACKGROUND:In vitro experiments have confirmed that human umbilical cord mesenchymal stem cells can be induced to differentiate into hepatocyte-like cells, thus which can be considered to function as liver repair.
OBJECTIVE:To observe the therapeutic effect of human umbilical cord mesenchymal stem celltransplantation on acute liver injuries in rats through in vivo animal experiments.
METHODS:Healthy Sprague-Dawley rats were randomly divided into three groups:normal control group without modeling, celltransplantation group, and PBS group. Rat models of acute liver injury were prepared by 10%CCl4-olive oil solution in the celltransplantation and PBS groups which were fol owed by intraperitoneal injection of 0.5 mL human umbilical cord mesenchymal stem cellsuspension and 0.5 mL PBS, respectively.
RESULTS AND CONCLUSION:Hematoxylin-eosin staining showed that pathological changes related to acute liver injury appeared at 24 hours after intraperitoneal injection of CCl 4 . Then, the liver structure recovered at 7 days after celltransplantation, but it did not recover til the 14th day after PBS injection. Compared with the normal control group, serum alanine aminotransferase and aspartate aminotransferase levels were significantly increased in the other two groups (P<0.05-0.01). In the celltransplantation group, the serum alanine aminotransferase and aspartate aminotransferase levels were significantly lower than those in the PBS group at 3 days after treatment (P<0.05-0.01), and recovered normal y after 7 days. cells positive for anti-human nucleoprotein antibody were found in the portal area of liver tissues in the celltransplantation group after 3 days of transplantation, and then cells positive for anti-human albumin antibody appeared after 7 days. These findings indicate that intraperitoneal transplantation of human umbilical cord mesenchymal stem cells can improve liver function and repair injured liver tissues after acute liver injury in rats to some extent.

3.Bacterial Endotoxin Test for Nicardipine Hydrochloride Glucose Injection

OBJECTIVE:To establish a method for the bacterial endotoxin test of nicardipine hydrochloride glucose injection.METHODS:The bacterial endotoxin test was carried out according to the bacterial endotoxin test in the addenda of China Pharmacopeia 2005 edition(second part).RESULTS:Nicardipine hydrochloride glucose injection showed interference on the bacterial endotoxin test when water was taken as solvent.The preliminary interference test and interference test revealed that using thinnerⅠas solvent,the sample injection in over 4-fold dilution had no interference on the bacterial endotoxin test.CONCLUSIONS:It is feasible for bacterial endotoxin test of nicardipine hydrochloride glucose injection be conducted in a limit value of 0.5 EU?mL-1.The bacterial endotoxin text can substitute pyrogen test in the detection of nicardipine hydrochloride glucose injection.
4.Study of clinical nurses scientific research integrity awareness scale reliability and validity

Objective To design the scale of scientific research integrity awareness among clinical nurses and study its reliability and validity.Methods The scale were completed by reviewing literature,collecting data and interviewing nursing specialists.A total of 286 clinical nurses were investigated by the scale for the research in its reliability and validity.Results The index of content validity was 0.885,and the reliability was 0.883 and the test-retest reliability was 0.878.Conclusion The scale is reliable and valid when used for assessing the integrity awareness of nurses.
5.A survey of oral health cognitive behaviors and impact factors of the elderly people in Jining

ObjectiveTo learn the oral health cognitive behaviors and impact factors of the elderly people in Jining.MethodsThe number of 1026 elderly people were investigated by stratified cluster sampling method using the self-made questionnaire of oral heahh behaviors of the elderly people.Results ①In the number of 1026 elderly people,denture lost teeth accounted for 5.3%.There was no gendr differece in denture lost teeth( χ2=0.162,P > 0.05 ) but age( t =- 9.089,P < 0.01 ).②Brush teeth every day of the test total 86.3%,56.4%after dinner would gargle,0.6% daily flossing,35.4% used shaft brush.③The oral health behavior was related to the elderly pepole' gender,cultural level,economic income and participate in the oral health education activities based on the binary logistic.ConclusionThe old oral health behaviors are affected by various factors.To improve oral health awareness and promote oral health,the further strengthening community propaganda and education about oral health and control measures should be reinforced on the basis of the old oral health cognitive and behavioral status.
6.The clinical value of measurement of urine nuclear matrix protein 22 and urinary bladder cancer antigen content in patients of bladder transitional cell carcinoma

Objective To assess the feasibility of nuclear matrix protein 22(NMP22)and urinary bladder cancer,antigen (UBC) for the early diagnosis of bladder transitional cell carcinoma and its influencing factors.Methotis 105 subjects,including 60 patients of bladder cancer,25 patients of urological benign disease and 20 normal (healthy)individuals were enrolled in this study.Urine NMP22 and UBC wag assessed by enzyme-linked immunosorbent assay(ELISA).Urine NMP22 and UBC as well as exfoliocytology were conducted for the purpose to compare the sensitivity,specificity,positive and negative predictive value of these three ways.Results The sensitivity of NMP22(88.3%)and UBC(86.7%)were significantly better than exfolioeytology(40.0%,P<0.01).The specificity of NMP22,UBC and exfoliocytology were 80.0%,84.0%and 92.0%,respectively, the positive predictive values were 91.4%,92.9%and 92.3%,and the negative predictive values were 74.1%.72.4%and 38.9%.Conclusions NMP22 and UBC are sensitive,specific,simple,feasible and noninvasive diagnostic markers for the early detection of urinary bladder transitional cell cancer.
7.Effect of mind mapping-oriented health education on patient's fluid intake during hemodialysis intervals

Objective To evaluate the effect of mind mapping-oriented health education on patient's fluid intake during hemodialysis intervals. Methods From June 2015 to April 2016, 72 patients with poor compliance with fluid intake were randomly divided into control group (n=36) and observation group (n=36). In the control group, routine health education was used and the observation group was treated with mind mapping-oriented health education. The two groups were compared before intervention and 6 months after that in terms of compliance with fluid intake and daily fluid intake, ultrafiltration volume and body weight gain/dry weight ratio and incidence of complications. Results In the observation group, the fluid intake compliance, daily fluid intake, ultrafiltration volume and body weight gain/dry weight ratio 6 months after intervention were superior to those of the control group (all P<0.05). The incidences of complications such as hypotension, hypertension, muscle spasms and cardiac arrhythmia were also significantly lower than those of the control group (P<0.05). Conclusion The health education based on mind mapping is helpful to enhance the degree of cognition of fluid intake compliance, improve self-efficacy of chronic diseases, control the growth of body weight during dialysis and reduce the incidence of complications.
8.Clinical value of TNF-α in serum of patients with colorectal cancer

Objective To investigate the application value of serum tumor necrosis factor alpha (TNF-α) expression in diagnosing colorectal cancer (CRC).Methods One hundred and thirty inpatients with CRC in our hospital from February 2015 to November 2016 were selected and divided into 90 cases of CRC confirmed by operation and histopathology(CRC group),including intestinal cancer with and without metastasis,and 40 cases of benign colorectal diseases(colonrectal benign disease group),at the same time 40 persons undergoing physical examination were selected as the healthy control group.The levels of TNF-α,carcinoembryonic antigen (CEA) and carbohydrate antigen (CA) 724 were collected in all subjects.The data of various groups were not normally distributed,and the nonparametric statistical method was used to analyze and compare.Results Serum TNF-α level in clinical TNM stage Ⅱ(median 9.47 pg/mL),Ⅲ(9.95 pg/mL) and Ⅳ(9.67 pg/mL) of the CRC group was significantly higher than that in TNM stage Ⅰ(6.59 pg/mL);which in the CRC with metastasis group (9.95 pg/mL) was higher than 8.03 pg/mL in CRC without metastasis group,the difference was statistically significant (P<0.05);the TNF-α expression had no obvious difference among different ages and between different genders(P>0.05).The area under the ROC curve in each test index:TNF-α was 0.868,CEA was 0.912,CA724 was 0.523,and which of their joint detection was 0.940.Conclusion The expression of TNF-α is increased in the occurrence and development process of CRC,its serum level detection has certain clinical application value,the TNF-α expression has no obvious difference in the subjects with different ages and different sexes and has wide suitability.The combine detection of serum TNF-α,CEA and CA724 in the patients with CRC significantly increases the level,the combined detection of 3-index can have complementary advantages to increase the sensitivity of CRC diagnosis,which serve as the significant indexes in CRC early diagnosis,therapeutic effect evaluation and prognosis monitoring.
9.The influence to airway responsiveness of asthmatic children treated with low dose azithromycin

Objective To investigate the influence to airway responsiveness by using low dose azithromycin in asthmatic children.Methods One hundred and four children with moderate persistent or severe persistent asthma were divided into Group A (include 56 cases) and Group B (include 48 cases) randomly.All of the patients were treated with seretide (50/100 μg/dose) 2 doses/d for4 weeks.Children with severe persistent asthma were added with montelukast 5 mg/d orally,children accompanied with allergic rhinitis were added with nasonex 100 ~200 μg/d intranasal.And four weeks later,all of the patients were given a 12-week observation.Group A kept on treating with seretide (the same dose as before),and azithromycin 7.5mg/ (kg·d) (the maximal dose was 250 mg/d) orally was administered intermittently twice a week.Group B kept on treating with seretide ( the same dose as before) only.At the end of the study,we assessed the number of days with asthmatic symptoms in two groups respectively.At the beginning and the end of the study,we measured the forced expiratory volume in one second (FEV1 % ),peak expiratory flow (PEF%),the PD20-FEV1 by histamine challenged at the 15 th day and at the end of the study in two groups.Twenty-four hours before the study and twenty-four hours after the study,we measured the 24-hour urinary 17-OHCS and 17-KS of the patients.Two weeks after the study,we measured the serumal ALT and AST of the patients.Results ( 1 ) The number of days with asthmatic symptoms was ( 2.13 ± 1.18 ) d and ( 2.25 + 1.19 ) d respectively in Group A and B,and there was no significant difference between two groups ( t =0.54,P=0.59).(2) The PEF% before and after the study in Group A was (49.77 ± 15.02)% and (82.73 ±7.81 ) % respectively ( t =16.59,P =0.000).The PEF% before and after the study in Group B was (52.69±13.90)% and (81.15±7.28)% respectively (t=12.37,P=0.000).The FEV1% before and after the study in Group A was (50.48 ± 15.08 ) % and ( 83.18 ± 6.61 ) % respectively ( t =16.25,P =0.000).The FEV1 % before and after the study in Group B was ( 53.29 ± 13.89 ) % and ( 82.73 ± 6.10 ) % respectively (t=12.83,P=0.000).(3) The PD20-FEV1 at the 15 th day and at the end of the study in Group A was (65.13 ±26.08) μg and (460.79 ±221.72) μg respectively (t =13.54,P =0.000).The PD20-FEVt at the 15th day and at the end of the study in Group B was ( 65.27 ± 25.75 ) μg and (65.66 ± 25.09 ) μg respectively (t =1.45,P =0.15).Compared the PD20-FEV1 between the two groups,there was significant difference at the end of the study (t =13.29,P =0.000).(4) The 24-hour urinary 17-OHCS was (14.27 ±3.41)nmol/L and (14.43 ±3.69) nmol/L respectively of Group A and was (14.31 ±3.66)nmol/L and ( 14.56 ± 3.37 ) nmol/L respectively of Group B before and after the study,and there was no significant difference between Group A and Group B.The 24-hour urinary 17-KS was ( 22.43 ± 5.69 ) nmol/L and (22.07±5.21 ) nmol/L respectively of Group A and was (22.40±5.04) nmol/L and (22.54 ±4.74)nmol/L respectively of Group B before and after the study,and there was no significant difference between Group A and Group B.(5) The serumal ALT of Group A and B measured 2 weeks after the study was (20.39± 9.12) U/L and ( 20.83 ± 7.83 ) U/L respectively ( t =0.26,P =0.79 ).The serumal AST of Group A and Group B measured 2 weeks after the study was (20.68 ±8.67) U/L and (21.44±8.60) U/L respectively (t =0.45,P =0.66 ).Conclusion It can significantly decrease the bronchial hyperreactivity of asthmatic children treated with low dose azithromycin orally for 12 weeks and seretide inhaled,but it can not decrease the number of days with asthmatic symptoms in the patients,can not improve the FEV1 and PEF significantly.There wasn't any influence on adrenal glands.function and liver function of the patients.
10.Computerized system validation of clinical researches.

Validation is a documented process that provides a high degree of assurance. The computer system does exactly and consistently what it is designed to do in a controlled manner throughout the life. The validation process begins with the system proposal/requirements definition, and continues application and maintenance until system retirement and retention of the e-records based on regulatory rules. The objective to do so is to clearly specify that each application of information technology fulfills its purpose. The computer system validation (CSV) is essential in clinical studies according to the GCP standard, meeting product's pre-determined attributes of the specifications, quality, safety and traceability. This paper describes how to perform the validation process and determine relevant stakeholders within an organization in the light of validation SOPs. Although a specific accountability in the implementation of the validation process might be outsourced, the ultimate responsibility of the CSV remains on the shoulder of the business process owner-sponsor. In order to show that the compliance of the system validation has been properly attained, it is essential to set up comprehensive validation procedures and maintain adequate documentations as well as training records. Quality of the system validation should be controlled using both QC and QA means.
