1.The clinical effect of POME optical treatment in patients with severe vulvovaginal candidiasis

Objective To evaluate the clinical effect of local POME optical treatmeat in patients with severe vulvovaginal candidiasis (VVC) . Methods Ninety-eight patients, who were diagnosed as severe vulvovaginal candidiasis from January to December 2009 in Beijing Wuzhou Women's Hospital gynecologicalclinic,were enrolled into this study and divided into control and physiotherapy groups randomly. The patients of control group (re =48) were treated with oral Fluconazole and had Miconazde Nitrate suppositories settlement into vagina, while in the physiotherapy group (n =50) the patients accepted local POME optical treatment combined with the same medications as control. The therapeutic effect were evaluated and compared according to the subjective symptoms and VVC scores in both groups. Results The clinical symptoms of physiotherapy group were relieved within 4 hours after the treatment, which were significantly depressed compared to before the treatment (itching score 1.4 ±0.3 vs. 2. 8 ±0. 5,P <0. 05;pain score 1.5 ±0.4 vs. 2. 8 ±0. 3,P <0. 05),while obvious improvements were observed in control group untill 48 hours later (itching score 1. 8 ± 0. 6 vs.2.9 ±0.3,P<0. 05;pain score 1.5 ±0.5 vs. 2. 8 ±0.4,P <0. 05). The VVC scores were significantly lower in physiotherapy group than control after treatment at different timepoints (24 h VVC Score 5. 3 ± 1. 6 vs. 9. 5 ±2. 1,P<0. 05;48 h Score 4. 8 ±1.0 vs. 7.9 ±2. 3,P<0. 05),but no significant difference was observed in the comparison of VVC Scores between two groups 72 h after treatment (P > 0.05). Conclusion Local POME optical treatment combined with drug treatment in severe VVC patients not only has better effect than drug treatment alone,but also relieves clinical symptoms more quickly.
2.Preliminary cvaluation of 3-D angiography in diagnosis of carotid stenosis and carotid stenting

Objective To summarize the initial application of three dimensional angiography(3D angio) in the diagnosis of carotid stenosis and carotid stenting.Methods 23 patients with carotid stenosis confirmed by conventional angiography (two dimensional angiography, 2D Angio) received 3D Angio including 16 accepted both 2D Angio and 3D Angio before and after carotid stenting and then undengone comparative study. Results 32 stenosed lesions were found, including 5 in common carotid artery, 19 in neck section of internal carotid artery, 3 in patrous section, 2 in caverous sinus and 3 in middle cerebral artery. 3D Angio could demonstrate the changes of stenosed segment from every point of view. The degree and extent of stenosis could be accuratly evaluated. The characterization of lesions including the diminution of lumen and coarse plaques could be clearly displayed. After stenting, the wall of stenosed segment became smooth and the plaques disappeared.Conclusions The diagnosis and treatment of carotid stenosis by 3D Angio is helpful to accurately estimate the extent of stenosis, the characterization of the lesions and monitor the results of stenting.
3.Experimental study of carotid sinus nerve block in prevention and treatment of hemodynamics instability caused by carotid sinus stimulation

5.Effects of glycomacropeptide in human milk and formula milk on proliferation ofbiifdobacterium infantis in vitro

ObjectiveTo explore the effects of glycomacropeptide (GMP) in human milk and formula milk on proliferation ofbifidobacterium infantis and their dose-response relationship.Methods Casein was isolated from the milk of 30 healthy postpartum women from Guangzhou Women and Children's Medical Center in September 2014, and hydrolyzed by rennet to obtain GMP, which was then purified by ultrafiltration and ion exchange chromatography. Human milk GMP and cow milk GMP (0, 250, 500, 1 000, 1 500, 2 000 and 3 000 mg/L) were added tobifidobacterium infantis liquid medium, and cultured under anaerobic conditions. Concentration of bacteria was measured by turbidimetric microplate assay (detection of OD600 nmvalue of medium). Difference of proliferative activities ofbiifdobacterium infantis in human milk GMP and cow milk GMP was compared with independent samplest-test.ResultsPurified human milk GMP concentration was 1 712.20 mg/L, with a purity of 80.3%. Increasing the cow milk GMP initial concentration in the culture medium at 250-2 000 mg/L could increase the concentration and proliferation rate ofbiifdobacteria infantis. When cultured at 36 h with GMP of various concentrations, the proliferation ofbiifdobacteria infantis maintained at a logarithmic phase. Therefore, 36 h was chosen as the test time point to compare the proliferation ofbifidobacterium infantis. At 36 h, when GMP in the medium was 1 000, 1 500, 2 000 and 3 000 mg/L, concentrations ofbiifdobacteria infantis in human milk GMP were 2.255±0.036, 2.583±0.088, 2.877±0.080 and 3.219±0.081, respectively, which were significantly higher than those in cow milk GMP (2.115±0.053, 2.312±0.064, 2.542±0.090 and 2.894±0.076;t=4.867, 5.569, 6.192 and 6.516; allP<0.01).Conclusions Both human milk GMP and cow milk GMP can promote the proliferation ofbiifdobacterium infantisin vitro, and the proliferative activity in human milk is greater than in cow milk at the same concentration of GMP.
6.Meta-analysis of antibiotics therapy for Brucellosis

Objective:To evaluate the effectiveness and safety of most common used regimens against brucellosis.Methods: Search PubMed and CENTRAL database of Cochrane library for all literatures written in English about treatment of brucellosis and CBM database for all RCTs for Brucellosis in Chinese from the year 1985 till now.Assess the quality of the included literatures using Cochrane Collaboration Risk of Bias form.Combine data of relapse,total therapeutic failure,and adverse reaction of regimens for treating human brucellosis.Results: 17 literatures were included.Combined antibiotic therapeutic regimens such as DR,DS,QR and DG were compared.Rate of total therapeutic failure(RRcb:2.53,95%CI:1.51-4.23) and relapse(RRcb:2.69,95%CI:1.46-4.98) of DS regimen was lower than those of DR regimen,while adverse reaction did not show any significant differences between them(RRcb:1.40,95%CI:0.97-2.01).No significant differences were seen in rate of relapse(RRcb:1.24,95%CI:0.67-2.30) and total therapeutic failure(RRcb:1.41,95%CI:0.86-2.32) between QR and DR regimen.QR regimen had lower rate of adverse reaction than DR regimen(RRcb:1.79,95%CI:1.17-2.74).Conclusion: DS regimen priors to DR regimen.QR equals DR in treatment outcome,has fewer adverse reactions meanwhile.Triple antimicrobial based on double regimens seemed to provide better outcomes without a significant increase in adverse reaction,but more clinical evidences are still needed.

8.Difference of gene expression in bone marrow mononuclear cells between a chronic myeloid leukemia patient and a healthy person

BACKGROUND: Chronic myeloid leukemia (CML) is characterized by the clonal expansion of hematopoietic stem cells. Without effective treat ment, individuals in the indolent, chronic phase (CP) of CML will undergo blast crisis (BC), the prognosis for which is poor. Therefore, it is important to clarify the mechanism underlying CML from a whole-genome perspec tive. OBJECTIVE: To investigate the gene expression profile of bone marrow mononuclear cells from CML with Applied Biosystems Expression Array System.DESIGN: Observation and controlled analysis.SETTING: Institute of Gene Engineering, Southern Medical University PARTICIPANTS: Samples of two cases of bone marrow (a chronic myeloid leukemia patient and a healthy person).METHODS: This experiment was conducted at the Institute of Gene Engineering, Southern Medical University from October 2004 to September 2005.The total RNAs were extracted and purified from bone marrow mononuclear cells derived from a CML patient and a healthy person. mRNAs were purified using an oligo (dT)-cellulose mRNA purification kits and labeled using reverse transcription, in vitro transcription (RT-IVT), then hybridized with microarray. Gene expression differentiation of the bone marrow mononuclear cells were examined by ABI 1700 Chemiluminescent Microarray Analyzer. Reproducibility of microarray results was assessed by comparing data sets obtained from the same sample and analyzed by two different arrays.MAIN OUTCOME MEASURES: ①Assessment of quality of total RNA and labled cRNA. ②Reproducibility of microarray. ③ Hybridization of array.④Results of semi-quantitative reverse transcription-polymerase chain reaction RESULTS: ①Using statistical data analysis tools, we identified 6 706 genes that were up- or down-regulated in CML patient compared with the healthy person. In these genes, we found that 17 genes were up-regulated while 51 genes were down-regulated among 68 genes closely related to CML. ②most differentially expressed genes in C/EBPalpha mediated path way and CD40L signaling pathway had reduced expression. ③Good repro ducibility of microarray was confirmed by analysis of correlation and detection concordance in technical replicates. The correlation coefficient of the detectable probe in technical replicates was 0.991 for the CML patient and 0.988 for the healthy person. ④The results of semi-quantitative RT-PCR experiments supported the reliability of our microarray analysis.CONCLUSION: By comparing expression patterns of CML with those of the healthy person, we identified a large number of genes that, were up- or down-regulated in CML patients. These data should provide useful information for finding candidate genes whose products might serve as molecular targets for treatment of CML patients.
9.The relationship between sperm DNA fragmentation and male infertility

Objective To explore the relationship between sperm DNA fragmentation rates and male infertil-ity.Methods Case -control study was used .Sperm DNA fragmentation rates were compared between the primary infertile males for unknown reasons (n=126) and the males whose spouses undergoing delivery in three months (n=100).Results Sperm DNA fragmentation rate of the research group was (11.95 ±4.89) %.Sperm DNA fragmenta-tion rate of the control group was (10.07 ±3.56) %.Significant difference was found between the two groups on sperm DNA fragmentation rate(t=-3.326,P=0.001).The 226 men were divided into group A with sperm DNA fragmentation rates<10%(n=130) and group B with sperm DNA fragmentation rates≥10%(n=96).The percent-age of male infertility in group A was significantly lower than that in group B (50.00%vs 61.62%) (χ2 =4.105, P=0.043).Conclusion Correlation is found between sperm DNA fragmentation rates and male infertility .
10.Detection of tumor cells in the peripheral blood of the patients suffered from gestational trophoblastic tumor

Objective To found sensitive and reliable method to identify trophoblastic tumor cells in the peripheral blood of the patients suffered from gestational trophoblastic tumor.Methods Given numbers of JAR cell from ten to million were mixed into 10ml non pregnant peripheral blood as a model. Detection of ? hCG mRNA with fluorescence quantitative reverse transcription polymerase chain reaction (FQ RT PCR) and then estimation of the numbers of tumor cell in the blood. Nine cases of peripheral blood were collected from the pretreatment patients of gestational trophoblastic tumor to assay ? hCG mRNA with FQ RT PCR, then to estimate the numbers of tumor cell in the circulation blood. Results FQ RT PCR could detect ? hCG mRNA when ≥10 2 JAR cells were mixed into 10ml non pregnant peripheral blood. Four cases of bloods had been detected ? hCG mRNA expression in 9 cases of gestational trophoblastic tumor, and the numbers of tumor cell from 10 4 to 10 7 per 10ml blood. Conclusion FQ RT PCR of which primers and probe are designed for ? hCG had been proved to be very sensitive detection means, it could be used to detect gestational trophoblastic tumor cells from patient preipheral blood; With FQ RT PCR the tumor cells had been detected in some patients of gestational trophoblastic tumor.
