1.Investigation on medicine taking in rural schizophrenia patients

ObjectiveTo investigate the status medicine taking in rural schizophrenic patients.MethodsThe families of 217 schizophrenic patients in a town of Shaoxing rural area were investigated with self-made questionnaire.Results and Conclusion125 cases routinely took medicine(57.6%). The main reason caused patients to resist taking medicine was absence of family or family's care. The medicine were obtained from hospital, drugstore, free drugs from mental preventing organization, mail-order, half or more of them were free. Only 53.46% of the objects would like to receive free drugs. The most of the others want to give up. The objects suggested mental preventing organization to provide more kinds and costly free drugs, simplify procedures of obtaining drugs, permit to receive more drugs at a time and be more kindly.
2.A New Method of 3-Dimensional Localization of Intraocular Foreign Bodies Using CT Imaging: A Role of Optic Nerve

Computed tomography (CT) is considered the most sensitive method for the detection of intraocular foreign bodies (IOFBs).The purpose of this study was to evaluate a new method of 3-dimentional (3D) localization of IOFBs that takes advantage of the anatomical structure of the optic nerve and to assess the clinical outcomes using this new method.Twenty-two trauma patients with IOFBs or suspected IOFBs admitted to our hospital were scanned with multislice CT (MSCT) between July and December 2003.All scanning was performed with a 16-row spiral CT in axial plane using a sequential scanning protocol.During the scanning,the eyeball of the patient was kept stable and was not allowed to rotate internally or externally.Section collimation was set at 16 mm × 0.75 mm.Table feed was 12 mm.Reconstruction index was 0.75 mm.After scanning,the reconstructed images were loaded into a workstation to create the multiplanar reconstruction images with the aid of the 3D software.We compared the localization results with the operative findings.Multiplanar reconstruction images showed IOFBs in all 22 patients.IOFBs occurred in the eyeball of 14 patients,in the wall of the eyeball of 5 patients and in the posterior orbits of 3 patients.Different surgical procedures were designed according to the localization by this new method and all IOFBs were successfully removed.All of these foreign bodies were metallic and the localization of IOFB using MSCT was consistent with that found by operative findings.It was suggested that MSCT is a simple and effective imaging modality for the localization of IOFBs.In our study,we localized the IOFBs more quickly and accurately by taking advantage of the fixed position of the intraocular segment of the optic nerve,and determined the necessary surgical parameters.
3.Effects of platelet-derived growth factor α receptor in experimental rabbit PVR

AIM:Receptor tyrosine kinase (RTK) activation is critical for growth factor-mediated cell proliferation. The present study was designed to determine the effect of the tyrphostin AG1295 and AG1296, a selective blocker of PDGF βand αRTK,on proliferative vitreoretinopathy (PVR) development.METHODS:Rabbit conjunctival fibroblasts (RCF) cells were cultured.The effects of AG1295, AG1296,PDGF-AA and PDGF-BB on RCF proliferation are evaluated by MTT assay.Homologous rabbit conjunctival fibroblasts were injected intravitreally to make animal PVR model, followed by injection of 100μmol/L of AG1295 or AG1296 respectively. The presence of tractional retinal detachment (TRD) was assessed to evaluate the effect of AG1295 and AG1296 in vivo .Electroretinography and histologic studies were performed after intravitreal injection of AG1295 into untreated eyes to evaluate toxicity. RESULTS: Both AG1295 and AG1296 (10μmol/L) significantly inhibited rabbit conjunctival fibroblast cell growth stimulated by PDGF-AA or -BB in vitro.Development of TRD was significantly reduced (P<0.05) with 100 μmol/L of AG1295 or AG1296 in vivo, but the effect of AG1295 only present till day 14. Inhibitive effect of AG1296 is longer than that of AG1295.No significant histologic or retinal functional damage was found in both drug-treated groups. CONCLUSION: PDGF αand βreceptor specific inhibitor AG1296 and AG1295 attenuated PVR without significant side effects in rabbits, and AG1296 was better than AG1295. The much longer and stronger therapeutic effect from PDGFαreceptor inhibitor indicated that PDGF α receptor is more important in the development of PVR, and inhibition of this pathway could be a useful treatment alternative to prevent PVR.

5.Rehabilitative nursing of patients with pectoralis minor muscle transplantation for recovery of thumb oppositional function

Objective We reported the rehabilitative nursing points of patients with pectoralis minor muscle transplantation for recovery of thumb oppositional function. Methods We selected 10 patients who underwent pectoralis minor muscle transplantation for recovery of thumb oppositional function and gave them perfect preoperative examination and psychological nursing. We also implemented rigorous monitoring postoperation and auxiliary electric stimulation and functional exercises. The emphasis was put on the exercise of thumb oppositional function. Results The thumb oppositional function of all the ten patients recovered. The myodynamia reached level 4 and the shape of thenar was satisfying after 6 to 12 months of follow-up visit. Conclusions Sufficient preoperative psychological nursing and effective postoperative rehabilitative treatment had pivotal significance for the success of operation and the recovery of thumb oppositional function.
7.Distribution of Specific Immunoglobulin E and Total Immunoglobulin E in Children with Respiratory Allergic Disease

Objective To study the distribution of sepecific immunoglobulin E(SIgE) and its relationship with the total immunoglo-(bulin) E(TIgE) in children with respiratory allergic disease.Methods Serum SIgE was measured in 209 children with asthma,allergic rhinitis,cough variant asthma and asthmatic bronchitis by Pharmacia UniCAP100E system.The SIgE included the SIgE antibodies to fx5E(egg albumen,milk,fish,wheat,peanut,soybean),hx2(house dust,dermatophagoides culinae,dermatophagoides pteronyssinus,cockroach),ex1(scurf of cat,horse,cow and dog),mx1(penicillium notatum,helminthosporium halodes,aspergillums furngatits,alternaria allcrnata,),f23(crab),f14(shrimp),w14(amaranthaceae),T21(cajeput) and i8(moth).Serum TIgE was also detected in 132 patients.Results 1.The positive detection rate of SIgE in patients aged from 3 to 14 years were 72.81% which were higher than that of aged from 40 days to 3 years old(57.89%).2.The SIgE detection rate of fx5E,hx2 and ex1 were 81.8%,32.73% and 16.36% respectively in patients aged from 40 days to 3 years old;but that of hx2,fx5E,i8,w14,f24 and ex1 were 78.31%,48.19%,24.10%,24.10%,15.66% and 15.66% respectively in patients aged from 3 to 14 years.3.The positive detection rate of SIgE was related with that of TIgE.The detection rate of combined SIgE and the TIgE was 83.33%.Conclusions 1.The lower detection rate of SIgE in patients aged from 40 days to 3 years may be due to the wheezing in infants partially induced by the acute infection and congenital airway disease.2.The most common allergens in children aged from 40 days to 3 years were fx5E,hx2 and ex1.But that of patients aged from 3 to 14 years were hx2,fx5E,i8,w14,f24 and ex1.3.The combined detection of the SIgE and the TIgE may increase the positive rate and diagnose the patients with respiratory allergic disease effectively.

Objective To explore the value of brainstem auditory evoked potentials (BAEP) in the evaluation of spastic cerebral palsy (CP). Methods Forty five cases of CP children were recruited and divided, according to the diagnosis standards, into three groups: diplegia, hemiplegia and tetraplegia groups. Twenty three healthy children served as control. BAEP was recorded from all the subjects and comparisons were done among the various groups. 10.Intensive care on one case of combined heart-liver transplantation

Objective To summarize the diagnosis and treatment of one case of combined heart liver transplantation. Methods On November 24, 2011, one case of combined heart-liver transplantation was performed on a patient with Ebstein's anomaly and tricuspid valve replacement after 5 years,complicated with congestive cirrhosis,liver failure dccompensation,preoperative heart failure Ⅲ degree and B grade of liver function Child-Pugh score. The operation was done with the graded cardiopulmonary bypass assisted mode:first creating the vena cava-aortic bypass to complete heart transplantation, second creating the femoral vein-ascending aorta bypass to complete liver transplantation,and third stopping and neutralizing.The aortic cross-clamping time was 54 min and the an hepatic phase was 38 min.The total time of three times of cardiopulmonary bypass was 199 min and the total time-consuming of operation was 517 min. The patient was given basiliximab +methylprednisolone for immune induction therapy, and tacrolimus + mycophenolate mofetil +prednisone solution for anti-rejection. After operation, liver protecting treatment, anti-infection therapy and nutrition support therapy were given.Results The recipient died of multiple organ failure after 78 days.The mechanical ventilation treatment duration for this recipient was 78 days and ECMO adjuvant therapy for postoperative hypoxemia time lasted 63 days.Conclusion The combined heart liver transplantation is an effective measures for treatment of heart and liver failure.
