1.Clinical analysis of elderly patients with osteoporotic hip fractures

Clinical data of 448 patients aged ≥60 years with osteoporotic hip fractures admitted in Shanghai Huashan Hospital between January 2010 and December 2014 were retrospectively analyzed.Among 448 cases,there were 143 males and 305 females;196 cases aged 80-89 years (43.8%) and 137 cases aged 70-79 years (30.6%);304 cases of femoral neck fracture (67.9%) and 144 cases of intertrochanteric femoral fracture (32.1%);the fractures caused by fall in 328 cases (73.2%) and comorbidity with hypertension and diabetes mellitus in 415 cases (92.6%).Two hundred and sixty eight patients were followed up for 2 years,among whom re-fractures occurred in 17 cases.The study indicates that health education is important for prevention of osteoporotic fractures in elderly people.

3.Efficacy of laparoscopic treatment for infertility caused by distal tubal obstruction.

Objective To study efficacy and its affected factors of laparoscopic treatment for infertility caused by distal tubal obstruction. Methods 113 infertile patients with occlusive distal tubal disease received leparoscopic surgery from January to December 1999.The postoperative pregnancy rate was observed. Results Occlusive tubal disease was presented in 64 cases (56.6%),Among them 46 cases had been followed up and 10 cases (21.7%) obtained interuterine pregnancy.The pregnancy rate of normal fimbria and unobstructed tube after lysis of periadnexal adhesion was 47.4%,26 cases of laparoscopic salpingostomy had no pregnancy. Conclusions laparoscopic surgery may be effective in the treatment of periadnexal adhesions in tubal infertility,and had no effect in salpingostomy.
4.Relationship between CX3CR1 polymorphisms and carotid artery stenosis

Objective To study the association of V249I and T280M polymorphisms of fractalkine receptor CX3CR1 with carotid artery stenosis (CAS). Methods 318 patients with CAS diagnosed using color Doppler ultrasound criteria were studied and compared with 292 subjects without CAS. V249I and T280M polymorphic genotypes of CX3CR1 were determined by polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP) and sequencing analysis. Results The genotypes of MM and TM were associated with reduced risk of CAS,the frequency of genotype MM + TM in the two groups being 22. 6% and 31.2% respectively ( OR = 0. 646, 95% CI 0. 451-0. 928, P = 0. 017), the frequency of M allele was significantly lower in patients with CAS than in those without CAS ( 13.8% and 19. 2% respectively, OR = 0. 677, 95%CI 0.499--0. 918,P=0. 010). No differences were observed in the Ⅱ, Ⅵ, or VV genotype, the frequency of genotype Ⅱ + Ⅵ in the two groups being 39. 0% and 43.8% respectively ( OR = 1. 012, 95% CI 0. 731-1. 403 ,P = 0. 940), the frequency of Ⅰ allele was significantly lower in patients with CAS than in those without CAS (28. 6% and 32. 7% respectively, OR = 0. 842, 95% CI 0. 660-1. 076, P = 0. 034). The genotypes of Ⅱ and Ⅵ in patients with stable plaques were more frequent than in vulnerable plaques( OR = 0. 610, 95% CI 0. 387-0. 962, P = 0. 033 ). Multiple Logistic regression analysis revealed that the genotypes of MM and TM were an independent risk factor for CAS ( OR = 1. 847,95% CI 1. 091- 3. 127 ,P = 0. 022). Conclusion The genotypes of MM and TM are independent risk factors for carotid artery stenosis, and the genotypes of Ⅱ and Ⅵ are associated with the stability of carotid artery plaques.

Objective To study the relationship between the expression of matrix metalloproteinase-9 in rat brain and pathogenesis of epilepsy.Methods The expression of matrix metalloproteinase-9 in hippocampus was observed by immunochemistry in normal and epilepsy rat.Meanwile,the effects of dexamethasone before epilepsy on the expression of MMP-9 in hippocampus was evaluated.Results The expression of MMP-9 in hippocampus were fewer in normal rat brain.The expression of MMP-9 had increased in hippocampus after epilepsy 24h.It had increased obviously in dentate gyrus(DG)after epilepsy 72h than 24h(P0.05).Dexamethasone suppressed the expression of MMP-9 in hippocampus.Conclusion The expression of MMP-9 in rat brain participates in immunopathogenesis of epilepsy by affecting brain immunoreaction,leading to the change of neuronal response and synapse remodeling in hippocampus,regulating brain nervous cell excitability.Dexamethasone plays a prominent suppressant role in the processes.

Objective:To investigate the correlation between NKG2A+NK cells and regulatory T cells in peripheral blood of patients with chronic hepatitis B virus infection, and explore the clinical significances.Methods: Forty-six patients with chronic hepatitis B virus infection and 17 health individuals were included in this study.HBV DNA levels were measured by Real-time quantitative PCR ( FQ-PCR) .NKG2A+NK cells and Treg in PBMC were quantitatively analyzed by flow cytometry.Results:According to HBV DNA levels,the CHB patients were divided into two groups:Low HBV DNA group(Low viral load group,300-104 U/ml)and High HBV DNA group( High viral load group,105-108 U/ml).We found that ALT levels of High HBV DNA group were obviously higher than Low HBV DNA group(P<0.05).The frequenies of CD56dim NK cells of High HBV DNA group were obviously higher than low HBV DNA group ( P<0.05 ), and similarly the percentages of NKG2A+CD56dim NK cells of High HBV DNA group were significantly higher than Low HBV DNA and control groups ( P<0.05).We also found that the percentages of regulatory T cells ( Treg) of High HBV DNA group were significantly higher than Low HBV DNA and control groups ( P<0.05).In addition,the proportions of NKG2A+CD56dim NK cells were positively correlated with High HBV DNA levels (r=0.59,P<0.05) and the percentages of Treg(r=0.53,P<0.05) in CHB patients.Conclusion:NKG2A+CD56dim NK cells may closely relate to the HBV-related immune escape and the progress of CHB.
9.Sustained negative pressure drainage for treatment of severe maxillofacial and neck space infection.

OBJECTIVEThis study observed the curative effect of sustained negative pressure drainage application on treatment of severe maxillofacial and neck space infection.
METHODSIncision and drainage were performed to treat 18 patients with severe maxillofacial and neck space infection. A small incision was made on the site of the most obvious swelling or fluctuations, and localized negative pressure was applied with a drainage device on the wound during suturing.
RESULTSAmong the 18 patients, 14 were healed, whereas 4 underwent dehiscence of the wound after the operation. Negative pressure was lost as the drainage tubes were removed, and non-negative pressure drainage method was used instead. During the negative pressure treatment, swelling and pain did not increase after the operation. Other complications, such as asphyxia, septic shock, or mediastinal abscess, did not occur. All the patients were healed and eventually discharged from the hospital.
CONCLUSIONSustained negative pressure drainage, which is a modified version of the traditional method of incision and drainage, is an alternative treatment for severe maxillofacial and neck space infection. Such treatment reduces patient pain and eases doctor exertion. Thus, this method provides a new therapeutic strategy for severe maxillofacial and neck space infection.
