2.Analysis of the etiology and the clinical characteristic of fever in older patients.

Objective To analyze the etiology and the clinical characteristis of fever in older patients in Wenzhou.Methods Totolly 87 cases admitted to the First Affiliated Hospital of Wenzhou Medical College from January 2001 to April 2004 were retrospectively analyzed;all cases displayed fever as the major clinical manifestation and met the criteria of fever well.Results The etiology of fever which was infectious disease was in 44 of the 87 cases,accounting for 50.6%,collagen-vascular diseases 4.6%,neoplasms 10.3% and miscellaneous diseases 5.7%.In 28.7% of the cases the etiology could not be found.And in the infectious disease the tuberculosis accounted for 13.6%(6/44). Ultrasonography ,X-rays,CT scan,MRI,the marrow puncture and biopsy contributed to the final diagnosis in 21.1%,16.9%,63%,0,16.7% and 57.1% of the cases,respectively.Conclusion Because the symptoms of the older patients'febrile diseases are atypical,the diagnosis is more difficult.Infectious disease still remains a major cause of the fever in the elderly.A thorough history of disease,full physical examination of the patient and routine laboratory studies,especially erythrocyte sedimentation rate,are very important in determining the etiology of febrile disease.Early and reasonable use of non-invasive imaging techniques and the essential invasive methods are helpful to diagnosis.
3.Simultaneous Determination of Chlorzoxazone and Paracetamol in Compound Chlorzoxazone Tablets by Kalman Filter Spectrophotometry

OBJECTIVE: To determine the contents of chlorzoxazone and paracetamol in compound chlorzoxazone tablets by Kalman Filter spectrophotometry. METHODS: The absorbabilities of the two components were detected in an interval of 1mn over a wavelength range of 248~296 nm. The data obtained were computed by Kalman Filter program. RESULTS: Good linear relations were achieved when the concentration ranges of chlorzoxazone and paracetamol were 2~20 ?g?mL-1 (r=0.999 9) and 3~30 ?g?mL-1 (r=0.999 7) respectively; the recovery rates were 99.1%(RSD=1.01%) and 100.3% (RSD=0.92%), respectively. CONCLUSION: This method is simple, rapid and accurate, and it can be used for the simultaneonsly determination of 2 components in compound chlorzoxazone tablets without separation.
4.Conventional and Contrast-enhanced Ultrasonography in Diagnosis of Thyroid Nodules:A Meta-analysis

Purpose There is no agreement for contrast enhanced ultrasound (CEUS) in differential diagnosis of benign and malignant thyroid nodules. This paper aims to evaluate the diagnostic value of the conventional and contrast-enhanced ultrasonography in the diagnosis of thyroid nodule by meta analysis. Materials and Methods Literature search was performed in PubMed, CNKI and Wanfang databases to identify relevant English and Chinese literatures by using pathological results as the gold standard from January 1990 to September 2014. The quality of each selected study was evaluated by QUADAS. The pooled weighted sensitivity, specificity and the corresponding 95% CI were calculated. Summary receiver operating characteristic curves (SROC), area under curve (AUC) and the Q* index were also calculated. Results A total of 14 literatures (1970 nodules) were included, the pooled weighted sensitivity and specificity of conventional ultrasonography were 0.74 and 0.70; those of contrast-enhanced ultrasonography were 0.86 and 0.82. The AUC by using the two methods were 0.84 and 0.93, respectively, and the Q* index were 0.77 and 0.86, respectively. Conclusion In the diagnosis of thyroid nodule, the accuracy of contrast-enhanced ultrasonography is better than conventional ultrasound; however, both of them have high clinical value in the diagnosis of thyroid nodule, thus they are helpful in avoidance of unnecessary surgical treatment in patients with thyroid disease.
5.Study of the fluid infusion model on the non-operative management of spleen rupture

Objective To compare the effect of two different fluid infusion models in the non-operative management of spleen rupture .Methods The clinical data of 46 cases of spleen rupture with non-operative management from 1995 to 2001 were analyzed retrospectively. Results In the non-operative management of spleen rupture, patients received continuous but rather slow fluid infusion had higher successful treatment rate(95.24%) and less intra- abdominal bleeding than those in patients received rapid fluid infusion (91.02%) . Conclusions In the non- opeative management of spleen rupture, continuous but rather slow fluid infusion model is superior to traditional rapid fluid infusion.
6.Effects of Exercise on Learning-memory of Space in Aged Rats after Cerebral Ischemia-reperfusion

Objective To study the effects and mechanism of exercise on learning-memory of space and in aged rats after cerebral ischemia-reperfusion. Methods 20 SD rats at the age of 20 months with cerebral ischemia-reperfusion were randomly divided into 2 groups. The experimental group (n=10) received regularly exercise, while the control group (n=10) exercised freely. Morris water maze was tested 1 week after exercise, and the brain-derived neurotrophic factor (BDNF) mRNA expression on the ischemic side of the hippocampus were detected.Results The latency in the 2nd round decreased (P<0.05) and the path line on the platform increased (P<0.05) in the experimental group in Morris water maze test (P<0.05), while the BDNF mRNA increased (P<0.01), compared with control group. Conclusion Regular exercise can improve space learning-memory ability of cerebral ischemia-reperfusion aged rats, which may associate with increased BDNF mRNA expression in hippocampus.
7.Theory analysis and clinical application of spirit-regulating and pain-relieving acupuncture method.

The theoretical foundation and scientific connotation of spirit-regulating and pain-relieving acupuncture method as well as its clinical application for pain are discussed. During spirit regulation, attention should be paid on regulating heart and brain, while acupoints should be selected mainly from the Heart Meridian, Pericardium Meridian and Governor Vessel. It has significant efficacy for refractory pain in clinical treatment. Spirit-regulating and pain-relieving acupuncture method is development of acupuncture treating spirit, and it is an important method for pain in clinic. Improvement on sensitization of pain center and brain function is considered as one of the mechanisms in spirit-regulating and pain-relieving acupuncture method.

8.Studies of cell sheet engineering on fibrin glue cultured with three kinds of rabbit corneal cells

AIM:To study whether three kinds of rabbit corneal cells can grow well on fibrin glue (FG) constructed in vitro, and investigate the feasibility of FG for the scaffold of cell sheet engineering. METHODS: Three kinds of corneal cells were seeded on FG which was produced in vivo. Cell growth on FG was examined as follows: (1) by inverted microscope; (2) histologically by hematoxylin and eosin; (3) by scanning electron microscopy. RESULTS: Fibrin glue prepared was smooth and transparent. With cell growth, FG degradated partly, then obtained cell sheet engineering only with a small amount of FG. Corneal cells generated well on the fibrin glue in vitro and maintained the physiological state of cells. Corneal epithelial cells formed unilaminar and stratified layers and cellular joins. Corneal endothelial cells formed round or polygon, the same size cells and lined up tightly. Corneal stroma cells formed triangle and arborization, cell-cell junction was obvious, and formed network link. CONCLUSION: Fibrin glue is well compatible with three kinds of corneal cells, which can construct tissue engineered cell sheet with fibrin glue, so as to reconstruct ocular surface.
9.Evaluation of the effect of photodynamic therapy combined with antifungal drugs and comprehensive nursing intervention in the treatment of kerion

Abstract: Objective To explore the efficacy and safety of photodynamic therapy (PDT) using 5-aminolevulinic acid (ALA) combined with antifungal drugs combined and comprehensive nursing intervention in the treatment of refractory kerion. Methods From January 2019 to December 2022,6 cases of refractory kerion who were treated at the Fifth People's Hospital of Hainan Province with photodynamic therapy (PDT) using 5-aminolevulinic acid (ALA) combined with antifungal drugs. A solution with 20% concentration of ALA solution was uniformly dropped on the cotton sheet, shielded from light for 3 h, and irradiated with (633±10) nm LED red light for 20-30 min. The treatment was performed once a week, with one treatment course consisting of three sessions. Prior to treatment, debridement and fire needles pre-treatment were performed. Provide timely and targeted psychological care during treatment to alleviate psychological negative disorders and health education. Follow-up examinations were conducted 6 months after the treatment to evaluate clinical efficacy, safety and patient satisfaction. Results Six children received standard ALA-PDT combined with antifungal drugs, supplemented by comprehensive nursing intervention. The cure rate of children reached 100%, and no disease relapse or atrophic scar was found. The satisfaction rate of patients and their families was 100%. Conclusions PDT combined with antifungal drugs has obvious clinical efficacy in the treatment of kerion, especially when combined with comprehensive nursing intervention. This approach can help to relieve patients' anxiety, improve treatment outcomes, enhances patient compliance, and further improve patients' satisfaction.
10.Oral lactitol combined with polyethylene glycol electrolyte powder for bowel preparation before colonoscopy in hospitalized patients

Objective To evaluate the efficacy of combined application of lactitol oral solution and polyethylene glycol electrolyte (PEG) powder compared with conventional method in bowel preparation before colonoscopy. Methods 205 patients who underwent colonoscopy were randomly divided into experimental group and control group. The experimental group (n = 102) were given lactitol and polyethylene glycol electrolyte powder, whereas the patients in control group (n = 103) were given polyethylene glycol electrolyte powder only. The visibility and adverse effects during colonoscopy were observed. Results The cleaning satisfaction rate was not statistically significant between the two groups. The proportion of cleanliness to grade 1 in experimental group was higher than that in control group. The incidence of adverse effects in experimental group was lower, and there was no effect on sleeping night. The compliance and tolerance of hospitalized patients were significantly improved. Conclusion Lactitol combined with polyethylene glycol electrolyte (peg) powder is safe, effective, with low incidence of adverse effect for bowel preparation in hospitalized patients.
