Purpose To investigate the diagnostic value of dual energy CT spectrum curve combined with CT morphology in central lymph node metastasis of papillary thyroid carcinoma (PTC).Materials and Methods Thirty-one PTC patients who accepted dual energy CT double-phase enhanced scan before surgery were analyzed.Central lymph nodes with short diameter ＞ 5 mm were labelled using axial plus 3D positioning.Preoperative labelled lymph nodes were collected and marked during operation.The CT morphology of metastatic lymph nodes was analyzed.The spectrum curve slope (K) difference between metastatic and non-metastatic lymph nodes of arterial-phase and venous-phase primary lesion was compared.The critical K value and the diagnostic efficacy of K value combined with morphology were obtained according to receiver operating characteristic (ROC) curve.Results A total of 73 central lymph nodes were obtained from 31 patients,among which 51 were metastatic and 22 were non-metastatic.There was no significant difference in K value between metastatic and non-metastatic lymph nodes of arterial phase group (P＞0.05).While,there was a significant difference in K value between metastatic and non-metastatic lymph nodes of venous phase group (P＜0.05).For venous phase,the sensitivity and specificity of the K value in diagnosing central lymph node metastasis were 62.7％ and 59.1％,respectively,and combined with morphology,the sensitivity and specificity reached 76.5％ and 81.8％,respectively.Conclusion The K value of CT spectrum curve is of certain significance in predicting PTC central lymph node metastasis,and the K value combined with CT morphology can improve the accuracy of diagnosis.

Objective To investigate the role of hypoxia-inducible factor-1α/ hexokinase Ⅱ (HIF-1α/HKⅡ) signaling pathway in the inhibition of hypoxia/reoxygenation (H/R)-induced apoptosis in the rat cardiomyocytes by hypoxic preconditioning (HPC).Methods Primarily cultured cardiomyocytes obtained from the neonatal rats were seeded in culture dishes at the density of 5× 105cells/ml.The cardiomyocytes were attached to the wall for 72 h and then randomly divided into 6 groups (n=15 each) using a random number table:control group (group C);group H/R;group HPC;HIF-1o inhibitor YC-1 group (group YC-1);group HPC + YC-1;dimethyl sulfoxide (DMSO) group.The ceils were exposed to D-Hank solution saturated with 95％ N2 and 5％ CO2 for 4 h,and then cultured in the normal culture atmosphere for 2 h.The cells in YC-1 and DMSO groups were incubated in the culture medium containing 10 μmol/l YC-1 and 0.1％ DMSO (100 μl) for 24 b,respectively.HPC was induced by 3 cycles of 10 min hypoxia followed by 30 min reoxygenation,and H/R injury model was then established in group HPC.In group HPC+YC-1,the cells were incubated for 5 min in the M199 culture medium supplemented with 10 μmol/L YC-1 and 10％ fetal bovine serum,and the other treatments were similar to those previously described in group HPC.After the end of treatments,the apoptosis in cardiomyocytes was detected by TUNEL,the expression of HIF-1α,HKⅡ and cytochrome c was detected by Western blot,and the mitochondrial membrane potential was quantitatively measured using fluorescence.Apoptotic rate was calculated.Results Compared with group C,the apoptotic rate was significantly increased,the mitochondrial membrane potential was significantly decreased,and the expression of HIF-1α,HKⅡ and cytochrome c was significantly up-regulated in group H/R (P＜0.05).Compared with group H/R,the apoptotic rate was significantly decreased,and the mitochondrial membrane potential was significantly increased,and the expression of HIF-1αt and HKⅡ was significantly up-regulated,and the expression of cytochrome c was significantly down-regulated in group HPC (P＜0.05).Compared with group HPC,the apoptotic rate was significantly increased,the mitochondrial membrane potential was significantly decreased,the expression of HIF-1α and HKⅡ was significantly down-regulated,and the expression of cytochrome c was significantly up-regulated in group HPC+YC-1 (P＜0.05).Conclusion The mechanism by which HPC inhibits H/R-induced apoptosis in rat cardiomyocytes is associated with activation of HIF-1α/HKⅡ signaling pathway.

Objective To investigate the sequence variations of mitochondrial D-loop region in chronic glomerular nephritis patients and family members and their possible associations with chronic glomerular nephritis.Methods 20 patients with chronic glomerular nephritis and 48 unaffected pedigree members,as well as 122 cases of normal control were recruited.mtDNA extracted from peripheral blood were examined by PCR-based assay for D-loop sequence variations,followed by sequencing analysis.Results Compared with the normal control and standard sequence,a total of 61 sequence variants were detected,among these,three high variations were found,and the base variation rate of patients with chronic glomerular nephritis(CGN)and unaffected pedigree members(UAPM)(0.88%,0.72%)significantly increased compared with the control group(0.61%).The distibutional difference of base variation rate in the experimental groups were significantly higher than those in the control group(x2=21.220,7.964,all P<0.05).Especially in microsatellite variation in D310 region,the mutation frequency and base variation rate in patients with CGN were 100.00% and 30.00%,respectively,which of UAPM were 81.30% and 17.95%,respectively,while those in the normal control group were 53.30% and 6.05%,respectively.Among them,the patients with CGN compared with the control group had statistically significant differences(x2=15.610,150.047,all P<0.05).Likewise,UAPM also had statistically significant difference compared with the control group(x2=11.347,66.188,all P<0.05).Conclusion D-Loop of mitochondrial genome mutations may be related to the development of chronic glomerular nephritis.

Objective To study the expression of sex hormone-binding globulin(SHBG),insulin signaling pathway and glucose transporter in placenta of pregnant women with gestational diabetes mellitus(GDM),and to explore its role in the pathogenesis of GDM. Methods A total of 10 full-term and non-obese(BMI<25 kg/m2)pregnancy women diagnosed with GDM and 10 normal pregnant cases were recruited for the study. Placental tissues were collected respectively. The expression of protein and mRNA of SHBG and insulin signal transduction(IRS-1,ISR-2,PI3K p85α)and glucose transporter proteins(GLUT-1,GLUT-3,GLUT-4)in placental tissue were detected by real-time PCR and Western blotting. Pearson and linear regression analysis were used to evaluate the correlation among the related indicators. Results In the placental tissue of non-obese women,there existed a decrease(P<0.05)in the expression of protein and mRNA of SHBG with a concurrent a decrease(P<0.05)in the expression of protein and mRNA of GLUT-4 in GDM women compared with normal controls. Furthermore ,a decrease(P<0.05)of GLUT-3 and IRS-1 protein expression with lower(P<0.05)IRS-2 mRNA expression was also observed in placental tissue from GDM patients. No changes were found in PI3K p85α and GLUT-1 at both protein and mRNA levels(P>0.05). Results of linear correlation analysis showed that there were positive correlations between SHBG mRNA and IRS-2 mRNA(P<0.05),SHBG mRNA and PI3K p85α mRNA(P<0.05),and SHBG mRNA and GLUT-4 mRNA(P<0.05). There was also a remarkable positive correlation between IRS-2 mRNA and GLUT-4 mRNA(P<0.01). There existed negative correlations between IRS-1 mRNA and PI3K p85α mRNA(P<0.05),and IRS-1 mRNA and GLUT-3 mRNA(P<0.05). There existed a remarkable positive correlation between IRS-2 mRNA and GLUT-1 mRNA(P<0.01). Conclusion The defective receptors of insulin signaling pathway are present in GDM placental tissue. Decreased expression of SHBG may be involved in the regulation insulin signaling ,leading to a concomitant decrease expression of relevant insulin signaling components in placental tissue ,implying insulin resistance and developing GDM finally.

Objective To study curative efficacy of granisetron in treatment of postoperative severe vomiting after posterior scleral reinforcement . Methods 84 patients of posterior scleral reinforcement who received therapy from January 2012 to December 2014 in our hospital were selected as research objects.According to random number table,those patients were divided into the control group (n=100) and the observation group (n=100), the control group were treated with ondansetron hydrochloride at the end of surgery, while the observation group were treated with granisetron at the end of surgery.Then postoperative sedation, analgesia, nausea, vomiting and so on.were compared.Results There were no significant differences in anesthesia time, operation time and remifentanil dosage between the two groups.The Ramsay scores of the observation group were (2.49 ±0.31), (2.23 ±0.34) and (2.10 ±0.28) points at 30 min, 1h and 2h after operation, respectively.In the control group, Ramsay scores were (3.02 ±0.42), (2.84 ±0.37), (2.45 ±0.34) at 30 min, 1h and 2h after operation, lower than the control group.The incidence of nausea and vomiting in the observation group were 9.52% ( 4/32 ) , 11.90% ( 5/42 ) respectively, and there was no significant difference between the two groups in the postoperative analgesia The total incidence of postoperative nausea and vomiting was 30.95% (13/42) and 30.95% (13/42) respectively, which were lower than the control group (P <0.05).Conclusion Granisetron is well for postoperative posterior scleral reinforcement, which can reduce the incidence of postoperative severe vomiting, it’s worthy of application and promotion.

Objective:To investigate the efficacy of early minimally invasive catheterization continuous abdominal lavage and drainage in treatment of severe acute pancreatitis (SAP) .Methods:170 SAP patients admitted to Quzhou People’s Hospital from Jan. 2016 to Jun. 2020 were divided into the observation group and the control group, with 85 cases in each group, according to the random number table method. The control group received comprehensive medical treatment, while the observation group received early minimally invasive catheterization continuous abdominal lavage and drainage intervention based on the control group. The efficacy and complications after 1 week of treatment were evaluated. Besides, the intra-abdominal pressure (IAP) and acute physiology and chronic health II (APACHEII) were scored, liver and lung function indexes [oxygenation indexes, oxygen partial pressure (PaO 2) , aspartate aminotransferase (AST) , alanine aminotransferase (ALT) ], and inflammation indexes [C-reactive protein (CRP) , tumor necrosis factor-α (TNF-α) , macrophage inflammatory protein-1α (MIP-1α) ] were measured before and 1 week after the treatment. Results:The total effective rate of the treatment in the observation group was 91.77%, which was significantly higher than 77.65% in the control group ( P<0.05) . The level of IAP, APACHEII score, AST, ALT, CRP, TNF-α, MIP-1α of the two groups after 1 week of treatment decreased significantly compared with those before treatment, while the oxygenation index and PaO 2 increased significantly. The levels of IAP, APACHEII score, AST, ALT, CRP, TNF-α, and MIP-1α in the observation group were lower than those in the control group after 1 week of treatment, while the oxygenation index and PaO 2 in the observation group were higher than those in the control group, with statistically significant difference ( P<0.05) . The incidence rates of multiple organ dysfunction syndrome (MODS) , sepsis and systemic inflammatory response syndrome (SIRS) in the observation group were: 8.24%, 11.76% and 15.29%, significantly lower than 21.18%, 29.41% and 30.59% in the control group ( P<0.05) . Conclusion:Early minimally invasive catheterization continuous abdominal lavage and drainage in treatment of SAP is safe and effective, which can significantly inhibit pro-inflammatory factors, and protect liver and lung function.

As a treasure resource of novel drug lead compounds, how to rapidly and high-efficiently screen and isolate active components from natural products is critical. Thanks to its high resolution, high automation and flexible integration, online two-dimensional liquid chromatography has great potential for screening active ingredients from complex matrices by integrating a highly specific bio-recognition process into a two-dimensional liquid chromatography system before, on or after the column separation. This review comprehensively summarized recent developments, applications and shortcomings of online two-dimensional liquid chromatography for natural product screening from different integration modes, including pre-column, on-column and post-column screening methods.

A 19-year-old female presented with a dark erythematous maculopapule measuring 2 cm in diameter on the left lower limb for more than one year.Physical examination revealed an enlarged cherry-like lymph node in the right inguinal region.Histopathology of the lesion revealed that the normal structure of skin disappeared absolutely,and there was a diffuse infiltrate of medium-sized lymphoid tumor cells with unclear nucleoli.Immunohistochemically,the tumor cells strongly expressed CD4,CD56 and CD123,and partially expressed terminal deoxynucleotidyl transferase (TdT).Flow cytometric analysis of bone marrow aspirates showed that abnormal cells amounted to 59.9％,and were positive for CD123st (strong),human leucocyte antigen-DRst (strong),CD56,CD304 (blood dendritic cell antigen-4),CD7,CD11b,CD33,CD36 and chemokine (C-X-C motif) receptor 4,CD13 (dim),CD4 (partial) and CD117 (partial).Transmission electron microscopy of bone marrow cells revealed that there were massive uniformly sized lymphoid cells with thick processes on cell surfaces,a high nucleus/cytoplasm ratio,centrally located round nuclei occasionally with deep gyrus-like notches,clumped marginated heterochromatin and multiple nucleoli.Moreover,a small amount of cytoplasm was observed in the lymphoid cells with the presence of mitochondria and endoplasmic reticulum arranged in concentric circles,which were characteristic of plasma cells.The features of both dendritic cells and plasma cells were found in the tumor cells through transmission electron microscopy.Based on the above findings,a diagnosis of blastic plasmacytoid dendritic cell neoplasm (BPDCN) was made.
