1.Further improving the result of multi-modality treatment for gastric cancer by optimizing scheme and measures.

The incidence of gastric cancer has declined worldwide in the past decades. However, due to the large number of population in mainland of China, the absolute number of newly-diagnosed patients in this developing country is still very high. Most of the major clinical centers have established multi-disciplinary team, but the strategy in treating this life-threatening disease still needs to be optimized, including the precise preoperative staging by various techniques, perioperative chemotherapy for locally advanced diseases, role of minimally invasive surgical approaches in treating patients with different stages, translational research to fill the gap between clinical trials and basic research, as well as the individualized treatment for gastric cancer. We believe that the biological behaviors and molecular events of gastric cancer in eastern countries are different from that of Western countries, so the clinical evidences derived from Eastern countries need to be strengthened to enrich the unmet demands in the clinical practice in China.

2.Synthesis of 10-Position Schiff Base Derivatives of Emodin and Preliminary Investigation of Anti-tumor Ac-tivity in vitro

Objective: To synthesize 10-position schiff base derivatives of emodin and study the anti-tumor activity. Methods:The 10-position schiff base derivatives of emodin were synthesized through emodin reacting with ammonia derivatives in absolute alcohol with acetic acid as the catalytic agent. Their structures were characterized by 1 H-NMR and ESI-MS, and the anti-tumor activity on k562 cells was determined by MTT method. Results:Three 10-position schiff base derivatives were synthesized and the structures were confirmed, and the IC50 on k562 cells was 4. 5, 3. 8 and 2. 0 μmol·L-1 , respectively. Conclusion:The 10-position schiff base de-rivatives of emodin can improve the anti-tumor activity and druggability of the parent compound, which are worthy of further study.
3.Early diagnosis and intervention in 0-9 months old infants with hearing loss.

OBJECTIVE:
To investigate the current situation of early diagnosis and intervention in 0-9 months old infants with hearing loss and analysis factors that will affect early diagnosis and intervention.
METHOD:
One hundred and eighty-six infants referred to the West China hospital from February 2014 to September 2014 were included. All 186 children were referred due to the fact that either they failed infant hearing screening or outer ear malformation. Early diagnosis and/or intervention were performed on those 186 children and their records of early diagnosis and intervention were analyzed.
RESULT:
Among the 186 infants, 167 (89.8%) were diagnosed with an average age at (4.0 ± 1.4) months. Among the 167 infants with final diagnosis, there were 31 (18.6%) infants diagnosed as conductive hearing loss (CHL), and 99 cases (59.3%) diagnosed as sensorineural hearing loss (SNHL), among whom, there were 75 (44.9%) bilateral SNHL and 24 (14.4%) unilateral SNHL. There were 2 cases (1.20%) with SNHL on one side and atresia on the other side. 5 (2.99%) of all conductive hearing loss cases with unilateral atresia and 2 cases with auditory neuropathy (AN) were found. 33 infants (19.8%) were found to have normal hearing. 30.7% (23/75) infants diagnosed as bilateral SNHL and 8.3% (2/24) infants diagnosed as unilateral SNHL were fitted with hearing aids. The fitting rate in infants with bilateral SNHL with mild, moderate, severe to profound degrees were 0 (0/23), 24.0% (6/25), 66.7% (6/9), 61.1% (11/18) respectively. The average intervention age was (5.0 ± 2.1) months.
CONCLUSION
Although the early diagnosis and intervention situation in this study are very close to international standard, there are still infants without final diagnosis and infants with hearing loss without hearing aid fitting. Further studies and efforts to promote early diagnosis and intervention in infants with hearing loss are needed.

5.Value of gastroendoscopy check-up early after non-surgical treatment of patients with gastroduodenal perforation

Objective To explore the value of gastroendoscopy check-up early after non-surgical treatment of patients with gastroduodenal perfolation.Methods The patients suspected of perforated gastroduodenal ulcer on hospital admission underwent non-surgical treatment were enrolled.After cured clinically,performed gastroendoscopy at different time periods.Results Among 133 patients underwent gastrointestinal endoscopy,129 cases(97.0%)were diagnosed gastroduodenal ulcer,3 cases(2.3%)were diagnosed gastric carcinoma,1 case (0.8%)Was confirmed duodenal diverticulum,no compilcations occurred due to endoscopy.The 6 cases that had surgical indications had be implemented the early surgical treatment. Conclusions After non-surgical treatment of patient with gastroduodenal perforation,timely gsstroendoscopy can truly show pathologic changes with gastroduodenal perforation.So endoscopy is beneficial under condition appropriate time seleetion for confirmed the eliology of PGD,treatment promptly targeted etiology and understanding the mechanism healing the perforation as non-surgical treatment.

8.The effect of simulate intraoral sandblasting on the band strength between enamel and composite resin

Objective: To study whether the bond strength between enamel and composite resin could be enhanced by intraoral sand abrasive. Methods: Ten human maxillary first incisior teeth were divided into 2 groups The experimental group was sandblasted with 30 ?m Al 2O 3 (CoJet Sand, pressure 300 kPa) from a distance of 5 mm for 5 seconds, and the control group were not sandblasted. The Herculite composite resin composite cylinders were bonded with Coltene system. Bonded specimens were stored in 37 ℃ distilled water for 24 h, then were subjected to shear force in a testing machine Stress at failure was calculated in Mpa, and mode of failure was recorded. The Student t test was applied to the data. Results: The shear bond strength of experimental groups was (33.0?1.8) MPa , and that of control groups was (26.7?5.2) MPa ,there was significant difference between these two groups. All the adhesive failures happened at the enamel composite resin interface , except that cohesive failure happened in one sandblasted specimen. Conclusion: Intraoral sandblasting could significantly enhance the shear bond strength between enamel and composite resin.
9.Diagnosis and analysis of residual gallbladder stone with choledocholithiasis

Objective To investigate the diagnosis/misdiagnosis reason/treatment of residual gallbladder stone with choledocholithiasis.Methods The clinical data of 34 patients with residual gallbladder,16 cases of them suffered from choledocholithiasis additionally were retrospective analyzed.Results 14 cases were treated by residual cholecystectomy +choledochotomy +T -tube drainage,2 cases were were treated by choledochotomy + T -tube drainage.Preoperative misdiagnosis of residual gallstones rate was 56.3% (9 /16 ),7 cases were found residual gallstones during operation.The other 2 cases were misdiagnosed residual gallstones.The rate of intraoperative and postoperative misdiagnosis was 12.5% (2 /16 ).18 cases with residual gallstones,16 cases underwent residual cholecystectomy,2 cases who were suspected with choledocholithiasis underwent residual cholecystectomy +choledo-chotomy,T -tube drainage.18 cases had no preoperative and postoperative misdiagnosis.Conclusion The clinical symptom of residual gallbladder stone with choledocholithiasis was similar to residual gallstones,often characterized by upper abdominal pain,fever,chills,jaundice,abdominal distension and other symptoms.It is difficult for preoperative imageological diagnosis that less than 3 cm residual gallbladder and small stones.When choledocholithiasis at the same time,needing more intraoperative careful dissect cystic duct and common bile duct,completes the cystic duct and common bile duct exploration,reducing misdiagnose residual gallstones.

This article analyzed the types and causes of clinical research misconduct including the definition and background.We then set up the research management system to control and prevent such misconduct behavior.Our strategy includes using various technical tools to actively carry out clinical research integrity related education and training program,establishing integrated control model,and clinical research misconduct handling mechanism to jointly cope with the problems,to promote a good atmosphere for clinical research and to establish the integrity of the research.Our goal is to promote the medical technology development and to improve the patient care.
