1.Thyroid microsomal antigen-antibody system and its clinical significance

Thyroid microsomal antigen-antibody(TMAg-Ab)system plays an important role in the pathogenesis of autoimmune thyroid diseases.TMAb are involved in the complementmediated cytotoxicity and antibody-dependent cytotoxicity,resulting in thyroid follicle epithelial cell and basement membrane injury,followed by thyroiditis and hypothyroidism,and they may also cause hyperthyroidism through some unknown mechanisms.In recent years,indirect hemagglutination,radioimmunoassay and enzyme-inked immunosorbent assay have been used to detect TMAb in thyroid disease patients'sera,indicating the incidence as high as 90%.Measurements of T3,T4 and TSH alsoreveal that serum TMAb level is related with the variation of T3,T4 and TSH.It ispostulated that TMAg might be the lipoprotein on smooth endoplasmic reticulum ofthyroid follicle epithelial cell.Due to the difficulties to purify TMAg,its nature stillremains unclear.
2.CLINICAL SIGNIFICANCE OF TMAb AND TGAb MEASURED WITH ENZYME-LINKED IMMUNOSORBENT ASSAY

The serum thyroid microsomal antibodies (TMAb) and thyroglobulin antibodies (TGAb) were measured in 103 normal persons and 183 patients with different thyroid diseases by enzyme-linked immunosorbent assay (ELISA). The serum T3 and T4 were also tested. The results showed that the incidence of these two autoantibodies were obviously increased in Hashimoto's thyroiditis, hyperthyroidism and hypothyroidism, and normal in simple goiter and thyroid adenoma. The TMAb and TGAb levels in subacute thyroiditis and thyroid cancer were moderately increased. There were also a negative correlation between TMAb and T3, T4, TGAb and T4 in Hashimoto's thyroiditis.
3.Clinical observation on nursing interventions for improving the quality of life after radiotherapy in nasopharyngeal cancer

Objective To investigate the effect of nursing interventions on the quality of life after radiotherapy in nasopharyngeal cancer. Methods 56 patients with nasopharyngeal cancer who received radiotherapy were randomly divided into control group and intervention group. All the patients were given conventional nursing interventions, and those in the intervention group were given accurate evaluation, enhanced psychological guide, reasonable dietary guide, and special nursing for preventing and treating complications. All the patients were evaluated by using quality of life questionnaire core 30 items(QOL-C30). Results The scores of the dimensions of somatic function,psychological function, social function and material life after interventions in the intervention group were significantly higher than those in the control group (P<0.05 ). Conclusion Nursing interventions can improve the quality of life, and the therapy can be completed smoothly.
4.Clinical Analysis and CT and MRI Diagnosis of the Solid-Cystic Papillary Tumors of the Pancreas

Objective To investigate the CT and MRI features of solid-cystic papillary tumors(SCPTs) of the pancreas.Methods Eight cases of SCPTs confirmed by operation and pathology underwent both plain and contrast-enhanced CT examination,and MR examination in three cases and contrast-enhanced MRI in two cases.Results In the eight patients,the mean tumor diameter was 7.6 cm and the lesions were found mainly in the body and tail of the pancreas,only one at the pancreatic head.On plain CT scan,the tumors appeared as huge solid-cystic mass with definite margin and an integritied capsul,the solid areas were papillary or irregular components,and the cystic areas in central were low attenuation.On contrast-enhanced CT,the cystic wall and solid part were slight enhancement in arterial phase,and moderate or obvious enhancement in parenchymal phase.Irregular ringed and punctate calcification could be seen in five cases,the acute hemorrhage occurred in one case,slight dilation of the pancreatic duct was noticed in one patient.MRI showed an ovoid solid-cystic mass of the pancreas with distinct border.On T1WI,heterogeneous iso-and hypo-intensity was observed.One case showing the high signal intensity in tumor was subacute hemorrhage.On T2WI,the tumors were mixed hypo-,iso-and hyperintensity.The papillary or flocculent solid portion and the cystic wall were enhanced after gadolinium was used.Conclusion SCPTs is of certain clinic,CT and MRI characteristic findings,which can aid in reaching a proper diagnosis.
5.The effects of resuscitation with limited fluid on rabbits with uncontrolled hemorrhagic shock

Objective To investigate the effects of resuscitation with limited fluid on rabbits with uncon-trolled hemordaagic shock. Method Uncontrolled hemorrhagic shock was produced in 40 rabbits. When the mean arterial pressure dropped to 40 manI-Ig and maintained for 30 minutes, resuscitation was initiated with Ringer solu-tion infusion into rabbits in large volume for group N3 (125 mL/kg) ,moderate volume for group N2 (92 mL/kg) and small amount for group N1 (57 mL/kg), and a drop of fluid was not given to rabbits of group N4. Besides, rob-bits of group N5 were controls without shock. MAP, fatality, volume of blood loss, SOD and Her were observed for 120 minutes or until death. Data were analyzed using SNK-q test and rank test. Results MAP, SOD and Het were higher, and fatality and blood loss were lower in group N5 than those in groups N1, N2,and N3 (P<0.01 ). MAP,fatality,blood loss and Het of group N4 were worse than those of groups N1,N2 and N3 (P<0.01). The blood loss and fatality in group N3 were significantly higher than those in groups N1 and N2 (P<0.01 ). SOD of kidney in group N3 was significantly lower than that in group N1 and N2(P<0.01). As the volume of fluid in-creased during resuscitation, Het of all animals was gradually becoming lower. As MAPS in groups N2, N3 and N4 were gradually becoming lower and lower, death increased. Conclusions Limited fluid resuscitation (57~92mL/kg) should be recommended to the therapeutic strategy for uncontrolled hemorrhagic shock.
6.Efficacy on hemiplegic spasticity treated with plum blossom needle tapping therapy at the key points and Bobath therapy: a randomized controlled trial.

OBJECTIVETo evaluate the efficacy on hemiplegic spasticity after cerebral infarction treated with plum blossom needle tapping therapy at the key points and Bobath therapy.
METHODSEighty patients were collected, in compliance with the inclusive criteria of hemiplegic spasticity after cerebral infarction, and randomized into an observation group and a control group, 40 cases in each one. In the control group, Bobath manipulation therapy was adopted to relieve spasticity and the treatment of 8 weeks was required. In the observation group, on the basis of the treatment as the control group, the tapping therapy with plum blossom needle was applied to the key points, named Jianyu (LI 15), Jianliao (LI 14), Jianzhen (SI 9), Hegu (LI 4), Chengfu (BL 36), Zusanli (ST 36), Xiyangguan (GB 33), etc. The treatment was given for 15 min each time, once a day. Before treatment, after 4 and 8 weeks of treatment, the Fugl-Meyer assessment (FMA) and Barthel index (BI) were adopted to evaluate the motor function of the extremity and the activity of daily life in the patients of the two groups separately. The modified Ashworth scale was used to evaluate the effect of anti-spasticity.
RESULTSIn 4 and 8 weeks of treatment, FMA: scores and BI scores were all significantly increased as compared with those before treatment in the two groups: (both P<0. 05). The results in 8 weeks of treatment in the observation group were significantly better than those in the control group (all P<0. 05). In 4 and 8 weeks of treatment, the scores of spasticity state were improved as compared with those before treatment in the patients of the two groups (all P<0. 05). The result in 8 weeks of treatment in the observation group was significantly better than that in the control group (P<0. 05). In 8 weeks of treatment, the total effective rate of anti-spasticity was 90. 0% (36/40) in the observation group, better than 75. 0% (30/40) in the control group (P<0. 05).
CONCLUSIONThe tapping therapy with plum blossom needle at the key points combined with Bobath therapy effectively relieves hemiplegic spasticity in the patients of cerebral infarction and improves the motor function of extremity and the activity of daily life.

7.Arthroscopic PCL reconstruction with an implant fixation of hamstring tendon knot and bottle-neck technique

Objective To evaluate arthroscopic PCL (posterior cruciate ligament) reconstruction with an implant fixaliun of hamstring tendon knot and bottle-neck technique.Methods Thirteen cases with PCL injury were treated with arthroscopic PCL reconstruction with an implant fixation of hamstring tendon knot and bottle-neck technique between April,2002 to June,2005.The patients were followed up fur at least seven months to evaluate the clinical effects by means of ROM of the joint,post drawer test,Lachman test,Lysholm and IKDC (International Knee Documents Committee) scores.Results The patients were followed up for an average of 13.4 months.No limitation of knee extension was found six months after operation.Eleven patients had normal knee flexion(>140?), one had knee flexion of 120?and one 100?.In posterior drawer test,11 patients were gradeⅠand two gradeⅡ.In Lachman test,11 patients were rated as within gradeⅠand two gradeⅡ.By Lysholm knee function scale,the average knee scores were 90.47?4.13 and 78.82?2.46 respectively before and after operation,and the dif- ference was of statistical significance (t=2.416,P=0.029).By IKDC scale,six were grade A,five grade B and one grade C.Conclusion Arthroseopic PCL reconstruction with an implant fixation of hamstring tendon knot and bottle-neck technique is a reliable method to restore knee function and will have a prospect of wide application be- cause of its good tissue biocompatibility,less invasion,and less operative expenditure.
9.Dynamic detection of T cell subsets and CD3+HLA-DR+T cells in peripheral blood with hand allograft recipients

Objective To study dynamic changes of T cell subsets and CD3+HLA-DR+T cells in peripheral blood of composite tissue transplantation-hand allograft recipients. Methods The levels of CD3+、CD3+CD4+、CD3+CD8+、CD3+HLA-DR+T cells and ratio of CD4/CD8 in peripheral blood of hand allograft recipients in different periods were examined by using flow cytometry.The recipients before transplantation were as the control groups.Results The first day after transplantation,the levels of CD3+、CD3+CD4+、CD3+CD8+、CD3+HLA-DR+T cells and ratio of CD4/CD8 all began to descend.The 3th to 5th day after transplantation,the levels were the lowest.The 8th day,the levels of CD3+、CD3+CD4+、CD3+CD8+、CD3+HLA-DR+T cells and ratio of CD4/CD8 were eventually rised and go to stable 15th day after transplantation.But the levels of CD3+、CD3+CD4+and value of CD4/CD8 were still lower than those of the contols,and the levels of CD3+CD8+、CD3+HLA-DR+T cells were higher distinctly.Conclusion Dynamic changes of T cell subsets and active T cells in peripheral blood of composite tissue transplantation-hand allograft recipients were accordant with that of renal allograft recipients with stable period and with stable clinical state of the hand transplantation recipients.
10.Advances in poly (N-isopropylacrylamide) based platforms for cell culture.

Poly (N-isopropylacrylamide) (PNIPAAm), a temperature-responsive polymer, can be potentially applied to replace enzymes or cell scrapers to recover attached cells. Taking full advantage of this unique function of PNIPAAm, cells can be protected from enzymatic hydrolysis and mechanical treatment, thereby to provide ideal seed cells with high quality for biomedical fields. In this review we describe the method to facilitate cell effective adhesion and rapid detachment on thermoresponsive two dimensional surfaces, including selecting special substrate, introducing hydrophilic group, adjusting reactant ratio, controlling polymer thickness/density, providing appropriate external force, so as to effectively improve adherent cell adaptability to thermoresponsive surfaces, depress the risk of bacterial contamination and reduce the effect of low-temperature treatment on the cells. The three dimensional cell culture systems involved in temperature-sensitive microcarriers, scaffolds and gels were briefly discussed. The application based on the platforms for cell culture was also presented.
