Objecttve To investigate the effects of snfentanil and ropivacaine used in labor combined spinalepidural analgesia(CSEA)and its influence on labor course and infants and its side effects. Methods 160 primiparas who chose deliver analgesia were randomly divided into two groups:group A intrathecal 5μg sufentanil plus saline up to 2.5ml,extradural 0.2μg/ml sufentanll plus 0.1% ropivacaine and saline up to 100 ml;group B extradural ropivacaine 1.5mg plus glucose 100g/L up to 2ml;group C have no analgesia. Then to investigate the effects of analgesia VAS scores, labor course duration, caesarean section rate, flooding volume, neonatal Apgar scores and its side effects. Results Compared with group C,the analgesia effects of group A and B were definitely(P<0.05,P<0.01);visual analogue score(VAS)at 5min,10min and 30min in group A were(3.4±1.2),(1.6 ±0.7)and(0.7 ±0.5)respectively,while in group B were(5.3±1.5),(4.3±1.8)and(3.1±1.7),which had significant difference in two groups(all P<0.01);they all could reduce the first labor course duration, caesarean section rate, and there were no difference with group C(P>0.05);and the flooding volume and neonatal Apgar scores had no differences(P>0.05).Conclusion Sufentanil and ropivacaine combined used in CSEA has stronger effect and longer duration without prolonged labor coulee and poor side effects, which would be a more perfect analgesia worth clinical application in obstetrics.

The peroneal artery chimeric bone flap combined with chimeric flap of lateral femoral circumflex artery was applied in the repair of a case defects of both ulna and radius arteries combined with large extensive ring-like soft tissue defects of forearm in November, 2018, causing no blood supply to the hand. In the one-stage surgical procedure, anastomosis of the ulnar and radial arteries with the pedicle of the 2 flaps by Flow-through anastomosis were performed and the blood supply of the forearm and hand was restored. Six months after the surgery, the rotation functions of forearm, wrist joint and fingers were recovered. The patient was able to carry out daily activities and some of precision tasks.

BACKGROUND:Tantalum rod implant technology is a new method of early osteonecrosis treatment. Current research on stress distribution before and after tantalum rod implant in different sizes of femoral head necrosis area is few. OBJECTIVE:To analyze the stress distribution before and after tantalum rod implantation in different sizes of necrotic femoral head area using three-dimensional finite element method. METHODS:Three-dimensional finite element models of normal femoral head and necrotic femoral head of 15, 20 and 30 mm diameterwere constructed. Eight measuring points were chosen on two tiers of each necrotic model to detect the stress distribution and its alteration before and after tantalum rod implantation. RESULTS AND CONCLUSION:(1) Stress concentration werefound on every necrotic femoral head, most pronounced on the one with 30 mm lesion. (2) Tantalum implant appeared to reduce the stress concentration generaly. Comparison of the peak points of these models indicated most significant benefit in 15 mm lesion, next in 30 mm lesion, last in 20 mm lesion. (3) Results indicate that larger lesion entails more concentrated stress distribution and more likely to colapse. Tantalum rod implantation can delay the development of necrosis of the femoral head, andismost effective in smal lesion.

Objective To establish the triplex Taqman probes real-time RT-PCR method for simultaneously detecting of EV71,CA16 and EV.Methods Retrospective study.Specific primers and probes were designed based on conserved regions of EV71,CA16 and EV.The sensitivity, specificity and reproducibility were assessed by the optimized reaction system.A total of 176 throat swabs as the experimental group were collected from children with suspected hand foot mouth disease (HFMD),who admitted from April 2012 to July 2012 in Nanjing Children's Hospital affiliated to Nanjing Medical University.During this time,10 cases of healthy children,10 cases of outpatients with flu-like symptoms and 90 cases of inpatients in pneumology department of our hospital were recruited as control group, whose throat swabs were also collected. All of 286 samples were tested by the triplex Taqman probes real-time RT-PCR for simultaneously detecting EV71,CA16 and EV.SPSS13.0 was used to analyze the results. Results The sensitivities of the triplex Taqman probes real-time RT-PCR was 1.0 × 103 copies per milliliter for EV71,CA16 and EV.It showed 100% specificity for 9 enterovirus and 3 non-enterovirus.Analysis with 1.0 × 103-1.0 × 105 copies per milliliter constructed plasmids demonstrated high reproducibility with coefficient of variation of 0.44%-1.04% for EV71,0.38%-0.73% for CA16,and 0.46%-0.90% for EV.More over 176 samples collected from children with suspected HFMD were detected by triplex Taqman probes real-time RTPCR and real-time RT-PCR.The results showed 97.2% (171/176)agreement and 0.94 Kappa value with high concordance. Conclusions The triplex Taqman probes real-time RT-PCR detecting EV71,CA16 and EV simultaneously has been established successfully. The assay, with high sensitivity and specificity, provide good basis for the rapid clinical diagnosis of EV71,CA16 and EV and open up broad prospects for clinical and relevant researches.

Objective To investigate the change of HepG2 liver tumor perfusion after microbubble enhanced ultrasound cavitation treatment and observe the related pathological injury. Methods Twenty eight Balb/c(nu/nu) nude mice transplanted subcutaneous HepG2 tumor were divided into three groups randomly, including the microbubble enhanced ultrasound cavitation group, the ultrasound group and the sham group.Microbubble enhanced ultrasound cavitation treatment was performed by 0.1 ml microbubbles intravenous injection combined with pulse ultrasound emission in experimental group, while in control groups only ultrasound exposure or microbubble injection were applied. The perfusion of tumors was imaged using contrast-enhanced ultrasonography before and after treatments.Time-intensity curve and peak intensity were analyzed. The tumors were then harvested for histological examination. Results The perfusion of HepG2 tumors almost vanished immediately after treatment in experimental group, with the peak intensity reduced from (26.9 ± 10.9)% to(8.2 ± 5.8)% (P <0.05).There was no significant changes before and after treatments (P > 0.05) in the two control groups.Histological findings were disruption of the endothelia, significant hemorrhage and increased intercellular fluid. Conclusions Microbubble-enhanced ultrasound cavitation can significantly reduce tumor blood perfusion and disrupt tumor vascularture. This new ultrasound therapy can potentially become a new physical anti-angiogenetic therapy for liver tumor.

Objective To observe the recent effects and toxicity in advanced gastric carcinoma by two kinds of neoadjuvant chemotherapies ( PF and FOLFOX).Methods A total of 60 patients with advanced gastric cancer ( Ⅲ ~ Ⅳ ),they were randomly divided into 2 groups; PF group ( 30 cases) and FOLFOX group ( 30 cases). All patients were given 3 cycles of docetaxel or L-OHP chemotherapy preoperatively,to observe the recent effects and toxicity in advanced gastric carcinoma. Results The total effective rate in PF group was 77.67%,while it was 50.00% in FOLFOX group,it had significant difference between in two groups ( x2 =4.59,P < 0.05 ).There is no difference in the incidence of neutropenia, transaminase elevation, nausea and diarrhea two groups. The incidence of hair loss in PF group is higher than in FOLFOX group, but there is no difference between two groups; The incidence of peripheral nerve paresthesia in FOLFOX group is higher than that in PF group( x2 =5.08,P <0.05).Conclusion PF as neoadjuvant chemotherapy is a very effective and well-tolerated regimen in patients with advanced gastric cancer.
