1.Effects of mycophenolic acid on the proliferation of rat pulmonary microvascular endothelial cells and the secretion of endothelin-1 and interleukin-6

Objective To investigate the effect of mycophenolic acid (MPA) on the proliferation of rat pulmonary microvascular endothelial cells (PMVECs) and the effects of MPA on the secretion of endothelin-1 (ET-1) and interleukin-6 (IL-6).Methods PMVECs were cultured by tissue-sticking method.Cells were divided into 5 groups,named as the control group,low-dose MPA group,moderate-dose MPA group,highdose MPA group and super high-dose MPA group,according to the final concentration of MPA (0,0.1,1,10 and 100 μmol/L,respectively).The proliferation of rat PMVECs was detected by cell counting kit-8 (CCK-8).The ET-1 and IL-6 secretion were assessed by radio-immunoassay.One-way analysis of variance was used to detect the differences between the group means.Results The cultured PMVECs of rats in vitro showed fusiform or polygonal in shape,and the expressed factor Ⅷ associated antigen stain was positive.At 24 h,the A values of the 5 groups (from control group to super high-dose MPA group) were 1.15±0.20,1.20±0.13,0.88±0.20,0.62±0.05 and 0.56±0.10,respectively,there was no significant difference in the A values on 450 nm between the control group and the low-dose MPA group (P=0.388),as well as the high-dose MPA group and super high-dose MPA group (P=0.292),but there were significant differences between any other two groups (P<0.01).At 48 h,the A values of the 5 groups (from control group to super high-dose MPA group)were 2.61±0.21,2.52±0.23,1.99±0.17,0.84±0.16 and 0.65±0.12,respectively,there was no significant difference in the A values on 450 nm between the control group and low-dose MPA group (P=0.094),but there were significant differences in the A values between any other two groups (P<0.01).At 72 h,the A values of the 5 groups (from control group to super high-dose MPA group) were 2.62±0.24,2.62±0.19,2.37± 0.38,0.79±0.17 and 0.61±0.17,respectively,there was no significant difference in the A values on 450 nm between the control group and low-dose MPA group (P=0.931),but there were significant differences in the A values between any other two groups (P<0.05).There were no significant differences in the ET-1 concentrations between the control group and low-dose MPA group (P=0.156),and high-dose MPA group and super high-dose MPA group (P=0.262),but there were significant differences in the concentrations between any other two groups (P<0.01).There were no significant differences in the IL-6 concentrations of the five groups between any of the two groups (P>0.05).Conclusion MPA can inhibit fetal bovine serum induced rat PMVECs proliferation and the secretion of ET-1 in a dose-dependent manner.
2.Biomechanical experiment of side-hole hollow bone cement nail in the femoral neck

OBJECTIVE:To observe the effect of the side-hole hollow bone cement nail on promoting bone fracture healing. METHODS:This experiment was performed in the Third Hospital of Hebei North College from July 2006 to December 2008. Femoral neck samples were obtained from 60 adult corpses of which the patients were dead one year at most with the mean age of 70 years. X-ray examination showed a significant osteoporosis,but no other abnormalities. Hollow nail was provided by Sanyou Medical Apparatus Co.,Ltd.,Shanghai; self-cohesion methyl methacrylate bone cement was provided by Dental Material Factory,the Second Medical University of Shanghai. In the maximal axial pulling-out force group,the hollow nails were directly implanted into 10 femoral neck samples after drilling at the axial center. The polymethyl methacrylate bone cement was inserted into other 10 femoral neck samples after drilling at the axial center,and the side-hole hollow nail were then implanted. The pulling out experiment was performed by AGS-10KN material testing machine to measure the maximal axial pulling-out strength of hollow nail and the maximal loading when the nail was loose during anti-bending cycle experiments. RESULTS:① The maximal axial pulling-out force of 10 femoral neck samples implanting the hollow nail was 471.5-1 070.5 N,with the mean value of (855.0 ? 207.5) N; the maximal axial pulling-out strength of femoral neck samples implanting the side-hole hollow nail added by poly-methyl methacrylate bone cement was 2 023.7-3 572.1 N,with the mean value of (2 943.3 ? 535.8) N. 3.Metabolic Effects of Nonsteroidal Anti-Inflammatory Drugs on Chondrocytes in Human Osteoarthritis

Objective To observe the effects of nonsteroidal anti-inflammatory drugs (NSAIDs) on the proliferation of chondrocytes and metabolism of human osteoarthritis (OA) in short-term (48 hours). Methods The human knee OA chondrocytes were cultured in the presence or absence of NSAIDs (celecoxib, diclofenac and ibuprofen). BrdU assays were used to evaluate the effects of NSAIDs on the proliferation of OA chondrocytes. ELISA was used to detect the contents of pro-teoglycan and type-Ⅱcollagen in chondrocytes and culture supernatant. The differences of those indexes were compared be-tween groups. Results The positive rate of BrdU labelling index of chondrocytes increased significantly in ibuprofen group than that of other groups (P<0.05). The content of type-Ⅱcollagen in the culture supernatant increased significantly in di-clofenac group than that of other groups (P<0.05 or P<0.01). The content of proteoglycan in the culture supernatant in-creased significantly in ibuprofen group than that of other groups (P<0.05 or P<0.01). There were no significant differences in the content of type-Ⅱcollagen and proteoglycan between chondrocytes of NSAIDs groups (P>0.05). Conclusion Ibu-profen may stimulate the proliferation and the secretion of proteoglycan of human OA chondrocytes. Diclofenac may stimu-late the secretion of type-Ⅱcollagen of human OA chondrocytes. There were no effects of celecoxib on the proliferation and metabolism of human OA chondrocytes.
5.A novel amorphous calcium phosphate drug-eluting stent:its biocompatibility after implantation into mini-swine coronary artery

BACKGROUND:As previously reported, poly-L-lactic acid stents have been mostly used as coronary artery stents, but this stent has a lack of early support force and produces some acidic products that wil induce local vascular inflammatory reactions.
OBJECTIVE:To explore the biocompatibility of a novel amorphous calcium phosphate drug-eluting stent, after implantation into mini-swine coronary artery.
METHODS:Twenty mini-swine were randomly equivalently divided into observation group and control group. The right femoral arteries of animals were separated layer by layer and the new type of biodegradable stent and poly-L-lactic acid stent were implanted, respectively.
RESULTS AND CONCLUSION:The vessel lumes in the two groups were smooth and unobstructed showed by coronary artery angiography immediately after implantation. At 4 weeks after implantation, the coronary angiography showed the patency of vessel lumens with no stent thrombosis and stenosis in both two groups. The C-reactive protein levels in the two groups had no significant differences before and 4 weeks after stent implantation. At 4 weeks after implantation, the number of lymphocytes and inflammatory score in the observation group were significantly higher than those in the control group, but the neointimal area and percentage of stenosis area had no significant difference between the two groups. These results indicate that the novel amorphous calcium phosphate drug-eluting stent after implantation into mini-swine coronary artery has good biocompatibility.
6.Treatment the fractures of the base of the first metacarpal with mini external fixator

Objective To introduced the clinical application and indication of treatment the fractures of the base of the first metacarpal with mini external fixator. Methods From October 2002 to December 2004, 37 cases of different typical fractures of the base of the first metacarpal were treated with mini external fixator and followed up, which included type Ⅰ (Bennett fracture) 19 cases, type Ⅱ (Rolando fracture) 10 cases, type Ⅲ 8 cases. Defferent operations were applicated accroding to the fresh degree and classification of fractures. In type Ⅰ and fresh type Ⅱ fractures, closed reduction and fixation with mini external fixator were applicated at first. If the reduction was insatisfied, open reduction and fixation with Kirschner wire and external fixator were necessary. In old type Ⅱ fractures, open reduction associating internal and external fixation was applicated. In fresh type Ⅲ fractures, close and external fixation were applicated. In old type Ⅲ fractures, open reduction and external fixation were done. Results All the cases were followed up average 15 monthes and all the fractures were union successfully. The average union time were 5.5 weeks and 7.5 weeks in the fresh and old fractures. Function evaluation was made according to pain degree, thumb joint activity and proportion of traumatic arthritis. The overall good-excellent rate was 94.6%, the clinical results were satisfactory. Conclusion Treatment of fractures of the base of the first metacarpal with mini external fixator is better than conventional methods. This method is simple and reliable, and may shorten heal time, decrerase joint pain, limitation of joint movement and proportion of traumatic arthritis. It is an effective method in treatment of fractures of the base of the first metacarpal.
7.Dynamic hip screw hole filled by bone cement: Anti-flexion and anti-torsion strength

OBJECTIVE: To evaluate the clinical outcomes of medical supporting bone graft following posterior approach and bone cement implantation in the hip joint in treating intertrochanteric fracture.METHODS: A computer-based online search of Science Direct, Ei databases was performed for English articles published between January 1960 and October 2009, with the key words "bone cement, intertrochanteric fracture". In addition, CNKI and CBM were searched for related Chinese articles published between January 1994 and October 2009, with the key words "intertrochanteric fracture, coxa vara, posterior approach of hip joint, bone cement implantation in major screw hole". Moreover,related books were manually searched. Treatment of intertrochanteric fracture, basic and clinical experiment of intertrochanteric fracture treated by bone cement was included.RESULTS: Intertrochanteric fracture treatment includes lateral or anterior approach for dynamic hip plate system, dynamic hip screw, and femoral proximal intramedullary screw internal fixation. Dynamic hip screw is standard internal fixation to treat intertrochanteric fracture, with strong anti-rotation function, and meets the biomechanical requirements. However, screw loosening,extraction and breakage frequently occur. Cancellous bone screw track enhanced by bone cement increases screw retention force,benefits screw compression, enhances bone-screw interface to transfer the stress to screw-bone regions, which significantly increases the anti-flexion and anti-torsion strength of dynamic hip screw and improves fracture stability. The mechanism involves the integral formation of cancellous bone, bone cement and screw by the micro-interlocking of bone trabecula and surrounding cancellous bone.CONCLUSION: Dynamic hip screw filled by bone cement significantly enhances the anti-flexion and anti-torsion strength of internal fixation and improves fracture stability.
8.Research on applied analysis on 64 row spiral CT post-processing technique in the occult fracture of rib

Objective:To explore the clinical application value of multi-slice spiral CT in the diagnosis.Methods: Twenty nine patients with clinically suspected rib fractures and X-ray showed no 64 row helical CT scanning in patients with chest trauma fractures after reconstruction, multiplanar reconstruction and VR imaging.Results: Twenty nine patients with occult fracture of rib on the MPR, VR imaging were rib image good, 28 cases were single, multiple rib fractures in 1 cases, the total of 4 cases with rib fracture, incomplete fracture in 25 cases.Conclusion: The 64 row helical CT for occult rib fracture detection rates, diagnostic accordance rate was superior to X-ray plain film, various post-processing techniques applied to timely, accurate, intuitive display rib fracture location, shape, extent of chest trauma, occult rib fracture first choice inspection method.
