1.The diagnosis and treatment of primary duodenal tumor

Objective To evaluate the preoperative diagnostic procedures and treatment choice of primary tumor of the duodenum (PTD).Method The clinical data of 52 cases with PTD in the last 10 years was analyzed retrospectively.Results The correct diagnostic rate of auxiliary examination was:duodenal endoscopy of 90.5%,air barium double radiography of 80%.Six primary benign tumors of duodenum (PBTD)were resected completely with a 5 year's survival of 100%.Among the 46 cases of primary malignant tumors of duodenum (PMTD),28 cases underwent pancreatoduodenectomy,4 cases received segmental duodenectomy,the remaining 13 cases in which the tumors were unresectable were treated bypass.The resection rate was 69.6% and the 5 years' survival rate was 32.6%.Conclusions Duodenal endoscopy and air barium double radiography are mainstays for the diagnosis of PTD.Segmental duodenectomy and simple tumor resection are curable for PBTD; while for PMTD,the therapy of choice should be pancreatoduodenectomy.

Objective Comparing the variability of four genes of HCMV in the population and the therapy process of single patient.Methods Using PCR to amplify the four genes and restriction enzyme to validate four fragments, These four genes are UL55,UL57,UL86 and UL122 whose coding products are glycoprotein B,single chain binding protein,main capsid protein and IE pp86 separately. Then we use AD169 to optimize the conditions of PCR and restriction enzyme. then we use these conditions to detect 200 blood specimens. 4.180° rotated maxillary central incisor: a case report.

The etiology of rotated maxillary central incisor is unclear, but it has a clear familial tendency. Except for the environmental factors, gene expression directly influences craniofacial growth and the types of malocclusion deformity. In this article, a case of 180° rotated left maxillary central incisor of a male with family history of the deformity was reported. Relevant literature was also reviewed.

Objective: To evaluate the effect of dietary fiber and phytic acid inositol hexaphosphate (IP6 or Insp6) on 1,2-dimethylhydrazine(DMH) induced colorectal carcinogenesis in Wistar rats. Methods: 86 four-week-old male Wistar rats were divided randomly into 6 groups and fed with either basal fiber free diet or the basal diet supplemented with 10% pectin, 10% cellulose, 2% Na-InsP6 supplemented water, 10% pectin in combination with 2% Na-InsP6 supplemented water, 10% cellulose in combination with 2% Na-InsP6 supplemented water. After four weeks, the rats were given a weekly injection of DMH (20mg/kg bw) for 20 weeks. The rats were killed after 26 weeks. The number of total macroscopically visible neoplasms was counted and the volume of individual neoplasmas was calculated. Proliferation cell nuclear antigen (PCNA) was analyzed. Results: There was no significant difference between the treatment group and the control group in the incidence of tumors. InsP6 significantly reduced the number of tumors in rats and tumor volume. Pectin and Pectin+InsP6 significantly increased the number of tumors in rats. The expression of proliferation marker PCNA was significantly down-regulated by InsP6 and significantly increased by pectin. Conclusion: A treatment regimen of 2% Na-InsP6 in drinking water was effective in significantly reducing the risk of colorectal cancer in Wistar rats. Diet supplemented with pectin could increase the risk of colorectal cancer.
8.Olive oil-based lipid emulsion for parenteral nutrition in patients after hepatectomy

Objective To assess the safety and efficacy of an olive oil-based lipid emulsion for parenteral nutrition in patients after hepatectomy.Methods Thirty-one postoperative patients with elective hepateetomy were randomized to receive isonitrogenous,isocalorie parenteral nutrition over 6 days after liver lobectomy(48-72hours)with either olive oil-based lipid emulsion(study group,n=15)or standard soybean oil emulsion(control group,n=16).The liver function and plasma protoins were assessed using peripheral venous blood collected before surgery,one day after surgery,and 7 days after surgery.The safety profiles of emulsion supports and postoperative rehabilitation were also assessed.Results The preoperative serum levels of total bilirubin,direct bilirubin,alanine amiotransferase,aspartate aminotransferase,alkaline phosphatase,total protein,albumin,and prealbumin were comparable between the two groups(all P>0.05).Although the Postoperative safety profile and liver function were not significantly different between two groups(all P>0.05),plasma total proteins,albumin,and prealbumin returned to the normal levels significantly faster in the study group than in control group[(57.57±9.84)g/L vs.(47.76±6.53)g/L,P=0.000;(31.29±3.11)g/L vs.(26.34±4.87)g/L,P=0.000;(0.188±0.059)g/L vs.(0.103±0.037)g/L,P=0.000]on the 7th Postoperative day,and the Postoperative hospital stay was also significantly shorter in the study group[(13.1±1.2)d vs.(15.2±1.1)d,P=0.041].The incidence of postoperative complications in study group and control group was 26.7%and 31.3%.respectively.Conclusions Treatment with the new olive oil-based lipid emulsion is weU tolerated in hepatectomy patients.It can speed up plasma proteins recovery and may shorten postoperative hospital stay,although it does not remarkably decrease the incidence of postoperative complications.
9.Advances in tissue and organ engineering

Tissue engineering has evolved as a dynamic research field that encompasses multidisciplinary approaches involving cytology,material science,bioreactor engineering,and medicine.The overarching goal is to construct human tissue and organs in the laboratory for tissue regeneration or replacement.It offers a potential solution to the donor shortage in organ transplantation and to the difficulties in regenerative medicine.The basic components for tissue engineering include seed cell selection,bio-scaffold construction,and ex vivo tissue formation via a bioreactor. Over the past decade,tremendous progress has been made in bioengineering cartilage,trachea,urinary bladder,heart valve and endocrine tissues. However,there remain significant challenges in creating complex and human-sized tissues and organs for clinical use.In this article,we review the basic components and strategies for bioengineering organs. We attempt to provide an overview of current progress and challenges in developing organ-level tissue engineering and in the successful translation of bioengineered tissue and organ products into daily clinical practice.
10.Cloning of novel gene GP1 associated with gastric cancer and its premalignant lesions

Objective To explore the differentially expressed genes of gastric cancer,matched normal gastric tissue and premalignant lesions. Methods The differentially expressed cDNA bands were isolated and identified by fluorescent differential display and then reamplified by PCR.After being cloned,all cDNA fragments were sequenced. Through BLAST,the results were compared with GenBank database for homology analysis. The expression of the cDNA fragment in different tissues was confirmed by Northern blot. SMART-RACE(rapid amplication of cDNA ends) was used to amplify the full length cDNA sequence. Bioinformatics analysis was performed to analysis its function. Results A differentially expressed cDNA fragment expressed lower in gastric cancers and premalignant lesions,no homology was found in GenBank. The novel cDNA fragment was given an accession number of EST(CD454989)in GenBank. A full length cDNA sequence of 1362 bp was acquired,who coded 267 amino acids,and was homologous with BAA91562.1,whose physiology function was unknown. Conclusion A differentially expressed gene was found and it may be involved in process of the gastric cancer.
