2.Effect of Paecilomyces hepiali mycelium on exhausted fatigue in mice

OBJECTIVE ToevaluatetheeffectandmechanismofPaecilomyceshepialimycelium
(PHM)onexhaustedfatigueinmice.METHODS Micewereforcedtorun30minadayforconsecutive 18 d using the wheel running fatigue apparatus, and the speed was gradually increased from 10.2 m·min -1 to 12.6 m·min -1 .On d19,these animals were forced to run 60 min to establish an exhausted fatigue model.Every day,40 min before the run training,the animals were ig administered with PHM (140,280,560 mg·kg -1 )and modafinil (13 mg·kg -1 ).The number of electric shocks was recorded on d5,d9,d1 3,d1 7 and d1 9,respectively,and the body mass of each mouse was recorded daily.The levels of muscle glycogen (MG),liver glycogen (LG),serum urea nitrogen (SUN),serum lactic acid (SLA)and serum creatine kinase (CK)were detected by commercially available kits. RESULTS Duringthelastexhaustedrunningond19,thenumberofelectricshocksofthemodelgroup was markedly larger than that of the drug administration groups.When compared to the normal control group,the model group showed a significant decrease in the level of either MG or LG,but a significant increase in the level of either SUN or CK.The nu mber of electric shocks of the PHM in the moderate and high dose groups was 133 ±55 and 1 15 ±41 times,respectively,which was significantly smaller than that of the model group 240 ±89 (P<0.01 ).The levels of MG and LG of the PHM560 mg·kg -1 group were (1 .28 ±0.24)mg·g -1 and (40.9 ±1 0.2)mg·g -1 ,respectively,which were significantly higher than those of the model group〔MG:(0.91 ±0.26)mg·g -1 ,LG:(22.2 ±2.9)mg·g -1;P<0.01〕,and the levels of SUN,SLA and CK of this PHM group were (5.8 ±1 .3)mmol·L-1 ,(5.7 ±1 .7)mmol·L-1 and (0.6 ±0.2)kU·L-1 ,respectively,which were significantly lower than those of the model group 〔SUN:(7.5 ±2.1 )mmol·L-1 ,SLA:(7.1 ±1 .8)mmol·L-1 ,CK:(0.8 ±0.3)kU·L-1;P<0.05〕.The number of electric shocks of the modafinil group was also significantly smaller than that of the model group (P<0.01 ),but there were not any significant differences between the modafinil group and the model groupinthelevelsofMG,LG,SUN,SLAorCK.CONCLUSION ThisstudyshowsthatPHMcanhelp relieve fatigue,which might be associated with its abilities to increase the storeage of energy sources,to enhance the aerobic metabolis m,and to decrease the accu mulation of metabolic products.
3.Advances in insertion depth and indwelling time of umbilical venous catheterization

Umbilical venous catheterization (UVC) is associated with many complications, such as displacement of the catheter, infection, exudation, thrombosis, pericardial effusion/pleural effusion, of which central line-associated bloodstream infections has been of long-standing interest. There is currently no optimal method for estimating the depth of UVC insertion. Ultrasound examination can be used to evaluate the position of the end of the tube after catheterization to avoid organ damage and complications caused by displacement and dislocation. However, whilst it is known that there is a correlation between the duration of UVC and central line-associated bloodstream infection, a consensus is yet to be reached regarding the optimal duration of UVC retention. More, high quality evidence through multi-center, prospective randomized controlled study is needed.
4.Microtensile bond strength and SEM evaluation of total-etch and self-etching adhesives to caries-affected dentine

Objective:To evaluate the microtensile bond strength of total-etch or self-etch adhesives to caries-affected dentine versus normal dentine. Methods:Twenty-four extracted human molars with mesial coronal carious lesions extending into mid-dentin were selected. The softened carious dentine was removed with a slow-speed, carbon-steel bur and Caries Marker was used as guides. Prepared cavity in sound dentine in the distal coronal part of the identical teeth was used as the control. Surfaces were bonded with either Prime&Bond NT, Adper Single Bond 2, Adper Prompt or XenoⅢ,according to manufactures' recommendations. A crown was built up using resin composite(Filtek P60).After storage in water (37 ℃,24 h),teeth were vertically serially sectioned into 1.0 mm thick slabs and trimmed to yield 1 mm2 test area that contained either caries-affected or normal dentine. Then the specimen were tested in tension in an Instron machine at 0.8 mm/min.The bonding interface between the dentine and adhesives was qualitatively evaluated under the observation of scanning electron microscope(SEM). 5.A Twenty Four Week Clinical Trial Using Didonosine, Stavudine and Nevirapine for Highly Active Antiretrioviral Therapy

Objective To obserre antiviral effect, immune rebuilding efficacy and side effects of didonosine, stavudine and nevirapine combination therapy for human immunodeficiency virus (HIV) type 1 infection. Methods Three medicines were administered for 8 HIV-1 infected cases. Plasma HIV RNA load, and the levels of CD 4 + T cell and CD 8 + T cell were detected before starting treatment and 4,12 and 24 weeks after starting treatment. Side effects and changes in laboratory's examinations were observed during the trial. Results After 12 and 24 week treatment, the plasma HIV-1 level reduced 2 28 logs and 2 63 logs, from a mean baseline of (283,125?187,217) copies/ml to (1,501?930) copies/ml and (669?477) copies/ml, the CD 4 + T cell counts increased from a mean baseline of (337?221) cells/ml to (393?301) cells/ml and (414?284) cells/ml, and CD 8 + T cell counts decreased from a mean baseline of (918?371) cells/ml to (823?315) cells/ml (812?305) cells/ml, respectively. Part of cases occurred mild or medium rash, nausea, headache, fatique, abdomina1 pain and diarrhea, and had blood transaminase increased, and leucocyte and hemoglobin decreased. Conclusions Didonosine, stavudine and nevirapine showed high anti-HIV and immune rebuilding effects in highly active antiretroviral therapy (HAART). The medicine regimen was well tolerated in a six-month clinical trial.

Objective To observe secretive ability of three C-C chemokines, macrophage inflammatory protein-1?(MIP-1?),MIP-1? and regulated-upon activation.normal T-cell expressed and secretory factor(RANTES)by lympocytes and moncytes of cord blood from chinese necnates and peripheral blood from chinese adults.Method Using Ficoll density-gradient centrifugation and gelatin-coated flask,the Iympocytes and monocytes were isolated from cord blood samples of seventeen Chinese neonates and peripheral blood samples of twenty Chinese adults.Purified lymphocytes and monocytes were incubated with PMA and LPS,respectively.Supernants were tested by ELISA for concentrations of MIP-1?,MIP-1? and RANTES.Results Concentrations of MIP-1?, MIP-1? and RANTES in cord blood lymphocyte and monocyte were (3920?730)pg/ml, (4910?590)pg/ml,(1470?410)pg/ml,(3240?980)pg/ml,(1960?1300)pg/ml and (240?120)pg/ml,respectively,and in adults peripheral blood were (6560?840)pg/ml,(5810?1150)pg/ml,(2250?570)pg/ml,(3010?1350) pg/ml,(2280?870)pg/ml,(690?430)pg/ml,respectively.Cord blood lymphocyte and monocyte showed diminished ability to secrete RANTES than adult peripheral blood.Conclusions The diminished RANTES secretion of cord blood lymphocyte and monocyte may be releted to the pathogenesis of Chinese neonatal HIV infection,and indicated some immunotherapies are feasible for preventing neonate from HIV mother-to-child transmission.
7.Risk factors for postoperative cognitive dysfunction after orthopedic surgery for adolescent idiopathic scoliosis

Objective To determine the risk factors for postoperative cognitive dysfunction (POCD) after orthopedic surgery for adolescent idiopathic scoliosis.Methods Sixty-five patients with adolescent idiopathic scoliosis,aged 11-18 yr,of American Society of Anesthesiologists physical status Ⅰ or Ⅱ,scheduled for elective orthopedic surgery with general anesthesia,were enrolled in this study.On admission to hospital (T0),before surgery (T1),at the end of surgery (T2) and on 2nd day after surgery (T3),blood samples were taken from the peripheral vein for determination of serum interleukin-10 (IL-10) and tumor necrosis factor-alpha (TNF-α) concentrations.Cognitive function was evaluated using a set of neuropsychological tests at 1 day before surgery and 7 days after surgery.Baseline patient characteristics such as age,gender,years of education and medical history,intraoperative fluid intake and output,surgery time,extubation time,postoperative visual analog scale score and related complications were recorded.Patients were divided into either POCD group or non-POCD group according to the diagnostic criteria of POCD.Logistic regression analysis was used to pick out the risk factors for POCD.Results Sixty-three patients completed this trial.The incidence of POCD was 24%.Compared with non-POCD group,the serum IL-10 concentration at T0 and serum TNF-α concentration at T3 were significantly increased,the IL-10/TNF-α ratio was decreased,and the length of postoperative hospital stay was prolonged in POCD group (P<0.05).The results of logistic regression analysis showed that increased concentrations of serum IL-10 at T0 and TNF-α at T3 were risk factors for POCD.Conclusion Increased serum concentrations of IL-10 on admission to hospital and TNF-α on 2nd after surgery are risk factors for POCD in the patients with adolescent idiopathic scoliosis.
8.Effect of auxiliary resistance forms on the resistance and marginal fitness of complete crowns for short molar preparations.

OBJECTIVETo evaluate the effect of different auxiliary resistance forms on the resistance and marginal fitness of complete crowns for short molar preparations.
METHODSA total of 70 Nissin resin teeth were prepared with 20° total occlusal convergence, 2.5 mm of occlusocervical height, and a shallow finish line on a milling machine. The milled preparations were then randomly assigned to 7 groups of 10. The first group was used as the control group. A total of 30 dies were modified by preparing interproximal grooves with angles of 0°, 6°, and 20° centered on the mesial and distal surfaces of the dies. The rest of the teeth were prepared with occlusal holes in the center of the occlusal surface milled with the same burs to form 0°, 6°, and 20° holes. Cobalt-chromium copings were fabricated for all specimens. The marginal gap of specific points on the axial surface was measured before and after cementation. The resistance of each specimen was evaluated by applying an external force at an angle of 45° to the long axis of the die by using a universal testing machine in a lingual to buccal direction. The maximum force applied before crown dislodgement was measured. Data were analyzed using the SAS 9.2 software.
RESULTSThe results showed that the 0° groove, 0° hole, and 6° hole were effective in improving the resistance of the complete crowns (P<0.05). The 0° groove, 6° groove, 0° hole, 6° hole, and 20° hole had significant difference with the control group in terms of marginal discrepancies (P<0.05).
CONCLUSIONAuxiliary resistance forms with less degree indicate greater resistance force but worse marginal fitness. In clinical practice, if the resistance of a preparation is enough, the auxiliary resistance forms should be avoided from being used.

9.Analysis of different endodontic sealers and strategies of root canal irrigation on the bond strength of fiber posts.

OBJECTIVETo evaluate the influence of eugenol-containing and resin-containing endodontic sealers on the bond strength of fiber posts using different strategies of root canal irrigation.
METHODSForty-eight mandibular premolars were endodontically treated. The specimens were randomly assigned into two groups according to different endodontic sealers. Group A used Endofil (eugenol-containing endodontic sealer), and group B used AH-plus (resin-containing endodontic sealer). After post space preparation, each group was randomly assigned into three subgroups according to the strategies of root canal irrigation (eight premolars in each subgroup). Group Al and B1: 0.9%NaCl irrigation; Group A2 and B2: 17% ethylene diamine tetraacetic acid (EDTA)+5.25%NaClO+0.9%NaCl irrigation; Group A3 and B3: ultrasonic agitation associated with 1 7%EDTA+5.25%NaClO+0.9%NaCl. One week after the cementation of fiber posts using RelyX™ Unicem, a push-out test was performed to measure the bond strength of the posts. The microstructure of the root canal surface was examined under scanning electron microscope (SEM).
RESULTSThe bond strengths of the six groups were as follows: Al (7.96±2.23) MPa, A2 (9.95±2.89) MPa, A3 (18.88±3.69) MPa, B1 (11.41±3.71) MPa, B2 (14.00±4.04) MPa, and B3 (19.14±3.27) MPa. Statistical analysis revealed a significant interaction between the different endodontic sealers and the strategies of root canal irrigation (P<0.05). Lower bond strength was found in group Al but not in group BI (P<0.05), and the same result was revealed when comparing group A2 and B2. No significant difference was observed between group A3 and B3 (P>0.05). SEM showed that the root canal in group A3 and B3 achieved the cleanest surface with nearly all dentine tubules opened.
CONCLUSIONThe eugenol-containing endodontic sealer can impair the bond strength of fiber posts compared with the resin-containing sealer when the root canal is irrigated by 0.9% NaCl or 17%EDTA+5.25%NaClO+0.9%NaC. No difference was observed between the two sealers when using 17%EDTA+5.25% NaCIO+0.9%NaCl combined with ultrasonic irrigation.

This is a retrospective study of 103 cases of early gastric cancer undergoing surgery during the years of 1974-1988 with a special discussion on surgical treatment.The lesions were localized to the mu-cosal layer in 54.3%,to submucosal layer in 45.7%,In 10% of patients there was lymphnode metastasis,all of them were in the first station.Operation consisted of radical subtotal gastrectomy in 94.2%.and total gastrectomy in 5.8%.The extent of lymphatic excision was:Ro in 12.6%,R1 in 61.2% and R2 in 26.2% Postoperative chemotherapy was given in 61.2%.However no statistical difference of 5 years survival rate was found in respect to the extent of lymphatic excision as well as postoperative chemotherapy.Since 60.2% of EGC lesions were of minute,multiple and plane type,preoperattve en-doscopy and intraoperative biopsy of gastric mucosa,if necessary,should be carefully done to ascertain that no lesion was overlooked in the remnant of the stomach.Follow-up rate was 96%,and the survival rates of 3 and 5 years were 97% and 93.7%.This makes the authors believe that a radical operation of R1 is justified and routine postoperative chemotherapy is unnecessary.
