Cystic neoplasms of the pancreas are rare lesions,usually without typical clinical manifestations.This article summarizes the clin-icopathological features and surgical treatment of common cystic neoplasms of the pancreas,including serous cystic neoplasm (SCN),mu-cinous cystic neoplasm (MCN),intraductal papillary mucinous neoplasm (IPMN),and solid pseudopapillary neoplasm (SPN).The clini-copathological features are independent prognostic factors for patients.More attention should be paid to correct diagnosis and appropriate sur-gical strategy for each cystic neoplasm of the pancreas.

5.Application of Realgar in Chinese Patent Medicine and Arsenic Dissolution

Objective To investigate the present status of Chinese patent medicine with realgar (As2S2) and to research arsenic dissolution in different pH values. Methods Data on the amount of Chinese patent medicine with realgar, the content of realgar, the method of taking medicine and so on were collected from the Chinese traditional medicine protection and criterions issued by the Chinese Ministry of Health in 2005, and evaluated the using status of Chinese patent medicine with realgar. The realgar powder was dissolved in solutions at different pH values (1, 3, 5, 7, 9, 11) in 37℃ water for 4 h, respectively, then arsenic was determined with cold trap hydride generation atomic absorption spectrometry to calculate the amount of dissolved arsenic. Results One hundred and twenty-one (3.13%) realgar medicaments were recorded in a total of 3 860 various Chinese patent medicaments, including 74 medicaments with both realgar and cinnabar (HgS) . Of all medicaments with realgar, 97 medicaments (80.17%) were for oral application, 10 medicaments (8.26%) were for external application, and 14 medicaments (11.57%) were for both oral and external application. 45 medicaments with realgar (1.17%) could be used in children; 108 medicaments with realgar (89.26%) contained arsenic 15%; The relative amount of arsenic in medicaments with realgar was from 0.46% to 27.52%. Daily intake of arsenic was 500 mg for 11 medicaments (9.09%). Only inorganic arsenic (iAs) was detected when 1 mg realgar was dissolved in solution with pH values of 1, 3, 5, 7, 9, 11. The levels of dissolved iAs seemed increased with the pH values, which were 1.58 ?g, 1.24 ?g, 1.57 ?g, 1.62 ?g, 2.28 ?g and 4.76 ?g, respectively. Conclusion Considering the common use of realgar in Chinese patent medicine and the high level of arsenic in realgar, the potential danger can not be ignored. It is possible that arsenic in realgar may be much easier to be absorbed in intestine than in stomach.
6.Dynamic variation of HCV hypervariable region gene and homology analysis

Objective To find out the dynamic variation on HCV genome to give the theoretic basis for diagnosis of post transfusion hepatitis C,and demonstrate the causality between donors and recipients suffering from post transfusion hepatitis C.Methods The high variable envelope gene fragments (HVR1 and HVR2) of HCV was selected for a cross sectional study and a one year follow up study was carried out on 3 HCV patients at every 3 month period to understand the dynamic variation of HCV HVR in each patient.Results The homology of HCV HVR genes among different types is 56.99%~60.88%,65.09%~68.80% among different subtypes,and 80.05%~96.40% among the same subtypes.It consists with the geographic distribution of HCV.The homology between the different strains isolated from the same patients is 92.10%~99.73%,95.36%~97.54 and 97.07%~99.20%,respectively.Conclusion The variation of different HCV strains in HVR is great.However,the variation of the same virus in the same patient during short infectious period is rather limited,which is no more than 7.9% per year.So the transmission pathway of some hepatitis C cases can be trailed at the molecular level by this way,and some non transfusion transmitted hepatitis C cases can be exclude
7.Pathogenic Bacteria Isolated from Subglottic Secretion in 43 Cases with Ventilator-associated Pneumonia: Their Distribution and Drug Resistance

CONCLUSIONS The pathogenic bacteria from VAP subglottic secretion are mostly Gram-negative rods, in which the dominant bacteria are A. baumannii, P. aeruginosa, S. maltophilia, K. pneumoniae and Escherichia coli. Most of them are multi-drug resistant. The kinds of pathogenic bacterial strains in subglottic secretion and lower respiratory tract secretion are not only consistent but also similar to each other in terms of composition and drug resistance.
8.Effect of heating on stored erythrocytes while stationary in an incubator

Objective To observe the effects of elevated temperatures(37~44.5℃)on the cell counts, hemolysis and osmotic fragility of stored red blood cells.Methods After 7~35 days of storage at 4℃, erythrocyte suspensions were heated by incubation at 37℃ and 44.5℃ for 30 min in an incubator and then examined for cell counts, hemolysis and osmotic fragility.Results Red blood cell, white blood cell and platelet counts, hemoglobin and potassium were unchanged following the heat treatment. In the osmotic fragility test, hemolysis remained within normal limits. Hemolysis of groups incubated at 37℃ and 44.5℃ began at (4.41?0.38)g/L and (4.3?0.2)g/L respectively while a complete hemolysis occurred at (3.42?0.36)g/L and (3.4?0.1)g/L , showing no significant difference compared to the room temperature control (4.5?0.18)g/L and (3.6?0.1)g/L. There was no significant distinction in the proportions of smooth spherocyte among the samples incubated at different temperatures, except the samples stored for 35 days, probably due to the long storage period.Conclusions Heating at temperatures between 37~44.5℃ for as long as 30 min does not cause hemolysis or other damage to red blood cells.
9.Structural characteristics of anatomical plates for the treatment of proximal humeral fractures

[Objective]To investigate the treatment effects of the anatomical plate on proximal humeral fractures and analyze the structural characteristics of the plate clinically.[Method]From June 2003 to June 2005,comminuted fractures of the proximal humerus of 32 patients(18 males and 14 females,with average age of 39 years)were treated with anatomical plates.Fractured proximal humerus was exposed by a standard deltopectoral approach after a dissection from the deltoid and pectoral muscle.Anatomical plates were placed on lateral sides of the humerus after reduction.The arm was immobilized in a sling,passive movement was begun on the first day after surgery,and active movement of the shoulder was started after three week.Active movement with resistance was allowed when there was radiological evidence of bone healing which usually occurred after six weeks.The patients were reviewed clinically and radiologically.[Result]All 32 patients were available at follow up with mean duration of 20 months (7~28 months).No complications such as nonunion of fractures,infection,the loosening and breakage of the plate were ever occurred.The clinical results were graded as excellent in 21 patients,good in 7,fair in 2,and poor in 2 according to the Neer's classification.The excellent and good rate was 87.5%.[Conclusion]Anatomical plate for the proximal humerus holds a 3-dimensional structure,which facilitates the fracture exposure,bone fragments reduction and fixation.It is concluded that rigid fixation of displaced fractures of the proximal humerus with an anatomical plate provides sufficient primary stability to allow early functional treatment.And is advocated that primary open reduction and rigid internal fixation using anatomical plates for the proximal humerus fracture.
10.Simple parathyroid adenomectomy through minimal incision for primary hyperparathyroidism caused by single adenoma

ObjectiveTo evaluate the result of direct parathyroid adenomectomy for the treatment of hyperparathyroidism caused by single parathyroid adenoma. MethodsClinical data of 13 cases were retrospectively analyzed from 1993 to 2000. Preoperative Bus, CT, 99mTc-MIBI localization was suggestive of a single adenoma and direct parathyroid adenomectomy through minimal incision was performed.ResultsHyperparathyroidism was cured in all cases. One case underwent second operation in which the pathology found malignancy, making the success rate of simple adenomectomy of 92.3%.Conclusions Simple parathyroid adenomectomy for primary hyper parathyroidism caused by single adenoma is effective?safe alternative to conventional neck exploration should preoperative diagnostic measures verify a single adenoma as the etiology of hyperparathyroidism.
