2.Different changes in glucose and lactate levels between brain extracellular fluid and plasma during hyperglycemia-hypoglycemia in rabbits

AIM: To study the difference in glucose and lactate levels between brain extracellular fluid (ECF) and plasma in rabbits in the different blood glucose levels. METHODS: Using intracerebral microdialysis technology, brain ECF levels of glucose and lactate were measured in every 10 min under basal conditions and during a hyperglycemia-hypoglycemia clamp study. RESULTS: Under basal condition, brain ECF glucose levels were markedly lower than ambient plasma levels (30% of plasma), whereas ECF lactate levels were substantially higher (165% of plasma). During the hyperglycemia-hypoglycemia clamps, the relationship between plasma and ECF levels of glucose remained similar, but changes in ECF glucose lagged about 30 min. There were no substantially changes in ECF levels of lactate during dynamical study. CONCLUSION: There are striking differences in glucose and lactate levels between brain ECF and plasma. Lactate may involve in the metabolic process of central nervous system.

4.Correlation study of carotid artery stenosis and the degree of homocysteine in ischemic stroke patients

Objective To study the relationship between the stenosis content of carotid artery and the degree of homocysteine (Hcy) by analyzing the level changes of Hcy in ischemic stroke patients.Methods One hundred and thirty patients of carotid artery stenosis with ischemic stroke were divided into four groups based on the degrees of carotid artery stenosis determined by ultrasound Doppler flow imaging:the slight stenosis group (32patients),the moderate group (35 patients),the severe group (33 patients) and the total occlusion group (30 patients).Meanwhile,30 healthy people were chosen as the control group.The levels of serum homocysteine (Hcy),total cholesterol (TC),triglyceride (TG) and low density lipoprotein (LDL) of each group were measured,and the results were compared with those of the control group.Results It was found that the serum Hcy level in ischemic stroke group were significantly higher than that in the control group (P<0.05),whereas the variations ofTC,TG,HDL and LDL levels between the ischemic stroke group and the control group could be neglected (P>0.05).An obviously increased order of serum Hcy levels was observed as the following,the control group,the slight stenosis group,the moderate group,the severe group as well as the total occlusion group.The difference between every two groups was statistically significant (P<0.05).The level of serum Hcy was positive correlated to the content of carotid artery stenosis (r=0.835,P<0.05).However,no correlation was found between the carotid artery stenosis and each level of TC,TG and LDL (r=0.031,0.012,0.018,respectively,P>0.05).Conclusions The serum Hcy level is closely related to the degree of carotid artery stenosis,and high level Hcy is suggested to be one of major risk factors for ischemic stroke.

7.Molecular epidemiological analysis of ECHO7 virus isolated from sewage water in Yunnan Province, China.

To investigate the epidemic and evolutionary trends of enterovirus (EV) in the external environment of Yunnan Province, China, molecular typing was performed on 4 EV strains that were isolated from environmental sewage in Yunnan. The VP1 region of isolates was amplified by RT-PCR using universal enterovirus primers, and the amplified VP1 region was sequenced for GenBank BLAST search and genotype analysis. The 4 EV strains were identified as ECHO7. Their nucleotide and amino acid homologies with the VP1 sequences of 68 ECHO7 strains retrieved from GenBank were measured by Mega software analysis. Our findings showed that ECHO7 strains from environmental sewage and population samples were in different evolutionary branches. These strains showed typical geographical and temporal differences; In addition, there were different transmission chains at the same time and in the same area. ECHO7 strains isolated from sewage water and patients with acute flaccid paralysis during the same period in Yunnan belonged to different clusters and evolved at different speeds. Special concerns are needed for this problem. Continuous molecular biological surveillance of human EV in the external environment of Yunnan will provide strong support for early warning of EV diseases.

8.Investigation of curative effect on renal cancer patients adopting thymopetidum combined with cytokine-induced killer cells in peroperative period

Objective To explore the change of cellular immune function in renal cancer patients adopting thymopetidum (TP-5) combined with cytokine-induced killer cells (CIK) in peroperative period,and evaluate its curative effect.Methods Forty-one cases of Ⅰ-Ⅲ stage renal cancer patients adopting CIK treatment postoperatively were enrolled in this study for retrospective analysis.According to applicating condition of TP-5,the patients were divided into two groups:16 cases of combination group (TP-5+CIK) and 25 cases of CIK group.The patients in combination group were given TP-5 intramscular injection everyday from preoperative 1 d to postoperative 3 weeks,once a day.Peripheral blood mononuclear cell was collected in the patients of two groups,which was cultivated for 2 weeks and then transfused.Fasting peripheral blood 1 week (before collected peripheral blood mononuclear cell),3 weeks (before CIK retransfusion),4 weeks(after CIK retransfusion for 1 week) and 5 weeks(after CIK retransfusion for 2 weeks) after operation were examined,the peripheral blood T cell subgroups (CD3+,CD4+,CD8+) and nature killer cell (NK cell) levels were detected using flow cytometry.Results At 1 week and 3 weeks (before CIK) after operation,the levels of CD3+,CD4+/CD8+ and NK cell in combination group were significantly higher than those in CIK group:1 week after operation:0.542 ± 0.063 vs.0.491 ± 0.054,0.94 ± 0.09 vs.0.90 ± 0.12,0.247 ± 0.025 vs.0.223 ± 0.033;3weeks after operation:0.641 ±0.058 vs.0.587 ±0.062,1.71 ±0.13 vs.1.02 ±0.07,0.319 ±0.038 vs.0.264 ± 0.047).There were significantly differences (P< 0.05).At 4 weeks after operation,the levels of CD3+,CD4+/CD8+ and NK cell in combination group were significantly higher than those in CIK group:0.698 ± 0.041vs.0.649 ± 0.050,2.01 ± 0.11 vs.1.64 ± 0.09,0.331 ± 0.029 vs.0.289 ± 0.034.There were significantly differences (P < 0.05).At 5 weeks after operation,the levels of CD3+,CD4+/CD8+ and NK cell in two grotups were no significant difference (P > 0.05).At 4 and 5 weeks after operation,the levels of CD3+,CD4+/CD8+ and NK cell in two groups were significantly higher than those of 3 weeks after operation (P < 0.05).None of patients occurred acratia,hyperpyrexia,shivering and so on.Conclusions The application of TP-5 treatment in peroperative period can promote the recovery of cellular immune function,which has synergistic effect with CIK.The value of TP-5 is worthy of promotion.
9.Importance of interventional neuroradiology curriculum in neurology standardizedresidency training

After more than3 years of pilot (since 2010), standardized training of resident physicians in Shanghai has made great progress, however, it still needs to be improved. Cerebral and spinal vascular diseases are main types of neurological diseases and they seriously influence people's life and health. Interventional neuroradiology plays an important role in the diagnosis and treatment of cerebral and spinal vascular diseases. However, in the current residency training system in Shanghai or other provinces of China, the training of interventional neuroradiology is not included. In this paper, the authors analyzed the important role of interventional neuroradiology in diagnosis and treatment of cerebral and spinal vascular disorders and discussed the feasibility, method, and time arrangement of including interventional neuroradiology curriculum into the schedule of neurology standardizedresidency training and hoped to provide reference for the relevant departments to formulate a future policy.
10.~(18)F-FDG PET/CT combining chest breath-hold spiral CT in the presurgical T staging of non-small cell lung cancer

Objective To assess the diagnostic value of ~(18)F-FDG PET/CT combining chest breath-hold spiral CT in preoperative primary tumor staging (T stage) of non-small lung cell cancer (NSCLC) before operation. Methods Ninety NSCLC patients underwent curative surgical resection after integrated ~(18)F-FDG PET/CT and chest breath-hold spiral CT examination. Two experienced radiologists blindedly staged all primary tumors in consensus by CT and PET/CT images. Surgical and histopathologic results served as the golden standard for determing the staging value of CT and PET/CT. T stage was assigned according to the seventh edition of TNM Classification of Malignant Tumors adopted by American Joint Committee on Cancer. Results According to pathological results, CT and PET/CT classified T stage accurately in 75.56% and 82.22%, respectively (P=0.03). CT and PET/CT accurately classified 3 and 6 patients respectively in 8 patients of central cancer with atelectasis, 2 and 4 metastases respectively in 5 patients at the ipsilateral lung. Conclusions Being able to describe the morphology, size and invasive region of primary tumor in NSCLC distinctly, CT is the main imaging method for T stage. ~(18)F-FDG PET/CT has unique value in differentiating center cancer with atelectasis and diagnosing pulmonary metastasis.
