2.Videothoracoscopy in the diagnosis and treatment of mediastinal tumors

Objective To explore the application of video-assisted thoracic surgery (VATS) in the diagnosis and treatment of mediastinal tumors. Methods Thirty-four patients with mediastinal tumors underwent VATS from June 1995 to December 2002. The patients comprised 9 men and 25 women, with a mean age 43 9 years (16~74 years). Entered into the study there were 14 cases of thymoma (9 benign thymomas and 5 malignant), 6 cases of neurogenic benign tumors, 5 bronchogenic cysts, 4 teratomas, 2 pericardial cysts and 3 other conditions (metastatic carcinoma, lymphoma and hemangioma racemosum, respectively). Maximum diameter of the tumors ranged 3 5 cm to 6 cm, mean 4 5 cm. Operations were performed under general anesthesia, double lumen endotracheal tube placement and single lung ventilation. The patients were maintained in the lateral decubitus position. Camera port was placed at the sixth or the seventh intercostals space on the midaxillary line. Sites of the other two ports depended upon the location of the tumors. Results Out of the 34 patients with mediastinal tumors, 25 patients underwent tumor removal by VATS alone and 6 patients received VATS combined with mini-thoracotomy, while the remaining 3 patients only underwent biopsy by VATS. Postoperatively, all patients recovered uneventfully, without severe complications or mortality. Conclusions VATS is the first choice for benign mediastinal tumors, typically those in the middle and posterior mediastinum.
3.Effect of early limb movement on motor function and the ability of daily life of stroke patients

Objective To investigate the effect of early limb movement on the motor function and the activities of daily life of stroke patients. Methods Eighty two stroke patients were divided into the control group(n=40)and intervention group(n=42). The former received conventional nursing care and the latter the intervention of early limb movement and routine nursing.The two groups were compared in terms of Fugl-Meyer movement function and Barthel index. Results After 12 weeks,the scores by Fugl-Meyer motor function and by Barthel index were significantly higher than those in the control group(all P<0.01).Conclusion The intervention by early limb movement is effective for promoting the recovery of limb motor function of stroke patients as well as their ability of daily life.
4.Inhibitory kinetics of amiloride on recombinant human protein kinase CK2 holoenzyme

AIM To study the direct effect of amiloride on recombinant human protein kinase CK2 holoenzyme and its kinetics. METHODS Recombinant human protein kinase CK2 ? and ? subunits were cloned,expressed and purified by gene engineering. The two subunits were mixed at the same molar ratio to form CK2 holoenzyme,which displayed the maximum biological activity. CK2 activity was assayed by detecting incorporation of 32 P of [? 32 P]GTP into the substrate in various conditions. RESULTS Amiloride inhibited the holoenzyme activity of recombinant human protein kinase CK2 with an IC 50 of 257 57 ?mol?L -1 . Kinetic studies of amiloride on recombinant human CK2 showed that the inhibition was competitive with GTP with the K i of 236 30 ?mol?L -1 ,and noncompetitive with casein with the K i of 163 63 ?mol?L -1 . CONCLUSION Amiloride inhibits the activity of cAMP or ATP dependent enzymes as well as the kinase with GTP as phosphate donor,and the recombinant human protein kinase CK2 may be used as a molecular target for simple screening and development of effective inhibitors of CK2.
5.Inhibitory Effects of Propofol on PC12 Cells Injury Induced by Glutamic Acid via MAPK/ERK Signal Pathway

OBJECTIVE:To study inhibitory effects of propofol on PC12 cells injury induced by glutamic acid via mitogen-acti-vated protein kinase/extra-cellular regulated kinase (MAPK/ERK) signal pathway. METHODS:PC12 cells were randomized into normal control group,model group(10 mmol/L glutamic acid),propofol low-concentrations,medium-concentrations and high-con-centrations groups(12.5,25,50 μmol/L+10 mmol/L glutamic acid). The optical density of cells,cell apoptosis,the phosphoryla-tion of ERK1/2 and the expression of c-fos,Bax,Bcl-2 were detected after treated with relevant medicine for 48 h. RESULTS:Compared with normal control group,optical density of cells,the phosphorylation of ERK1/2 and Bcl-2 decreased in model group (P<0.01);apoptotic rate,the expression of c-fos and Bax increased (P<0.01). Compared with model group,optical density of cells,the expression of Bcl-2 and the phosphorylation of ERK1/2 increased in propofol group (P<0.01);apoptosis rate,the ex-pression of c-fos and Bax decreased (P<0.05 or P<0.01). There were statistical significant between the different concentrations (P<0.01). CONCLUSIONS:Propofol can inhibit the apoptosis of PC12 cells induced by glutamic acid,which is associated with the up-regulation of ERK1/2 phosphorylation.
7.Expression of inducible co-stimulator in peripheral blood T lymphocytes in the patients with systemic lupus erythematosus.

The expression of inducible co-stimulator (ICOS) in peripheral blood T lymphocytes from the patients with systemic lupus erythematosus (SLE) and the role in the pathogenesis of SLE was investigated. By using two-color immunofluorescent staining and flow cytometric assay, the expression levels of ICOS in peripheal blood T lymphocytes from 33 patients with SLE and 16 healthy volunteers were detected. SLE diseases activity index (SLEDAI) of the patients with SLE was used to evaluate the disease activity. The correlation between the ICOS expression and SLEDAI was analyzed among the groups. The results showed that the expression levels of ICOS in T lymphocytes in active SLE group was markedly higher than those in the control and inactive SLE groups (both P< 0.01). There was no significant difference in the expression levels of ICOS between the inactive SLE and the control groups (P>0.05). In active SLE and inactive SLE groups, positive linear correlation was found between the levels of the ICOS expression in T lymphocytes and SLEDAI (r=0. 711, P=0.001; r=0.561, P=0.03). It was suggested that the expression of ICOS in peripheral blood T lymphocytes from the patients with active SLE was up-regulated and and ICOS might be related to the pathogenesis of SLE.

8.Expression of CD23 and CD19 on peripheral blood lymphocytes and its association with sreum total IgE in patients with allergic rhinitis

Objective:To explore the expression of CD23,CD19 on peripheral blood lymphocytes as well as its association with serum total IgE levels and nasal allergic symptoms in patients with allergic rhinitis(AR). Method:Symptom scores were evaluated in 46 AR patients,expression of CD23,CD19 on peripheral blood lymphocytes were measured by flow cytometry,and serum total IgE levels were determined by immune chemiluminescence. Thirty two healthy individuals were enrolled as controls.Result:The percentage of CD23~+,CD19~+ and CD23~+/CD19~+,on peripheral blood lymphocytes in AR patients were 11.6±1.9,22.8±3.3,10.2±1.7,respectively.Higher frequencies of CD23~+,CD19~+,and CD23~+/CD19~+ were found in AR patients compared with controls (P<0.05).There were positive correlations between expression rates of CD23~+,CD19~+,CD23~+/CD19~+ and levels of serum total IgE,nasal allergi symptom scores,respectively.CD23~+ /CD19~+ demonstrated greater correlations with serum total IgE and nasal allergic symptom(r=0.65 and 0.49,P<0.05)than CD23~+ and CD19~+ did.Correlation between CD23~+/CD19~+ and nasal allergic symptom scores was greater than the corresponding correlations of serum total IgE(r=0.33,P<0.05).Conclusion:CD23 and CD19 are important factors that associated with serum total IgE in the pathogenesis of AR,Analysis on the expression of CD23~+/CD19~+ on peripheral blood lymphocytes is helpful for evaluating the severity of AR.
9.Analysis of Serum Inflammation Related Factors of Influenza A (H1N1) Patients with Epidemic Febrile Damp-heat Syndrome and Warm-heat Syndrome

Objective To analyze the serum inflammation related factors of influenza A ( induced by sub-type H1N1 virus) patients with epidemic febrile damp-heat syndrome and warm-heat syndrome. Results Fifty-six patients confirmed as influenza A ( H1N1) were differentiated into epidemic febrile damp-heat syndrome ( 36 cases) and epidemic febrile warm-heat syndrome (20 cases) . And then we observed the serum immune globulins IgM and IgG, interleukin-8 (IL-8), tumor necrosis factor alpha (TNF-ɑ), tumor growth factor beta 1 (TGF-β1), lipolysaccharide binding protein (LBP), Toll-like receptor 4 (TLR4) and soluble CD14 (sCD14) levels of the influenza A patients. Results Insignificant differences of serum IgM, IgG, IL-8, TNF-ɑand TGF-β1 levels were shown between epidemic febrile damp-heat syndrome patients and epidemic febrile warm-heat syndrome patients (P>0.05), but the differences of LBP, TLR4 and sCD14 were significant (P<0.05) .Conclusion Influenza A (H1N1) patients with epidemic febrile damp-heat syndrome have higher serum LBP, TLR4 and sCD14 levels than patients with epidemic febrile warm-heat syndrome, indicating that inflammatory reaction is severer in influenza A (H1N1) patients with epidemic febrile damp-heat syndrome.
10.Study of the introduction of the student as standard patient in the clinical teaching of endocrinology

Student as standard patient(SSP) was defined as student receiving relative medical training,who was willing to act as a human analogue as a patient and could be used as not only a patient but also a teacher in clinic practice.The results indicates that the using of SSP in the clinical teaching of endocrinology could promote the students'level in practice and in the meanwhile,it could be useful to testing the learning result.
