1.Otoacoustic Emissions and Auditory Brainstem Response in High Risk Neonatal Hearing Screening

Objective To investigate the character 、difference and clinical meaning of distortion product otoacoustic emissions(DPOAE) and auditory brainstem respons(ABR) in high risk neonates with.Methods DPOAE and ABR were recorded with amplaid MK 22 and GSI60 in 200(400ears) high risk neonates, then compared the results.Results Of 200(400ears) newborns, the presence of DPOAE was 64%(256/400 ears), the presence of ABR was 88.25%(353/400 ears). The co-negativity and co-positivity of DPOAE and ABR were 61.50%(246/400 ears)and 9.25%(37/400 ears). The false negative and false positive of DPOAE were 3.91%(10/256 ears)and 74.31%(107/144 ears).Conclusion The DPOAE test is simple,quick,harmless,sensitive and objective. Because it only reflects the function of the cochlea and has high false positive, we can't make the diagnosis of hearing loss immediately when newborns don't pass the DPOAE test.The ABR test is a reliable method for neonatal hearing screening. The combination of DPOAE and ABR in high risk neonatal hearing screening could increase the accuracy and reliability of the test and retest results.
2.Experimental study on the effect of intravitreal nonsteroid antiinflamatory drug on the structure and function of retina

Objective To investigate the retinal toxicity and verify the safe dose of intravitreal injection of nonsteroid anti inflamatory drug, diclofenac sodium. Methods Twenty eight healthy adult white rabbits were divided at random into 7 groups and received in every right eye the intravitreal injection of a single dose of diclofenac sodium solution ranging from 0.4~1.0 mg/0.1ml respectively,the left eyes were regarded as control ones. Before injection and on the 1st, 3rd, 7th, 14th, 21st, and 28th day after injection the electroretinography on both eyes was examined. On the 28th day after injection the retinas of two rabbits of every group were examined by using light microscopy. On the 10th and 30th day after injection the retinal tissues around the optic nerve disk of two eyes from every group at random were tested by using transmission electron microscopy. Results The ratio of amplitude of b wave of electroretinography in 0.4 mg and 0 5 mg groups had no significant difference from groups before injection,the retinal tissues showed no structural changes in light and electron microscopy examination. 3.The changes of multifocal electroretinogram of the affected and fellow eyes of the patients with central serous chorioretinopathy

ObjectiveTo evaluate the retinal function with central serous chorioretinopathy (CSC) and the fellow eyes (mf-ERG).MethodsTwenty patients (40 eyes) with unilateral CSC, diagnosed by fluorescein angiography, were examined on each eye of the patients by mf-ERG multifocal electroretinography. The patients aged from 31 to 53 (13 men, 7 women; mean age, 41 years), and the CSC eyes had visual acuities of 0.2 to 0.8. None of the patients had significant abnormality in the fellow eyes with visual acuities of 0.8 to 1.5. Twenty-four healthy people (29 normal eyes with visual acuities of 1.0 and more and without any retinal diseases) were studied simultaneously as the normal control. The stimulus matrix of 61 hexagonal elements were grouped into 5 concentric rings from the fovea outward to 30? of the degree of visual field. The first-order responses of mf-ERG were recorded. The first negative wave (N 1-wave) and positive wave (P-wave) of the mf-ERG were measured. Amplitude intensities and latechies for the two configurations were calculated and compared.ResultsCompared with the control eyes, the responses were not only depressed severely in P 1- and N 1-wave amplitude intensities in the ring 1~3 in the CSC eyes with CSC (P0.05). There were no significant differences in N 1- and P 1-wave latencies in 5 concentric rings among the control eyes, the fellow eyes or the CSC eyes.ConclusionsCSC is a kind of macular disease involving both eyes. It may cause the decrease of retinal function in macular area of CSC eyes and also influence the retinal function in macular area of the fellow eyes. mf-ERG is a sensitive way in examining visual function.
4.Judging the improvement of medical safety control from hospital appraisal standards

In light of the medical safety control standards for general hospital appraisal in Zhejiang and the principles followed in designing the standards, the authors put forward some suggestions for medical institutions to improve their medical safety control. ①Medical safety occupies a key position in hospital appraisal standards. Some points are presented for discussion with regard to the criteria for malpractice-caused deduction of marks. ②The focal point of medical safety control lies in prevention. Medical institutions and their staff ought to have a correct understanding of malpractice and try to remove hidden dangers. The directive function of plans for preventing and handling malpractice ought to be brought into full play. In addition, nationally uniform and comprehensive criteria and routines for diagnostic, treatment and nursing procedures ought to be set up speedily. ③ It is imperative to make use of advanced means so as to ensure the smooth, timely and efficient reporting and feedback of medical safety information. ④The idea of service ought to be reinforced and a risk assessment mechanism ought to be set up.
5.Expression and significance of CRKL in thyroid papillary micro-carcinoma

Objective To investigate the expression of CRKL in thyroid papillary micro-carcinoma and its clinical significance.Methods 120 patients with thyroid tissue specimens were collected,in which 30 cases of diam-eter >1 cm of papillary thyroid carcinoma,30 cases of thyroid papillary micro-carcinoma,30 cases of nodular goiter, 30 cases for specimen of thyroid disease patients without diabetes.Immunohistochemical(SP)method was used to de-tect samples in CRKL expression.Results In thyroid papillary micro-carcinoma and thyroid papillary cancer group, CRKL expression positive rates were 30.12% and 29.87% respectively,which were higher than that of nodular goiter and normal thyroid group 30.03% and 28.57%(χ2 =52.102,P <0.05);The average absorbance(A)value in thy-roid papillary micro-carcinoma and thyroid papillary carcinoma group which were respectively (0.516 ±0.100)and (0.496 ±0.201),were higher than that in nodular goiter and normal thyroid group (0.246 ±0.050)and (0.117 ±0.015),the difference was statistically significant(F =149.105,P <0.05).Conclusion CRKL is highly expressed in papillary thyroid micro-carcinoma and the clinical detection of CRKL is helpful to determine the surgical plan for papillary thyroid micro-carcinoma.

As the development of robotic surgery in urology, the simulation training platform is playing an important role in the training of urologists on robotic surgery training.Currently, the available training simulators for urological robotic surgery are composed of virtual simulators such as VR and AR, and physical (mechanical) simulators such as dry-lab and wet-lab.The reality and validity of various simulation training platforms have been validated in different degree by many researches, which demonstrated the useful and helpful effect on the training and improving of robotic surgery skills.However, we need further study to develop more useful and precise training modules, to integrate advantages of different simulators, and to establish more reasonable learning curve.
8.Development of Chinese and Western Ethics and Relevant Ethical Review Construction

Through the analysis on different development paths of western and Chinese medical ethics, this article proposed to construct the Chinese medical ethical review system. Although the medical ethical review system was originated in western countries, due to different origins of medical ethical thoughts, different cultures and beliefs among races, as well as different medical and science development models, the application of western medical eth-ical review system should not be mechanically used in China. The application should follow the idea of har-monization with differences. This article discussed on current status and significance of the construction of an ethical review platform which is suitable to the national conditions of China. It was also emphasized on the im-portance of ethics training among researchers.
