1.Value of gastroendoscopy check-up early after non-surgical treatment of patients with gastroduodenal perforation

Objective To explore the value of gastroendoscopy check-up early after non-surgical treatment of patients with gastroduodenal perfolation.Methods The patients suspected of perforated gastroduodenal ulcer on hospital admission underwent non-surgical treatment were enrolled.After cured clinically,performed gastroendoscopy at different time periods.Results Among 133 patients underwent gastrointestinal endoscopy,129 cases(97.0%)were diagnosed gastroduodenal ulcer,3 cases(2.3%)were diagnosed gastric carcinoma,1 case (0.8%)Was confirmed duodenal diverticulum,no compilcations occurred due to endoscopy.The 6 cases that had surgical indications had be implemented the early surgical treatment. Conclusions After non-surgical treatment of patient with gastroduodenal perforation,timely gsstroendoscopy can truly show pathologic changes with gastroduodenal perforation.So endoscopy is beneficial under condition appropriate time seleetion for confirmed the eliology of PGD,treatment promptly targeted etiology and understanding the mechanism healing the perforation as non-surgical treatment.
2.Characteristics and resistance patterns of pathogens for nosocomial pneumonia in patients with central nervous system disease.

Objective To investigate the pathogenesis, distribution of the pathogens and resistance pattern of nosocomial pneumonia in patients with diseases of central nervous system(CNS).Methods Bacterial typing was conducted on the 59 strains separated from the sputum of 59 patients of nosocomial pneumonia with CNS diseases in our hospital in 2002.MIC drug sensitive test in vitro was conducted with the most common 15 antibiotics.Results The most common pathogens of nosocomial pneumonia in the diseases of CNS were:Staphylococcus aureus 23.7%,Pseudomonas aeruginosa 22.0%,Acinetobacter baumannii 18.6%,Klebsiella pneumoniae 8.5% Xanthomonas maltophilia 6.8%?According to the drug sensitive test,antimicrobial-resistant gram-negative bacilli were increasing.None of the vancomycin-resistant MRSA variant was seen.Conclusion The risk factors of nosocomial pneumonia in the diseases of CNS are long hospital stay,respiratory tract invasive operation and unreasonable antibiotic therapy.The most common pathogens were Staphylococcus aureus,Pseudomonas aeruginosa,Acinetobacter baumannii,Klebsiella pneumoniae and Xanthomonas maltophilia.Effective and infection-control procedures,surveillance systems and appropriate antimicrobial selection are key methods in limiting nosocomial pneumonia and antimicrobial-resistant pathogen occurrence.
4.(18)~F-deoxyglucose-hPET in the detection of recurrence in non-small-cell lung cancer

Purpose:To evaluate ~(18)F-deoxyglucose dual-head tomography with hPET in the detection of suspected recurrence in non-small cell lung cancer, as compared with CT/MRI imaging. Methods:21 patients with clinically suspected recurrences in non-small-cell lung cancer underwent ~(18)F-FDG hPET and CT/MRI imaging. The final diagnosis of recurrence was proved by pathology or clinical following-up. Results:The ~(18)F-FDG hPET sensitivity, specificity,accuracy,postive predictive value in defining local recurrence were 94.1%,75%,90.5 %,94.1%,respectively ; and those of CT/MRI were 60%,66.7%,61.9%,81.8%, respectively. Conclusions:In comparison with CT/MRI,~(18)F-FDG hPET possesses a higher sensitivity、specificity and accuracy in detecting recurrence in non-small-cell lung cancer.~(18)FDG hPET is quite valuable methd to define the tanget in the radiotherapy of recunent non-small-cell lung cancer,and is less expensive than PET.It is thus recommended for clinical use.
5.Role of the Arthroscopy in Diagnosis and Treatment of Osteoarthritis of the Knee Joint

Objective To evaluate the role of arthroscopy in diagnosis and treatment of osteoarthritis of the knee.Methods The data of 108 patients (118 knees) with osteoarthritis of the knee and received arthroscopic surgery were analyzed retrospectively including sign, X-ray and MRI examinations of knee, the average stage in hospital, diagnoses before and after operation, and prognosis.Results Arthroscopic surgery was of benefit to diagnosis and treatment of osteoarthritis of knee. The average stage in hospital was 11±7 days. The following-up of 98 patients showed that 82 patients were excellent, 16 patients good and only 1 patients not up to standard.Conclusion Arthroscopic surgery has following advantages such as less invasive surgery, shorter stage in hospital, good curative effect in the near future.
6.Comparision and application of detective methods for Her-2 gene in invasive breast carcinoma tissue

Objective Through comparision of Her-2 gene in invasive breast carcinoma tissues assayed by IHC, CISH and FISH respectively, to explore an optimal detective methods. MethodsIHC, CISH and FISH were used in detection for Her-2 gene of 100 cases, respectively.ResultsCISH identified no Her-2 gene amplification in IHC score negative rumors (6/6) and gene amplication in 7.1%(1/14) of IHC score (+) and in 32 % of IHC score (++) tumors(l6/50) and in 80 % of IHC score (+++) (24/80), Concordant rate between CISH and FISH was 100 %(20/20). ConclusionThere are advantage and disadvantage among IHC,CISH and FISH. CISH is an accurate and dependable methods and would be easy to apply for detection of Her-2 gene.
7.Progress of diagnosis, treatment and prognosis of four special kinds of gastric cancer

8.Clinicopathologic features of 112 patients with mantle cell lymphoma

Objective:To explore the clinicopathologic features of 112 patients with mantle cell lymphoma (MCL). Methods:Da-ta from 112 MCL cases were collected, and immunohistochemical assay was conducted. A break in the CCND1 gene was detected by fluorescence in situ hybridization (FISH). The t-test was used in the statistical analysis. Results:All tumor cells in the 112 cases ex-pressed B cell-related antigen, including 1 blastoid subtype and 1 polymorphic subtype. Among all the cases, 106 expressed CD5 and 104 expressed cyclinD1. A break in the CCND1 gene was not found in 3 cases with CD5-MCL. IgH/CCND1 polyploid was found in 2 classical cases. Conclusion:MCL is a type of special immunophenotypic B-cell lymphoma. The prognoses of blastoid and polymorphic subtypes are poor. Special subtypes should be classified during diagnosis.
