1.Characteristics of 42 cases of incidental discovery of pheochromocytoma

We have retrospectively analyzed the medical files of 86 consecutive patients who were diagnosed as cases of adrenal or extra-adrenal pheochromocytoma at Changhai hospital between 2001 and 2009. Patients with incidentally detected pheochromocytoma often had lower prevalence of hypertension and lower mean arterial blood pressure compared with those patients suspected of pheochromocytoma on clinical grounds.
2.PRELIMINARY REPORT ON THE NUTRITIONAL TREATMENT OF CHRONIC UREMIC PATIENTS

In non-dialytic composite treatment of chronic renal failure, wheat (or corn) starch should be used for low protein (20-40 g/day) diet in substituting grains, to increase high biological value protein intake (about 50-70% of total protein intake). This principle has been used in 30 Patients (33 times) for 1-2 monthes. Daily total calorie intake should be maintained within 2000-3000 kcal. According to the status of the patient, EAA-TR2 (13.8 g/day I.V.) and other supporting measurements may be used for most patients. In all the patients, except six (2 cases useless, observation of 4 cases discontinued), uremic manifestations were ameliorated, nutritional status was improved and azotemia markedly alleviated, life span may be prolonged. According to nitrogen balance studied, in 6 cases it is indicated that nitrogen balance can be maintained if protein intake is not less than 0.5 g/kg/day (with sufficient calorie intake) for chronic renal failure patients (Cr clearance 5-10 ml/min).
3.Topical cell growth factors for wound healing after fractional CO2 laser therapy: an experimental study

ObjectiveTo compare the accelerating effect of topical recombinant human epidermal growth factor (rhEGF) and recombinant bovine basic fibroblast growth factor (rb-bFGF) on wound healing after fractional CO2 laser therapy.MethodsTwenty male guinea pigs were included in this study.After hair removal and irradiation with fractional CO2 laser,the back of each guinea pig was divided into 4 regions to be topically treated with rhEGF of 10 μg/cm2 (rhEGF group),rb-bFGF of 262.51 IU/cm2 (rb-bFGF group),the combination of rhEGF and rb-bFGF (combination group),or normal saline (control group),twice daily until the healing of wound.Skin physiology parameters including elasticity index and melanin index were detected before the irradiation,7,14 and 28 days after the irradiation,and compared between the 4 groups by analysis of variance.Tissue specimens were obtained from 4 mice at the above time points and subjected to pathological examination for the observation of collagen fibers and quantification of fibroblasts.ResultsAfter fractional CO2 laser therapy,the crusts fall off completely in growth factor-treated regions,while partly in the control regions,within 3 to 7 days; the wounds healed completely in 14 to 28 days in all the groups,with the regenerating tissue being more tender and redder compared with the surrounding unirradiated tissue.The wound surface was smaller in area and redder in color in the 3 growth factor-treated groups than in the control group.At 28 days after the irradiation,the elasticity index was 262.29 ± 62.40 in the combination group,202.00 ± 65.62 in the rhEGF group,188.86 ± 35.02 in the rb-bFGF group,167.14 ± 42.49 in the control group.Statistical difference was observed in elasticity index,but not in skin melanin index among the 4 groups.Pathological examination showed a dense and organized arrangement of collagen fibers in the combination group but a sparse and disorganized arrangement of collagen fibers in the control group.ConclusionThe combined application of rhEGF and rbbFGF can accelerate the healing of wound and increase the elasticity of regenerating tissue after fractional CO2 laser therapy.
4.Pedicle screw system in the treatment of unstable fractures of thoracolumbar

Objective To discuss pedicle screw system in the treatment of unstable thoracolumbar fraetures of the results of surgical treatment.Methods Pedicle screw system for thoracolumbar unstable fracture of 46 cases.Results 39 cases were followed up for an average of 17 months after the intervertebral space height and thoracolumbar physiological curvature satisfactory recovery.The average Cobb angle and the average spinal stenosis index has been markedly improved,Frankel raise a grade:three.Conclusion Posterior short section of the pediele screw system is the treatment of thoracic and lumbar fractures of the effective ways is satisfied with the reduction,reliable fixation.
5.Relationship between Surface EMG and Angle of Elbow Joint

Objective To explore the correlation between the moving angle of elbow joint and surface electromyogram signal(sEMG)activities of biceps and triceps.Methods The eigenvalue of sEMG during elbow joint moving under constant load were calculated with means of root mean square(RMS)and integrated electromyogram(iEMG)in time domain.Results The experimental results showed that both the chavacteristics of RMS and iEMG for biceps differed from those of triceps.The sEMG activities of biceps increased with the increment of elbow joint angle,while those from triceps reached to the maximum values when elbow joint angle was 90?.Furthermore,the sEMG activities of triceps at the elbow angle of 45? was similar with that when elbow angle was 135?.Conclusion The sEMG from biceps and triceps can reflect the moving angle of elbow joint under constant load.
6.Status quo of medical application software for chronic diseases management and its prospect

Objective To analyze and assess the major functions of medical APP by investigating its application in self management of chronic diseases. Methods The 3 major functions of APP were analyzed, including data record and condition assessment, medical consultation, and life therapy. Results The medical APP possesses its basic functions for chronic diseases management. However, it lacks of practicality and convenience and its homogenization is severe. Conclusion The APP +hardware +cloud medical management model should be promoted by making use of the Internet + medical technology, the supervision of mobile medical APP should be strengthened, the mobile information management mechanisms of chronic diseases should be perfected, and the self management efficiency of patients with chronic diseases should be improved.
7.Molecular mechanism of dexamethasone affecting podocyte motility

Objective To investigate the potential roles of dexamethasone (Dex) in podocyte motility,and to explore the mechanism of modulating α-actinin-4,nephrin.Methods Podocytes were divided into three groups:Dex group [1 μmol/L Dex +50 mg/L puromycin aminonucleoside (PAN)],PAN group (50 mg/L) and normal control group.Scrape wound assay and Transwell migration assay were used to detect cell motility.Filtering ratio of podocytes was measured by FITC labeled BSA.Real-time PCR and Western blotting were used to examine the expressions of c-actinin-4 and nephrin.Results From the scrape wound assays,the ability of wound repair in podocytes of PAN group was significantly increased (P<0.01),and the number of migrating cells in this group also rose (P<0.01).Compared to PAN group,podocytes in Dex group did not enhance the motility after treatment with the same dose PAN (P<0.01).Real-time PCR and Western blotting showed that Dex could significantly inhibit the up-regulated expression of α-actinin-4 and nephrin induced by PAN.Conclusions Dex can relieve the enhanced motility induced by PAN.Its mechanism may be associated with the modulation of the expressions of α-actinin-4 and nephrin.
8.Diagnostic and therapeutic strategy of traumatic pulmonary pseudocyst

Objective To evaluate the diagnostic and therapeutic strategy of traumatic pulmonary pseudocyst (TPP).Methods Fifteen patients who were diagnosed and treated as TPP between January 2000 and November 2011 were studied retrospectively.Results Nonpenetrating chest trauma was the underlying cause in all cases.A typical sign shown on chest radiograph was a thin-walled cavitary lesion in 9 patients,6 patients accompanied by traumatic wet lung,with or without an air-fluid level.Serial radiological images of CT showed high resolution of the above lesions.Single TPP lesion occurred in 9 patients,and multiple TPP lesions in 6 patients.The size of the lesions was 5 -75 (32 ± 17) mm.The pseudocyst was located in the left lung in 5 patients(33%),located in the right lung in 7 patients (47%),located in bilateral lung in 3 patients (20%).All TPP patients were treated conservatively with no occurrence of complications.Conclusions TPP is an uncommon benign lesion secondary to thoracic trauma.CT scan is an optimal option for diagnosis and evaluation of TPP.Uncomplicated cases can take conservative treatment.For complicated patients,theraneutic strategy should be made individually.
9.Associations of blood pressure level with carotid intima-media thickness and plaque among middle-aged and elderly Chinese hypertensives.

OBJECTIVETo analyze the associations of blood pressure (BP) level with carotid intima-media thickness (CIMT) and plaque in a middle-aged and elderly hypertensive population.
METHODSBy block randomized sampling, an epidemiological investigation was conducted among hypertensives aged 45 - 75 years in the northern rural area of Jiangsu Province. Blood pressure was measured, and carotid CIMT and plaque were determined using a coloured ultrasonograph. The relationship between blood pressure level and CIMT or plaque was analyzed using a multivariable linear regression or logistic regression model.
RESULTSA total of 524 subjects (male 242) were enrolled. CIMT and plaque prevalence were (0.7 ± 0.1) mm and 46.3% in males and (0.7 ± 0.1) mm and 34.0% in females. After adjustment for relevant variables, CIMT increased 0.001 12 mm with 1 mmHg systolic BP increase in males (P < 0.001) but this trend was not significant in females. The risk of plaque number > 1 was significantly higher in those with grade III hypertension compared to grade I hypertensives (OR: 2.136, 95%CI: 1.138 - 4.012, P = 0.018).
CONCLUSIONIn this middle-aged and elderly hypertensive population, carotid CIMT is positively in relation to systolic BP, especially for male patients. Higher BP is associated with higher risk of plaque occurrence. Hypertension is thus an independent risk factor for the formation of atherosclerosis.

10.Ischemic preconditioning induced functional opening of myocardial capillary beds

AIM:To explore the protective effect of ischemic preconditioning on myocardial capillary bed. METHODS: Anesthetized open-chest dogs in ischemic preconditioning (IP) group ( n =6) were subjected to 5 min ischemia and 5 min reperfusion for 4 times in left descending coronary artery (LDA) while the dogs in sham-operated (SO) group ( n =6) were observed for 40 min without any stimulating.
