1.Detection of Trichomonas Vaginalis Infection in Male Patients With Non-gonococcal Urethritis by InPouch TV Culture and Polymerase Chain Reaction

Objectives To study the incidence of Trichomonas vaginalis(TV)infection in Chinese male patients with nongonococcal urethritis(NGU),to evaluate the sensitivity and specificity of urine-based and urethral swab polymerase chain reaction(PCR)detection,and to set up a method of non-invasive detection for male TV infection.Methods One hundred and five male NGU patients were collected from the STD clinic.Two urethral swabs were obtained from each patient,one for InPouch TV culture system,the other for PCR;one urine specimen(first-void urine or first-catch urine)was also collected for PCR detection.Culture was considered as the"gold standard".Compared with the culture,the sensitivity,specificity,positive predictive value(PPV)and negative predictive value(NPV)of two PCR detection were calculated.Results The positive rate of InPouch TV system detection was4.76%(5/105);the positive rate of urine-based PCR and urethral swab PCR detection was3.81%(4/105)and4.76%(5/105),respectively.In comparison with the culture,the sensitivity,specificity,PPV and NPV were80%,100%,100%and99%for urine-based PCR and80%,99%,80%and99%for urethral swab PCR.Conclusions TV is one of the etiological agent for male NGU,the positive rate of TV is4.76%in our study.The urine-based PCR detection has a higher sensitivity and specificity and provides a noninvasive method which is feasible in practice.

3.Correlation between serum 25-hydroxyvitamin D level and gestational diabetes mellitus

Objective To investigate the relationship between serum 25-hydroxyvitamin D level and gestational diabetes mellitus (GDM).Methods The fasting plasma glucose,fasting insulin,25-hydroxyvitamin D,and high sensitive C-reactive protein (hs-CRP) levels in the normal glucose tolerance group and gestational diabetes mellitus (GDM) group were measured.The homostasis model assessment insulin resistant (HOMA-IR) and homostasis model assessment β cell (HOMA-B) were calculated for assessing the insulin resistance and β-cell function.The relationship between 25-hydmxyvitamin D and hs-CRP,HOMA-IR,and HOMA-B were analyzed.Results The level of 25-hydroxyvitamin D in the GDM group [(2.16 ± 0.38) nmol/L] was significantly lower than that in the normal glucose tolerance group [(4.13 ±0.79) nmol/L](P =0.001).Serum 25-hydroxyvitamin D level was negatively correlated with hs-CRP (r =-0.448,P =0.002) and HOMA-IR (r =0.396,P =0.004),but was positively related with HOMA-B (r =0.495,P =0.001).Conclusions The serum 25-hydmxyvitamin D level remarkably decreases in GDM patients.Vitamin D deficiency may cause the functional damage of beta cell of islet,increase insulin resistance,and thus contribute to the development of GDM.
5.Research progress on radiation-induced osteosarcoma

The extensive application of radiotherapy and the long survival of patients with malignant tumors have led to an in-creased incidence rate of radiation-induced osteosarcoma (RIOS), one of the most critical radiation-induced complications. Compared with primary osteosarcoma, RIOS has higher grade and poorer prognosis, which reduces the survival of patients with cancers. However, RIOS has not been fully understood in the clinical setting. This paper summarizes the latest progress at home and abroad, focusing on the incidence rate, risk factor, latency, and diagnostic criteria of RIOS. The molecular mechanism of RIOS is associated with the muta-tion of p53, the activation of the Wnt/β-catenin signaling pathway, and the superposition of multiple alleles. Furthermore, we discuss the differences in clinical characteristics, imaging findings, treatment, and prognosis between primary osteosarcoma and RIOS.
6.Clinical Efficacy of Modified Colon Instillation in the Patients with Severe Acute Pancreatitis and Intestinal Paralysis

Objective: To observe the clinical effect of modified colon instillation in the patients with severe acute pancreatitis ( SAP) and intestinal paralysis. Methods:Totally 63 cases of patients with SAP and intestinal paralysis were randomly divided into the treatment group (32 cases) and the control group (31 cases), and they were treated with different enema methods for 15 days. The pe-ripheral venous blood was collected for the detection of serum amylase (AMS), C reactive protein (CRP) and tumor necrosis factor ( TNF-α) before and after the treatment. The abdominal pain, relief time of abdominal pain, recovery time of gastrointestinal function and complications were observed. Results:Compared with those before the treatment, the serum levels of AMS, CRP and TNF-αwere decreased in both groups after the treatment, and the decrease in the treatment group was more notable than that in the control group ( P<0. 05, P<0. 01). The duration of abdominal pain, relief time of abdominal pain and the recovery time of gastrointestinal function in the treatment group were shorter than those in the control group (P<0. 01, P<0. 05). The incidence of complications in the treatment group was 12. 50%, while that in the control group was 35. 48% (P<0. 05). Conclusion:Modified colon instillation can improve the clinical efficacy, shorten the recovery time of gastrointestinal function and reduce the incidence of complications, which is worthy of clinical promotion.
7.Study on the clinical practice teaching of respiratory medicine

After years of respiratory medicine clinical practice teaching , we have discovered that there exist some problems such as teachers' job burnout, teachers' lowprofessional knowledge level of clinical teaching and lack of teaching ability, medical students' fear, low self-esteem, psycho-logi-cal fear and emotional weariness breeding as well as the patients and their family members ' refusal of practice because of their worrying about being a test and so on. To solve these problems , we have put forward some proposals such as recognizing and rewarding teachers to affirm their teaching levels , organizing teachers to participate in academic conferences, seminars, etc. to expand their professional knowledge in order to improve their overall quality. And at the same time, we have adopted PBL teaching method, applied multimedia technology and network platforms, put emphasis on basic skills training, the improvementof departmental rotation assessment, the establishment of teaching files and doctor-patient communication and other ways to improve clinical teaching quality and develop interns' clinical thinking ability to help them transitto clinicians.

9.Detection and antimicrobial resistance of hypermutable Pseudomonas aeruginosa strains

Objective To investigate the isolation and antimicrobial resistance of hypermutable Pseudomonas aeruginosa strains.Methods The cultured bacteria were identified by API 20NE system.The susceptibilities of bacteria were detected by disk diffusion method and the results were confirmed with the rules of NCCLS/CLSI.The hypermutable strains were identified byhigh concentration of rifampin-resistant growth assay.Results Among analyzed 100 strains,37 strains(37.0%) were hypermutable.The resistant rates of hypermutable strains to imipenem,meropenem,cefoperazone/sulbactam,piperacillin/tazobactam,ceftazidime,cephfime,aztreonam,amikacin and ciprofloxacin were significantly higher than those of non-hypermutable strains.The multidrug resistance rates of hypermutable strains were also higher than those of non-hypermutable strains.Conclusion The hypermutable strains of Pseudomonas aeruginosa sourced from chronic respiratory tract infections dominantly.The hypermutable strains were hyper-resistant and multidrug resistant to many antibiotics,so strengthened monitoring should be encouraged to control the prevalence of hypermutable Pseudomonas aeruginosa strains.

Objective To examine the effect and mechanism of sodium ferulate inhibiting P38 MAPK in macrophage on neurotoxicity which was mediated by fribrilar-amyloid-induced.Methods The isolated peritoneal macropohages were cultured.P38 MAPK protein in nuclear extracts was analyzed by Western blot and production of tumor necrosis factor-?(TNF-?) was detected by ELISA.For neuron-macrophage co-cultures,Microtubute-associated protein-2(MAP-2) expression was detected by immunocytochemical technique.The level of LDH in the medium was measured.Results The production of TNF-? and P38 MAPK protein in nuclear extracts increased significantly by incubation with A?25-35.LDH efflux in neuron-macrophage co-culture medium increased and MAP-2 expression decreased significantly(P
