Journal of Audiology and Speech Pathology 2004;0(06):-
Objective To investigate the relationship between Alloiococcus otitidis (A. otitidis) and Secretory Otitis Media (SOM) in adults. Methods The study included 39 adult patients suffering from SOM and 42 samples (middle ear effusion, MEE)were collected. We detected the four organisms(A. otitidis ,S. pneumoniae, H.influenzae and M. catarrhalis) by mutiplex PCR, for comparing the bacterial findings in the patients with or without cold history, with or without antibiotic treatment, with effusion of different characters and durations by chi-square test. Results The DNA of A. otitidis was detected from 5 MEEs(11.9%).To acute SOM, only 1 sample was PCR-positive for A. otitidis; and to chronic SOM,4 samples were positive. There was a significant difference between them. There were 3 serous MEEs and 2 mucous MEEs, which were PCR-positive for A. otitidis, and there was a significant difference between them. Antibiotic therapy didn't contribute to PCR-positive for A.otitidis. Conclusion There is A. otitidis in the MEEs of adult SOM and it may be a pathogenic factor in SOM. A. otitidis may play an important role in chronic SOM.

Sterols are one of the active classes of compounds in Inonotus obliquus for their effective therapy of many diseases. In field environment, this fungus accumulates large amount of sterols. In cultured mycelia, however, this class of compounds is less accumulated. For analyzing the factors responsible for differing sterol composition, the field-grown and cultured mycelia were extracted with 80% ethanol at room temperature and total sterols were prepared using silicon gel column chromatography followed by identification using either GC-MS or spectroscopic methods. For culturing Inonotus obliquus, the seed culture was grown either in basic medium consisting of glucose (2%), yeast extract (0.5%), KH2PO4 (0.01%), MgSO4·7H2O (0.05%) and distilled water at pH 6.5, or the basic medium supplemented with serial concentrations of AgNO3. The results indicated that field-grown mycelia contained lanosterol and inotodiol (comprised 45.47% and 25.36% of the total sterols, respectively) and other 10 sterols (comprising the remaining 30.17%) including ergosterol biosynthetic intermediates such as 24-methylene dihydrolanosterol, 4,4-dimethylfecosterol, 4-methyl fecosterol, fecosterol and episterol. Column chromatography also led to the isolation of lanosterol, Inotodiol, trametenolic acid, foscoparianol B and a new triterpenoid foscoparianol D in field-grown mycelia. In comparison, the cultured mycelia only contained three sterols with ergosterol as the predominant one (82.20%). Lanosterol only accounted for 3.68%. Supplementing Ag+ into the culture at 0.28 μmol·L-1 greatly enhanced content of lanosterol (accounting for 56.81%) and decreased the content of ergosterol (18.5%) together with the presence of intermediates for ergosterol biosynthesis. These results suggested that the sterol composition in mycelia of the fungus can be diversified by supplementing substances inhibiting enzymatic process towards the synthesis of ergosterol. Harsh growth conditions in field environment (I.e. temperature variation, UV irradiation etc.) can delay the synthesis of ergosterol and hereby diversify the sterol composition in the mycelia of Inonotus obliquus.

Objective To explore the difference between the stapes prosthesis surgery with microscope and with otoscope. Methods Fifty-four patients with otoselerosis accepted stapes prosthesis surgery with the same kind of stapes prosthesis, done in the same operative method by the identical surgeon. All the operations were done with microscopes or with otoscopes. The operative difference and therapeutic effects between microscope surgery and otoscope surgery were analysed. Results There was no difference in the postoperative heating improvement between microscope surgery and otoscope surgery; the operative time of microscope surgery [(30±7)] min was much shorter than that of otoscope surgery [(60±5)] min, with significant statistical difference. Conclusions Stapes prosthesis surgery with microscope, which has advantages of bimanualness, easiness, and short operative time, is much better than that with otoscope. Thus, microscope is still the preferred magnifying equipment in stapes prosthesis surgery.

Objective To detect the mutations of pigment epithelium derived factor (PEDF) gene in a human malignant melanoma cell line A375. Methods A375 cells and control melanocytes obtained from circumcised prepuce were cultured, genomic DNA was extracted from these cells. All eight exons of PEDF gene were scanned by single strand conformation polymorphism analysis ofpolymerase chain reaction products (PCR-SSCP) in both A375 cells and control melanocytes. DNA sequencing was performed for the PCR products separated into electrophoretic bands with altered mobility. Results Altered mobility was observed with SSCP analysis in amplicons of exon 3, 4, 5, 6 and 7, with the most obvious alteration occurred in exon 5 and 6. DNA sequencing revealed mutations in both exon 5 and 6. The common type of mutations was single base-deletion in exon 5 and single base-substitution in exon 6. Conclusion Mutations of PEDF gene may contribute to the development of human malignant melanoma.
