1.Clinical study on pelvic reconstruction surgery in the treatment of pelvic organ prolapsey

Objective To study the efficacy of pelvic reconstruction surgery in the treatment of pelvic organ prolapsey (POP).Methods 18 patients with POP underwent pelvic reconstruction surgery were selected.Clinical parameters during peri-operation were recorded.After 3 months and 6 months,pelvic organ prolapse quantitative examination(POP-Q) and anatomic improvement in these patients were analyzed.Comparisons of pelvic floor distress impact questionnaire short form 7 (PFIQ-7) and quality of sexual life of chinese women questionnaire in these patients before and after surgery were used to evaluate quality of life and quality of sexual life.Results Median operation time was (90.0 ± 13.2) minutes and median blood loss was (30.0 ± 1.2) ml,all the patients were followed.The objective cure rate was 100%.The mean score of post-operative PFIQ-7 were significantly lower than those of pre-operation (P < 0.01),and those of qualify of sexual life of chinese women questionnaire was no significantly difference (P >0.05).The rate of mesh crosion was 0,and there was no case of de novo urinary incontinence and de novo dyspareunia.Conclusion Pelvic reconstruction surgery is a safe and efficacious management.It can improve qualify of life remarkably and have no influence on sexual quality of life.
2.Familial disorders of steroid 17?-hydroxylase deficiency

Objective To study and to improve the understanding of the etiology, diagnosis and treatment of familial congenital adrenal hyperplasia(CAH) due to 17? hydroxylase deficiency. Methods 2 rare cases of 17? hydroxylase deficiency in a family were studied with review of the literature. Results 2 cases were definitely diagnosed,one being male pseudohermaphroditism and the other female hypogonadism.Treatment consisted of replacement doses of glucocorticoid hormones and supplemented by estrogen therapy.The serum potassium became normal and the blood pressure was kept below 150/90 mm Hg. Conclusions 17? hydroxylase dificiency is a kind of hypertensive forms of CAH and is inherited as an autosomal recessive trait,its prevalence being rare.It is associated with hypogonadism or pseudohermaphroditism, hypertension or hypokalemia and with absent/decreased 17 OHCS and 17 KS.Hypoaldosteronism and positive dexamathasone inhibition test are the outstanding features.
3.Optimization of the Extraction Technology of Thrombolytic Components from Phyllanthus urinaria by Orthogonal Experiment

OBJECTIVE: To optimize the extraction technology of thrombolytic components predominantly as corilagin from Phyllanthus urinaria.METHODS: The extraction technology was optimized by orthogonal experiment with solvent amount,extracting time and extracting times as factors,and with the comprehensive score of the yield of extract and corilagin content as indexe.The impurity-removing efficacy by centrifugation,acid/base method or ethanol precipitation was evaluated.RESULTS: The optimal extraction technology was as follows: using 8-fold volume of 30% ethanol as solvent for reflux extraction of 3 times(2 hours/ time).The best impurity-removing efficacy was achieved when 90% ethanol was used for precipitation.CONCLUSION: The extraction technology is simple and stable with high yield of active components,and it provides theoretic basis for the industrial production.
4.Synthesis of oligopeptide of HDV antigen and its application in diagnosis

A 27-peptide,a fragment of hepatitis delta antigen(HDAg),was synthesized and used to develop an ELISA method for ihe detection of anti-HD.It was found that positive anti-HD reaction occurred between the coated 27-peptide and a stored sample of serum which was known anti-HD positive.Absorption test revealed that the synthetic peptide competed with natural HDAg for anti-HD,suggesting that the peptide possessed the antigenicity similar to that of natural HDAg.The antigenicity of the synthetic peptide was quite specific wihtout cross reaction with normal human and mouse sera and with anti-HA.anti-HB and anti-HC sera.Among 300 blood donors,there was only 1 case(0.33%)anti-HD positive with an ALT level 2 times higher than normal.In 41 cases of non-B hepatitis and 52 cases of HAV hepatitis,none was anti-HD positive.In 211 cases of various types of HBV hepatitis,21 were(9.95%)anti-HD positive,among whom 2/82(2.5%)werehealthy HBV carriers,6/43(13.95%)were patients with a-cute icteric hepatitis,6/60(10.00%)were patients of chronic active hepatitis,4/18(22.20%)were patients of severe hepatitis,and 3/8(37.50%)were those with liver cirrhosis.These results were consistent with those in our previous reports.
5.Analysis of the patient's death cause in hospital after cardiac valve replacement

Objective: The aim of our study was to analyse the death cause and related factors of death in hospital of patients after cardiac valve replacement (CVR). Methods: Regression analysis of 34 patients' data was executed. Results: Death rate was 9.9 %, death cause: (1) L.ow cardiac output syndrome is the first complication and death cause (2.6 % ), which was closely related to cardiac function (grade Ⅳ ) before operation, LVEDD>8.0 cm, LVEF<0. 40, GR >80%, emergency operation, long time of arterial block, etc; (2) Ventricular tachycardia、 ventricular fibrillation is the second cause of death (2. 1%); (3) Respiratory failure (4) Post-operation hemorrhage inducing pericardial tamponade and shock; (5) Postoperation infection of replaced valve and brain complications. Conclusion: The success rate of operation can be increased by selecting patients strictly, improving operation methods, enhancing periopertive monitor.
6.A reasearch and analysis of psychological nursing intervention of primiparas

Objeetive To detect the influence of psychological nursing intervention in 215 Prlmiparas Methods Taking whole course psychological nursing intervention of 215 primiparas who were making appointments on obstetrical outpatient department.Take psychological care,education of in-hospital and person to person care of in-delivery room as experiment group.and choose 205 Primiparas who were delivery in ordinary way at random.Compare the rate of viginal delivmy and the rate of cesarean section and neonatal asphyxia and the rate of postpartum hemorrhage.Evaluate the effect of Psychological nursing intervention to Primiparas.Results The rate of、riginal delivery on experiment group Was higher than the control group.It also shorten the duration of labor,and control the rate of cesarean section,neonatal asphyxia and postpartum hemorrhage effectively.There ale significant difference between two groups(P<0.05).Conclusion The psychological nursing intervention is positive to the primiparasis in labor.It can increase the of viginal delivery shorten the duration of labor.and decrease the rate of cesarean section neonatal asphyxia and postpartum hemorrhage.
7.Serum Dickkopf-1 levels in patients with ankylosing spondylitis not affected by using tumor necrosis factor-α inhibitor

Objective Dickkopf-1 (DKK-1) is an antagonist of the Wnt signaling pathway of bone formation and it is the target gene of TNF.When DKK-1 is inhibited,osteoprotegerin (OPG) would increase,OPG can antagonize bone erosion caused by receptor activator of NF-κB ligand (RANKL).Detection the levelsof serum DKK-1,OPG,RANKL and TNF-α of patients with AS before and after applying TNF-α antagonist,the effects of anti-TNF-α treatment on signaling pathway of bone formation was explored.Methods Medical records and serum samples of 43 AS patients receiving TNF-α receptor-Ⅱ IgG Fc fusion protein for 12 weeks were collected and the levels of serum DKK-1,OPG,RANKL and TNF-α were detected by enzyme-linked immunosorbent assay.Paired t-test and Person's correlation analysis were used for data analysis.Results Disease activity indexes in AS patients with anti-TNF-α treatment,including VAS score,time of morning stiffness,BASDAI,BASFI,ESR and CRP,which were [(2.7±2.0) cm,(7±4) min,1.1±1.1,0.8±1.2,(10±9) mm/1 h and (16±26) mg/L] respectively,were reduced when compared with those before treatment,[(7.6±1.9) cm,(46±33) min,6.0±1.3,4.4±2.0,(39±29) mm/1 h and (76±43) mg/L] respectively,and the differences were statistically significant (P<0.05).Serum levels of DKK-1,(3.3±1.5) μg/L vs (3.2±1.3) μg/L,OPG,(144±71) pg/ml vs (136±62) pg/ml,and RANKL,(71±37) pg/ml vs (68±30) pg/ml,were not changed (t=14.043,8.031,20.166,13.521,7.679,9.563,all P>0.05).The levels of TNF-α increased from (1.9±1.3) pg/ml to (55.0±50.6) pg/ml (t=6.951,P<0.05).There were no relations between DKK-1 and TNF-α,RANKUOPG,patient VAS,duration of morning stiffness,BASDAI,BASFI,ESR and CRP,allP>0.05.Conclusion Clinical indicators are improved significantly by anti-TNF-α therapy; however,it should not be withdrawal suddenly.There is no relationship between serum levels of DKK-1 and level of inflammation in AS patients.
8.Subclinical thyroid diseases and the risks or outcomes of ischemic stroke

Subclinical thyroid dysfunction including subclinical hypothyroidism and subclinical hyperthyroidism, subclinical hypothyroidism is more common in the general population. Subclinical thyroid dysfunction may increase the incidence of risk factors of stroke, like hypertension, diabetes melitus, dyslipidemia, atherosclerosis, atrial fibrilation, homocysteinemia and so on. Subclinical thyroid dysfunction may be associated w ith the outcomes in patients after acute ischemic stroke.
9.Stress analysis of one-piece spiral implant

砄bjecat:To study the correlatiom between stress and structure of one piece spiral implant.Methods: The morphology of alveolar bone was selected by CT around the first molar of mandible and around the incisor of maxilla of a healthy man in the position of normal occlusion. Cortical bone with the thickness of 1~2 mm was used to make the alveolar bone model. The one piece spiral implants with root, in the shape of cylinder, length of 10,12, 13 and 14 mm, corona height of 4 and 9 mm,diameter of 3.6,3.7, 3.8,3.9 and 4.0 mm were prepared respectively. The implants were put into the alveolar bone models and a load of 200 N was applied vertically, or 45 ℃ obliquely or 45℃ non center obliquely. The stress on the implants was measured and studied with 3D FEM. Results:The stress on the implants was inversely correlated to the length and diameter in the place of front teeth and molars except non centeraly obliquely loading to molar. The greatest stress was found around the neck of the implants. The distribution and intensity of the stress were related to the way of loading and the elastic modulus of bone. Conclusion:The length and diameter of one piece implant are the factors correlated to the stress on the implant and surrounding bone.
10.Improved Compliance with Recommended Infection Control Procedures among Beijing Dentists in 2000 vs 2005

OBJECTIVE To investigate changes in Beijing dentists′ infection control(IC) practices in 2000 vs 2005.METHODS Data from questionaires of 647 dentists in 2000 and 592 dentists in 2005 were compared by using descriptive statistics and Person′s ?2.RESULTS Their response rates were 95.00%(2000) and 93.00%(2005).There were improvements in 2005 vs 2000 in reports of routine use of gloves(94.59% vs 73.31%);masks(56.78% vs 13.94%);and protective gown(38.87% vs 14.51%);change of gloves after each patient(93.47% vs 63.25%);vaccination for hepatitis B virus(HBV)(73.60% vs 32.66%);autoclave sterilization of dental handpieces after each patient(95.07% vs 41.24%);flushing waterlines after each use(70.78% vs 42.01%);and high-volume suction(44.92% vs 11.19%).CONCLUSIONS The compliance with recommended infection control procedures among Beijing dentists is improving in 2005 vs 2000.More education and inspection is needed to develop the infection control practices in dental health care settings.
