2.Teaching practice and normalization construction of the elective course of hyperbaric oxygen medicine

The importance of course arrangement and standardization construction for the course of hyperbaric oxygenation medicine was expounded in this review.In addition,the measures and experiences on textbook choosing,teaching content assignment,teaching methods designing,assessment means innovation and teaching personnel training were also discussed.The teaching system of the course of hyperbaric oxygenation medicine can be improved and references for thc dcvelopment of clinical hyperbaric oxygenation medicine courses can be provided through these measures.
3.Microsurgical repairment of finger-tip defection

Objective To investigate the methods to repair the defection of finger tip.Methods During Feb.2004 to July 2008,in 98 cases,23 fingers were reconstructed with 2nd toe transfer which was reformed with digital flap or trilobed flap from 2nd toe,33 fingers were coverd with wrap around flap or half wrap around flap from great toe,or combined wrap around flap from great with pulp flap from 2nd toe,28fingers were repaired with fabular flap from great toe combined with tibial flap from 2nd flap,and 27 fingers were repaired with nail flap or enlarged nail flap from great toe. Results Vessel articulo took place in 5 cases,and all cases recovered with wonderful outward appearance Conclusion Microsurgical technic is an ideal method to repair the finger-tip defection.

5.Clinical study about effects of the acute fasting hyperglycemia and type 2 diabetes mellitus on ischemic preconditioning in patients with the first-ever acute anterior circulation infarction

Objectives To assess whether prodromal transient ischemic attack (TIA) has beneficial effects on type 2 diabetic patients and those had acute fasting hyperglycemia with the first-ever acute anterior circulation infarction.Methods One hundred and seventy patients with first-ever ischemic acute anterior circulation infarction were involved.According to whether they had type 2 diabetes mellitus,patients were divided into group A (non-diabetic patients) and group B (diabetic patients).Mean infarct volume was also compared between patients with pro-TIA and without pro-TIA in group A and B,and each group with acute hyperglycemia too.Results In group A,prodromal TIA was associated with a smaller infarct volume (M: 2.61 cm3 vs 5.99 cm33 Z=-2.69,P=0.007).On the contrary,in group B,there was no significant difference in infarct volume (M: 3.25 cm3 vs 11.0 cm3,Z=-1.699,P=0.08) between patients with and without prodromal TIA.In group A patients absent with acute fasting hyperglycemia,infarct volume significantly smaller in patients with prodromal TIA than in those without (M: 2.69 cm3 vs 6.46 cm3,Z=-2.34,P=0.019) ; In patients with acute fasting hyperglycemia,there was no a significant difference in infarct volume (M: 2.61 cm3 vs 5.69 cm3 ,Z=-1.218,P=0.27).However,among group B patients present or absent with acute impaired fasting glucose,there was no significant difference in infarct volume in patients with or without prodromal TIA.Conclusion Diabetes mellitus and the acute fasting hyperglycemia might prevent the ischemic preconditioning.
6.Clinical analysis of 15 infants with citrin deficiengcy

Objective To analyze the clinical characteristics and treatment outcome of citrin deficiengc in infants. Methods The clinical data of 15 patients with citrin deficiengcy were analyzed retrospectively and the diagnoses were confirmed by gas chromatography-mass spectrometry(GC/MS). Results The patients consisted of 9 male and 6 female infants,whose age of onset ranged from 2 to 10 days excepted 1 case later than one month. The main clinical manifestations were jaundice and swelling liver,laboratory data showed increased total and direct bilirubin,bile acid,alanine aminotransferase,γ-glutamyltransferase,blood ammonia,lactic acid and α-fetoprotein;7 paitients had prolongation of prothrombin time and 9 had dyslipidaemia;G C/MS analysis of urine samples showed elevated galactose ,galactitol and 4 - hydroxyl phenyllactic acid;citrin gene were positive in 6 cases ,cytomegalovirus infections were found in 13 cases. After the therapy of fat-soluble vitamins(A,D and K)and lactose free formula,the jaundice relieved rapidly and dispeared within 2 weeks in all cases. The 6-month follow-up data showed no symptoms in 6 cases. Conclusions The clinical manifestations of citrin deficiengcy are nonspecific,GC/MS and lactose free formula treatment can be used to confirm the diagnosis.
7.3.0 T conventional MRI and MR arthrography in the evaluation of acetabular labral tears

Objective To compare the diagnostic value of conventional MR imaging with MR arthrography for acetabular labral tears at 3.0 T MR imaging Methods Forty-four patients suspected of acetabular labral pathology were prospectively scanned with conventional MR imaging and MR arthrography of the affected hip joints. All MR data were retrospectively studied. Each acetabular labrum was divided into three regions of anterior, superior, and posterior, and the acetabular tear and its classification were recorded. Five patients underwent subsequent hip arthroscopy. Wilcoxon signed ranks test was used to compare the difference between MR imaging and MR arthrography, and Kappa value was adopted for the evaluation of agreement of these two techniques. Results One hundred and sixteen regions of acetabular labrum were evaluated to be concordant at MR imaging and MR arthrography. Only 16 regions had different results at MR imaging and MR arthrography, in which 9 were diagnosed to have tears at MR arthrography but non-tear at MR imaging, and 6 had tears at MR imaging but non-tear at MR arthrography, and the other one had tears at both exams but of different classifications. In 5 patients confirmed by hip arthroscopy, both the evaluations of MR imaging and MR arthrography were accordant with that of arthroscopy. There were no statistical difference between MR imaging and MR arthrography ( Z = 0. 347, P > 0. 05 ) for diagnosing the acetabular labral tears, and good agreements (Kappa = 0. 781 ,P < 0. 01 ) were found between these two methods. Conclusion Conventional MR imaging of hip joints at 3.0 T shows similar diagnostic value with MR arthrography in evaluating the acetabular labral tears.
8.Progress in treatment of clinical stage N2 non-small cell lung cancer

Although the optimal treatment protocol for clinical N2-stage non-small cell lung cancer (NSCLC) is still uncertain, the recommended standard treatment protocol goes to radical concurrent chemora diotherapy. Concurrent chemoradiotherapy after induction chemotherapy is not superior to standard concurrent chemoradiotherapy. Neoadjuvant concurrent chemoradiotherapy followed by pulmonary lobectomy can improve overall survival ratio comparaed with preoperative neoadjuvant concurrent chemoradiotherapy. The role of concurrent chemoradiotherapy followed by consooldation chemotherapy is still controversial. Gefitinib or erlotinib failed to bring about the survival benefit from the maintenance treatment after concurrent chemoradiotherapy. Prophylactic cranial irradiation play a role merely in decreasing the risk of brain metastases, but not in the improvement of overall survival.

The evaluation of biomaterial biocompatibility is a key step before the clinical application.With the rapid development of molecular biology,scientists have begun to evaluate biomaterial biocompatibility at molecular level and proposed the concept of molecular biocompatibility.Researchers'main tasks at present are to identify more molecular markers using molecular biology technology and to establish standards for evaluating molecular biocompatibility of biomaterial,which will provide the guidance for the design of better biomaterials.
10.β-catenin mutation in pediatric malignant tumors

β-catenin is an important signaling transduetion and adhesion molecules.Mutation of the betacatenin gene,CTNNB1,is a common case in pediatric tumors,which may induce development and metastasis.In hepatoblastoma,48%have CTNNB1 mutation.In Wilms tumor,mutation only occurs in cases with WT1 gene mutation.In neuroblastoma,mutation is also found.
