Objective To analyze the clinical and pathological features of Wegener granulomatosis (WG) in children in order to improve the understanding of this disease. Methods Nine cases of WG,including two cases from our hospital and seven cases throughout the country, were analyzed in aspects of their clinical characteristics, laboratory examinations,X-ray and treatments. Results Of the nine cases, six cases were male and three cases were female with average age of twelve years old. The average time from the initiation of clinical symptom to diagnosis was 8.3 months. Patients had multiple systems and organs involvements. Lung and kidney involvement were the most common. Six out of seven were positive in c-ANCA and anti-PR3 antibody. Biopsy was conducted in eight patients, and pathological manifestations involved parenchymal necrosis, granulomatosis, vasculitis and so on. Most patients improved significantly after being treated with glucocorticoids and cyclophosphamide. Conclusion WG is a syndrome which involves many organs without distinctive clinical manifestation. An ANCA test should be performed to make an early diagnosis.

The afsRS(cla) global regulatory genes from Streptomyces clavuligerus activate the production of two antibiotics in Streptomyces lividans. In this study, we gained an increase of 38% in the production of natamycin (3.56 g/L) in an industrial strain Streptomyces gilvosporeus TZ1401 through the integration of pHL851 that bears the afsRS(cla) global regulatory genes into its genome. We discovered by quantitive real-time reverse transcription PCR (qRT-PCR) that the expression of 6 genes of the natamycin biosynthetic gene cluster were improved from 1.9 to 2.7 times. This suggests that afsRS(cla) improve the production of natamycin through increased transcription. This study provides a good example for applying afsRS(cla) in high yield breeding of industrial antibiotic producers.

BACKGROUND:The incidence of open fracture of the elbow joint is increased rapidly with an increased number of accidents. Fractures are often accompanied by severe soft tissue injuries. At present, a combined therapy of autologous skin grafting, vacuum sealing drainage and external fixation for the treatment of open fractures has been widely reported. OBJECTIVE:To investigate the characteristics of the combined therapy of autologous skin grafting, vacuum sealing drainage, and external fixation for repair of complex and severe open fracture of the elbow joint. METHODS:A total of 41 patients with complex and severe open fracture of the elbow joint who received the combined therapy of autologous skin grafting, vacuum sealing drainage and external fixation during January 2009 to December 2013 in General Hospital of Tianjin Medical University, China were selected. Al patients were treated with routine debridement, external fixation and vacuum sealing drainage to cover wound during the first period. During the second period, autologous skin grafting or flap transfer was used to repair wound when the fresh granulation tissue covered the wound. RESULTS AND CONCLUSION:The postoperative folow-up period of these 41 patients was 8-22 months, with an average period of 13 months. External fixation period was 8-13 months with an average period of 9.8 months. The open wounds were completely healed after autologous skin grafting or skin flaps transfer. Fractures were healed in 31 patients during the first period. There were 7 cases of delayed union, 3 cases of nonunion. Al fractures were completely healed during 8-12 months after iliac bone grafting. The excelent and good rate of limb function recovery was 73.1% among 41 patients with severe open fracture of the elbow joint. The limb function of Gustilo II patients better recovered. These results suggest that the clinical effect of the combined therapy of autologous skin grafting, vacuum sealing drainage and external fixation for the treatment of complex and severe open fractures of the elbow joint is satisfactory.

Objective: To compare the effects of bare-handed cardiopulmonary resuscitation (CPR) and cardiopulmonary resuscitator in emergency treatment. Methods: A total of 185 patients received CPR in our hospital from 2013-01 to 2015-10 were enrolled. The patients were divided into 2 groups: Bare-handed CPR (Bare-handed ) group, n=101 and Cardiopulmonary resuscitator (Mechanical) group, n=84;meanwhile, taking 30 min as border time, each group was further divided as short time CPR subgroup and long time CPR subgroup. High quality CPR was conducted in all patients. The success rates, time and blood levels of lactic acid after CPR were compared among short time subgroup, long time subgroup between Bare-handed group and Mechanical group respectively. Results: For short time CPR patients, the success rates, time and blood levels of lactic acid after CPR were similar between Bare-handed group and Mechanical group; for long term CPR patients, the success rates and time were similar between Bare-handed group and Mechanical group, while blood levels of lactic acid after CPR in Mechanical group (6.88 ± 1.82) mmol/L was lower than Bare-handed group (9.39 ± 2.39) mmol/L, P<0.05. Conclusion: The success rates of bare-handed CPR and mechanical CPR were similar for emergency treatment, both methods were effective; while mechanical CPR may save the manual labor, achieve better metabolic result which should be recommended in clinical practice.
