Neonatal diabetes mellitus (NDM) is a rare monogenic disorder primarily caused by insufficient insulin secretion resulting from mutations in the KCNJ11 and ABCC8 genes. Sulfonylureas, represented by glibenclamide, have become the standard therapy for this type of NDM by precisely closing the mutated ATP-sensitive potassium channels in pancreatic β cells, thereby restoring insulin secretion. Clinical studies confirm that sulfonylureas enable over 90% of patients to successfully transition from insulin to oral treatment, achieving long-term stable glycemic control and improving neurological outcomes to a certain extent. In terms of safety, severe hypoglycemia induced by sulfonylureas is relatively rare and gastrointestinal reactions are mild; moreover, sulfonylureas show good long-term tolerability, and have no adverse effects on child growth and development. In the future, by further refining the full-chain management pathway of “rapid genetic diagnosis-early intervention-specialized dosage forms-long-term follow-up”, the clinical application of sulfonylureas is expected to provide NDM patients with an optimized treatment regimen and maximize their health benefits.

OBJECTIVE To investigate the effects of subanesthetic dose of esketamine on postoperative anxiety and recovery in patients undergoing laparoscopic cholecystectomy. METHODS A total of 200 patients scheduled for laparoscopic cholecystectomy at Suzhou Ninth Hospital Affiliated to Soochow University from January 2023 to December 2024 were randomly assigned to control group (n=100) and observation group (n=100). One minute before the initiation of anesthesia, patients in the control group received intravenous injections of Propofol emulsion injection, Sufentanil citrate injection, and Succinylcholine chloride injection. On this basis, patients in the observation group received an intravenous injection of Esketamine hydrochloride injection. The anxiety status of patients in both groups was compared, along with their general intraoperative conditions (including sufentanil dosage, duration of pneumoperitoneum, operative time, anesthesia time, and extubation time), postoperative recovery, incidence of adverse reactions, and the need for dezocine rescue analgesia. Heart rate and mean arterial pressure, entropy index (state entropy and response entropy), inflammatory marker levels [interleukin-6 (IL-6) and C-reactive protein (CRP)], numerical rating scale (NRS) for pain intensity were compared between the two groups at different time points. RESULTS No significant differences were found between the two groups in pneumoperitoneum duration, operative time, anesthesia time, extubation time, incidence of postoperative dry mouth, entropy index or length of stay in the post-anesthesia care unit (P>0.05). Compared with the control group, the observation group showed significantly lower postoperative STAI-S scores, reduced intraoperative sufentanil consumption, decreased incidence of postoperative nausea, vomiting, and shivering, the need for dezocine rescue analgesia, as well as lower plasma IL-6 and CRP levels at 24 h after surgery, and NRS (P<0.05). The heart rate and mean arterial pressure of patients in the observation group at the start of surgery, end of surgery, and during extubation were all significantly higher than those in the control group (P<0.05). CONCLUSIONS Subanesthetic dose of esketamine can effectively alleviate postoperative anxiety, reduce intraoperative opioid consumption, suppress postoperative inflammatory response, relieve postoperative pain, and promote recovery in patients undergoing laparoscopic cholecystectomy.

Objective:To evaluate the diagnostic value of intestinal tissue metagenomic next-generation sequencing (mNGS) in severe diarrhea following haploidentical allogeneic hematopoietic stem cell transplantation (allo-HSCT) .Methods:Sixteen patients who developed severe diarrhea or hematochezia after haploidentical allo-HSCT at the First Affiliated Hospital of Fujian Medical University (June 2023–August 2024) were enrolled. All underwent gastrointestinal endoscopy and mNGS for microbial detection. Clinical, endoscopic, pathological, and microbiological data were analyzed to evaluate the diagnostic value of mNGS and treatment outcomes following targeted therapy. Results:The study included 16 patients (12 males, 4 females; median age 32.5 years, range 3–60 years). Diarrhea occurred a median of 3.93 months post-transplant (range 1.63–10.40 months). Stool cultures were negative except for one case with Candida. One patient tested positive for Clostridium difficile antigen. Endoscopy revealed mucosal congestion, edema, erosion, and bleeding, with focal inflammation on pathology. mNGS detected pathogens in 87.5% (14/16) of cases, including mixed infections in 78.5% (11/14). Common pathogens were Klebsiella pneumoniae, Enterococcus faecium, Escherichia coli, Rhizopus microsporus, EBV, and CMV. Targeted treatment adjustments led to symptom improvement in 87.5% of patients.Conclusion:Allo-HSCT patients are prone to infectious diarrhea due to immunosuppression. Molecular analysis of endoscopic biopsy tissues using mNGS can accurately identify pathogens, guide targeted therapy, and improve clinical outcomes.

Objective:To evaluate the clinical feasibility and safety of uterine suture with tourniquet binding during cesarean delivery for placenta previa with placenta accreta spectrum (PAS) grades 2-3a.Methods:This retrospective cohort study included 62 patients with placenta previa and PAS grades 2-3a who underwent cesarean section at the First Affiliated Hospital of Kangda College of Nanjing Medical University (the First People's Hospital of Lianyungang) from June 2018 to June 2023. Participants were divided into two groups according to vascular occlusion method: 31 patients receiving uterine suture with aortic balloon and/or uterine artery embolization comprised the intervention group, while 31 patients undergoing uterine suture with rubber tourniquet binding constituted the tourniquet group. Clinical characteristics and outcomes were compared, including operative duration, 24-hour postoperative blood loss, postoperative hospitalization, costs (hospitalization and 24-hour transfusion), early complications (late postpartum hemorrhage, fever, disseminated intravascular coagulation, organ injury), and late complications (menstrual changes, pelvic pain, urinary tract infections within 6-12 months). Statistical analyses employed two independent t-tests, Mann-Whitney U tests, and Chi square tests. Results:(1) Baseline characteristics, including maternal age, gestational weeks at delivery, gravidity and parity, number of previous cesarean deliveries, hemoglobin levels within 24 hours before surgery, neonatal birth weight, or proportion of emergency surgeries, showed no significant differences between the two groups (all P>0.05). (2) The tourniquet group demonstrated shorter operative duration [(118.3±38.2) vs. (180.7±66.6) min, t=3.87, P<0.001] and postoperative hospitalization [(5.9±1.7) vs. (7.6±2.2) d, t=3.04, P=0.002], with lower hospitalization costs [20 000 (15 000-23 000) vs. 44 000 (34 000-52 000) CNY, Z=5.92, P<0.001] and 24-hour transfusion costs [1 300.0 (1 207.5-2 400.0) vs. 2 828.0 (1 634.5-5 657.7) CNY, Z=2.90, P=0.004]. Early complication rates were significantly reduced [6.5% (2/31) vs. 35.5% (11/31), χ2=7.88, P=0.005], while 24-hour blood loss [1 574.2 (900.0-2 000.0) vs. 1 990.3 (1 000.0-2 500.0) ml, Z=1.37, P=0.172] and late complication rates [25.8% (8/31) vs. 32.3% (10/31), χ2=0.48, P=0.399] showed no significant differences. Conclusion:For placenta previa with PAS grades 2-3a, tourniquet-binding uterine suture effectively controls hemorrhage, reduces operative time, hospitalization duration, and costs, representing a safe and efficient surgical approach.
