Hypertension is a common chronic disease with elevated arterial pressure as the main clinical manifestation, which can cause lesions in important organs such as heart, brain and kidney. In recent years, the prevalence of hypertension has increased, and its incidence is also increasing year by year. Combining the theory of traditional Chinese medicine (TCM) with personal clinical experience,Academician TONG Xiaolin put forward the specific pathogenesis of "disease-stage-state" of hypertension based on the state-target theory. Lipophagy plays an important role in the pathogenesis of hypertension by interfering with lipid metabolism, atherosclerosis, arterial plaque formation, vascular remodeling, vascular endothelial injury and other aspects of hypertension. Through modern medical and biological research,on the one hand,it is believed that the dynamic pathological changes in lipophagy are closely related to the "stage" of hypertensive state-target differentiation and treatment, corresponding to the time points of disease occurrence and development in the three stages of hypertension. On the other hand,it is believed that lipophagy is an important microscopic manifestation of the "state" of hypertensive state-target differentiation and treatment. From the perspective of lipophagy, this paper discusses the biological connotation of the 'disease-stage-state' model of state-target differentiation and treatment of hypertension and draws on the modern medicine to understand the law of disease development, matching the macro differentiation and treatment of TCM and the micro physico-chemical indicators of modern medicine to achieve precise TCM treatment. This study provides new ideas for the early prevention and treatment of hypertension and its complications.

Objective:To investigate the application value of double contrast-enhanced ultrasonography (DCEUS) combined with serum pepsinogen (PG) in the diagnosis of early gastric cancer(EGC).Methods:Eighty-two patients suspected of EGC from July 2020 to July 2022 in the People′s Hospital of Guangxi Zhuang Autonomous Region, and preoperative DCEUS examination and PG test were performed, and the patients were divided into benign lesion group(13 cases), early gastric cancer group(57 cases) and progressive gastric cancer group(12 cases) using postoperative pathology as the gold standard. Parameters for comparison included time to peak (TTP), peak intensity (PI), enhanced intensity (EI), serum pepsinogen Ⅰ (PGⅠ), serum pepsinogenⅡ (PGⅡ) and their ratio (PGⅠ/PGⅡ). The sensitivity, specificity, and accuracy of DCEUS and PG alone and in combination for the diagnosis of EGC were analyzed by plotting the ROC curve, and its diagnostic value was compared. Results:In the comparison of DCEUS parameters, PI and EI values were higher in the malignant group than in the benign lesion group and TTP was the opposite, with statistically significant differences (all P<0.05). In the comparison of PG detection, PGⅠ and PGⅠ/PGⅡ were lower in the malignant group than in the benign lesions, and lower in the progressive gastric cancer than in the EGC, while PGⅡ was the opposite, with statistically significant differences (all P<0.05). As shown by the ROC curve results, the sensitivity of DCEUS and PG alone and in combination for the diagnosis of EGC was 80.7%, 73.7% and 87.7%, respectively; the specificity was 76.0%, 72.0% and 80.0%, respectively; and the accuracy was 79.3%, 73.2% and 85.4%, respectively. The area under curve (AUC) of the two modalities alone and combined were 0.784 (95% CI=0.669-0.898), 0.728 (95% CI=0.606-0.850) and 0.839 (95% CI=0.734-0.943), respectively, and the combined diagnosis had a higher diagnostic value than the single diagnostic modality. Conclusions:The combined diagnostic modality of DCEUS and PG can further improve the diagnostic efficacy of EGC and reduce its underdiagnosis rate, which has good application value.

Objective:To analyze the impacts of volume-based procurement(VBP) of artificial joints on the hospital costs for discharged patients who underwent total hip arthroplasty. Methods:The discharge data of 515 total hip replacement patients at a tertiary public hospital in Baise city was collected, comprising 353 cases pre-VBP (January 2021 to March 2022) and 162 cases post-VBP (April to October 2022). Data was analyzed using interrupted time series method. Between October to December 2022 and July to August 2023, one-to-one unstructured qualitative interviews were conducted with the director, doctors, and nurses of the orthopedic department, as well as staffs of the medical equipment department and the health insurance office. The aim was to gather insights about the implementation details of the VBP policy for artificial joint implants, VBP policy's impact on hospitalization costs and its underlying causes. Results:After the implementation of the VBP for artificial joint implants, the hospitalization expenses of surgical patients immediately dropped by 22 566 yuan ( P<0.001), but the change in the slope of the long-term trend was not statistically significant ( P=0.430). From the perspective of cost structure, the cost of disposable surgical materials decreased immediately by 26 889 yuan ( P<0.001), with a long-term trend slope change of -515 yuan per month ( P<0.001). Meanwhile, there were varying degrees of immediate increases in surgical treatment costs, non-surgical item treatment costs, and non-surgical disposable medical material costs ( P<0.05). Specifically, the long-term trend slope for surgical treatment costs and non-surgical disposable medical material costs decreased by -637 yuan per month and -372 yuan per month, respectively ( P<0.001). However, the long-term trend slope of non-surgical item treatment costs did not show statistical significance ( P=0.196). The out-of-pocket expenses immediately decreased by 7 195 yuan ( P<0.001), but the change in the slope of the long-term trend was not statistically significant ( P=0.550). The interview results showed that there were not much overall changes found in the use and surgeries of artificial joints after centralized procurement, but there were delivery delays, and the changes in some cost indicators of hospitalization expenses were also related to the reform of diagnosis related groups payment methods. Conclusions:The implementation of VBP effectively reduced the hospitalization cost for total hip arthroplasty patients, primarily by decreasing artificial joint cost. In addition, it optimized the cost structure, alleviated the financial burden on patients and saved medical insurance expenditures. Future initiatives should focus on improving the medical insurance payment reform to form a policy synergy, creating a cohesive policy framework that enhances the effectiveness of VBP policy, thereby facilitating the high-quality development of public hospitals and medical services.

Objective To create a model based on meteorological data to predict the regions at risk of schistosomiasis during the flood season, so as to provide insights into the surveillance and forecast of schistosomiasis. Methods An interactive schistosomiasis forecast system was created using the open-access R software. The schistosomiasis risk index was used as a basic parameter, and the species distribution model of Oncomelania hupensis snails was generated according to the cumulative rainfall and temperature to predict the probability of O. hupensis snail distribution, so as to identify the regions at risk of schistosomiasis transmission during the flood season. Results The framework of the web page was built using the Shiny package in the R program, and an interactive and visualization system was successfully created to predict the distribution of O. hupensis snails, containing O. hupensis snail surveillance site database, meteorological and environmental data. In this system, the snail distribution area may be displayed and the regions at risk of schistosomiasis transmission may be predicted using the species distribution model. This predictive system may rapidly generate the schistosomiasis transmission risk map, which is simple and easy to perform. In addition, the regions at risk of schistosomiasis transmission were predicted to be concentrated in the middle and lower reaches of the Yangtze River during the flood period. Conclusions A schistosomiasis forecast system is successfully created, which is accurate and rapid to utilize meteorological data to predict the regions at risk of schistosomiasis transmission during the flood period.

Objective:To investigate the etiology of necrotizing pneumonia (NP) in children and the clinical characteristics of NP caused by different pathogens in China. Methods:A retrospective, case-control study was performed in children with NP who were admitted to 13 hospitals in China from January 2008 to December 2019. The demographic and clinical information, laboratory data, etiological and radiological findings were analyzed. The data were divided into three groups based on the following years: 2008-2011, 2012-2015 and 2016-2019, and the distribution characteristics of the pathogens in different period were compared. Meanwhile, the pathogens of pediatric NP in the southern and northern China were compared. And the clinical characteristics of the Mycoplasma pneumoniae (MP) NP and the bacterial NP were also compared. T-test or Mann-Whitney nonparametric test was used for comparison of numerical variables, and χ 2 test was used for categorical variables. Results:A total of 494 children with NP were enrolled, the median ages were 4.7 (0.1-15.3) years, including 272 boys and 222 girls. Among these patients, pathogens were identified in 347 cases and the pathogen was unclear in the remaining 147 cases. The main pathogens were MP (238 cases), Streptococcus pneumoniae (SP) (61 cases), Staphylococcus aureus (SA) (51 cases), Pseudomonas aeruginosa (13 cases), Haemophilus influenzae (10 cases), adenovirus (10 cases), and influenza virus A (7 cases), respectively. MP was the most common pathogen in all three periods and the proportion increased yearly. The proportion of MP in 2016-2019 was significantly higher than that in 2012-2015 (52.1% (197/378) vs. 36.8% (32/87), χ 2= 6.654, P=0.010), while there was no significant difference in the proportion of MP in 2012-2015 and that in 2008-2011 (36.8% (32/87) vs. 31.0% (9/29), χ2=0.314, P=0.575).Regarding the regional distribution, 342 cases were in the southern China and 152 in the northern China. Also, MP was the most common pathogen in both regions, but the proportion of MP was higher and the proportion of SP was lower in the north than those in the south (60.5% (92/152) vs. 42.7% (146/342), χ 2=13.409, P<0.010; 7.9% (12/152) vs. 14.3% (49/342), χ 2= 4.023, P=0.045). Comparing the clinical characteristics of different pathogens, we found that fever and cough were the common symptoms in both single MP and single bacterial groups, but chest pain was more common (17.0% (34/200) vs. 6.1% (6/98), χ 2=6.697, P=0.010) while shortness of breath and wheezing were less common in MP group (16.0% (32/200) vs. 60.2% (59/98), χ 2=60.688, P<0.01; 4.5% (9/200) vs. 21.4% (21/98), χ 2=20.819, P<0.01, respectively). The white blood cell count, C-reactive protein and procalcitonin in the bacterial group were significantly higher than those in the MP group (14.7 (1.0-67.1)×10 9/L vs. 10.5 (2.5-32.2)×10 9/L, 122.5 (0.5-277.3) mg/L vs. 51.4 (0.5-200.0) g/L, 2.13 (0.05-100.00) μg/L vs. 0.24 (0.01-18.85) μg/L, Z=-3.719, -5.901 and -7.765, all P<0.01). Conclusions:The prevalence of pediatric NP in China shows an increasing trend during the past years. MP, SP and SA are the main pathogens of NP, and the most common clinical symptoms are fever and cough. The WBC count, C-reactive protein and procalcitonin in bacterial NP are significantly higher than those caused by MP.

China Journal of Chinese Materia Medica 2019;44(7):1464-1474
To predict the targets of active ingredients of Kuihua Hugan Tablets by network pharmacology, and explore the "multi-component-multi-target-multi-pathway" hepatoprotective mechanism of action. First, through traditional Chinese medicine systems pharmacology(TCMSP) and TCM Database@Taiwan Database, main active ingredients of Kuihua Hugan Tablets were screened out based on oral bioavailability(OB), drug-likeness(DL) and effective half-lives(HL). The targets of active ingredients of Kuihua Hugan Tablets were predicted based on the PharmMapper method. Then, the prediction was conducted by screening the target genes associated with chronic hepatitis and early cirrhosis through CooLGeN and GeneCards databases. Target gene functions and signal pathways were analyzed by bioinformatics annotation database Metascape. Cytoscape software was used to construct the Kuihua Hugan Tablets ingredient-target and ingredient-target-pathway network. String database combined with Cytoscape software was used to construct the networks of component-target and component-target-pathway. STRING database was combined with Cytoscape software to draw protein-protein interaction(PPI) network and conduct network topology analysis. Finally, Systems Dock Web Site software was applied in verifying the molecular docking between active ingredients and potential protein targets. A total of 26 compounds and 509 potential targets were screened out from Kuihua Hugan Tablets in the experiment. The results of PPI network analysis indicated that albumin(ALB), insulin-like growth factor 1(IGF1), matrix metalloproteinase-9(MMP9), matrix metalloproteinase-2(MMP2), non-receptor tyrosine kinase proto-oncogene(SRC), estrogen receptor 1(ESR1) and cancer-signal transduction-inflammation-drugs metabolism-related biological processes and metabolic pathways were closely associated with the active ingredients in Kuihua Hugan Tablets. The effects of Kuihua Hugan Tablets in alleviating chronic hepatitis and early cirrhosis indicated the multi-component, multi-target, and multi-pathway characteristics of traditional Chinese medicines, providing new ideas for further research and development of Kuihua Hugan Tablets.
