1.Research progress of surgery for age-related macular degeneration with cataract

Age-related macular degeneration (AMD) and cataract are the most common causes of low vision worldwide.Nowadays, there is still a controversy about whether cataract surgery should be taken in patients combined with AMD and when should the surgery be taken.The aim of this review is to assess the influence of cataract surgery on the occurrence and development of AMD, to analyze the risk factors, to explore the occasion of cataract surgery in patients with AMD, and joint with anti-vascular endothelial growth factor (VEGF) treatment, also including the development and application of intraocular lens.It helps to avoid and postpone the development and progression of macular degeneration after cataract surgery and get good visual outcome.
3.CLINICAL STUDY ON COMPARISON OF DIFFERENT LOADING-DOSE OF CLOPIDOGREL ON ACUTE CORONARY SYNDROME

Objective To compare the efficiency and safety of clopidogrel of different loading-dose in acute coronary syndromes(ACS) patients with percutaneous coronary intervention(PCI) therapy, and, give an assessment of security about higher loading dose of clopidogrel. Methods 120 patients from January 2008 to January 2009 in our hospital were included.They were randomly divided into 3 groups(na=nb=nc=40)and were given 600 mg,450 mg and 300 mg clopidogrel on the 6 hour before PCI. There was no significant difference between the three groups' basic clinical information(p>0.05),such as age, sex and so on. Four points were selected to observe: the platelet aggregation induced by ADP with 5umol/L before and two hours、four hours, six hours after taking medicine; Primary endpoint events, which include shock during hospitalization, death, target revascularization, myocardial infarction, recurrent angina, stroke in 30 days,and Postoperative Bleeding events and adverse Reactions after Surgery in 30 days. Results compared to the standard dose of 300 mg clopidogrel, 600 mg doses clopidogrel can give a greater degree of inhibition on platelet activating than 450 mg doses in the pre-6 h after administration. Increase Clopidogrel loading dose can reduce the occurrence of major cardiovascular events rate. Bleeding events and the occurrence of adverse events was no significant difference among three groups. Conclusions compared to the standard dose of 300 mg clopidogrel, a higher loading dose of clopidogrel can produce much more faster, greater platelet inhibition, and similar security.
4.Significance of Ligustrazine on acute cerebral infarction rat model and its changes on hydrogen sulfide, plasma bilirubin and oxidized low density lipoprotein level

Objective To explore significance of Ligustrazine on acute cerebral infarction rat model and its effects on hydrogen sulfide, plasma bilirubin and oxidized low density lipoprotein level.Methods 100 healthy SD rats, ( 250 ±30 ) g, male or female, were divided into five groups ( control group, sham operation group, model group, western medicine group, and Ligustrazine group) ,each had 20.Experimental groups prepared focal cerebral ischemia model with suture method.Normal group, model group and the sham group once daily intravenous injected sterile saline 10 mL, Ligustrazine intravenous injection group injected Ligustrazine 10 mL/(kg? d), the WM group intravenous injected atorvastatin 10 mL/(kg? d).Half of the rats were sacrificed at 10 days of the experiment to detect the relevant indicators, at 20 days, the remaining rats in each group were sacrificed.Blood hydrogen sulfide, plasma bilirubin and oxidized LDL levels were detected, and brain pathology change were observed.Results Compared with control group and sham group, in Ligustrazine group and western medicine group,hydrogen sulfide and plasma bilirubin levels increased significantly, and the difference was statistically significant (P<0.05);after 20 days of treatment, compared with western medicine group Ligustrazine group hydrogen sulfide and plasma bilirubin levels rose more significant (P<0.05).Compared with control group and sham group, in Ligustrazine group and western medicine group, OxLDL levels decreased significantly, the difference was statistically significant (P<0.05); after 20 days of treatment, compared with western medicine group, OxLDL levels in Ligustrazine group decreased more significant ( P<0.05 ) .The rats brain histopathology examination showed that, Ligustrazine group:few nerve cell death, cytoplasm loose and swelling reduced significantly;WM group:condensation nuclei rare, brain edema relieve;model rats: brain tissue surrounding the nerve yuan swelling and the emergence of shrinkage, nuclear condensation within the infarct cells and vascular necrosis of normal tissue disappeared, fuzzy structure, interstitial edema; the control group and the sham-operated rats: normal cell morphology, distribution, no cortical Ministry pale infarction.Conclusion Ligustrazine can increase acute hydrogen sulfide, plasma bilirubin levels in rats with cerebral infarction, elevate level of oxidized low-density lipoprotein, has clinical significance.
5.Effect of propofol on a-amino-3-hydroxy-5-methyi-4-isoxa-zolep-propionate receptors AMPA GluR1 subunit and long-term potentiation (LTP) in hippocampal slices in aged rats

Objective To observe the effect of propofol on phosphorylation of a-amino-3-hydroxy-5-methyl-4-isoxa-zolep-propionate receptors (AMPARs) GluR1 subunit and long-term potentiation (LTP) in cultured hippocampal neurons in aged rats.Methods A total of 30 18-month-old rats were decapitated,the brains were rapidly removed and hippocampal slice were prepared.The slices were randomly divided into control group (perfused with artificial cerebrospinal fluid,n=10),propofol-treated group (perfused with propofol in artificial cerebrospinal fluid,n=10)and propofol+ phorbol-12-myristate-13-acetate (PMA)-treated group (perfused with propofol and phorbol ester in artificial cerebrospinal fluid,n=10).Extracellular excitatory postsynaptic potentials (EPSP) were recorded from the CA1 region of hippocampal slices.After perfusion for 20 min,LTP was induced using higher-frequency stimulation (HFS,100Hz,400 pulse) by the Schaffer-collateral pathway.The phosphorylation of AMPA-GluR1 subunit was assayed in cultured rat neurons by Western blot.Results The value of EPSP in propofol-treated group (105.50 ± 3.77) was much lower than in control group (242.10±14.68) and in propofol+ PMA-treated group (239.40±8.98) (F=2.90,P<0.05),and there was no significant difference in the value of EPSP between control group and propofol+ PMA-treated group (P>0.05).The level of P-Glu1/Glu1in propofol-treated group (0.68±0.15) was much lower than in control group (1.67±0.20) and in propofol+PMA-treated group (1.57±0.18) (F=6.84,P<0.05),while there was no difference in the level of P-Glu1/Glu1 between control group and propofol + PMA-treated group (P > 0.05).There was no difference in the value of GluR1/β-actin among the three groups (F=0.31,P>0.05).Conclusions Propofol possesses the ability to inhibit LTP induction and attenuate AMPA receptor GluR1 subunit phosphorylation through modulation of PKC pathway.
6.The occurrence and treatment of atrioventricular block during and after transcatheter occlusion of ventricular septal defect

Objective To investigate the cause and development of atrioventricular block (AVB) occurred during and after transcatheter closure of ventricular septal defect (VSD), and to explore its feasible prevention and treatment. Methods From Mar, 2005 to Dec, 2005, 157 patients who suffered from congenital VSD underwent transcatheter interventional occlusion. Transient Ⅲ degree AVB occurred in 2 patients during the transcatheter therapy and Ⅱ degree AVB in 2 patients and Ⅲ degree AVB in 3 patients were observed 4 hours to 8 days after the therapy. Two of them suffered from Adams-Stokes syndrome. All patients were treated by intravenous injection of glucocorticosteroid, diuretic and dehydrator. One of them was also treated with temporary pacing. Results AVB did not re-occurred after the transcatheter closure in 2 patients who suffered from transient Ⅲ degree AVB during the transcatheter interventional therapy. The cardiac rhythm in 3 patients who suffered from Ⅲ degree AVB after the transcatheter closure reverted to sinus rhythm in 7, 8 and 18 days after the operation respectively. However, 2 of them suffered recurrent Ⅲ degree AVB after discharge, and one of them could not be restored to sinus rhythm. Three patients who suffered from the Ⅱ degree AVB resumed to sinus rhythm in 5, 7 and 8 days after the transcatheter closure respectively, and the type Ⅰ and the type Ⅱ of Ⅱ degree AVB alternated before the rhythm completely reverted. Conclusion AVB is a frequent complication during and after transcatheter closure of VSD. AVB occurred after transcatheter occlusion should be treated actively, or it may become permanent AVB. Improving the cardiac transcatheter interventional device and technique can prevent the high degree AVB in certain degree.
7.The relationship between sigmoid colon propagating motility and cholecystokinin and motilin in patients with irritable bowel syndrome.

Objective To investigate the relationship between the sigmoid colon propagating contractions and the gut hormones cholecystokinin(CCK)and motilin(MTL)in the patients with irritable bowel syndrome(IBS),and to reveal the gut hormones' modulation to the sigmoid colon motility.Methods Twenty diarrea-predominent IBS patients(D-IBS),and sex/age matched 20 constipation-predominent IBS patients(C-IBS)and 15 healthy volunteers were enrolled in the study.From June 2005 to Mar.2006,the high amplitude propagating waves and loger time of the sigmoid colon was measured by means of a liquid-perfused capillary tube connected to a computer.Meanwhile,the concentrations of CCK,MTL of blood plasma and sigmoid colon mucosa were tested by means of immunoassay technology.Results The D-IBS group had higher amplitude of the sigmoid colon propagating waves and their time percentage than that of the C-IBS group and the control group(P

3-Hydroxy-3-methylglutaryl-coenzyme A reductase (HMGR) is the first rate-limiting enzyme for sesquiterpene synthesis in the mevalonate (MVA) pathway. The specific primers were designed according to the transcript sequence of AsHMGR2 from the Aquilaria sinensis (Lour.) Gilg transcriptome database. The full-length cDNA of AsHMGR2 was cloned by RT-PCR and rapid amplification of cDNA ends (RACE) technology, and was analyzed at bioinformatics levels; AsHMGR2 expression profiles in different tissues and in responds to different treatments were analyzed by real-time PCR. The length of AsHMGR2 Open Reading Frame (ORF) was 1 749 bp, encoding 582 amino acids. The GenBank accession number is KC140287. Tissue expression analysis indicated that AsHMGR2 was mainly expressed in root and shoot tips, followed by stem, and was lowest in leaves. Inducible-experiments showed that the genes were induced by mechanical wound as well as chemical liquid induction, and reached the highest expression level at 6 h and 8 h, separately. The full-length cDNA of AsHMGR2 and its expression patterns will provide a foundation for further research on its function in agarwood sesquiterpene biosynthesis.
10.Cathepsin L expression in plasma after acute myocardial ischemia and ischemia-reperfusion in rats.

OBJECTIVE:
To test cathepsin L as a biomarker of myocardial ischemia by examination of cathepsin L expression in plasma after myocardial ischemia and ischemia-reperfusion in rats.
METHODS:
The rat models were established and divided in acute myocardial ischemia model (myocardial ischemia 30 min, 1 h, 2 h groups), ischemia-reperfusion model (ischemia-reperfusion group), and isoflurane-pretreated ischemia-reperfusion model (isoflurane-pretreated group), respectively. Normal control group and sham-operated group were established as contrast. The contents of cathepsin L in plasma were examined by ELISA and myocardial infarction areas were measured after TTC staining.
RESULTS:
No statistical significant changes were found among the experimental groups compared with the normal control group and sham-operated group (P>0.05). The cathepsin L from the ischemia-reperfusion group increased to 2.37 times compared with the normal control group (P<0.05). The cathepsin L and myocardium infarction size of isoflurane-pretreated group decreased compared with the ischemia-reperfusion group (P<0.05).
CONCLUSION
The cathepsin L in plasma is not a promising biomarker of acute myocardial ischemia. Isoflurane preconditioning can reduce the cathepsin L in plasma caused by ischemia-reperfusion injury.
