3.STUDY ON CLINICAL CHARACTERISTICS OF HYPERTENSION IN PATIENTS WITH PRIMARY ALDOSTERONISM

To investigate the clinical characteristics of hypertension in patients with primary aldosteronism (PA), 112 cases of PA confirmed by pathological examination after operation were studied retrospectively. These included 111 cases of PA with unilateral adrenal adenoma and 1 case of bilateral adrenal adenoma. Incidence of hypertension in patients with PA was 100%. 14 3%, 37 5% and 48 2% of patients were at stage 1, 2 and 3 of hypertension, respectively. No relationship between hypertension and the age or duration of hypertension and the size of tumor. Complications of hypertension were found in 31 3% of patients with PA. Stroke was found in 4 5% patients, 3 6% patients with cerebral hemorrhage and 0 9% patients with cerebral infarction. Coronary artery disease as myocardial infarction was found in 1 8% of patients. Proteinuria and renal insufficiency were found in 22 3% of patients and 2 7% of patients, respectively. Complications of hypertension were independent of the age, duration of hypertension, the size of tumor, or preoperative highest systolic pressure and diastolic pressure. 83 9% of patients had taken antihypertensive drugs before operation, and 46 5% of patients still had persistent hypertension after operation. The results suggested that the incidence of hypertension in patients with PA was extremely high, and the majority of patients were suffering from moderate and severe hypertension. Complications of hypertension were common in patients with PA. Hypertension was difficult to control by using antihypertensive drugs before operation, and still persisted after operation in some of the patients with PA.

This paper aims to discuss the distribution status of insulin resistance in TCM syndromes of Type 2 diabetes mellitus and to provide objective indicators for TCM syndrome differentiation of Type 2 diabetes mellitus.The research methods of the paper involve:Select 180 cases of Type 2 diabetes mellitus as subjects to be investigated and divide them into three groups of yin-deficiency dryness-heat, dampness-heat distressing spleen and yang-deficiency dampness-stasis.Another 60 healthy cases are selected as comparison group. Indicators such as height,body mass,asting plasma glucose (FPG),fasting insulin (FINS), glycosylated hemoglobin (HbA1C), three acids glyceride(TG),cholesterol (TC) etc,are tested and body mass index (BMI) ,insulin sensitivity index (ISI) are calculated.The comparisons are made between such objective indices and TCM syndromes to discuss the features and distribution rules of TCM syndromes.The results show that the dampness-heat distressing spleen type is the most common; the yin-deficiency dryness-heat type has the shortest course of diseases.The ISI in each group is lower than that of the healthy comparison group (P

Continuous blood purification therapy, one of the most popular multiple organ protection and life support technology, has been widely used in the treatment of critical illness.After the initial introduction of the concept of dialytrauma or CRRT trauma in 2012, the related complications are getting more and more attention.The loss of heat caused by CBP may lead to hypothermia.Howev-er, to critically ill patients, controversy still reigns on the influence of this issue.There is no guideline developed through clinical prac-tice to define such issue.This article reviews the methods as well as results of related clinical studies to discuss how to manage the hy-pothermia during CBP.
6.Treatment of osteoarthritis using colony-forming cells in stromal vascular fraction of adipose tissue

BACKGROUND:Stromal vascular fraction digested from adipose tissue is a mixture of a variety of cel s, and clone-like cel components may have the real ability to maintain the stemness of mesenchymal stem cel s OBJECTIVE:To separate, passage and culture colony-forming cel s in the stromal vascular fraction of adipose tissue, and to test its ability to repair the knee cartilage in animal models. METHODS:Stromal vascular fraction isolated from fresh adipose tissue was cultured to col ect clone-like adherent cel s that were subject to digestion and passage until the third generation. A rabbit model of osteoarthritis was established surgical y, and the third generation of colony-forming cel s were transplantated into the articular cavity of model rabbits. Model rabbits with no cel transplantation served as controls. Cartilage regeneration in the knee joint of model rabbits were observed and compared. RESULTS AND CONCLUSION:At 3 days after transplantation of stromal vascular fraction cel s, clone-like cel colonies appeared in adherent cel s;at 5 days, the amount of clone-like cel colonies and surrounding single cel s were both increased. At the passage 3, the clone-like cel colonies were also visible, which were fusiform in shape. Gross observation and MRI examination showed that these colony-forming cel s could promote cartilage repair in the joint cavity, increase secretion of synovial fluid, and help to control inflammation and narrow ulceration area. Experimental findings are beneficial for optimizing the isolation and culture of adipose-derived mesenchymal stem cel s and provide a new way for the clinical treatment of osteoarthritis.
7.Fluorescent supermagnetic nanoparticles-labeled adipose-derived mesenchymal stem cells in the three-dimensional culture system

BACKGROUND:Fluorescent magnetic nanoparticles with the properties of quantum dots and magnetic particles have good biocompatibility, and can label cels effectively through endocytosis. OBJECTIVE:To validate the feasibility of fluorescent magnetic nanoparticles in labeling human adipose-derived mesenchymal stem cels. METHODS:Healthy human adipose tissue was extracted and adipose-derived mesenchymal stem cels were isolatedin vitro by type I colagenase digestion. Passage 6 cels were incubated with the fluorescent magnetic nanoparticles overnight. Prussian blue staining and laser scanning confocal microscope were used to observe labeled adipose-derived mesenchymal stem celsin vitro after co-culturing with fluorescent magnetic nanoparticles. The tracing effect of labeled adipose-derived mesenchymal stem cels in vivo was detected by fluorescence imaging system. RESULTS AND CONCLUSION:Prussian blue staining showed that the fluorescent magnetic nanoparticles dispersed in the cytoplasm of adipose-derived mesenchymal stem cels in the form of blue particles. Under the laser scanning confocal microscope, the nuclei of adipose-derived mesenchymal stem cels were dyed blue by Hoechest33258, and the cytoplasm was dyed green. The fluorescence imaging results showed that labeled human adipose-derived mesenchymal stem cels had good imaging results. Therefore, as an efficient tracer, the fluorescent magnetic nanoparticles can label human adipose-derived stem cels in vitro and provide a new method for transplantation and transformation of adipose-derived mesenchymal stem cels.
8.Vascularization for tissue engineering organs:research progress and application prospect

BACKGROUND:An efficient blood vessel system has a decisive effect on the survival and function expression of cells in three-dimensional tissues. Therefore it has been a hot research field in tissue engineering to find an appropriate vascularization strategy.
OBJECTIVE:To summarize and discuss the theory and research progress in vascularization strategies.
METHODS:Literature search was performed in PubMed database for English literatures published from 2003 to 2013. The key words are“tissue engineering, vascularization, endothelial cell, scaffold”in English. Then, the papers were further analyzed and reviewed in line with the theme.
RESULTS AND CONCLUSION:A total of 124 papers were searched. At last, 41 papers were selected according to the titles and objectives. Vascularization is the focus and pressing issue in tissue engineering field. There are many vascularization strategies, such as growth factor delivery, cellco-culture, dynamic-culture by bioreactor, scaffolds or decellularized scaffolds. But none of them is recognized as an effective strategy to achieve functional anastomosis with the host and sustain grafts survival for a long time in vivo. It wil be a big breakthrough in the future to co-culture pluripotent stem cells with other stem cells, combine with growth factors and optimize culture conditions for the differentiation in vivo.
9.Analysis of the effectiveness of remediation of antibacterials in a third-grade class-A hospital

Objective To explore the effectiveness of remediation of antimicrobials in a third-grade class-A hospital .Methods Using retrospective analysis to analyze the clinical application of antimicrobials related data during July .2011 to Nov .2011(before remediation) and July .2012 to Nov .2012(after remediation) .Results The ratio of antimicrobial cost ,the proportion of outpatient antimicrobial prescribing ,inpatient antimicrobial usage and AUD fell respectively from 13 .73% ,16 .97% ,59 .93% and 72 .71 DDDs/100 persons day before remediation to 7 .37% ,13 .83% ,44 .88% and 40 .08 DDDs/100 persons day after remediation ;indica-tors of antibacterial use in clean surgery improved greatly ;the structure of antibacterials changed significantly .Conclusion After extensive antimicrobial special rectification ,the clinical use of antimicrobials is rationalized ,a number of indicators are in line with the Ministry of Health ,its management experience is worth using for reference .But there are still some problems which call for at-tention .
10.Rheumatoid leptomeningitis: a case report and literature review

To report the clinical, radiological and neuropathological findings of a patient with rheuma-toid meningitis. The patient was a 71-year-old Chinese man with a two-year history of rheumatoid arthritisand no other significant medical history, who presented to our hospital recurrent weakness of his left ex-tremities, dysarthria and a continuous bilateral hand tremor. Cerebrospinal fluid (CSF) and serumexam-inations were normal apart from a mildly raised serum perinuclear antineutrophil cytoplasmic autoantibody(p-ANCA). Brain magnetic resonance imaging (MRI) showed leptomeningeal enhancementin both fron-tal and parietal lobes, in addition to several old white matter infarcts. Meningeal biopsy showed numerousinfiltrating macrophages and lymphocytes within the leptomeninges. The patient responded clinically andradiologically to corticosteroid and cyclophosphamide therapy. The patient subsequently developed herpeszoster over his left chest as a complication of his immunosuppressive treatment. His cyclophosphamidewas ceased and intravenous immunoglobulin (IVIG) therapy was commenced, with good clinical responseto both the herpes zoster and meningitis. According to the result of the biopsy, aseptic meningitis wasconsidered the MRI results and the patient’s clinical history were given, and a diagnosis of rheumatoidmeningitis was made. The patientwas p-ANCApositive. Although there was no evidence for cerebral vas-culitis on biopsy, it remains a possibility that the patient’s recurrent minor cerebral infarcts visible onMRI were vasculitic in nature.
