1.THE OPTIMAL COMPUTER AIDED DESIGN FOR LAG SCREW INTERNAL FIXATION IN THE POSTERIOR COLUMN OF THE ACETABULUM

Objective To investigate a special optimized technique for computer aided measure,and provide an anatomical basis for the placement of lag screw in the posterior column of the acetabulum. Methods Eighty accurate hemi-pelvis models were reconstructed from 40 CT data sets.The optimization objective function,for improving the placement of lag screw,was built by adaptively sampling the weighted distance of screws to the acetabulum boundary according to surgical requirements,and the two end points of the screw were modified iteratively to reduce the objective value under constraints.The statistical measure data were provided according to new anatomical reference landmarks for clinical use. Results The optimal entry point for lag screw fixation in the posterior column was located(18.90?1.19) mm above the line connecting the anterior borderline of the sacroiliac joint with the notch between the anterior superior iliac spine and the anterior inferior iliac spine,and the connecting line was divided by the perpendicular project point with a proportion of 2∶3.The mean inclination of the lag screw was(85.99?2.04)? for angle with the line connecting the entry point with the notch between the anterior superior iliac spine and the anterior inferior iliac spine,and(37.54?1.55)? for angle with the line connecting the entry point with the perpendicular project point;the mean length of the lag screws in the posterior column was(133.07?3.22)mm.Conclusion The automatic optimized technique for the computer aided measure is very efficient and has many advantages over the conventional manual dissection methods,and is convenient to design new anatomical reference landmark systems for clinical use.
2.Meta analysis on diagnosis value of narrow band imaging and iodine staining in early esophageal cancer and precancerous lesions

Objective To systematically evaluate the value of narrow band imaging(NBI)and iodine staining in early diagnosis of esophageal cancer and precancerous lesions.Methods The databases of PubMed,Embase,CNKI,VIP technological journals and Wanfang digital journals were retrieved by computer for searching the literatures on the value of NBI and iodine staining in diagnosing early esophageal cancer and precancerous lesions.The Meta-Disc 1.4 software was used to conduct the meta analysis and calculate the pooled sensitivity,specificity and 95% confidence interval(95% CI).Summary receiver operating characteristic(SROC)curve was drawn and the area under curve(AUC)was calculated,thus the diagnostic values of the two methods were evaluated.Results Totally 13 articles were included,including 975 patients.The pooled sensitivity and specificity for NBI were 0.88(95%CI=0.86 to 0.90)and 0.79(95%CI=0.76 to 0.82).The pooled sensitivity and specificity for iodine staining were 0.95(95%CI=0.93 to 0.97)and 0.45(95%CI=0.40 to 0.49).AUC of SROC for the two methods were 0.938 6 and 0.952 9 respectively.There was no statistically difference in the AUC values between the two methods(Z=0.519,P>0.05).Conclusion NBI and iodine staining are both effective ways in the diagnosis of early esophageal cancer and precancerous lesions.And both of them have a certain clinical value.

4.Application of Roy adaptation mode in post-operative self-image disorder and reformation of patients suffered from breast cancer

Objective To explore the application value of Roy adaptation mode in self-image disorder and reformation of patients suffered from breast cancer after operation.Methods 178 cases of postoperative patients suffered from breast cancer were separately divided into the experimental group (90 cases)and the control group(88 cases) at random according to the order of hospital admission.The experimental group adopted nursing of the Roy adaptation mode based upon operation and normal treatment,while the control group received general nursing besides operation and routine treatment.The investigation was carried out using the form of questionnaires.The occurrence rate of self-image disorder,life quality by using GQOLI-74 questionnaire,psychological quality by using emotion scale were compared between the two groups.Results The occurrence rate of the self-image disorder of the patients of the experimental group was evidently lower than that of the control group after the nursing measure was taken.Scores of all items of life quality and emotional scale of the experimental group were evidently higher than those of the control group.Conclusions Applying Roy adaptation mode to nurse post-operative patients suffered from breast cancer is a very good nursing mode,it can facilitate psychological suitability,improve psychological quality and promote their effective adaptability.It has pivotal significance to lower the occurrence rate of the self-image disorder of the patients,reform the self-image and make them return to society early.
5.The willingness to family care bed service and its influencing factors among the elderly with chronic diseases

Objeetive To investigate the willingness to family care bed service and its influencing factors among the elderly with chronic diseases. Methods A three-stage random sampling method was adopted to carry out face-to-face questionnaire investigation in 652 community elderly patients with chron-ic diseases. Results The willingness to family care bed service was 57.2%.Its influencing factors includ-ed age,educational level,family economic income,activity of daily living,whether or not received treatment within 2 weeks and hospital stay within 1 year,whether or not understood the meaning of family care bed service or received it or similar at home service. Conclusions More than a half of the elderly with chronic diseases were willing to accept family care bed service.The community health service center or hos-pital should attaeh importance to and actively carry out family care bed service.They should enhance propanda work to induct the utilization of family care bed service by edlder people with chronic disease and tackle and satisfy their health requirement.
6.Research on medical instrument information integration technology based on IHE PCD.

Integrating medical instruments with medical information systems becomes more and more important in healthcare industry. To make medical instruments without standard communication interface possess the capability of interoperating and sharing information with medical information systems, we developed a medical instrument integration gateway based on Integrating the Healthcare Enterprise Patient Care Device (IHE PCD) integration profiles in this research. The core component is an integration engine which is implemented according to integration profiles and Health Level Seven (HL7) messages defined in IHE PCD. Working with instrument specific Javascripts, the engine transforms medical instrument data into HL7 ORU message. This research enables medical instruments to interoperate and exchange medical data with information systems in a standardized way, and is valuable for medical instrument integration, especially for traditional instruments.

7.Effect of Danhong Injection on Thyroid Hormone Level in Patients with Angina Due to Coronary Heart Disease

Objective To observe the effect of Danhong Injection(DI)on the thyroid hormone level in patients with angina due to coronary heart disease(CHD).Methods Sixty CHD angina patients were equally randomized into the treatment group and the control group.The two groups received routine western medicine such as aspirin,angiotensin convert enzyme inhibitor(ACEI),statins,beta-blockers and nitrate esters,and the treatment group was additionally given intravenous drip transfusion of DI 20 mL,qd.Twenty days constituted one treatment course.After treatment,the therapeutic effect was evaluated and the changes of serum thyroid hormone level were observed.Results In the control group,13 patients were markedly effective,9 effective,8 ineffective,and the total effective rate was 73.3%;in the treatment group,19 patients were markedly effective,6 effective,5 ineffective,and the total effective rate was 83.3%;the difference between the two groups was significant(P
8.Clinical Efficacy of Therapeutic Method of Eliminating Phlegm, Tranquilizing and Calming Wind on Essential Hypertension and Influence on Biochemical Parameters

Objective To observe the clinical efficacy of therapeutic method of eliminating phlegm, tranquilizing and calming wind on essential hypertension (EH) with accumulation of phlegm-wetness and influence on biochemical parameters. Methods One hundred cases of EH were selected and randomly divided into the treatment group and the control group (50 cases for each group). The two groups were given the same western treatment. The therapeutic method of eliminating phlegm, tranquilizing and calming wind (with recipe of eliminating phlegm, tranquilizing and calming wind) was given to the treatment group additionally. The course of treatment was 60 days. Grade of phlegm-wetness syndrome, blood pressure (BP), rhythm of the heart (HR), blood lipid, uric acid (UA), blood-glucose, fasting insulin and C-reactive protein of the two groups before and after treatment were observed and compared. Results BP in the two groups were improved after treatment, and BP in the treatment group was improved more than that in the control group (SBP P=0.000, DBP P =0.049). HR in the treatment group was improved notably than that in the control group (P =0.000). After treatment, the blood-glucose, fasting insulin and C-reactive protein in the treatment group were improved notably compared with those in the control group (the former P =0.001, the latter two P=0.000). UA, TC, LDL-C and HDL-C in the treatment group after treatment were improved in comparison with those before treatment (P =0.000). Conclusion On the basis of western treatment, the therapeutic method of eliminating phlegm, tranquilizing and calming wind can improve BP, HR and C-reactive protein in the patients of EH with accumulation of phlegm-wetness, and it can probably improve the blood lipid and UA to some extent.
9.Vancouver classification-based treatment of periprosthetic femoral fracture after total hip arthroplasty in 15 cases

Fifteen cases of periprosthetic femoral fractures of after total hip arthroplasty (THA) in Department of Orthopedic Surgery, Guangdong People’s Hospital between May 1997 and May 2007 were retrospectively analyzed. According to the classification of Vancouver, there was 1 case of type AG, one of type AL, 3 of type B1, 5 of type B2, 3 of type B2, and 2 of type C. Two cases of type A were treated with conventional method, and cases of type B1 and C were treated with open reduction and internal fixation. Cases of type B2 and B3 were treated with a long-stem cemented prosthesis revision plus allograft cortical strut bone. Through 3-12 months follow-up, all the fractures were healed, and bone allografting was successful, but with high rate of complications which were curable. The new prosthesis stem was fixed well. Excellent and good rate of Harris scores for hip joint was 73.3%. Therapy selection based on Vancouver classification for periprosthetic femoral fracture after total hip arthroplasty is an effective method.
10.Preliminary Study of Dengzhan Xixin Injection on Prevention and Intervention of Cardiovascular and Cerebrovascular System Disease of Older People Caused by Metabolic Disorder

Objective To observe the effect of Dengzhan Xixin injection on prevention and intervention of cardiovascular and cerebrovascular system disease of older people caused by metabolic disorder. Method A Group with metabolic syndrome were treated with a mixture of 30~40 mL of Dengzhan Xixin injection in 250 mL of physiological saline by intravenous injections (n=52). A treatment session was one injection once a day for 10~14 days. Each session was repeated every six months for a duration of three years. Cardiovascular and cerebrovascular disease symptoms, and changes of high sensitivity C-reactive protein (hsCRP) were measured compared with the control group (n=50).
