The main feature of acute steroid-resistant rejection (ASRR) after liver transplantation is a lack of response to steroids.Although some effective immunosuppressants are available,there is no uniform treatment presently available.This paper will focus on exploring the mechanism of ASRR and formulating individually approached strategies.
2.Application of subaxillary curved mini-incision thoracotomy in thoracic operations

Objective To investigate the clinical application of subaxillary curved mini-incision thoracotomy in thoracic operations. Methods Through a subaxillary curved mini-incision between the latissimus dorsi muscle and the serratus anterior muscle (length of incision, 8~14 cm), thoracic operations were performed in 65 patients with thoracic diseases, including pneumothorax or hemopneumothorax (13 patients), pulmonary benign tumor (23 patients), mediastinal tumor (4 patients), and esophageal benign or malignant tumor and cardiac cancer (25 patients). Results All the patients were successfully operated on. No surgery-related deaths occurred. The incidence of complications was 6.2% (4/65), including pleural effusion in 1 patient, pulmonary infection in 2 patients, and anastomotic leakage of esophagogastrostomy in 1 patient.Conclusions Subaxillary curved mini- incision thoracotomy, characterized by minimal invasion, little blood loss, mild influence on cardiopulmonary functions, quick recovery, and good cosmetic outcomes, is an ideal option for the treatment of thoracic diseases.
4.Problems in Community-based Rehabilitation during the Period of 12th Five-year Plan in China

The development of community-based rehabilitation (CBR) needs to improve to meet the change of environment during the period of 12th Five-year Plan. The problems of CBR development included 3 major contradictions: administration of CBR and marketization of medical services; professionals demand of CBR and their professional development; and the demand of CBR and capacity of services. It is necessary to establish some market-oriented CBR project, pay attention to professional development, and promote the capacity of the community rehabilitation agencies.
5.Research of radiation dose rate around patients underwent renal dynamic imaging of SPECT with 99Tcm-DTPA

Objective:To study the radiation dose rate around patients underwent renal dynamic imaging of SPECT with 99Tcm-DTPA, so as to provide the basis for ensuring the radiation safety of the surrounding environment and the general public.Methods: 109 patients who underwent renal dynamic imaging were enrolled in this study, and the radiation dose rates of different time and different distance after 99Tcm-DTPA was injected patients were measured. And then, the changes of dose rate following time and distance were analyzed. Results: At the 0.5h, 1h, 2h, 3h, 6h, and 7h after radiopharmaceuticals were injected patients underwent renal dynamic imaging, the radiation dose rates were 0.76-8.86 μSv/h, 0.68-7.27 μSv/h, 0.57-4.52 μSv/h, 0.56-3.90 μSv/h, 0.23-2.07 μSv/h and 0.21-1.05 μSv/h, respectively, between 0.5 m and 4 m away from patients. At 24h after radiopharmaceuticals were injected, all of radiation dose rates at different distances around patients were same with background values.Conclusion: The radiation dose rates around patients underwent renal dynamic imaging will rapidly decrease with the increasing of time and distance. At 24h after radiopharmaceuticals were injected, all of radiation dose rates at different distances away from patients were same with background values.

7.Discussion about the sampling positions of the bag-type infusion sets for single use in the ethylene oxide residues detection.

This paper selects the bag-type infusion sets for single use as samples, which are produced by different manufacturers and based on the ethylene oxide sterilization. The ethylene oxide sterilization residues in different parts of samples are detected by colorimetric analysis. Combined the comparison of the ethylene oxide residues testing results in the different parts of the same sample with the actual situation in clinical use, more reasonable sampling positions are found to detect the ethylene oxide sterilization residues. The result of this experiment will play a guiding role in the detection of the actual samples.

8.Mechanism and prevention of the formation of hypercoagulable state in patients with malignant tumor

The majority of patients with malignant tumors have hypercoagulable state,which easily leads to thrombosis,and is closely related with recurrence of tumor metastasis.The formation mechanism of hypercoagulable state is related to tumor,tumor treatment,complications etc.The main diagnostic indicators are the platelet,cruor and fibrinolysis,P-selectin and lysosomal protein,blood rheology.Tumor patients with the high risk of thrombus should use low molecular weight heparin.Thromboembolism should be prevented in bedridden patients with tumor and tumor associated operation.Tumor patients with venous thromboembolism should be given thrombolytic treatment.Tumor patients with hypercoagulable state should be treated by antico-agulant therapy combined with chemotherapy.
9.Construction of Eukaryotic Expression Vector for Tumor-Associated Gene SNC90 and Transfection into Cancer Cells

Objective: To construct eukaryotic expression vector of tumor-associated gene SNC90 and transfect it into human colorectal cancer cell lines. Methods: A 1.5 kb tumor-associate gene SNC90 full length cDNA was inserted into a mammalian expression vector pREP9 to make recombinant vector pREP9-SNC90, which was then introduced into three kinds of colorectal cancer cell lines, SW1116, COL0205 and SW620, by lipofection or electroporation. The cells resistant to G418 drug were selected. Results: Cells transfected with pREP9-SNC90 showed fewer G418-resistant colonies than those transfected with void vector, the inhibitory rates are 72.2%, 74.2% and 59.7%, respectively. Conclusion : SNC90 may play a negative role in regulating growth of colorectal cancer cell.
10.Endoscopic greater saphenous vein harvest in coronary artery bypass operation: one-year follow-up analysis

ObjectiveTo summarize the one-year follow-up clinical result of patients undergoing Endoscopic vein harvest (EVH) technology to collect greater saphenous vein( GSV )in coronary artery bypass operation (CABG),and to assess the related factors influencing the outcome.Methods248 patients underwent off-pump coronary artery bypass grafting (OPCAG) from May 2009 to May 2010.Among these patients,136 patients with coronary artery disease received EVH technology to gain GSV,and 112 patients received conventional open vein harvesting (OVH).Then 71 EVH (group 1 ) patients and 64 OVH ( group 2 ) patients had one-year follow-up analysis.We compared and evaluated the data of two groups about operation information,lower limb wound complication,bridge blood vessels patency rate,and psychologic status.ResultsThe date of wound surface size,wound healing and appearance,wound infection rate,the second debridement rate and the overall lower limb wound complication rate show that EVH was better than OVH.After one-year follow-up,we contrasted the patency rate of bridge blood vessels between two groups.The patency rate of arterial bridge blood vessels was 96.8% ( venous bridge blood vessels was 85.7% )in group of EVH when we followed up 63 patients;while the patency rate of arterial bridge blood vessels was 94.9% ( venous bridge blood vessels was 86.4% ) in group of OVH that we followed up 59 patients.We compared the outcome between two groups,and found that there were no statistical significance in angina recurrence rate,patency rate of venous graft and patency rate of arterial graft.But the psychologic status was different,the EVH group was better than OVH group.ConclusionCompared to OVH,EVH technique has more advantages.Through taking a more secure method in the collection process of the bridging vein material,EVH group also maintained a satisfactory one-year graft patency rate.
