3.Advances in molecular targeted therapy of adenocarcinoma of the esophagogastric junction

Adenocarcinoma of the esophagogastric junction is a special type of tumor,not only in the special location,but also in the biological characteristics and clinical manifestations are unique.Surgery is the main treatment,but surgery alone sometimes result in poor prognosis,as a new direction of cancer treatment,Molecular targeting therapy is playing an increasingly important role.Some molecular targets such as vascular endothelial growth factor,hepatocyte growth factor receptor,epidermal growth factor receptor,fibroblast growth factor receptor 2,human epidermal growth factor receptor-2 in the treatment of esophageal gastric adenocarcinoma show a broad prospect.This review summarizes the current status and progress of molecular targeted therapy of adenocarcinoma of esophagogastric junction.
4.Effects of MyD88 and Trif on production of donor-specific antibodies and memory T cells in mouse cardiac transplant model

Objective To determine the roles of MyD88 and Trif,critical adaptor proteins for TLR signaling,in production of donor-specific antibodies (DSA) and memory T cells in a presensitized mouse cardiac transplant model.Methods Skin grafts from Balb/c mice were transplanted into either wild type B6 mice or B6 Myd88 and Trif double knockout mice (Myd88/Trif DKO).The recipients were subsequently transplanted heterotopically with cardiac grafts from the same donors two weeks after skin transplantation.Plasma DSA levels and spleen phenotypical analysis were performed prior to heart transplant or at time of cardiac rejection by using flow cytometry.Results Recipients presensitized with skin grafts developed accelerated cardiac allograft rejection in the absence of Myd88 and Trif.However,plasma DSA,especially IgG2,was significantly decreased (P<0.05) in Myd88/Trif DKO mice,compared to that in Wild Type mice at 2nd week after skin transplantation.The production of DSAs including all IgG subtypes was further reduced 3 days following heart transplantation in the Myd88/Trif DKO.In addition,MyD88/Trif DKO mice had impaired ability to generate memory T cells,as percentages of both CD44hi CD4+ and CD44hi CD8+ were significantly lower in the DKO than in Wild Type mice (P<0.01,P<0.05).Conclusion Simultaneous ablation of MyD88 and Trif in recipients significantly decreases the production of serum DSAs and spleen memory T cells following allogeneic skin and heart transplantation,supporting a crucial role of TLR signaling in adaptive immune responses in organ transplantation.
5.Detection of human cytomegalovirus UL97 gene mutations conferring ganciclovir resistance

Objective To explore human cytomegalovirus UL97 mutations related to ganciclovir resistance in hematopoietic stem cell transplant (HSCT) recipients.Methods A total of 43 patients,including 24 males and 19 females,with an average age of 21 years old,who had HCMV DNAemia for more than two weeks after HSCT between 2008 and 2010 in Peking University People's Hospital,were included in this prospective study.UL97 GCV resistance mutations were investigated in 49 plasma specimens collected from those patients.GCV resistance mutations such as UL97 M460V/I,H520Q,A591V,A594V,L595S/F,and C603W,were analyzed by modified PCR-RFLP methods.UL97 mutations related to GCV resistance were assayed by the method of PCR-direct sequencing (PCR-DS).An amplified refractory mutation system real-time PCR (ARMS RT-PCR) was developed for the detection of UL97 A594V mutation.Results Eight known UL97 ganciclovir resistance mutations were not detected by PCR-RFLP and PCR-DS.Four new UL97 mutations like UL97 R494P,T502A,N558D,and G561S,were detected by PCR-DS.The ARMS RT-PCR for detecting of UL97 A594V was established successfully.The lower limit of detection of the method was at least 7.5 × 102 copies/ml combined with the use of nucleic acid extraction reagent.UL97 A594V resistance mutation was identified by the method of ARMS RT-PCR in two HSCT recipients.The rate of UL97 A594V mutation was 4.7% (2/43) in HSCT recipients.Conclusion The ARMS RT-PCR assay represented a sensitive method for the identification of UL97 A594V mutation.

Objective:Rudimental contrast media in the subclavian vein often influence the display of carotid on dual-source computer tomography angiography.This article aimed to investigate this influence,and find a best solution.Methods:One hundred patients were randomly divided into Groups A(n=52) and B(n=48).The former received traditional imaging,with 65 ml contrast media,4.0 ml/s injection flow and caudocranial scanning direction,while the latter kept all the parameters unchanged except the scanning direction,from caudocranial to craniocaudal.Each group was graded by original axial images,maximum intensity projection(MIP),silhouette images and VR images for vascular delineation of the carotid.Results: The carotid artery was clearly displayed in Group B,while the subclavian vein was unsatisfactorily revealed in Group A,with statistically significant differences in extent scores between the two groups(P
7.Double tractors swing microendoscopic discectomy technique for multi-segmental lumbar disc herniation

Objective To evaluate the effect of double tractors swing microendoscopic discectomy technique in multi-segmental lumbar disc herniation. Methods From December 2006 to November 2009,153 patients with multi-segmental lumbar disc herniation were treated with double tractors swing microendoscopic discectomy. They included 85 cases of multi-segmental disc herniation, 53 cases of degenerative canal stenosis and 15 cases of lumbar instability. Among them, 2, 3, 4 and 5 fenestrations were performed in 105,33, 13 and 2 cases respectively and interbody fusion was done with "quanhe" inflation cage and screws in 15 cases. The results were evaluated with Macnab scale. Results All the 153 patients were followed from 3to 36 months, with an average of 16 months. The mean operative time was 45, 61, 83 and 110 min for 2,3,4 and 5 fenestrations respectively, with a mean blood loss of 150 ml. And it took extra 92 min to finish interbody fusion. Complications included dural sac tears in 1 case,canda equina slight lesion in 1, superficial incision infection in 1, the formation of deep venous thrombosis in 1, and revision for"quanhe"inflation cage in 1. The mean hospital stay was 10 days. Excellent results were obtained in 117 cases, good in 32 and fair in 4. One hundred and thirty-two patients returned to their work or normal activities in 3 weeks. One hundred and forty-nine cases were satisfied with the therapeutic effect. Conclusion This technique not only can reach adjacent intervertebral space easily but also disperse pressure on the nerve root effectively. This technique can provide thorough decompression and good results.
8.Effects of stromal cell-derived factor-1α on senescence of endothelial stem cells from peripheral blood

OBJECTIVE To investigate whether stromal cell-derived factor-1α (SDF-1α) might be able to prevent senescence of endothelial stem cell (ESC) and also study its effects on the telomerase activity. METHODS Total mononuclear cells (MNCs) were isolated from peripheral blood by density gradient centrifugation, and then the cells were plated on fibronectin-coated culture dishes. After cultured for 4 d, attached cells were divided into control and SDF-1α 1, 10, 50 and 100 μg·L~(-1) groups. ESC became senescent as determined by acidic β-galactosidase staining. The proliferation of ESC was assessed by MTT assay and colony-forming capacity. Telomerase activity was measured by telomerase-PCR ELISA and the phosphorylation of Akt was determined by using Western blotting. RESULTS Ex vivo prolonged cultivation of ESC led to rapid onset of ESC senescence. Compared with control group, SDF-1α concentration-dependently inhibited the onset of ESC senescence, maximum at 100 μg·L~(-1) (40.8±7.1 vs 17.5±3.0; P<0.01). Moreover, SDF-1α 100 μg·L~(-1) increased ESC proliferation (0.22±0.02 vs 0.39±0.04; P<0.01) and ESC colony-forming activity (7.8±2.2 vs 22.4±3.4). Compared with control group, SDF-1α 100 μg·L~(-1) also increased telomerase activity (0.34±0.05 vs 0.57±0.09; P<0.01). In addition, SDF-1α treatment of ESC stimulated a concentration- and time-dependent Akt phosphorylation. CONCLUSION SDF-1α-induced prevention of ESC senescence leads to the potentiation of proliferative activity, and clonal expansion, which may be related to the activation of telomerase and Akt phosphorylation.
9.Hepatic arterial infusion of antibiotics for the treatment of pyogenic liver abscess unsuitable for puncture drainage management

Objective To compare the clinical effect of hepatic artery perfusion of antibiotics with that of intravenous administration of antibiotics in treating pyogenic liver abscess which is unsuitable for puncture drainage management. Methods Between October 2010 and October 2013, a total of 32 cases with bacterial liver abscesses which were unsuitable for puncture drainage management were encountered at the Third Affiliated Hospital of Wenzhou Medical University. The patients were divided into group A (n = 15) and group B (n = 17). Hepatic arterial perfusion of antibiotics was carried out in the patients of group A, while intravenous administration of antibiotics was employed in the patients of group B. Percutaneous puncture drainage was adopted in the patients when their imaging examination showed liquefaction within the lesion. Results The hospitalization time, recovery time of body temperature, hemogram recovery time and liquefaction extent of the lesion in group A were much better than those in group B , and the differences were statistically significant (P < 0.05). Conclusion Hepatic arterial perfusion of antibiotics is an important and effective treatment for bacterial liver abscesses.
10.Diagnosis of intracranial aneurysms: accuracy of digital subtraction bone removal CT angiography and dual energy bone removal CTA

Objective To evaluate the diagnostic accuracy of digital subtraction bone removal CT angiography (DS-BR-CTA) and dual energy bone removal CTA (DE-CTA) compared with conventional and rotational DSA. Methods A total of 264 patients underwent 3D-DSA were enrolled, 190 of them underwent also DS-BR-CTA, while 74 underwent DE-CTA as well. Aneurysms detected with DS-BR-CTA and DE-CTA were analyzed compared with 3D-DSA on per-patient and per-aneurysm basis. The sensitivity, specificity, positive predictive value and negative predictive value of two methods for diagnosis of aneurysm were calculated. Results Taking DSA as the standard, the sensitivity and specificity of DS-BR-CTA to detect intracranial aneurysms was 96.90% and 98.36% on per-patient basis, while 97.37% and 99.97% on per-aneurysm basis, and the sensitivity and specificity for the detection of aneurysm ≤3 mm was 88.89% and 98.36%. The sensitivity and specificity of DS-BR-CTA to detect intracranial aneurysms was 98.25% and 100% on per-patient basis, 95.52% and 100% on per-aneurysm basis, and the sensitivity and specificity for the detection of aneurysm ≤3 mm was 90.91% and 100%. Conclusion DE-CTA has a comparable diagnostic accuracy with DS-BR-CTA to diagnose intracranial aneurysms, and can be used in the routine workflow for detection of intracranial aneurysms.
