1.A preliminary clinical study of polyester spacer-enhanced pancreatico-intestinal anastomosis in pancreaticoduodenectomy

Objective:To investigate the safety and utility of using polyester spacers in conventional pancreatico-enteric anastomosis in pancreaticoduodenectomy to achieve prevention of postoperative pancreatic fistula.Methods:The clinical data of 82 patients with pancreaticoduodenectomy completed by the same physician in Anyang People's Hospital from August 2018 to August 2023 were retrospectively analyzed, including 52 males and 30 females, aged (62.21±9.75) years. They were divided into two groups, test group ( n=40) and control group ( n=42) according to whether polyester spacers were used in pancreatico-intestinal anastomosis, the perioperative data of the two groups were compared, and Logistic analysis was used to analyze the risk factors affecting postoperative pancreatic fistula. Results:There was no statistically significant difference between the two groups in terms of biliary fistula, intestinal fistula, abdominal infection, abdominal bleeding, and postoperative mortality rate (all P>0.05). The incidence of postoperative pancreatic fistula in the test group was 20.0% (8/40) which was lower than that in the control group 45.2% (19/42), and the difference was statistically significant ( P=0.015). Soft pancreas texture ( OR=16.595, 95% CI: 1.891~145.657) was an independent risk factor for postoperative pancreatic fistula, while improved pancreatic enterostomy with polyester spacers ( OR=0.332, 95% CI: 0.114~0.969) could reduce the risk of postoperative pancreatic fistula. Conclusion:Use of polyester spacers to reinforce the pancreatico-enteric anastomosis during pancreaticoduodenectomy reduces the incidence of postoperative pancreatic fistulae with good safety and practicality.
2.The role of serum miRNA-30c in early diagnosis of gastric cancer

Objective To test the value of miRNA-30c(miR-30c)in early diagnosis of gastric cancer. Methods Serum miR-30c expression levels were detected by using quantitative real-time polymerase chain reaction in 80 patients with advanced gastric cancer, 35 patients with early stage gastric cancer and 35 healthy controls in the Affiliated People's Hospital of Inner Mongolia Medical University from August 2014 to August 2017. The receiver operating characteristic (ROC) curves and the area under the curve (AUC) were analyzed to test the efficacy of the miR-30c in distinguishing advanced gastric cancer, early stage gastric cancer and healthy controls. Results The expression level of serum miR-30c in advanced gastric cancer (0.45±0.11) was lower than that in early stage gastric cancer (0.54±0.15) (t = 5.2, P < 0.05). Compared with the healthy controls (0.61±0.12), miR-30c was down-expressed in the serum of early stage gastric cancer patients(t=6.7,P<0.05).Compared with the traditional tumor markers,the AUC of miR-30c was the biggest (0.92±0.03) in early stage and advanced gastric cancer groups, and the sensitivity and specificity in the diagnosis of gastric cancer were 90 % and 84 %, respectively. The AUC of miR-30c was 0.87±0.04 in early stage gastric cancer and healthy controls, and the sensitivity and specificity in the diagnosis of gastric cancer were 70 % and 86 %, respectively. Conclusion miR-30c might be used as a potential serum biomarker for the early diagnosis of gastric cancer.
3.Efficacy observation and imaging evaluation of arthroscopic repair of degenerative medial meniscus injury in middle-aged and elderly patients

Objective To observe the clinical effect of arthroscopic repair of isolated degenerative medi-al meniscus injury in patients over 45 years old and to evaluate the healing after more than two years of fol-low-up through MRI.Methods A retrospective study was conducted on the clinical effects of 51 patients with isolated degenerative medial meniscus injury who underwent arthroscopic repair from January 2016 to Decem-ber 2020.The meniscus was sutured by total internal suture or total internal suture combined with internal-to-external technique.IKDC score,Tegner score and Lysholm score were recorded before operation and at the last follow-up(at least 24 months).The type and location of meniscus injury during operation,and the microscopic classification and location of cartilage injury were recorded.At the same time,MRI was used to observe the healing of meniscus and ICRS cartilage injury grading.On MRI,0-2 degree injury was defined as meniscus healing,and 3 degree injury was defined as nonunion.The clinical repair failure was judged according to the Barrett evaluation criteria.Results The average age of the patients was(57.3±8.4)years,and the average follow-up time was(55.0±15.3)months.There were 47.1%complex fissures,31.3%horizontal fissures and 21.6%other types.The Lysholm score increased from 53.5±6.4 preoperative to 87.6±7.9 postoperative,the IKDC score increased from 33.6±4.7 preoperative to 72.8±5.3 postoperative,and the Tegner score increased from 1.0±0.1 preoperative to 4.0±0.5 postoperative.Three patients were judged as clinical repair failure,and five patients had progressed cartilage injury after surgery.At the last follow-up,the meniscus healing was observed on MRI:0 degree in two cases(3.9%),1 degree in 17 cases(33.3%),2 degree in 11 cases(21.6%),3 degree in 21 cases(41.2%).Conclusion For middle-aged and elderly patients with isolated de-generative medial meniscus injury,if conservative treatment is not effective or there are mechanical obstruc-tion factors,arthroscopic meniscus repair can achieve good clinical results and low clinical repair failure rate.
