1.Study on the Identification and Potency Determination of L-asparaginases from 2 Kinds of Strain

OBJECTIVE: To discuss the identification method of L-asparaginases prepared from E.coliASI.357 and Erwinia carotovora and the optimal enzymatic reaction conditions of potency determination.METHODS: HPLC method and isoelectric focusing electrophoresis were applied for the identification of L-asparaginase from 2 kinds of strain.The effects of category of buffer solution and pH value on enzymatic reaction of potency determination of L-asparaginase were investigated.RESULTS: HPLC chromatogram of L-asparaginases from E.coli ASI.357 was different from that from Erwinia carotovora.The retention time of the peaks were 11.0 min and 11.8 min.The isoelectric point (PI) of L-asparaginase produced from E.coliASI.357 was within 4.65~5.1 and that produced from Erwinia carotovora was within 7.1~8.20.The optimal enzymatic reaction conditions of potency determination of L-asparaginase produced from E.coliASI.35 were Tris-HCl (pH=9.0) as buffer and that produced from Erwinia carotovora was 0.2 mol?L-1 phosphate (pH=8.0) as buffer.CONCLUSION: The isoelectric point (PI) of L-asparaginases produced from 2 kinds of strain is different from each other as well as their optimal enzymatic reaction conditions of potency determination.The L-asparaginases from 2 kinds of strains should be controlled as 2 different categories.
2.SIGNIFICANCE OF EXERCISE ECHOCARDIOGRAPHY FOR EVALUATING THE LEFT VENTRICULAR PUMPING FUNCTION

To resolve the problem of methodology of exercise echocardiography andthe criterion and value for evaluating left ventricular (LV) pumping func-tion, 68 healthy persons (age 54.8?8.9 years) and 48 patients with coronary artery disease (age 57.6?5.7 years) were studied by echocardiography bothat rest and during bicycle ergometer exercise in the supine position. Mostsubjects (76.8%) having clear echocardiograms at rest also acquired cleartracing during exercise. Fifty percent of patients had abnormal responsesto exercise. There were no increase in ejection fraction (EF%), percentchanges of diameter (△D%) and stroke volume index (SVI). All these weresensitive indices for determining LV pumping function reserve. Six indicesincluding △D%, PEP/LVET, mVcf at rest, exercise, increment of EF%, △D%and SVI were used as comprehensive assessing criterion of LV pumpingfunction. Compared with the indices of the rest state, the abnormal ratioof LV pumping dysfunction found in patients were raised from 21% to 37%by the indices of the exercise increment and further to 56% by the compre-hensive assessing criterion. This was not in the case of healthy persons.It showed that exercise echocardiography and comprehensive assessing cri-terion might enhance the sensitivity of evaluating LV pumping functionwithout affecting its specificity. It is suggested that they might be adaptedin the research works in rehabilitation medicine and sports medicine.
3.Study on Thoracic Explosive Injury from Explosive Device

Objective To observe the characteristic, mechanisms and the change of pathophysiology about thoracic explosive injury from an explosive device. Methods Seventy-three millimetres prefabricated steel-ball bomb was used to be the center of explosion. According to the distance from each group to the center of explosion, 30 sheep were randomly divided into 5 groups. The injuries of thoracic wall, lung and surrounding organs were observed from fragments and blast wave. Results The mortality of thoracic explosive injury on the ground testing was 56.7%.Most of fragment wounds were blind wound. The track of wounds was often complicated and curved. The major associated injuries were myocardial contusion, splenic-hepatic rupture and gastric-enteric rupture. There were much severe pulmonary hemorrhage and lung edema. The parameters of trauma were well correlated with the severity of thoracic injury. Conclusion The parameters of trauma were well correlated with the severity of thoracic injury. The characteristic of thoracic injury from explosive device was more complicated and severe.
4.Images change of pulsed Doppler and tissue Doppler in evaluation of right ventricular diastolic function in patients with essential hypertension

ObjectiveTo assess right ventricular diastolic function in patients with essential hypertension using tricuspid valve flow spectrum and tricuspid annulars motion dectected by echocardiography. MethodsAccording to Ganau′s categories, 124 patients with essential hypertension were enrolled,which were divided into four groups:normal left ventricular configurational group (group Ⅰ, n =32 ), centripetal reconstruction group (group Ⅱ, n =30), centripetal hypertrophy group ( group Ⅲ, n =32), eccentric hypertrophy group ( group Ⅳ, n =30 ). Control group consisted of 35 normal subjects pulsed wave Doppler(PW) data and tissue Doppler imaging(TDI) data were acquired from tricuspid and its' annulus respectively on apical four chamber view. ResultsEPA > 1 was observed in control group as well as group Ⅰ and Ⅱ ,while E/A < 1 in group Ⅲ and group Ⅳ ,there were significant differences between them. Em/Am < 1 was observed in all of patients with hypertension,there were significant differences among them( P < 0.05 ) .ConclusionTissue Doppler imaging was more sensitive than pulse Doppler in evaluating decreased right ventricular diastolic function in patients with essential hypertension, and the decrement of the function was different in degree based on variant left ventricular configurations.
5.Investigation and analysis of anxiety in relatives of patients with severe brain injury at acute period

Objective To investigate the anxiety in the relatives of patients with severe brain injury at acute period and discuss the effective solving pathway.MethodsA questionnaire-based survey was conducted in 120 relatires of patients with severe brain injury during March 2006 to March 2007 by using state-trait anxiety inventory (STAI).Results Comparison of anxiety score between relatives of patients and normals,the scores of state anxiety and trait anxiety in the relatives of patients[(52,40±8.42),(46.73±7.56)]were significantly higher than those in normal people[(38.97±8.45),(41.31±7.54),P<0,01].And between relatives of man and woman,the Scores of state anxiety and trait anxiety in the relatives of patients in female were significantly higher than those in male ones(P<0.01).The scores of state anxiety and trait amv,iety in the 51~65 old group were significantly higher than ihosein 31~50 old group and 17~30 old group(P<0.01).The scores of state anxiety and trait anxiety in the little educated group were significantly higher than that higher educated group(P<0.05,P<0.01),and higher in those having self-medical service than in those having public medical service.The scores for,mate is signkfigantly higher than that other roles(all is P<0.05).Conclusion The relatives of patients with severe brain injury have anxiety of different degrees.Medical staff should pay attention to the relatives anxiety,and so to better the relationship of medical staff and patient
6.Relapse Factors of Acute Central Nervous System Inflammatory Demyelinating Disease in Children

Objective To explore influential factors and features of relapse of presenting with an initial episode of acute central nervous system inflammatory demyelinating disease(IDDCNS) in children.Methods By group-control study,20 relapsing patients with IDDCNS compared with 50 single episoding patients with IDDCNS whose case history longer than 0.5 year,whose age at onset,gender,preceding infection or immunization,neurologic syndromes at presentation,therapeutic response,and final neurologic outcome after initial episode were analyzed by mono-factor analysis and multi-factor nonconditional Logistic regression model.Results 1.The risk of relapse increased in the near future,75.0% relapsed before 6 months.There were 18 cases diagnosed multiple sclerosis(MS),1 case neurooptic myelitis and 1 case immaturity myelitis in relapsing patients.There were 39 cases diagnosed acute disseminated encephalomyelitis in single episoding patients.2.Mono-factor analysis:8 factors were not statistically significant,including age at onset,gender,seizure,palsy,conscious disturbance,ataxia,natural relief,sequelae after the first attack;6 factors were statistically significant,including infectious or vaccination agents before episode,fever,visual disorder,multiple neurological signs,positive pathologic reflex,with marked improvement to cortical hormone.3.In the multi-factor stepwise regression analysis,visual disorder(OR=42.50)and positive pathologic reflex(OR=8.666)were the independent influencing factors,multiple neurological signs(OR=0.068)was independent protecting factor.Conclusions The rate of relapse in IDDCNS is lower than abroad and most relapse before 0.5 year,the main diagnosis of relapsing case is MS.When visual disorder and positive pathologic reflex presenting in the fist episode of IDDCNS,the opportunity of relapse will increase,and will decrease when multiple neurological signs presenting.
7.Immune suppression induced by auto-keratinoc-ytes in intermingled skin grafting

Aim To explore the mechanism of the immune tolerance induced by the auto-islets in the intermingled skin grafting. Methods The mixed epidermal cell and lymphocyte culture (MELC) was established, in which auto-keratinocytes were added to mimic the effect exerted by the auto-islets in vivo. Results Auto-keratinocytes could inhibit the lympho-proliferation induced by alloantigen. Conclusion The immune tolerance induced by auto-islets may be related to the function of auto-keratinocytes as antigen presenting cells.
8.The anti-oxidant effect and the possible mechanism of plcroside Ⅱ in cerebral ischemia/reperfusion injury in rats

Objective To investigate the anti-oxidant effect and the possible mechanisms ofpicrodide II in cerebral ischemia/reperfusion injuries in rats. Methods A total of 90 adult, healthy, mmale Wistar rats were used to established the middle cerebral artery occlusion reperfusion (MCAO/R) models by intraluminal monofilament suture on the left external-internal carotid artery. The treatment group and the positive control group were respectively injected with 1.0% picroside II (10 mg/kg, 250 μl) and salvianic acid A sodium (10 mg/kg, 250 μl) via the tail vein, and the negative control group and sham-surgery group were injected with 0.1mol/L phosphate buffer saline (PBS) 250 μl. The neurological deficit scores were evaluated with Bederson's test. The cerebral infarction volume was observed with tetrazolium (TTC) staining. The apoptosis positive cells were counted by terminal deoxynucleotidyl transferase dUTP nick-end labeling and the expressions of inducible nitric oxide synthase (iNOS) and superoxide dismutase (SOD) were detected with immunohistochemical assay.The concentration of iNOS and SOD proteins in brain tissue was detected by enzyme linked immunosorbent assay.Results Neurological behavioral malfunction appeared in all the rats with MCAO/R. The infarction focuses emerged in the ischemic hemisphere following the MCAO/R injuries. The number of apoptotic cells and the expression of iNOS increased while the SOD reduced after MCAO/R. After the treatment of picrodide Ⅱ, the nervous behavioral function (1.28±0.38)improved, the infarction volume(68.73±4.46)% reduced, the number of apoptosis positive cells(6.10± 1.26), the expressions and the concentrations in brain tissue of iNOS(4.67+0.51)decressed while those of SOD (0.53 ±0.14) increased significantly compared with the negative control groups(t=3.16、 2.51、 4.15、3.12、 3.25, P<0.05). Conclusion PicrodideⅡ might play a neuroprotective effect by inhibiting the neuronal apoptosis and the expressions of iNOS and SOD after cerebral ischemia/reperfusion injuries.
9.Value of percutaneous transhepatic cholangiobiopsy in the diagnosis of obstructive jaundice

Objective To investigate the value of percutaneous transhepatic cholangiobiopsy (PTCB) in the diagnosis of obstructive jaundice.Methods The clinical data of 826 patients with obstructive jaundice who received PTCB at the First Affiliated Hospital of Zhengzhou University from April 2001 to December 2011 were retrospectively analyzed.The pathological results,positive rates of PTCB and complications were analyzed.The safety and efficacy of PTCB and the pathological features of malignancy causing obstructive jaundice were summarized.The difference in the positive rates of PTCB for biliary and non-biliary malignancies was analyzed by chi-square test.Results A total of 826 patients received PTCB,and the success rate was 100%.Eighty-six patients had complications postoperatively,including transit bilhaemia in 47 patients,bile leakage in 11 patients,temporary biliary hemorrhage in 28 patients,no severe complications occurred.There were 740 patients were with malignant biliary stricture and 86 with benign biliary stricture.Seven Hundred and twenty-seven patients were with positive results of PTCB (641 were with cancerous stricture and 86 with inflammation of biliary tract or fibrogenesis),and 99 patients were with false negative results.The overall positive rate of PTCB was 88.01% (727/826).Malignant neoplasm accounted for 89.59% (740/826) of the factors causing obstructive jaundice,and well-,moderate-and poor-differentiated neoplasms were accounted for 57.88% (371/641),19.97% (128/641) and 22.15% (142/641).Biliary adenocarcinoma was the main pathologic type,which was accounted for 96.41% (618/641).The positive rates of PTCB for biliary and non-biliary neoplasms were 89.50% (469/524)and 79.63% (172/216),with significant difference (x2 =12.87,P < 0.05).Conclusions PTCB is a safe,feasible and easy way to diagnose obstructive jaundice.Biliary neoplasms are the best indications for PTCB.Well differentiated neoplasm is the main pathological type causing the obstructive jaundice.
