1.Effect of Clinical Pharmacists' Intervention on the Quality of Life of Patients with Type 2 Diabetes Mellitus

OBJECTIVE:To explore the approach to improve the target-meeting rate in patients with type 2 diabetes.METHODS:A total of 116 patients with type 2 diabetes were randomly assigned to either trial group (with intervention of clinical pharmacists;n=60,19 males and 41 females) or control group(with routine therapy;n=56,20 males and 46 females).The levels of fasting plasma glucose (FPG),HbA1c,BP,HDL-C,TG,LDL-C and the SF-36 scores for quality of life in both groups were recorded at 0 and 3 months,respectively.The target-meeting rates after treatment were computed in accordance with China Diabetes Control Guideline in which the target value of HbA1c control was no more than 7%.RESULTS:After three months,the target-meeting rates of FPG,HbA1c,BP,HDL-C,TG,LDL-C and the SF-36 scores in the trial group were significantly better than in the control group.CONCLUSION:Intervention of clinical pharmacists is a effective approach to improve the quality of life of patients with type 2 diabetes.
2.Analysis of amygdalin of Ephedra spp.and Apricot Kernel Cough Lozenge by HPLC

Objective: Assay of amygdalin of Ephedra spp. and Apricot Kernel Cough Lozenge (ephedrine hydrochloride, Semen Armeniacae Amarum, etc.) by HPLC. Methods: Amygdalin was extracted with methanol. The separation was performed by HPLC and amygdalin was determined with UV detector. Results: The contents of amygdalin of Ephedra spp. and Apricot Kernel Cough Lozenge was 0.476% and 1.53mg per lozenge. Conclusion: The method was simple, accurate, rapid and with good reproducibility. It can be used for quality control of Ephedra spp. and Apricot Kernel Cough Lozenge.
3.On the Optimization of Extraction Conditions of Tetrandrine and Demethyltetrandrine from Fourstamen Stephania (Stephania tetrandra)

Several factors influencing the extraction of tetrandrine and demethyltetrandrine from Chinese medicinal herbs, Stephania tetrandra S. Moore were studied by means of Ortogonal method. The optimum Condition was eight hours steeping, in 33.3% ammonia in absolute alcohol for four hours under ultrasonic agitation. Tctrandrine aud demethyltetrandrino were detected by HPLC. The contents were 0.678% and 0.616%, the detection limits were 3.87?10-3and 0.48?10-3 mmol/L respectively.
4.Moxibustion Improved Transcutaneous Oxygen Tension and Exercise Capacity in Lower Limbs of Peripheral Arterial Disease.

OBJECTIVETo observe the effects of moxibustion and treadmill exercise on transcutaneous oxygen tension and exercise capacity in lower limbs of peripheral arterial disease (PAD).
METHODSTotally 58 mild-to-moderate PAD patients were assigned to the control group (18 cases), the moxibustion group (20 cases), and the treadmill exercise group (20 cases) by random digit table. Patients in the control group received conventional drug therapy for 12 weeks. Patients in the moxibustion group and the treadmill exercise group additionally received moxibustion [at Zusanli (ST36), Sanyinjiao (SP6), Yongquan (KI1)] and treadmill exercise respectively, once per day, 5 times per week for 12 weeks in total. Ankle-Brachial Index (ABI) , transcutaneous oxygen tension (TcPO₂), 6-min walking test (6MWT), and walking impairment questionnaire (WIQ) were assessed before and after treatment.
RESULTSCompared with the control group and the same group before treatment, there was no statistical difference in ABI in the moxibustion group and the treadmill exercise group (P > 0.05). But TcPO₂, 6MWT, and WIQ were obviously elevated (P < 0.01). Besides, 6MWT and WIQ assessment of the treadmill exercise group were better than that of the moxibustion group (P < 0.01) after intervention.
CONCLUSIONMoxibustion and treadmill exercise could improve the exercise capacity and TcPO₂of lower limbers in PAD patients.

5.Fillet flap and postoperative pressure combined with magnet pieces and triamcinolone acetonide injection therapy for keloid in aural region

Objective To investigate the effect of Fillet flap and postoperative pressure combined with magnet pieces and triamcinolone acetonide injection therapy for keloid in aural region.Methods From January 2009 to January 2011,87 patients (102 side ears) with keloid at the aural region were treated.Fillet flap repaired for keloid in aural region,pressure treating by magnet pieces began at 14 days after operation,and continued for more than one year.Triamcinolone acetonide injection therapy began 1 months after operation if keloid recurrence tendency.Results After operation,80 patients (94 lateral ears) healed by first intention.Hematoma under flap appeared in 7 patients (8lateral ears),and healed by changing dressings.And 87 cases (102 lateral ears) were followed up for 1 year.The effect of operation and pressure therapy group and operation,pressure and injection therapy group was significantly higher than operation group,with statistically significant differences.Effect of operation,pressure and injection therapy group was higher than that of operation and pressure therapy group,but there was no significant difference.Conclusions Fillet flap and postoperative pressure combined with magnet pieces and triamcinolone acetonide injection therapy can effectively treat aural region keloids.

In recent years, tumor has become one of the major diseases that endanger human health. It is of great significance to explore the pathogenesis of tumors in the prevention, diagnosis, treatment and prognosis of tumors. microRNA (miRNA) is a class of small non-coding RNA, which is involved in post transcriptional processing, epigenetic inheritance, cell growth and individual development and other important life activities. miRNA, charactered by oncogenes and anti-oncogenes function, participates in the development, invasion and metastasis of tumors. Telomeres, located at the end of chromosomes, are closely related to the malignant transformation of cells and the aging of human. To understand the pathogenesis of tumors and to provide a novel direction for the diagnosis, treatment and prognosis of tumors, the role and the relationship of miRNA and telomere in tumors are reviewed in this paper.
9.Experience of total parenteral nutrition during perioperative period in abdominal surgery

53 cases who underwent major and moderate abdominal surgery were administered total paraenteral nutrition(TPN) by double energy resources and "all in one" irffusion way since December 1998. Non-protein energy was 105~150KJ/kg·d. Nitrogen was 0.16 ~0.35g/kg·d, Fat was 30%~40% of total energy. Serum protein of the cases were approximately or exceeded preoperative level. Body weight remained stable or increased (0.5~3kg). Positive nitrogen equilibrium was achieved 5~7d postoperatively. TPN improved the patients nutritional status, facilitated postoperative recovery and reduced the occurence of complications. We have the experience: (1) Appropriate nutrition support helps the critical patients of abdominal surgery tide over the long critical course and enhance curative rate. (2) It is necessary for the patients of obvious malnutrition and poor tolerance for operation to receive nutritional support for 5~7d. Postoperative TPN support should not be less than 1 week. (3)Infusion of nutritional ingredients should follow the principle of "metabolic support". (4)TPN can improve not only nutritional condition and immunity,but anticaneerous function of patients with gastrointestinal tract tumor combined with chemotherapy. (5)TPN support can reduce intestinal fluid leakage, facilitae fistua healing and prevent infection for patients with intestinal fistula. (6)TPN support can supply appropriately body needed nutrition and reduce exocrine secretion of pancreas for patients with severe pancreatitis.
10.UPLC-MS/MS determination of content of three iridoids of xingnaojing oral preparation in rat brains and study on their brain pharmacokinetics.

To establish a UPLC-MS/MS method for the simultaneous determination of geniposide, genipin 1-O-beta-D-gentiobioside and geniposidic acid in rat brains and study the brain pharmacokinetics of the three iridoid glycosides in stroke rat after the oral administration of Xingnaojing. In this experiment, brain samples were precipitated with protein for twice. Acquity BEH C18 column was adopted, with acetonitrile-0.1% formic acid-water as the mobile phase for gradient elution. ESI source was adopted for mass spectra; multiple reaction monitoring (MRM) was conducted to detect negative ions. The time for sample analysis was 3.5 min. the results showed good linear relations among the three iridoid glycosides, with the extraction recovery between 99.6% and 114.3%, good intra- and inter-day precisions and accuracies and stability in line with the requirements. The t1/2 and MRT in the three components were similar in brains of stroke rats. Geniposide and genipin 1-O-beta-D-gentiobioside showed double peaks; where as geniposidic acid showed a single peak. In conclusion, the method is so specific, sensitive, accurate and reliable that it can be used to study the brain pharmacokinetics of Xingnaojing oral preparation.
