1.The correlation between estrogen and diabetic retinopathy of the postmenopasual patients

Objective To observe the correlation between postmenopausal estrogen levels and diabetic retinopathy (DR) in women.Methods Thirty-nine menopause female patients with type 2 diabetes mellitus and 17 menopause subjects (control group) were enrolled in this study.Control subjects aged from 53 to 82 years,with the mean age of (69.80± 8.32) years.Diabetes mellitus patients aged from 56 to 84 years,with the mean age of (70.50±8.27) years;diabetes duration ranged from 3 to 23 years,with the average course of diabetes (11.40± 7.97) years.DR diagnosis was according to the results of fundus fluorescein angiography,and thus the 39 patients were divided into DR group (19 patients) and non-DR (NDR) group (20 patients).There was no significant difference in age and menopause duration between the three groups (t=0.347,0.485;P>0.05).There was significant difference in diabetes course (t=2.748,P<0.05).Compared with NDR group,fasting blood glucose (FBG),glycosylated hemoglobin (HbAlc),total cholesterol (TC),triglyceride (TG),low density lipoprotein cholesterol (LDL-C) were significantly increased (t=6.130,5.322,4.574,2.426,4.033),high density lipoprotein cholesterol (HDL-C) was significantly lower (t=3.917),the difference was statistically significant (P<0.05).The level of estradiol (E2) was measured by radioimmunoassay.The differences of E2 levels between the three groups were compared.Logistic regression analysis was used to analyze the influencing factors of DR.Results The levels of E2 in control group,DR group and NDR group were (42.38 ±8.64),(21.49 ± 9.81) and (32.72 ± 10.51) pg/ml,respectively.The level of E2 in DR group was significantly lower than that in NDR group and control group (t=3.443,10.110;P<0.05).Logistic regression analysis showed that the duration of diabetes mellitus [coefficients =0.166,odds ratio (OR)=1.181,P=0.016],FBG (coefficients=1.162,OR=4.014,P=0.001),TC (coefficients=3.212,OR=10.820,P=0.002),TG (coefficients=1.649,OR=5.203,P=0.030) and LDL-C(coefficients=1.605,OR=4.976,P=0.003) were the risk factors for DR;E2 (coefficients=-0.100,OR=0.904,P=0.004) and HDL-C (coefficients=-4.460,OR=0.012,P=0.002) were the protective factors for DR.Conclusion The estrogen level of postmenopausal women have a certain correlation with the development of DR,it may be one of the protective factor of DR.
4.Effects of Blood-cooling and Blood-stasis-removing Decoction on Autoantibody and Serum Levels of Interleukins 6, 17, 21 in MRL/lpr Experimental Rats

Objective To observe the effect of blood-cooling and blood-stasis-removing Decoction on anti-double-strain DNA ( anti-dsDNA) antibody, anti-nucleosome antibody ( AnuA) and serum levels of interleukin ( IL) -6, 17, 21 in spontaneous systemic lupus erythematosus ( SLE) MRL/lpr experimental rats. Methods The experimental rats were divided into blank control group, model group, and high- and low- dose Chinese medicine groups ( 25.2, 12.6 g/kg respectively) , the treatment lasting 8 weeks. At the end of the experiment, the blood taken from the orbital veins was separated for obtaining serum, and then the serum anti-dsDNA antibody, AnuA, IL-6,17,21 levels were tested by enzyme-linked immunosorbent assay ( ELISA). Results The serum autoantibody and IL-6, 17, 21 levels of rats in the model group were increased significantly as compared with the blank control group ( P<0.01). The serum antibody and cytokine levels of Chinese medicine groups were reduced as compared with the model group, the difference between high-dose Chinese medicine group and model group being significant ( P<0.01). Conclusion Blood-cooling and blood-stasis-removing Decoction has certain effect on reducing the serum levels of anti-dsDNA antibody, AnuA, and IL-6,17,21, which may coniribute to one of its therapeutic mechanisms for SLE.
5.Applicated effect of systematic nursing intervention in patients with severe pneumonia

Objective To discuss the applicated value of systematic nursing intervention in patients with severe pneumonia.Methods 100 patients with severe pneumonia were randomly divided into the study group and the control group,50 cases in each group.The control group was given routine nursing measures.The study group received systematic nursing intervention on the basis of routine nursing measures.The hospitalization time,satisfaction degree, treatment compliance and mentality were observed.Results The SAS scores of the study group before and after nurs-ing were (56.67 ±5.99)points and (38.92 ±5.31 )points,there was statistically significant differences between before and after treatment (t =9.235,P <0.05).The SDS scores of the study group before and after nursing were (62.58 ±6.14)points and (41.15 ±5.26 )points,there was statistically significant difference (t =8.623, P <0.05 ).The hospitalization time ,satisfaction degree and treatment compliance of the study group were (32.9 ± 6.6)days,(8.21 ±1.29)days,6.00%,86.00%,74.00%,which were significantly lower than (45.1 ±10.2)days, (13.56 ±1.77)days,16.00%,52.00%,48.00% of the control group (t =5.254,3.829,χ2 =5.236,8.292, 8.1561,all P <0.05 ).Conclusion Hsystematic nursing intervention can significantly improve the postoperative anxiety and depression,and improve the quality of life of patients with severe pneumonia.
6.Association between platelet-derived growth factor-B and in-stent restenosis in elderly patients

Objective To investigate the association of platelet-derived growth factor(PDGF)-B gene single nucleotide polymorphism(SNP) and plasma PDGF-BB level with in-stent restenosis(ISR) in elderly patients.Methods 157 patients who had undergone coronary artery stenting for more than half year were divided into ISR group(n=74) and NISR group(non-ISR,n=83) according to the angiographic diagnosis of in-stent restenosis (ISR).Plasma level of PDGF-BB was measured by enzyme-linked immunosorbent assay(ELISA).DNA was isolated from leukocytes.Two SNPs of the PDGF-B gene(rs1800818 and rs1800817) were determined by Taqman Quantitative Real-Time PCR with TaqMan-MGB probe.Results There were no significant differences in genotype frequency of rs1800818 AA,AG,GG between ISR group and NISR group(x2 =4.48,P>0.05).The frequency of rs1800818A allele was much higher in ISR group than in NISR group(x2 =5.33,P<0.05).There were no significant differences in genotype frequency of SNP rs1800817 AA and AC(x2 =0.06,P> 0.05) and allele frequency of SNP rs1800817 A and C(x2 =0.06,P>0.05) between ISR group and NISR group,while genotype CC was not found.The plasma level of PDGF-BB was higher in ISR group than in NISR group [(6.53±3.65) ng/L vs.(5.07±2.45) ng/L,t=2.92,P<0.01].Plasma level of PDGF-BB in patients with rs1800818 AA genotype was significantly higher in ISR group than in NISR group [(9.94 ± 4.60) ng/L vs.(5.90 ± 2.98) ng/L,t =2.69,P<0.05].Multivariable logistic regression analysis showed that plasma PDGF-BB level was the risk factor for ISR (OR =1.187,95.0% CI:1.054 1.337,P<0.01).Conclusions High plasma PDGF-BB level is the risk factor for ISR,but PDGF-B gene SNPs rs1800818 and rs1800817 are not associated with the occurrence of ISR.
7.Dead Reasons and Related Factors of 42 Cases with Acute Leukemia in Chinldren

Objective To explore the dead reasons and related factors of acute leakemia(AL)in children.Methods The clinical data of 42 dead cases of children with AL,who were admitted to Guangzhou Children's Hospital between Jan.1999 and Mar.2007 were reviewed.Nineteen cases were boys,23 cases were girls;the aging from 4 months to 12 years old and 5 months.Of 42 children,21 cases were acute lymphoblastic leukemia(ALL),18 cases were acute myeloid leukemia(AML),3 cases were unclassified AL.Results Ten out of 12 early-dead cases died of bleeding.Among 30 cases who received chemotherapy,4 cases died of bleeding,others of myelosuppression combined with infection,14 of whom died from sepsis,3 cases with fungal infection,1 case with virus infection,4 cases with unknown pathogens.Sixteen children relapsed among those who got remission,half of them relapsed within 1 year since diagnosed.Treatment-related mortality(TRM)was 90%(27 out of 30 cases).The main causes of TRM were infection and hemorrhage.Hyperleukocytosis and bleeding were 2 risk factors related to early death in new-diagnosed children.Conclusions More emphasizes should be put on bleeding tendency during early treatment in children with AL and treatment of infection complications during induction chemotherapy to reduce TRM.
8.Clinical research of limbal relaxing incision during implantable collamer lens surgery

To evaluate the efficacy and safety of limbal relaxing incision ( LRl) for correcting corneal astigmatism during implantable collamer lens ( lCL) surgery.METHODS:A total of 185 eyes of 105 patients with high myopia and corneal keratometric astigmatism were included in the study. lCL surgery with concomitant relaxing incision was performed in 105 eyes of 60 patients in LRls group ( Group A) . Eighty eyes of 45 patients only underwent lCL surgery were in control group ( Group B) . All patients undergone ophthalmic examination that included uncorrected visual acuity ( UCVA ) , best -corrected visual acuity ( BCVA ) , Pentacam analysis system to observe the changes of corneal astigmatism before and 1wk, 1 and 3mo after surgery.RESULTS: Respectively comparing UCVA between two groups in 1 and 3mo postoperatively, the P values were considered statistically significant ( P < 0. 05 ). But, respectively comparing BCVA between two groups in 1wk, 1 and 3mo postoperatively, the P values were considered no statistically significant ( P > 0. 05 ). Preoperative corneal astigmatism was 1. 52 ± 0. 55D in group A and 1. 48 ± 0. 57D in group B, there was no statistically significant difference (P>0. 05). One week postoperatively, the astigmatism was 0. 55 ± 0. 41D in group A and 1. 20 ± 0. 48D in group B. One month postoperatively, the astigmatism was 0. 60 ± 0. 38D in group A and 0. 93 ± 0. 47D. Three months postoperatively, the astigmatism was 0. 51 ± 0. 32D in group A and 0. 96 ± 0. 40D in group B. The difference between the two groups were statistically significant ( P<0. 05 ). The difference value of corned astigmatism before surgery and 1wk, 1 and 3mo after surgery had statistical significance ( P<0. 05). ln LRls group, at preoperative and postoperative time points, the average corneal astigmatism changes were also considered statistically significant difference (P<0. 05). CONCLUSlON: LRls performed during lCL surgery appeared to be an effective and safer procedure to reduce pre-existing corneal astigmatism and improve UCVA as well as the visual quality.
