BACKGROUND: Severe acute pancreatitis (SAP) can result in intestinal mucosal barrier (IMB) dysfunction. This study was undertaken to demonstrate the effect of IGF-I on the intestinal mucosal barrier in rats with SAP and its possible mechanisms. METHODS: Seventy-two male Wistar rats were randomly divided into three groups: a sham operation (SO group,n=24), a SAP group not treated with IGF-I (SAP group,n=24), and a SAP group treated with IGF-I (IGF-I group,n=24). SAP was induced in the rats by injecting 5.0％ sodium taurocholate into the biliary-pancreatic duct. The SO rats were given an infusion of normal saline instead. The rats in the IGF-I group underwent the SAP procedure and were given a subcutaneous injection of IGF-I at 30 minutes before the operation and at 3 hours after the operation. Eight rats in each group were sacrificed at 6, 12 and 24 hours after operation. Apoptosis of mucosal cells in the small intestine was determined by TUNEL. The levels of endotoxin and DAO and serum amylase were also measured. Pathologic changes in the small intestine were monitored. Changes of bax and bcl-2 mRNA expression in the small intestine were determined by reverse transcription polymerase chain reaction (RT-PCR). RESULTS: The levels of serum amylase were lower in the IGF-I group than in the SAP group at all three time points (P<0.05). The levels of endotoxin in the IGF-I group were higher than those in the SAP group at 6 hours, but lower in the IGF-I group than in the SAP group at 12 and 24 hours (P<0.05). The levels of diamine oxidase were higher in the IGF-I group at 6 hours but lower than those in the SAP group at 12 and 24 hours. The pathological score of the small intestine was lower in the IGF-I group than in the SAP group, and the difference was statistically significant at 12 and 24 hours. The pathologic changes observed under electron microscopy were better in the IGF-I group than those in the SAP group. The apoptosis index of intestinal epithelial cells was significantly decreased in the IGF-I group compared with the SAP group. Compared with the SO group, the mRNA expression levels of bax were increased at each time point in the SAP group, and were significantly decreased in the IGF-I group as compared with the SAP group at each time point (P<0.05). The expression levels of bcl-2 were weak and not different between the SO group and the SAP group (P>0.05). They were significantly increased in the IGF-I group versus the SO and SAP groups (P<0.05). The ratio of bax and bcl-2 mRNA expression levels at each time point in the SAP group were significantly higher than those in the SO group, but they were obviously decreased in the IGF-I group. CONCLUSIONS: Exogenous IGF-I seems to protect mucosal cells in the small intestine against SAP-induced apoptosis and could alleviate SAP-induced injury of the intestinal mucosa. The underlying mechanisms include enhanced mRNA expression of bcl-2 and inhibition of bax mRNA expression.

Anti-cancer targeted therapy “(targeted” cancer therapy) is the use of small molecule drugs or antibodies themselves, guided by specific targeting tools, to attack tumor cells and their microenvironment or activate the body’s immune response. The rapid development of targeted therapy has enabled specific anti- tumor therapies to be achieved, which has taken us another step closer to our ultimate goal of treating tumors, that is, to effectively kill tumors without harming normal tissues. This article reviews the related contents of tumor targeted therapy, such as tumor-associated antigens, antibodies, and targeting strategies and also elaborates on the research in this field in combination with the study of our team.

In an attempt to overcome the limitations of titanium in dental and orthopaedic clinical applications, a new method has been developed to prepare calcium carbonate coatings on sandblasted and acid-etched (SA) titanium implants. The purpose of this study was to investigate the effect of calcium carbonate-SA (CC-SA) implants on osseointegration in vivo. The surfaces of SA and CC-SA implants were characterised for surface morphology and surface chemistry. Subsequently, these two kinds of implants were implanted in the femoral condyles of rabbits. The implants were retrieved and prepared for histological and histomorphometric evaluation 1, 2, 4, 8 and 12 weeks after implantation. Significantly higher values of bone-to-implant contact of the entire implant except the gap area (BIC_ALL) and the bone-to-implant contact of the gap area (BIC_GAP) were found in animals with the CC-SA implants than in those with the SA implants at 4 weeks. Higher values of total gap bone were found in those with the CC-SA implants than in those with the SA implants at 1, 2 and 4 weeks. In conclusion, the current findings demonstrate that the calcium carbonate coating can improve and accelerate the early ingrowth of bone and osseointegration at the early healing phase. This may reduce clinical healing times and thus improve implant success rates.

OBJECTIVETo analyse the relationship between coping strategy and occupational stress in rail freight dispatchers.

METHODS115 rail freight dispatchers were investigated by using group sampling method, investigation contents included coping strategies, occupational stressors, strains and personalities.

RESULTSThe proportion of using coping strategy in rail freight dispatchers is lower. The scores of job future ambiguity, type A behavior and work locus of control in workers with insufficient coping strategy were higher than those in workers with sufficient strategy (P < 0.05 or P < 0.01), but the score of organization commitment score in workers with insufficient coping strategy was lower than those in workers with sufficient strategy (P < 0.05), the differences of scores of some occupational stressor, strain and personality variables between workers with insufficient and those with sufficient in social support, job-family balance, job involvement coping factors of coping strategy were remarkable significant (P < 0.05 or P < 0.01), the differences of scores of occupational stressor, strain and personality variables between workers with insufficient and those with sufficient in ask, logic and time management factors of coping strategy weren't significant (P > 0.05). Logistic regression analysis showed that risk of being job dissatisfaction and daily life stress in workers with insufficient social support coping was three or four times than those with sufficient coping (OR = 3.06 or 4.38, respectively), risk of being daily life stress in workers with insufficient job involvement coping was three times than those with sufficient coping (OR = 3.26).

CONCLUSIONThe proportion of using coping strategy in rail freight dispatchers is lower. Coping strategy has influence on the individual's perception of occuaptional stressors, strains and personalities.

The purpose of the present study was to explore whether substance P (SP) modulates the response mediated by GABAA receptors. Experiments were carried out on cultured hippocampal pyramidal neurons of rats. GABA-activated inward currents were recorded using the whole-cell-patch-clamp techique. The majority of the neurons examined (66/92, 72%) were sensitive to both GABA and SP. When the neurons were treated with SP prior to application of GABA, the GABA-activated current (IGABA) was inhibited markedly, which was concentration-dependent and could be blocked by spantide, an NK1 receptor antagonist. With 10－8, 10－7, 10－6 and 10－5 mol/L SP, IGABA was inhibited by 18%, 24.8%, 25.9% and 28% respectively. Intracellular application of H7, a potent inhibitor of PKC, abolished inhibition of IGABA by SP, suggesting that the inhibition of IGABA by SP may be a result of intracellular phosphorylation of the GABAA receptor.

This report documents the treatment of a giant aneurysm of the internal carotid artery bifurcation with a fistula to the cavernous sinus, which appeared following closed head trauma. A 39-year-old man suffered from a blunt head trauma in an automobile accident. Two weeks after the trauma, progressive chemosis of left eye was presented. Four months after the trauma, digital subtraction angiography showed an internal carotid artery bifurcation aneurysm, with drainage into the cavernous sinus. The lesion was successfully obliterated with preservation of the parent artery by using coils in conjunction with Onyx. Follow-up angiography obtained 3 months postoperatively revealed persistent obliteration of the aneurysm and fistula as well as patency of the parent artery. Endovascular treatment involving the use of coils combined with Onyx appears to be a feasible and effective option for treatment of this hard-to-treat lesion.

OBJECTIVETo explore clinical effect of manipulative reduction and percutaneous K-wires fixation in treating supracondylar fractures of the humerus in children.

METHODSFrom July 2010 to December 2012, clinical data of 52 children with supracondylar fractures of the humerus, which treated with manipulative reduction and percutaneous K-wires fixation, were retrospectively analyzed. Among them, there were 35 males and 17 females with an average age of 6.7 (ranged from 2.5 to 12) years old. All fractures were type Garland II - III fractures, and 51 cases were extension type and 1 case were flexion type. Flynn evaluation standard of elbow performance score were applied to evaluate clinical effects.

RESULTSAll patients were followed up from 12 to 18 months with average of 16 months. According to Flynn evaluation standard of elbow performance score, 41 cases obtained excellent result, 8 good and 3 moderate.

CONCLUSIONManipulative reduction and percutaneous K-wires fixation for the treatment of supracondylar fractures of the humerus in children has many advantages, such as minimally invasive, rapid recovery, stable fixation. It could prevent osteofascial compartment syndrome, Volkmann Contracture and cubitus varus.

Objective To explore the protective effect of saponins in stems and leaves of Panax notoginseng onmyocardial ischemic reperfusion injury in rats. Methods The rats were divided into six groups (the sham operation group, model group, DAXXK group, PNSSL 40, 80, 160 mg·kg-1 group) and continuous oral administration for 7 days. The ratmodel of myocardial ischemic reperfusion injury (MIRI) was developed by ligation of the left anterior descending coronaryartery the electrocardiogram (ECG), myocardial infarct size, staining with hematoxylin and eosin were observed. Theactivities of LDH、CK、AST、ALT in serum and SOD、GSH-Px、MDA in cardiac muscle tissue were detected by kit.Results Saponins in stems and leaves of Panax notoginseng in each dose (40, 80, 160 mg · kg-1) group significantlyreduced the myocardial infarct size and improved the ECG on ST segment, the myocardial damage was obviously reducedfrom the pathological section. Stems and leaves of Panax notoginseng in a dose of 160 mg · kg-1 lowered the serumactivities of LDH、CK and AST, contents of SOD and GSH-Px in a dose of 80 mg·kg-1 significantly lowered. Conclusions The results indicates that PNSSL have the effect against myocardial ischemic reperfusion injury and that the mechanismof pharmacological action related to the improvement of ECG changes, the stability of cell membrane, eliminate oxygenfree radicals and the reducing of inflammatory infiltration.

OBJECTIVETo evaluate the efficacy and toxicity of Yanshu injection (a compound Chinese traditional medicine from Sophora flauescens Ait) combined with concomitant radiochemotherapy in patients with stage III nasopharyngeal carcinoma.

METHODSSixty patients with stage III nasopharyngeal carcinoma were randomized into Yanshu group and control group (n = 30, each). Patients in the Yanshu group received Yanshu injection in addition to intensity modulated radiation therapy (IMRT) and concomitant chemotherapy, and those in the control group were treated with IMRT and concurrent chemotherapy.

RESULTSThe 1-year, 2-year, 3-year and 4-year overall survival rates were 100%, 93.3%, 86.7%, 80.0% for Yanshu group, and 96.7%, 90.0%, 83.3%, 76.7% for the control group, respectively, with no significant difference between the two groups (P = 0.565). The 1-year, 2-year, 3-year and 4-year progression-free survival rates were 96.7%, 90.0%, 83.3%, 70.0% for Yanshu group, and 90.0%, 86.7%, 76.7%, 66.7% for control group, respectively, with no significant difference (P = 0.554). However, the reaction of mucosa of oral cavity, myelosuppression and thrombocytopenia in the Yanshu group were significantly lower than that in the control group (P < 0.05). The quality of life of the patients in the Yanshu group was significantly higher than that in the control group (P < 0.05).

CONCLUSIONSYanshu injection combined with radiochemotherapy in patients with stage III nasopharyngeal carcinoma show a good efficacy and can reduce the side effects of radiochemotherapy of nasopharygeal carcinoma, and improve the quality of life of the patients.
