2.Clinical application of perforator flap in cubital fossa to repair skin defect in fingers

Objective To discuss the clinical application and evaluate the effect of repairing finger injuries using the perforator flap in forearm cubital fossa.Methods From July,2012 to December,2013,8 cases of finger injuries with totaled defect area of 2.5 cm × 4.0 cm-5.5 cm × 7.0 cm were reviewed.Among them,6 cases had phalangeal fracture,7 cases had neurovascular injury and 4 cases combined with tendon injuries.Cubital fossa flaps based on the inferior cubital perforator of radial artery were transplanted to repair the defects.Results All of the 8 flaps survived.One of them experienced distal end necrosis of epidermis and 1 cm long wound dehiscence.But it was healed by dressing change.All of the 8 cases were followed up for an average of 11 months (range,6-20 months).Sensory quality of (S) + was present in all of the flaps with two-point discrimination ranging between 7.5 mm and 9.8 mm with an average of 8.5 mm.All flaps were seen with good appearance,texture and colour.Moreover,function and appearance in donor sites were satisfactory.Conclusion The perforator flap in forearm cubital fossa is a satisfying choice in repairing small skin defects in fingers and other paas in that it is fixed,easy to dissect and leaves small injuries in targeted area.
3.Clinical application of bilobed deep inferior epigastric perforator flap to repair degloving injury of hand

Objective To discuss the method and clinical effect of bilobed DIEP flap for repair of degloving injuries of the hand.Methods From June, 2013 to January, 2015, the bilobed DIEP flap were designed to repair 6 cases of degloving injuries of hand, of which the small leaf flap repaired separately defect of thumb, larger leaf flap repair 2nd-5th finger and palm and dorsum.The bilobed DIEP flap design were based upon anatomic study on deep inferior epigastric artery, superior epigastric artery, intercostal artery and its anastomosis.Results The largest area of main flap was 35 cm × 12 cm, and the deputy leaves of flap was 10 cm× 7 cm.All of the 6 flaps survived.One of them experienced distal end necrosis of epidermis.But it was healed by dressing changes.All of the 6 cases were followed up for average of 12 months (range, 6-20 months).All flaps were seen with good appearance, fully recovered protectve sensation and the active motion of the thumb was recovery.Moreover, the donor sites were closed directly and the appearance were satisfactory.Conclusion The bilobed DIEP flap is a satisfying choice in repairing degloving injuries of the hand in that it has reliable blood supply, can obtain the larger area of skin and leaves small injuries in targeted area.

5.Factors influencing neuropathic pain in patients with spinal cord injury

Objective To investigate the neuropathic pain (NP) accompanying spinal cord injury (SCI),and to analyze the related factors to provide references for its prevention and treatment.Methods Seventy SCI patients with NP were selected using the DN4 scale.Their age,gender,occupation,education level,monthly income,injury position,marital status and other data were surveyed using a general questionnaire.Their NP situation was surveyed using a simplified McGill pain questionnaire (SF-MPQ).Results The patients' average visual analogous scale (VAS) score was 4.37.Their average pain rating index (PRI) according to the SF-MPQ was 8.23,with the PRI-sensory and PRI-emotional components 5.23 and 3.00 respectively.The average degree of present pain intensity was 1.86,between mild pain and discomfort,and discomfort was the description most commonly used.The most common pain descriptor was prickling pain,followed by burning pain and bulge pain.85.7% of the patients felt that their pain had an adverse effect on their affective state,and exhaustion occurred more often than any other descriptive words.Univariant analysis showed that the degree of injury,education level,marital status,monthly income,family support and medication history were all factors correlated with NP perceptions.Multi-variate logistic regression analysis showed that being unmarried and severity of injury were independent protective factors against NP.No family support,no medication and low income were independent risk factors for NP.Conclusion The type of neuropathic pain varies in patients with spinal cord injury.The intensity of the pain is mostly at a medium level.The emotional state of most patients was affected.Neuropathic pain involves many factors.Being unmarried and severely injured are independent protective factors,while lack of family support,no medication,and having low income were independent risk factors.

7.Research on control system of brain-computer interface based on the alpha waves in EEG

A system design of brain-computer interface based on the alpha waves in human electroencephalography(EEG) is presented in this paper.With the effects on the alpha wave amplitudes of human eye's open and close involved in,the selection control of four direction targets can be performed on a computer screen.The system speed and accuracy rate are investigated through the experiments involving 5 subjects.It is shown that the system is easy to operate and needs no complex learning and biofeedback training.The studying results provide a good technical foundation for the development of BCI control panel and the realization of the system integration.It has the potential application for clinical engineering and is valuable for further research.
8.Research on selective vascular edge detection in coronary angiogram

A new method is developed to detect vascular edge-line in coronary angiograms.It firstly searches the path through vessel segment of interest,then detects vascular edges along profiles perpendicular to the path.The experiments demonstrate that the method performs well in coronary angiograms with less nearby or overlapping structures and branching vessels.
9.Effects ofShenqi YizhiGranules on Ability of Learning and Memory and Content of Aβ1-42 of Cerebral Tissue in 5XFAD Mice with Alzheimer’s Disease

Objective To study the effects ofShenqi YizhiGranules (SQYZ) on learning and memory and content of Aβ1-42 of cerebral tissue in 5XFAD mice with Alzheimer’s disease; To discuss its mechanism on improving learning and memory ability of 5XFAD mice.Methods Four-month-old C57BL?6 wild type mice were randomly divided into NS control group and SQYZ control group, and the 5XFAD mice were randomly divided into model group, SQYZ group and huperzine-A (HupA) group, 15 mice in each group. Each group were given same volume for gavage for 60 d. After treatment, the learning and memory ability were evaluated by nesting test, passive avoidance and Morris water maze test. The senile plaques and content of Aβ1-42, ionized calcium binding adapter molecule 1 and glial fibrillary acidic protein in cerebral cortex and hippocampus were detected by immunohistochemical staining and immunofluorescence, respectively.Results Compared with NS control group, the score of nesting test in model group significantly decreased; the step-through latency in passive avoidance was shortened and the escape latentcy in Morris water maze test was prolonged; the quantity of senile plaques and content of Aβ1-42 increased in cerebral cortex and hippocampus; the activation of glial cells significantly increased. In the SQYZ group, the above-mentioned indexes reached or approached the level of wild type control mice. The difference between SQYZ group and model group was statistically significant (P<0.05,P<0.01).Conclusion SQYZ improved learning and memory ability in 5XFAD mice, which may be related to reduction of senile plaques, inhibition of over activation in glial cells and reduction of content of Aβ1-42 in cerebral cortex and hippocampus.
10.Clinical diagnostic value of serum amyloid A in patients with lung cancer

Objective To evaluate the clinical value of SAA by detecting their expression levels in patients with lung cancer and the analysis of the relativity of SAA for early diagnosis.Methods There are 243 cases specimens obtained from lung cancer patients who were newly diagnosed and without any treatment in Zhejiang Cancer Hospital from April 2014 to June 2015.The 243 lung cancer individuals were 147 male, and 96 female, their ages ranged from 29 to 85 years, with an average age of 63 years.The distribution of pathological type was as follows:95 patients were adenocarcinoma, 102 patients were squamous carcinoma, and 46 patients were small cell carcinoma.The distribution of TNM staging systems was as follows: 59 patients in stage 1and stage 2, 54 patients in stage 3, and 130 patients in stage 4.While 179 cases physical examination as the control.There were 94 individuals male, and 85 individuals female.Their ages ranged from 26 to 86 years, with an average age of 61 years.By using latex enhanced immune turbidimetric method, serum SAA concentrations in patients with lung cancer and healthy controls were checked on the Hitachi-7600 automated chemistry analyzer ( Hitachi ).The comparisons of all analyses values between healthy controls and lung cancer were estimated by two independent samples nonparametric tests ( Mann-Whitney U).The association between SAA and lung cancer prognostic factors such as age, smoking status and metastasis, was evaluated by spearman correlation and multivariate analysis.Results The median and interquartile spacing of SAA concentration was 42.36 mg/L (9.35, 74.22) in lung cancer patients.While 24 mg/L ( 3.25, 21.45 ).The median level of SAA in lung cancer patients (42.36 mg/L) were significantly higher than in healthy controls (11.24 mg/L), and difference reached statistically significant (Z=-2.403,P=0.006).Nevertheless, there was no significant difference in SAA concentrations among the different pathological types(Z=-1.013, P=0.339), ages (Z=0.578, P=0.458) and gender(Z=0.726, P=0.246) of lung cancer patients.While the level of SAA in has smoking status (Z=-2.282, P=0.013) and distant metastasis (Z=-2.138, P=0.017) of lung cancer was higher.By drawing ROC curve, the cut off value of SAA in distinguishing lung cancer with healthy control was 14.48 mg/L.Meanwhile, the AUC was 0.811, the accuracy is 89.12% and the sensitivity was 88.73%.Serum concentration was positively related with smoking status ( r =0.331, P =0.018 ) and distant metastasis ( r =0.372, P=0.015 ) by Spearman correlation analysis .Conclusion Serum SAA concentrations may contribute to the auxiliary diagnosis of lung cancer, evaluate the clinical stage and distant metastases of lung cancer.
