OBJECTIVETo observe the effects of moxibustion and treadmill exercise on transcutaneous oxygen tension and exercise capacity in lower limbs of peripheral arterial disease (PAD).

METHODSTotally 58 mild-to-moderate PAD patients were assigned to the control group (18 cases), the moxibustion group (20 cases), and the treadmill exercise group (20 cases) by random digit table. Patients in the control group received conventional drug therapy for 12 weeks. Patients in the moxibustion group and the treadmill exercise group additionally received moxibustion [at Zusanli (ST36), Sanyinjiao (SP6), Yongquan (KI1)] and treadmill exercise respectively, once per day, 5 times per week for 12 weeks in total. Ankle-Brachial Index (ABI) , transcutaneous oxygen tension (TcPO₂), 6-min walking test (6MWT), and walking impairment questionnaire (WIQ) were assessed before and after treatment.

RESULTSCompared with the control group and the same group before treatment, there was no statistical difference in ABI in the moxibustion group and the treadmill exercise group (P > 0.05). But TcPO₂, 6MWT, and WIQ were obviously elevated (P < 0.01). Besides, 6MWT and WIQ assessment of the treadmill exercise group were better than that of the moxibustion group (P < 0.01) after intervention.

CONCLUSIONMoxibustion and treadmill exercise could improve the exercise capacity and TcPO₂of lower limbers in PAD patients.

Adjuvant and neoadjuvant therapy are the important therapeutic methods for rectal cancer.Neoadjuvant concurrent chemoradiotherapy and postoperative adjuvant chemotherapy play a crutial role in rectal cancer treatment.New chemotherapy drugs and targeted therapy drugs could further improve the therapeutic effect of rectal cancer.

AIM: To select optimum macroporous resin and adsorption conditions of crocin in Fructus gardeniae. METHODS: The content of crocin was determined by UV spectrophotometer with crocin Ⅰ as standard. Property of adsorption and desorption, adsorption dynamics and factors affecting the adsorption were investigated by static adsorption method. RESULTS: The macroporus resin of HPD series had the best performance; the adsorption quantily of HPD300,HPD450,HPD400、HPD600 was 100.60 、 91.15 , 100.95 、 72.27mg?g -1 , desorption efficiency 84.05% , 87.22% , 93.83% , 78.38% , time of adsorption equilibrium was 4,3,3,2 hours respectively; the purer the sample, the higher the adsorption quantity was; the adsorption quantity had no remarkable difference when pH was less than 8 and concentration of ethanol was less than 1.5% . CONCLUSION: HPD400 was the optimum macroporous resin; the appropriate adsorption conditions consisted of 15% alcoholic sedimentation after extraction in neutral solution.

Objective To elucidate the deleterious diabetogenic effect of tacrolimus on Chinese Han people.Methods According to patients' whole blood trough levels,the concentration of tacroli- mus was 5,15 and 25?g/L.Human islet cells cultured in vitro were exposed to various concentra- tions of tacrolimus for 72 h respectively,then stimulated by glucose at low (2.8 mmol/L) and high (16.7 mmol/L) levels.Glucose-stimulating insulin secretion during subsequent static incubation was measured using the ultrasensitive human insulin ELISA kit.The assessment of islet cell viability was studied with acridine orange (AO)-propidium iodide (PI).According to fasting plasma glucose,461 subjects were divided into normal glucosetolerance group (FPG＜5.55 mmol/L),impaired fasting glu- cose group (5.55 mmol/L＜FPG＜6.88 mmol/L) and posttransplant diabetes mellitus group (FPG≥6.89 mmol/L).A retrospective review of 461 non-diabetic kidney recipients completing at least 12 months of follow-up was performed to determine risk factors,incidence,and characteristics of post- transplant diabetes mellitus (PTDM).Results In vitro,lower tacrolimus concentration had not dele- terious effect on insulin secretion and viability of islet;high tacrolimus impaired?-cell survival as well as insulin secretion.Of the 461 patients with no history of diabetes at transplantation over the study period of 12 months,123 received tacrolimus and 338 received cyclosporine A (CsA).For patients receiving tacrolimus the cumulative incidence of PTDM was 13.8%,compared with 7.7% for pa- tients receiving CsA,showing no significant difference between them.The reduced tacrolimus level within the therapeutic window resulted in an increase in better glucose metabolism.Older age,family history of diabetes,acute rejection episodes and pre-transplant impaired fasting glucose were identified as risk factors for PTDM.Conclusion The diabetogenic effect of TAC is dose-dependent,and in the majority is reversible.Low initial doses,more rapidly tapering and lower maintenance levels of tacroli- mus decrease the incidence of PTDM in patients receiving tacrolimus.

[Objective]To explore the treatment method of Spinal tuberculosis with focus-healed kyphosis and compression of spinal cord in thoracolumbar region.[Method]Seventeen cases were treated surgerically by single stage procedure,including posterior fusion,posterior instrumentation,decompression of spinal canal and interbody autografting.[Result]All patients were followed up from 9 months to 7 years postoperatively.The solid fusion was showed radiologically in 2.8 months at posterior autografting and in 3.3 months at interbody autografting on the average.The anteriolateral decompression of spinal canal was performed on 6 cases,completely recovered in 1 case,partially recovered in 3 cases and bad result in 2 cases.[Conclusion]Spinal tuberculosis with focus-healed kyphosis and compression of spinal cord in thoracolumbar region may show little spinal symptom clinically.The purpose of treatment is to provide rigid fixation and good stability of spine.

Aim To investigate the effects of PB-19 on action potential (AP) of ventricular cells in vitro and ischemia and reperfusion induced arrhythmias in vivo in rats. Methods ① We established the model of isolated rat right ventricular wall and perfused the models with normal Tyrode’s solution and PB-19.Microelectrode was inserted into the cells to record the AP. ② We ligated the left anterior descending coronary artery of rats to study the effects of PB-19 on ischemia and reperfusion induced arrhythmias. 50 rats were divided into 5 groups randomly and were given iv. injection with normal saline and PB-19 of different concentrations respectively. Objective To investigate the efficacy of patient-controlled epidural analgesia(PCEA)with ropivacaine plus fentanyl for postoperative analgesia after suprapubic prostatectomy(SPP).Methods Thirty-six ASAⅡorⅢpatients aged 62-78 yrs weighing 52-84 kg undergoing SPP were randomly divided into 3 groups with 12 patients in each group:groupⅠreceived PCEA with 0.12% ropivacaine and fentanyl 1?g?ml~(-1);groupⅡreceived PCEA with 2% ropivacaine and fentanyl 1?g?ml~(-1) and groupⅢreceived patient-controlled intravenous analgesia(PCIA)with morphine 0.2 mg?ml~(-1).Postoperative pain was assessed using VAS(0=no pain,10= worst pain).The degree of motor blockade(modified Bromage scale)was evaluated at 6,24,48 and 72 h after operation(T_(1-4)).Bladder spasm episodes were recorded.Side effects including nausea,vomiting and pruritus and the total number of RBC in rinse solution were also recorded.Results(1)Bromage score(0=no motor block,3 =inability to flex ankle joint)was significantly higher in groupⅠandⅡthan in groupⅢat T1(6 h after op.), but significantly lower in groupⅠandⅢthan in groupⅡat T_(2-4)(24-72 h after operation).(2)VAS score(0= no pain,10=worst pain)was not significantly different among the 3 groups without bladder spasm but significantly higher in groupⅢthan in other 2 groups during episodes of bladder spasm.(3)The number of bladder spasm episodes was not significantly different among the 3 groups within 6h after operation,but was significant larger in groupⅢthan in the other two groups during 6-72 h after operation.During 6-24 h after operation more bladder spasm attacks occurred in groupⅠthan in groupⅡ.(4)The incidence of side effects was significantly lower and the total number of RBC in rinse solution was significantly smaller in groupⅠandⅡthan in groupⅢConclusion PCEA with 0.12% or 2% ropivacaine plus fentanyl can effectively reduce the number of bladder spasm attack and postoperative bladder bleeding.Different concentrations of ropivacaine should be used for PCEA during different postoperative periods.

The DNA fragment RM07 was isolated from halophilic archaea Halobacterium halobium, which can function as promoter not only in halophilic archaea, but also in Escherichia coli as eubacterial promoter. Sequencing analysis indicated that it possessed the typical consensus sequences (-35 and -10) of bacterial gene promoter, which was confirmed by further deletion analysis: With its -35 sequence deleted and -10 sequence left,DNA fragment RM07a nearly cannot initiate transcription;With its both -35 and -10 sequences,RM07b DNA fragment could be active as promoter at a level even higher than RM07. Our research also showed that the promoter function of RM07 fragment in Escherichia coli was under the control of environmental factors,especially its positive correlation with the increasing concentration of sodium chloride. Therefore, RM07 DNA fragment may be potential1 novel promoter source for constructing double-function vectors. It also has special significance in elucidating the issues of the fusing characteristics of archaea and lateral gene transfer between archaea and bacteria.
