1.Exploration and Consideration on the Model of Two-way Referral Care for Diabetes Patients in TCM Hospital

In recent years, the incidence of diabetes has been increasing year by year. Along with it, the problems of high medical cost and health resource shortage also make the diabetes the key chronic disease of health service management in our country. There are significant features and advantages in treatment of diabetes with traditional Chinese medicine. Therefore, the construction mode of two-way referral care for diabetes patients between TCM hospitals and community medical institutions is just unfolding. Based on the status quo of TCM treatment for diabetes, this article reviewed the present basic situation of the two-way referral care for diabetes patients in TCM hospitals and community medical institutions, discussed the significance of this mode for diabetes patients in TCM hospital, emphasized the feasibility and validity of implementing this mode, and provided beneficial exploration and consideration for further development of this mode for diabetes patients between TCM hospitals and community medical institutions.

Objective To study the management of female urethral syndrome complicated by mental diseases in the aged. Metho ds 103 patients were randomly grouped into A and B.Nilestrioli,2mg e very 2 weeks,was instituted to the patients in Group A whereas to patients in Gr oup B,sedatives was instituted in addition to nilestrioli.The therapeutic result was evaluated in the 4th and 8th week.In patients with symptoms unrelieved,diaz epom injection and urethral perfuson of lidocaine were undertaken and the therap eutic effect was studied in the first,second,third and the fourth week. The inhibitory effect of cGMP on currents of voltage-gated potassium channel in PASMCs from both normal and chronic hypoxic rats may be probably through the phosphorylation of voltage-gated potassium channel.

Objective:To evaluate the clinical experience and working method of the department of stomatology,otorhinolarynogology and ophthalmology during the peacekeeping period in Liberia.Methods:Five hundred twenty five patients with stomatological,otorhinolaryngological or ophthalmological diseases during the peacekeeping period were reviewed.Results:The patients with stomatogical disease(455),NEN disease(38) and eye disease(32) were treated by a stomatological without mistake during 7 months. Conclusion:Stomatological doctors should be versatile to accomplish their peacekeeping mission.
5.The combination of MRI dynamic contrast enhancement and diffusion weighted imaging in the diagnosis of breast mass lesions

Objective To explore the combination of dynamic contrast enhancement and diffusion weighted imaging in the diagno-sis of breast diseases.Methods 67 patients with breast mass proven by operation or biopsy underwent dynamic contrast enhance-ment and diffusion weighted imaging.Fischer scoring system and BI-RADS were used to distinguish the benign lesions from the ma-lignant mass.The optimal boundary value of ROC curve was set as the threshold in DWI.The independent and conj unctive diagnos-tic efficacy of DCE and DWI were calculated and compared.The conj unctive methods included combinated properly method and DCE used as the chief diagnostic method while DWI as the adj unctive method.Results the best diagnostic boundary of ADC values was 1.1×10-3 mm2/s,The sensitivity,specificity and diagnostic accuracy of DWI and DCE were 71.4%,88.6%,80% and 100%, 80%,90%,respectively.The value was 85.7%,94.3%,90%,when using DCE chiefly and DWI adjunctively,while it was 91.4%respectively in combinating properly method.Conclusion DWI can improve the specificity of MRI in diagnosing breast lesions while it can reduce the sensitivity,which can results in the missed diagnosis of malignant diseases.DWI can used as an auxiliary examina-tion method to increase the diagnostic confidence of DCE method.
6.Analysis of 4 cases of sclerosing angiomatoid nodular transformation of spleen

Objective To investigate the clinicopathologic features,diagnosis,differential diagnosis and treatment of sclerosing angiomatoid nodular transformation(SANT). Methods The clinical data,pathologic characteristics,immunophenotype and postoperative follow-up of SANT were analysed. Results There were no specific findings in the clinical manifestations of the 4 cases of SANT.Grossly,the cut surface of the masses was gray-white and vague nodularity was observed.Microscopically,it was characterized by the multinodular angiomatoid appearance in a fibrosclerotic stroma.The nodules were composed of slit-like,sinusoid-like vascular spaces and were interspersed with a population of spindly or ovoid cells.It was revealed by immunohistochemistry that the expression of CD34 in some vessels' endothelial cells was positive,and CD8 was negative.While in another vessels' endothelial cells,CD8 was positive and CD34 was negative.The expression of SMA,Actin,Vimentin,Collage IV and CD68 was positive in all of the 4 cases,while that of CD21,Desmin and NSE was negative.No relapse or metastasis was found during the follow-up.Conclusion SANT is a rarely encountered benign lesion of the spleen,which should be distinguished from the malignant tumor of the spleen.The diagnosis counts on the pathologic and immunohistochemical findings.It could be cured by splenectomy with a favourable prognosis.
7.Application of Direct Gene Disruption Method in Recombinant Pichia pastoris

Proteolytic degradation has been a severe problem when Pichia pastoris is employed to express recombinant proteins.One alternative method to circumvent this problem is to construct protease gene disruptant.However,the main study of gene disruption is focused on nonrecombinant Pichia pastoris rather than recombinant strain.In our study,we established two different methods to directly disrupt PRC1 and KEX1 gene in recombinant Pichia pastoris.On the basis of this,we further discussed and compared the application and advantages of both methods.

Peritoneal dialysis (PD) is an established form of renal replacement therapy.Long-term PD leads to morphologic and functional changes to the peritoneal membrane (PM),which is defined as peritoneal fibrosis,a known cause of loss of peritoneal ultrafiltration capacity.Inflammation and angiogenesis are key events during the pathogenesis of peritoneal fibrosis.This review discusses the pathophysiology of peritoneal fibrosis and recent research progress on key fibrogenic molecular mechanisms in peritoneal infiammation and angiogenesis,including Toll-like receptor ligand-mediated,NOD-like receptor protein 3/interleukin-1β,vascular endothelial growth factor,and angiopoietin-2/Tie2 signaling pathways.Furthermore,novel strategies targeting peritoneal inflammation and angiogenesis to preserve the PM are discussed in depth.
9.The correlation between Val66Met polymorphism in BDNF gene and Alzheimer's disease

Apoptosis is one of the important forms during cerebral ischemia.Phosphoinositide-3 kinase (PI3K)/serine/threonine kinase (Akt) is the important cell survival signaling pathway,while c-jun N-terminal kinase (JNK) is the important pro-apoptotic signaling pathway.The dynamic equilibrium of the two signal transduction pathways maintains cell survival and apoptosis under the physiological state.Stimulation during cerebral ischemia breaks this physiological balance and results in the apoptosis of massive neurons.A variety of proved neuroprotective factors are associated with the amplification of enhancement of cell survival signal or inhibition of apoptosis signal,and thrus maintain the balance between the two signal pathways.
