1.To intensify our understanding about management of severe burn infection.

Nowadays, it is necessary to emphasize the three basic inseparable elements in the treatment of severe burn infection, which are systemic care, burn wound care, and rational use of antimicrobials topically or systematically. Systemic care has been shifted from simple nutritional support to maintaining the systemic homeostasis, including balancing immune-inflammatory response, and protecting organs from dysfunction. Some work focused on regulating systemic immune response in the initial phase and the balance of inflammatory response after occurrence of severe burn infection have been reported. These results at least broaden our thinking to recognize that treatment should not only destroy microbes, but also balance the response of the body. Escharectomy in earlier phase has been a consensus. Currently, we turn our vision into how to use "damage control surgery (DCS)" concept in management of severe burn. DCS in burn care includes the evaluation of perioperative situation more accurate to make a more appropriate surgical decision. Meanwhile, an overall strategy should be established to confront the rapidly increasing drug resistance of the pathogens. The release of endotoxin after use of antimicrobials, which has been studied widely, should be explored further.

4.Analysis of ADR/ADE cases caused by Sanqi Panax notoginseng for injection

Objective To analyze the related factors of ADR/ADE cases caused by Sanqi Panax notoginseng for injection(lyophilization),in order to provide reference for clinical rational use of Sanqi Panax notoginseng for injection.Methods Adopted the method of retrospective study,this paper statistically analyzed ADR/ADE cases caused by Sanqi Panax notoginseng for injection(lyophilization)in the database of Adverse Drug Reaction Monitoring Center in Qingdao from January 2015 to December 2015.Results There were altogether 123 ADR/ADE cases caused by Sanqi Panax notoginseng for injection(lyophilization)in 2015,with 90 cases in general,12 new cases, 21 severe cases;more female patients than male patients;more middle -aged and old patients;irrational use of the drug accounting for 45.53% of the total number of the report;and there was a higher proportion of irrational drug use in grass -roots medical institutions.Conclusion Irrational drug use is an important factor leading to ADR/ADE cases caused by Sanqi Panax notoginseng for injection(lyophilization),so the injection of Sanqi Panax notoginseng (lyophilization)should be regulated in order to reduce the occurrence of ADR/ADE cases.

6.Relationship between defense and SSP measured personality in obsessive-compulsive disorder patients

Objective To study the relationship between personality and defense mechanism of obsessivecompulsive disorder (OCD) patients.Methods Thirty-four OCD patients were tested with Swedish University Scales of Personality(SSP) and defensive style questionnaire( DSQ), and the result was compared with that of thirty-one normal controls.Results The SSP scores of STA,PsTA,SS,E,TI and M of OCD patients were significantly higher than that of normal controls (t = 2.06 ~ 5.10, P < 0.05 ).Comparing with normal controls, OCD patients tended to use immature and intermediate mechanism such as projection, passive aggression, unconscious emerge, phantasy, withdrawl and reaction formation, but less used mature mechanism such as sublimation, suppression and humor (t = -2.27 ~ 6.73, P< 0.05 ).All subscales of SSP in OCD patients, excluding SD and D, had positive correlation with total score of immature mechanism (r = 0.375 ~ 0.719, P < 0.05 ).The OCD patients' SSP scores of STA had positive correlation with mature mechanism (r = 0.378, P < 0.05 ).The OCD patients' STA, PsTA, I, AS, E, M,VTA and PhTA score of SSP had positive correlation with intermediate mechanism (r= 0.381 ~0.548, P< 0.05 ).All subscales of SSP in OCD patients, excluding SD, had positive correlation with total score of concealment (r =0.342 ~ 0.530, P < 0.05 ).Conclusion OCD patients have the neurotic and aggressive features of personality;and the personalities of OCD patients have significant correlation with defense mechanism.
7.Effect of Scoparone on Th1/Th2 cytokines and IgE in the experimental allergic rhinitis rats.

OBJECTIVE:
To observe changes of serum Th1/Th2 cytokines and IgE of rats with experimental allergic rhinitis (AR) treating with Scoparone (20 ml/kg, the concentration is 168 mg/L).
METHOD:
Forty male SD rats were randomly divided into 4 groups : group NC, group AR, group Sco, group Dxm. The rats were sensitized with OVA and were then treated with Scoparone, during the treatment, the behaviors were observed and the change in nasal mucosa were recorded. The level of serum cytokines IFN-gamma, IL-4, IL-5 and IgE were determined by ELISA.
RESULTS:
The behavior scores of group Sco were significantly lower than those of group AR, the difference was statistically significant (P < 0.01). Comparing to group Dxm and group NC respectively, the difference was not statistically significant (P > 0.05). The nasal inflammation of group Sco was significantly less than that of group AR. The level of serum IFN-gamma of group Sco was significantly higher than that of group AR, while the levels of serum IgE, IL4, IL-5 of group Sco were significantly lower (P < 0.01). There were no significantly difference between group Sco and group NC, group Dxm, the difference was not statistically significant (P > 0.05).
CONCLUSION
Scoparone is highly effective in treating allergic rhinitis by regulating the expression of Th1/Th2 cytokines and IgE.

8.Expression and Correlation of Sex Hormone-binding Globulin,Insulin Signal Transduction and Glucose Transporter Proteins in the Gestational Diabetes Mellitus Placental Tissue

Objective To study the expression of sex hormone-binding globulin(SHBG),insulin signaling pathway and glucose transporter in placenta of pregnant women with gestational diabetes mellitus(GDM),and to explore its role in the pathogenesis of GDM. Methods A total of 10 full-term and non-obese(BMI<25 kg/m2)pregnancy women diagnosed with GDM and 10 normal pregnant cases were recruited for the study. Placental tissues were collected respectively. The expression of protein and mRNA of SHBG and insulin signal transduction(IRS-1,ISR-2,PI3K p85α)and glucose transporter proteins(GLUT-1,GLUT-3,GLUT-4)in placental tissue were detected by real-time PCR and Western blotting. Pearson and linear regression analysis were used to evaluate the correlation among the related indicators. Results In the placental tissue of non-obese women,there existed a decrease(P<0.05)in the expression of protein and mRNA of SHBG with a concurrent a decrease(P<0.05)in the expression of protein and mRNA of GLUT-4 in GDM women compared with normal controls. Furthermore ,a decrease(P<0.05)of GLUT-3 and IRS-1 protein expression with lower(P<0.05)IRS-2 mRNA expression was also observed in placental tissue from GDM patients. No changes were found in PI3K p85α and GLUT-1 at both protein and mRNA levels(P>0.05). Results of linear correlation analysis showed that there were positive correlations between SHBG mRNA and IRS-2 mRNA(P<0.05),SHBG mRNA and PI3K p85α mRNA(P<0.05),and SHBG mRNA and GLUT-4 mRNA(P<0.05). There was also a remarkable positive correlation between IRS-2 mRNA and GLUT-4 mRNA(P<0.01). There existed negative correlations between IRS-1 mRNA and PI3K p85α mRNA(P<0.05),and IRS-1 mRNA and GLUT-3 mRNA(P<0.05). There existed a remarkable positive correlation between IRS-2 mRNA and GLUT-1 mRNA(P<0.01). Conclusion The defective receptors of insulin signaling pathway are present in GDM placental tissue. Decreased expression of SHBG may be involved in the regulation insulin signaling ,leading to a concomitant decrease expression of relevant insulin signaling components in placental tissue ,implying insulin resistance and developing GDM finally.
9.Comparison of two flushing suction methods in SpyGlass choledochoscopy system

Objective To evaluate the applicant effect of improved flushing suction method for SpyClass choledochoscope operation. Methods Seventy-three patients who underwent SpyGlass choledocho-scopy from February 2013 to January 2015 were enrolled. Negative pressure biopsy channel was used in group A(n= 34)during conducting SpyGlass choledochoscopy with the tube directly connected to the conveying pipe and combined with duodenoscope suction.In group B(n= 39),the biopsy channel of disposable syringe jacket was first connected to a delivery catheter,then the negative pressure pipe was put in the injector coat, combined with duodenoscope suction. Adverse reactions,theⅠ image acquisition time after the placement of SpyGlass delivery catheter to pancreaticobiliary duct,the total operation time,and the occurrence of compli-cations after operation were recorded. Results Sinus bradycardia,arrhythmia,agitation occurred in 7 cases (20. 6%),5 cases( 14. 7%),and 10 cases( 29. 4%) respectively in group A,those were 1 case (2. 6%),0 case(0/ 39),2 cases(5. 1%)respectively in group B with significant difference(P<0. 05). The time to acquire a clear image and the total operation time in group A were 6. 2 min and 24. 9 min,and 3. 3 min and 16. 8 minutes in B group with significant difference(P<0. 05),postoperative cholangitis oc-curred in 5 cases in group A(14. 7%),1 case(2. 6%)in group B with significant difference(P<0. 05). Conclusion New irrigation and suction method has obvious advantages in the SpyGlass system.
10.Treatment of telangiectasia and varicose vein of lower limb: experience with foam sclerotherapy and the long-pulsed 1064 nm Nd∶YAG laser

Objective The decease of varicose vein and telangiectasis of lower extremity has always been difficult to treat.Regular treatments and medicines are invalid.This research aimed to study the effect of foam sclerotherapy and the long-pulsed 1064 nm Nd∶YAG laser in treatment of reticular veins and telangiectasis of lower extremity.Methods From January 2012 to June 2012,excluding deep and superficial veins valve insufficiency of the lower extremity through duplex uhrasonography,72 patients with simple reticular veins and telangiectasis of the lower extremity were treated with foam sclerosing agent injection combined with Nd∶YAG 1064 nm laser therapy.Results Of the 72 patients,cure was achieved in 38 cases,significantly effective in 32 cases,effective in 2 cases,and total effective rate was 100%.Follow-up showed that after 3 months 1 case was dissatisfied,3 cases were discontent,6 relatively satisfied,32 satisfied,and 30 very satisfied;after 12 months no cases were very dissatisfied,1 cases were discontent,9 relatively satisfied,28 satisfied,and 34 cases very satisfied.There were no severe complications in all cases.Conclusions Foam sclerotherapy and Nd:YAG 1064 nm laser can be used for different stages of the treatment process and different caliber of blood vessels.Treatment with foam sclerotherapy and Nd∶YAG 1064 nm laser for reticular veins and telangiectasia of lower extremity is a safe,simple and effective way.
