2.Effects of psychological stress on pathogenetic condition and rapeutic effect of ulcerative colitis

AIM: To investigate the effects of psychological stress on ulcerative colitis rats and to explore the mechanisms of this;meanwhile to investigate the effects of anxiolytic drug on symptoms of ulcerative colitis rats under psychological stress and the mechanisms of it.METHODS: eighty male Wistar rats were randomly divided into four groups(n=20): UC control group;UC stress group;UC+clonazepam group;UC stess+clonazepam group.After UC model was established with immunization,these rats were fightened by cat to set up psychological stress model.During establishing stress model,each groups were respectively received physiologic saline or clonazepam by intragastric administration.After 15d treatment,all rats were sacrified simultaneously.Colon mucosal inflammationand damage were assessed by measuring colon mass,morphologic damage score,colonic MPO and NO activity levels.Morphologic damage score was examined under microscope.Colonic MPO and NO was measured by spectrophotometric method.Serum hydrocortisone was determined by radioimmunoassay(RIA).RESULTS: Morphologic damage score,colonic MPO and NO,serum hydrocortisone of UC stress group were significantly increased compared with those of UC control group;morphologic damage score,colonic MPO and NO,serum hydrocortisone of UC+clonazepam group were decreased markedly vs UC stress group;Little difference was observed between UC+clonazepam group and UC control group.CONCLUSION: Psychological stress can aggravate symptoms of UC.Although benzodiazepine have no anti-inflammatory action,it relieve effects on internal organs of psychological stress and aggravation of symptoms of UC by to effect limbic system and formatio reticularis.
3.The distribution and drug resistance of respiratory tract infection pathogens isolated from patients of pediatric CICU

Objective To analyze the common respiratory tract infection pathogens distribution and their drug resistance in pedi-atric cardiac intensive care unit(CICU),so as to provide reference for clinical rational use of antibiotics.Methods 1 350 cases of sputum specimens from lower respiratory tract infection patients of pediatric CICU in the medical center between January 2011 and December 2012 were cultivated and drug susceptibilities were tested.The results were retrospectively analyzed.Results 490 patho-genic strains were isolated from 1 350 cases of sputum specimens and identified,including Gram negative bacilli 288 strains (58.78%),Gram positive coccus 140 strains(28.57%),fungi 62 strains(12.65%,mainly Candida albicans ).Gram negative bacilli was given priority to with Klebsiella pneumoniae (62 strains,12.65%),followed by Branhamella catarhalis ,Pseudomonas aerugi-nosa and Escherichia coli .The rates of extended-spectrum beta-lactamases(ESBLs)-producing strains among Escherichia coli and Klebsiella pneumoniae were 73.33% and 66.13%,respectively.Gram positive coccus was given priority to with Staphylococcus aureus (65 strains,13.27%),followed by Streptococcus pneumoniae .Methicillin-resistant Staphylococcus aureus (MRSA)accounted for 24.62%.Conclusion Staphylococcus aureus ,Streptococcus pneumoniae and Klebsiella pneumoniae are main pathogens of re-spiratory tract infection in pediatric CICU.And there is multiple drug-resistant bacteria infection.Rational applicattion of antibiot-ics according to the test results of isolation and drug susceptibility is an effective way to control the infection of critical children and reduce the emergence of resistant strains.
4.Surgery experience in microvascular decompression for trigeminal neuralgia

Objective To explore the efficacy of microvascular decompression (MVD)on trigeminal neuralgia and the anatomical characteristics of the responsible blood vessels.Methods The clinical data of 25patients with idiopathic trigemine neuralgia(ITN) undergoing MVD were analyzed retrospectively.The origins of the responsible blood vessels and sites of compression at the trigeminal root were observed and determined.Meanwhile,the anatomical characteristics of the responsible blood vessels and the efficacy of MVD were investigated.Results The offending vessels which were responsible for ITN were identified at root exit zone (REZ) in 21 cases (84%).The most common responsible vessel was the superior cerebellar artery in 13 cases (52%),followed by anterior inferior cerebellar artery in 6 cases (24%).Seventy six percent of the offending vessels were arteries and 8% were rock vein.No vascular compression but evident increase in the thickness of arachnoid mater covered by the trigeminal nerve was observed in 4 cases ( 16% ).All patients were relieved for the acute pain and the overall cure rate was 96%.Conclusion MVD is a safe,microinvasive and effective surgical modality for ITN.The key for better outcomes and less complications of MVD is to improve microsurgical techniques and deal with all the involved vessels.
5.Research on non-rigid registration of multi-modal medical image based on Demons algorithm.

Non-rigid medical image registration is a popular subject in the research areas of the medical image and has an important clinical value. In this paper we put forward an improved algorithm of Demons, together with the conservation of gray model and local structure tensor conservation model, to construct a new energy function processing multi-modal registration problem. We then applied the L-BFGS algorithm to optimize the energy function and solve complex three-dimensional data optimization problem. And finally we used the multi-scale hierarchical refinement ideas to solve large deformation registration. The experimental results showed that the proposed algorithm for large de formation and multi-modal three-dimensional medical image registration had good effects.

7.Applications of real-time three-dimensional transesophageal ultrasound technology in the diagnosis and treatment of artificial mitral paravalvular leak

Objective To assess the application values of real-time three-dimensional transesophageal ultrasound (RT3D-TEE) technology in the diagnosis and treatment of artificial mitral paravalvular leak (PVL). Methods Fifty-one cases of artificial mitral PVL patients were received two-dimensional transesophageal ultrasound (2D-TEE) and RT3D-TEE examination before and after surgery. Accuracy analysis was performed between the two kinds of test. Results The diagnostic accuracy of RT3D-TEE technique in patients with PVL drain port positionwas significantly higher than that of 2D-TEE technology , and the data of PVL drain port size by RT3D-TEE technique is significantly larger than that of 2D-TEE technique (P < 0.05, respectively). Conclusion The PVL laboratory diagnosis based on RT3D-TEE is accurate and reliable , and RT3D-TEE can assist doctors to diagnose before operation , and can provide real-time ultrasound information from intraoperative monitoring , and monitor the postoperative efficacy of patients .
8.Pathogenic Bacteria Distribution and Antimicrobial Susceptibility of Children with Acute Otitis Media

Objective To analyze pathogenic bacteria distribution and antimicrobial susceptibility in children with acute otitis media(AOM ) .Methods Otorrhea samples from 146 episodes of AOM were cultured .The antimi‐crobial susceptibility of the main pathogenic bacteria was determined .The results were analyzed by SPSS19 .0 .Re‐sults 1) The strains of bacteria were isolated from 109 children with the positive rate of 74 .66% .Streptococcus pneumoniae (SP ) was the major bacteria(64 episodes ,58 .72% ) ,followed by staphlococcus aureus(SA) (19 epi‐sodes ,17 .43% ) .2) Sp was all sensitive to vancomycin ,levofloxacin ,moderate to penicillin ,amoxicillin ,cefo‐taxime ,and highly resistent to erythromycin and clindamycin .Staphlococcus aureus were all sensitive to vancomy‐cin ,tetracycline ,and Amy card ,and moderate to amoxicillin clavulanic acid potassium ,cefoxitin ,and oxacillin ,all resistent to penicillin and ampicillin .3) The strains of SP in age≤1year ,>1 -3years ,and >3 years respectively were 31(50 .82% ) ,25(56 .82% ) ,8 (19 .51% ) .There were significant differences between them(χ2 =14 .073 ,P=0 .001) .4)The strains of SP in 2012 ,2013 ,2014 respectively were 16(30 .19% ) ,22(48 .89% ) ,26(54 .17% ) ,There were significant differences between them(χ2 =6 .557 ,P=0 .038) .The antimicrobial susceptibility of SP had no sig‐nificant differences among 2012 ,2013 ,2014 ,but a yearly resistance decreasing trend was seen .Conclusion SP was the main bacterial contributor for AOM in Wuhan children .SP detection rate increases every year ,mainly in chil‐dren less than 3 years old .T he antimicrobial susceptibility is stable .
9.Protective effect of rhein lysinate on liver of diabetic rats and its mechanism

Objective To investigate the protective effect of rhein lysinate (RHL)on the liver of the models with diabetic rats,and to provide basis for research on treatment of fatty liver in the patients with diabetes mellitus. Methods The models of diabetic rats were established by intraperitoneally injecting streptozotocin(STZ).40 rats were divided into control,model,25 mg·kg-1 RHL,and 50 mg·kg-1 RHL groups(n=10).The levels of malonaldehyde (MDA)and the activities of superoxide dismutase (SOD)and glutathione peroxidase (GSH-Px) were detected by thiobarbituric acid method, pyrogallol autoxidation method, and NADPH coupling method, respectively.The pathological changes of liver tissue were observed by hematoxylin and eosin (HE)staining;the content of fat in liver tissue was observed by Nile red staining;the expression levels of fat synthesis-related proteins were detected by Western blotting method.Results Compared with control group,the body weight of the rats in model group was decreased and the levels of blood glucose,total cholesterol(TC)and triglyceride(TG)were increased (P<0.05);the activities of SOD and GSH-Px in liver tissue were decreased (P<0.05);there were a plenty of fat vacuoles and fat accumulation in liver tissue. The signal pathway of fat synthesis-related ERK1/2-SREBP-1c was activated in model group;compared with model group,it was inhibited in 25 and 50 mg· kg-1 RHL groups (P<0.05).Compared with model group,the blood glucose,TC and TG of the rats in 25 and 50 mg ·kg-1 RHL groups were decreased (P<0.05);the activities of SOD and GSH-Px were increased (P<0.05);however the body weight had no change. Compared with model group, the fatty vacuoles and the fatty accumulation of liver tissue in 25 and 50 mg·kg-1 RHL groups were decreased. Conclusion The hepatic protection of RHL is correlated with the inhibition of oxidative stress, fat degeneration and fatty accumulation of liver tissues.
10.Expression of telomerase in skin lesions of condyloma acuminatum,bowenoid papulosis,Bowen's disease and vulvar squamous carcinoma

Objective To study the expression of telomerase and its possible clinical significance in patients with condyloma acuminatum,bowenoid papulosis,Bowen's disease and vulvar squamous cell car- cinoma.Methods The human telomerase reverse transcriptase (hTERT) was detected by immunohisto- chemistry technique in all patients.Histopathological analysis was done in patients with bowenoid papulosis or Bowen's disease.Results There was a significant difference in the expression of hTERT among normal tissue,condyloma acuminatum and squamous carcinoma,and the expression rate increased successively from normal control to squamous carcinoma.In bowenoid papulosis,the expression of bTERT was significantly higher as compared to that in condyloma acuminatum,significantly lower as compared to that in squamous carcinoma,and was as the same as that in Bowen's disease.Histopathologically,the degree of cell atypia was significantly higher in Bowen's disease than that in bowenoid papulosis.Conclusions The expression of telomerase increases successively from normal skin,benign tumors to malignant carcinomas,which sug- gests that the expression of telomerase may play an important role in both cell proliferation and immortaliza- tion.
