1.Qualitative and quantitative detection of Poria cocos by near infrared reflectance spectroscopy.

OBJECTIVEThe present study is concerning qualitative and quantitative detection of Poria cocos quality based on FT-near infrared (FT-NIR) spectroscopy combined with chemometrics.
METHODThe Poria cocos polysaccharides contents were determined by UV. Transmission mode was used in the collection of NIR spectral samples. The pretreatment method was first derivation and vector normalization. Then principal component analysis (PCA) was used to build classification model and partial least square (PLS) to build the calibration model.
RESULTThe results showed that conventional criteria such as the R, root mean square error of calibration (RMSEC), and the root mean square error of prediction (RMSEP) are 0.944 0, 0.072 1 and 0.076 2, respectively. The misclassified sample is 0 using the qualitative model built by PCA.
CONCLUSIONThe prediction models based on NIR have a better performance with high precision, good stability and adaptability and can be used to predict the polysaccharose content of Poria cocos rapidly, which can provide a fast approach to discriminate the different parts of Poria cocos.

2.Emergency department in patients with acute mgocardial infarction rescue hursing implementation of the new project

Objective To investigate the clinical rescue and nursing plan for the acute myocardial infarction (AMI) patients in emergency department.Methods Totals of 120 patients with acute myocardial infarction were selected and divided into control group (n =60) and experimental group (n =60) randomly.A new rescue nursing scheme was taken in experimental group,while routine nursing was taken in control group.Then,the effect of treatment was observed and compared between control group and experimental group.Results In experimental group,58 cases were cured,2 improved,no cases was dead,and the cure rate was 96.67%.And 45 cases were cured,12 were improved,3 cases were dead,and the cure rate was 75.00% in control group.The success rate of the experimental group was significantly higher than that of the control group (x2 =12.857,P < 0.05).Conclusions The new nursing scheme has good effect on rescuing patients with acute myocardial infarction.

Functional magnetic resonance imaging (fMRI), a newly developing technique, contains function, anatomy and image, which makes the real-time, dynamic and non-invasive measurement of the functional brain imaging availability. This paper summarized the characteristics of fMRI in health and stroke populations, introduced the advances of fMRI in neuroplasticity, rehabilitation assessment and prognosis in hand movement dysfunction in patients with stroke, and discussed the difficulty fMRI faced in rehabilitation assessment and the further researches.

Objective To explore the relationship between the test of drug-induced electroencephalography(EEG) and epilepsy.Methods324 patients with epilepsy were randomly divided into 2 groups, the benadrly group(n=162) and chloral hydrate group(n=162) as control. EEG was examined by evoking during sleep to observe epileptiform discharge and which stage of non-rapid eye movements (NREM) epileptiform discharge occurred more frequently.ResultsThe epileptiform discharge was 31.61% in benadrly group, and 18.52% in chloral hydrate group. There was significant difference between the two groups (P<0.01). The detection rate of epileptiform discharge at the stage I~II of NREM was significantly higher in benadrly group (83.7%) than in chloral hydrate group(63.3%)(P<0.05), which suggested that epileptiform discharge frequently occurred at the stage I~II of NREM.ConclusionBenadrly is superior to chloral hydrate in the test of drug-induced EEG of epilepsy.
7.Effect of low-dose glucocorticoid on corticosteroid insufficient patients with acute exacerbation of chronic obstructive pulmonary disease

BACKGROUND: This study aimed to investigate the prevalence rate of critical illness-related corticosteroid insufficiency (CIRCI) and the effect of low-dose glucocorticoid on prognosis of CIRCI in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). METHODS: Since January 2010 to December 2012, 385 patients, who met the criteria of AECOPD, were enrolled in the Intensive Care Unit (ICU) of the First People's Hospital and Municipal Central Hospital of Xiangtan City. The AECOPD patients complicated with CIRCI screened by an adrenalcorticotrophic hormone test within 12 hours after admission to ICU were divided into a treatment group (n=32) and a control group (n=31) for a prospective, randomized and controlled clinical trial. Hydrocortisone (150 mg/d) or normal saline was injected intravenously for 7 days. The patients were followed up for 28 days after injection. The endpoint included 28-day survival time, non-shock time, ICU stay and the period of non-mechanical ventilation. The markers of inflammation C-reactive protein, tumor necrosis factor-α, interleukin 6 and procalcitonin were measured at baseline and 7 days after treatment. The variables were analyzed by Student's t test, the non-parametric statistical test, the Chi-square test or the Kaplan-Meier method with SPSS18.0 statistic software. A P value <0.05 was considered statistically significant. RESULTS: Totally 63 patients were diagnosed with CIRCI by an adrenalcorticotrophic hormone test and the prevalence rate was 16.4%. The shock rate of the AECOPD patients complicated with CIRCI was higher than that of the AECOPD patients without CIRCI (23.8% vs. 8.7%, P<0.01). Kaplan-Meier analysis revealed that the 28-day survival time of the treatment group was obviously longer than that of the control group (P<0.05). Compared with the control group, shock-free days within 28 days was longer in the treatment group (18.2±9.5 vs. 25.8±4.1, P<0.05). Treatment with low-dose glucocorticoid obviously decreased the markers of infection and inflammation (P<0.01), such as C-reactive protein (13.2±5.5 mg/L vs. 8.3±3.1 mg/L for the control group; 13.5±5.9 mg/L vs. 5.1±2.3 mg/L for the treatment group), tumor necrosis factor-α (26.1±16.2 g/L vs. 17.5±11.7 g/L for the control group; 25.0±14.8 g/L vs. 10.4±7.8 g/L for the treatment group) and procalcitonin (3.88 g/L vs. 2.03 g/L for the control group; 3.77 g/L vs. 1.26 g/L for the treatment group). Furthermore, the markers in the treatment group decreased more obviously than those in the control group (P<0.01). CONCLUSION: The prevalence rate of CIRCI was higher in the patients with AECOPD in the department of critical medicine, and low-dose glucocorticoid treatment for one week reduced the 28-day mortality, shock time and markers of infection and inflammation.
8.Inhibitory effect of genistein on the proliferation of the anterior pituitary cells of rats

The effects of tyrosine kinase inhibitor genistein on the proliferation of rat anterior pituitary cells and mouse AtT-20 cells were studied using techniques of cell culture, ~3H-TdR incorporation, flow cytometric analysis and electron microscope. Genistein significantly inhibited the proliferation of rat anterior pituitary cells and mouse AtT-20 cells. Genistein (50 and 100 μmol/L ) blocked the proliferation of AtT-20 cells at G0/G1 and G2/M phases and evoked an apoptotic peak of these cells with an apoptotic ratio of 19.9% and 36.4%. The apoptotic cells were also observed under the electron microscope. In consequence, genistein, as a tyrosine kinase inhibitor, can significantly inhibit the proliferation of pituitary cells possibly by inducing apoptosis, and the tyrosine kinase activity may play a key role in the proliferation and differentiation of pituitary cells.
