2.Clinical diagnosis and microsurgery of 12 patients with parasellar cavernous hemangiomas

Objective To investigate the clinical features and microsurgical outcomes of patients with parasellar cavernous hemangiomas.Methods The clinical data and prognosis of 12 patients with parasellar cavemous hemangiomas treated with microsurgery were retrospectively analyzed.Magnetic resonance imaging showed that cavernous hemangiomas appeared slight hypointensity on T1WI,hyperintense on T2WI,and significant contrast on enhancing.Results Total resection and sub-total resection was achieved in 9 and 3 patients.No death occurred postoperatively.After operation,all patients were released from intracranial hypertension.In 6 patients who had visual disorder,5 patients improved and 1 patient had no change.In 5 patients who had eyeball motor disorder,3 patients improved and 2 patients had no change.In 4 patients who had pituitary gland dyshormonism,2 patients improved and 2 patients had no change.At postoperative early stage,diabetes insipidus and serum electrolyte disorder occurred in 3 patients,2 patients developed oculomotor paralysis.Patients were followed up (30.51 + 2.57) months.Three patients recovered from diabetes insipidus and serum electrolyte disorder,and 1 patient recovered from oculomotor paralysis.No patient suffered tumor recurrence or regrowth.Conclusions The correct diagnosis of parasellar cavernous hemangiomas can be achieved according to the preoperative MRI.Microsurgery is the first-line therapy for parasellar cavernous hemangiomas.
3.Time of lower extremity deep venous thrombosis after hip arthroplasty in senile patients with osteoporotic femoral neck fractures

BACKGROUND: Lower extremity deep venous thrombosis is commonly seen after hip arthroplasty. The time of thrombosis after arthroplasty in senile patients with osteoporotic femoral neck fractures is little reported.OBJECTIVE: To investigate the time of perioperative lower extremity deep venous thrombosis in senile patients with osteoporotic femoral neck fractures undergoing hip arthroplasty. METHODS: The data of 178 patients with femoral neck fractures undergoing hip arthroplasty were analyzed, and were then allocated to anticoagulant and non-anticoagulant groups according to the preoperative use of low-molecular-weight heparins or not. Doppler ultrasonography of both lower extremity arteries was conducted at 1, 3, 5 and 5-7 days after injury, and 2, 5, 7 and 14 days postoperatively to monitor the lower extremity deep venous thrombosis, and the number and incidence of thrombosis in the two groups were recorded at different time points. The time of thrombosis occurred frequently after injury and surgery was determined through drawing the frequency distribution table and line chart.RESULTS AND CONCLUSION: (1) The incidence of thrombosis showed significant difference between anticoagulant (n=6, 8%) and non-anticoagulant (n=22, 21%)) groups (P < 0.05). (2) Thrombosis frequently occurred at 3 days after injury and 1 day postoperatively. (3) These results suggest that the thrombosis in patients with femoral neck fractures frequently occurs at 1-3 days after injury and 1 day after replacement. Additionally, active prevention measures after injury can significantly reduce the incidence of lower extremity venous thrombosis.

5.Comparison of endoscopic retrograde cholangiopancreatography performed without radiography and with ultrasound-guidance in the management of acute pancreaticobiliary disease in pregnant patients

Background Currently,the recommendation when treating acute biliary or pancreatic disease during pregnancy is to perform endoscopic retrograde cholangiopancreatography (ERCP) without radiation exposure,either empirically (with no radiographic guidance) or with ultrasound guidance.However,few published studies compared these two ways.This study aimed to compare ultrasound-guided ERCP with the procedure without radiographic guidance in the treatment of acute pancreaticobiliary disease in pregnant patients.Methods The clinical data of 68 pregnant patients with acute pancreaticobiliary disease admitted to our hospital between January 2004 and May 2010 were reviewed retrospectively.ERCP was performed without radiographic guidance in 36 cases (group A) and with ultrasound guidance in 32 cases (group B).Data on the following variables were compared between the two groups:surgical success rate,rate of complete stone removal,time to resolution of clinical manifestations and laboratory indicators,length of hospital stay,complications,outcome and differences in efficacy of ERCP during different stages of pregnancy.Results In group A,the rates of surgical success and complete removal of stones were 69% and 60%,respectively; the corresponding values were 91% and 89% in group B (P <0.05).Postoperatively,clinical manifestations improved rapidly in all patients; there was no statistically significant difference between the groups (P >0.05).Leukocyte counts and liver function had improved significantly after one week in all patients; they recovered more quickly in group B ((8.64±1.83)days vs.(14.57±3.74) days,(14.29±4.64) days vs.(20.00±5.40) days,P <0.01).The hospital stay was shorter in group B ((16.28±7.25) days vs.(28.00±6.83) days,P<0.001).The complication rate was 14% in group A and 3% in group B (P <0.05).There were no significant differences between the two groups in the procedure's efficacy during different stages of pregnancy.Conclusions In the treatment of acute pancreaticobiliary disease during pregnancy,ultrasound-guided ERCP is safer and more effective than performing the procedure empirically without radiographic guidance when performed by experienced practitioners.Its more widespread use is recommended.
6.The investigation of HEV epidemiology, inferior clinical infection and viral-toxemia in blood donors in Quzhou area.

OBJECTIVETo investigate the infection of HEV in Quzhou area of Zhejiang Province.
METHODSAll sera from blood donors in the central blood bank of Quzhou from April 2006 to April 2007 were used. Anti-HEV IgG and anti-HEV IgM were measured by EIA. RT-PCR was also performed to the samples with positive anti-HEV IgM. Genotype and sequence homology were analyzed after sequencing.
RESULTSThe positive ratio of anti-HEV IgG was 40.60%, in which the male infection ratio was higher than the female significantly (43.09% VS 36.09%; chi2=22.6; P < 0.01). The infection ratio was increased with age. The positive ratio of anti-HEV IgM was 0.43%. The positive ratio of anti-HEV IgG and the titers of antibody were higher in the inferior clinical infectors with positive anti-HEV IgM than the negative ones (P < 0.05). Two samples were positive in HEV PCR among 21 samples with positive anti-HEV IgM. The toxemia ratio was 0.4% of all the donors. And the genotype of the two samples with toxemia were both HEV-IV.
CONCLUSIONThe HEV infection was correlation with age and sex significantly and the infection occurred in the adults mainly in Quzhou area. HEV toxemia was not infrequency in the blood donors.

7.The psychological effect of minesweeping infrasonic field.

OBJECTIVETo investigate the effect of infrasonic field of minesweeper on psychology of minesweeper crews.
METHODSAn experimental ship was selected to conduct infrasonic minesweeping, a control ship of the same type was selected to operate likewise with the infrasound generator turned off, and another group of coast-servicemen was chosen as blank control. Attention-span test, digit memory test, two-digital numbers list and profile of mood states (POMS) were used to test the ship crews and coast-servicemen.
RESULTSThere were no significant differences in psychological parameters between the two ship crews before the experiment. But the scores of two-digit figures list in ship crews were significant lower than that in coast-servicemen, (31.2 +/- 11.8, 36.4 +/- 14.5 respectively vs 45.8 +/- 13.9, P < 0.05). POMS showed that the scores of anger-hostility, fatigue-inertia, and confusion-bewilderment in both ship crew groups [(15.5 +/- 6.4, 18.3 +/- 6.8), (12.1 +/- 5.0,12.3 +/- 4.9), (11.6 +/- 4.4, 12.5 +/- 4.8), respectively] were higher than those in coast-servicemen (13.9 +/- 7.0, 7.6 +/- 4.1, 8.2 +/- 4.3, respectively, P < 0.05 or 0.01), while vigor-activity in experimental group (15.0 +/- 5.9) was lower than that in coast-servicemen (19.7 +/- 4.7). After the experiment, the scores of experimental crews in digit memory test (14.5 +/- 5.0), vigor-activity (12.2 +/- 5.8), fatigue-inertia (15.8 +/- 6.2) were significantly different from those of control crews (17.3 +/- 4.8, 16.5 +/- 4.6, 11.3 +/- 5.1).
CONCLUSIONSThe infrasonic field from minesweeper may injure the crew's psychological health and some neurobehavioral function.

8.Research progress on the traditional Chinese medicine-pharmaceutical drug interaction mediated by the ABC transporter family

ABC transporters on the intestinal barrier, blood-brain barrier and on tumor cells will affect drug bioavailability, transport across the blood-brain barrier and multidrug resistance. The active ingredients of traditional Chinese medicines can affect the function and expression of ABC transporters. When combined with pharmaceuticals the potential interaction between the two can change the efficacy of the medicines. We review the ABC transporter superfamily and their distribution with regard to their relationship and interactions with traditional Chinese medicine on the intestinal barrier and the blood-brain barrier, as well as their role in tumor multidrug resistance mediated by ABC transporters. We summarize the research progress over the past five years.
9.Environment surveillance of filamentous fungi in two tertiary care hospitals in China.

BACKGROUNDInvasive fungal infections have constituted an increasingly important cause of morbidity and mortality in immunocompromised patients. In this study, a surveillance project was conducted in three different intensive care units of two large tertiary hospitals in China.
METHODSA one-year surveillance project was conducted in two tertiary hospitals which located in northern China and southwest China respectively. Air, surfaces and tap water were sampled twice a month in a central intensive care unit, a bone marrow transplant unit, a neurosurgery intensive care unit and a live transplant department. Environmental conditions such as humidity, temperature and events taking place, for example the present of the visitors, healthcare staff and cleaning crew were also recorded at the time of sampling.
RESULTSThe air fungal load was 91.94 cfu/m(3) and 71.02 cfu/m(3) in the southwest China hospital and the northern China hospital respectively. The five most prevalent fungi collected from air and surfaces were Penicillium spp., Cladospcrium spp., Alternaria spp., Aspergillus spp. and Saccharomyces spp. in the southwest China hospital, meanwhile Penicillium spp., Fusarium spp., Aspergillus spp., Alternaria spp. and Cladospcrium spp. in the northern China hospital. The least contaminated department was intensive care units, and the heaviest contaminated department was neurosurgery intensive care unit. Seventy-three percent of all surfaces examined in the northern China hospital and eighty-six percent in the southwest China hospital yielded fungi. Fifty-four percent of water samples from the northern China hospital and forty-nine percent from the southwest China hospital yielded fungi.
CONCLUSIONSThese findings suggested that the fungus exist in the environment of the hospital including air, surface and water. Air and surface fungal load fluctuated over the year. Air fungal load was lower in winter and higher in summer and autumn, but seldom exceeded acceptable level. The higher values were created during May to August in the northern China hospital and May to June and September to October in the southwest China hospital. A correlation between air fungal load and humidity, as well as personnel was observed.

10.Short-term therapeutic outcomes of robotic-assisted laparoscopic radical prostatectomy for oligometastatic prostate cancer:a propensity score matching study

Background:The role of local treatment in oligometastatic prostate cancer (PCa) is gaining interest with the oligometastases hypothesis proposed and the improvement of various surgical methods and techniques.This study aimed to compare the short-term therapeutic outcomes of robotic-assisted laparoscopic radical prostatectomy (RALP) for oligometastatic prostate cancer (OPC) vs.localized PCa using propensity score matching.Methods:Totally 508 consecutive patients underwent RALP as a first-line treatment.The patients were divided into two groups according to oligometastatic state:the OPC group (n =41) or the localized PCa group (n =467).Oligometastatic disease was defined as the presence of two or fewer suspicious lesions.The association between the oligometastatic state and therapeutic outcomes of RALP was evaluated,including biochemical recurrence (BCR) and overall survival (OS).A Cox proportional hazards model was used to assess the possible risk factors for BCR.Results:Totally 41 pairs of patients were matched.The median operative time,the median blood loss,the overall positive surgical margin rate,the median post-operative hospital stays,and the post-operative urinary continence recovery rate between the two groups showed no statistical significance.The 4-year BCR survival rates of the OPC group and localized PCa group were 56.7% and 60.8%,respectively,without a significant difference (P=0.804).The 5-year OS rates were 96.3% and 100%,respectively (P =0.326).Additionally,the results of Cox regression showed that oligometastatic state was not an independent risk factor for BCR (P =0.682).Conclusions:Our findings supported the safety and effectiveness of RALP in OPC.Additionally,oligometastatic state and sites did not have an adverse effect on BCR independently.
