Objective To investigate the relationship between microalbuminuria (MAU) and insulin resistance syndrome. Methods A total of 240 patients(130 of them with hypertension) were stratified by oral glucose tolerance status 〔normal glucose tolerance(NGT), impaired glucose tolerance(IGT), and type 2 diabetes mellitus(DM)〕. Morning urine albumin concentration and anthropometric indices, serum lipid, fasting plasma true insulin concentrations were also measured. 2.Application of native hapten-gel diffusion test in differential diagnosis of Brucella-infected antibody and S2 or Rev.1-vaccinated antibody

The aim of the present study was to work on the efficiency of differential diagnosis of native hapten-gel diffusion assay (NH-GD) on the background of vaccination with S2 or Rev.1.The conditions of NH-GD assay was firstly optimized,its sensitivity,specificity,repeatability and ability of differential diagnosis were determined respectively,and its test result was compared with that of fluorescence polarization assay (FPA).The results showed NH-GD assay with good specificity and repeatability could differentiate Brucella-infected antibody from vaccinated antibody after vaccination with S2 or 122 days after vaccination with Rev.1.And the result of NH-GD assay was highly consistent with that of FPA,which was simple to operate and needed a few simple equipment.Therefore,NH-GD assay was a good method for sheep brucellosis surveillance in China and especially suitable for application in grass-roots areas.
3.Antimicrobial resistance profile and homology of carbapenem-resistant Klebsiella pneumoniae isolated from patients and related surroundings

Objective To understand the homology of clinical isolates from patients with carbapenem-resistant Klebsiella pneumoniae(CRKP)infection and isolates from environment in a medical institution.Methods One CRKP strain isolated from a patient in this hospital and 4 strains of Klebsiella pneumoniae(K.pneumoniae)isolated from patient's surroundings were collected,susceptibility of 5 strains to commonly used antimicrobial agents was detected,production of carbapenems in 5 strains were detected by modified Hodge testing and carbapenem inactivation method(CIM),homology analysis was performed by pulsed-field gel electrophoresis(PFGE).Results Antimicrobial susceptibility testing results showed that 5 strains of K.pneumoniae(1 from patient,4 from the patient's ward surroundings,including hands of nursing aides,solution bottle opening,handle for lifting and dropping bed,and bedrail)were all resistant to other antimicrobial agents except to cephamycin and aminoglycosides.The modified Hodge testing and CIM confirmed that 5 strains all produced carbapenemases;PFGE results showed that electrophoretogram of CRKP isolated from solution bottle opening of ward,bedrail,and handle for lifting and dropping bed were the same as CRKP isolated from patient,while electrophoretogram of CRKP isolated from hands of nursing aides had 2 different bands,there was a close relationship between the strains.Conclusion The same type of CRKP were isolated from patient and his surroundings,it is necessary to implement healthcare-associated infection(HAI)control system,isolate infected patient,and strengthen environmental cleaning and disinfection,so as to avoid the outbreak of HAI.

5. Preliminary evaluation of 10-hydroxycamptothecin loaded human serum albumin nanoparticles prepared by drug-liquid compound method

Objective To characterize and evaluate in vitro and in vivo of the 10-hydroxycamptothecin (HCPT) loaded human serum albumin nanoparticle (HSA-NP) prepared by drug-liquid compound method. Methods The HCPT-HSA-NP was prepared with low weight polyethylene glycol drug-liquid compound and blank albumin nanoparticle. Then the in vitro evaluations were conducted with tests of entrapment efficiency, solution stability, accumulative release, morphous investigation and X-ray powder diffraction. At the same time, the primary pharmacodynamics comparison between HCPT injection and the nano preparation-8 mg/kg) was carried out on animal tumor model. Results The obtained HCPT-HSA-NP fitted to the basal features of nano preparation. The entrapment efficiency was averagely higher than 99% for each sample and the solution was stable. In vitro accumulative release study showed that the preparation had long-term release pattern over 100 hours. In vivo pharmacodynamics study showed that the HCPT-HSA-NP was significantly more effective than HCPT injection (P<0.01). Conclusion The drug-liquid compound method can be used to prepare HCPT-HSA-NP.
6.The investigation of BMP-2 and VEGF dual gene activated nanobone enhanced the vertebral bone of ovariectomized goat

Objective To verify BMP-2 and VEGF gene activated nanobone paste can effectively enhance the vertebral bone of ovariectomized goat.Methods From January,2011 to May,2016,the goats had been neutered by ovariectomy 6 months earlier to induceosteoporosis.Then surgery to established the model of vertebral bone defected with nanobone implanting,and the operation vertebrae divided randomly into 3 groups:control group,nanobone group and double gene activated nanobone group.Three months after operation the goats were sacrificed and removed the vertebrae.Micro CT analysis of micro three-dimensional structure of trabecular bone,scanning electron microscope (SEM) analysis of the two-dimensional structure of the vertebrae,the structure of trabecular bone was evaluated by movat pentachrome staining.Results The dual gene activated nanobone group compared with the nanobone group,the bone volume fraction (BV/TV) significantly increased (85% at 1.2 mm vs 43% at 1.2 mm,P < 0.05);the dual gene activated nanobone group compared with nanobone group,in the largest ROI (1.2 mm),TbTh increased 10.9% (374 ± 26.2 μm vs 337 ± 22.3 μm,P < 0.05);Trabecular distribution coefficient (TbPf) was significantly decreased (7.519 ± 0.184 mm-1 vs 7.529 ± 0.261 mm-1,P < 0.05);In the largest ROI (0.8 mm),trabecular distribution coefficient (TbPf) was significantly decreased (283 ± 36.4 μm vs 327 ± 31.2 μm,P < 0.05),In the largest ROI (0.8 mm),the trabecular bone volume (Tbn) was increased 20%(1.404 ± 0.283 mm-1 vs 1.173 ± 0.224 mm-1,P < 0.05);Cortical thickness over the implantation area showed asignificant increase of 43% in vertebrae(P < 0.05);The histological analysis revealed a more extensive osseointegration of the dual gene activated nanobone group,with the presence of anabundant osteoid tissue and an osteoblastic celllining.Conclusion BMP-2 and VEGF gene activated nano bone paste can effectively enhance the vertebral bone of ovariectomized goat.
7.Effect of Astragalus Polysaccharide in Combination with Three-dimensional Conformal Radiotherapy on Elderly Patients with Non-small Cell Lung Cancer

Objective: To observe the therapeutic efficacy of astragalus polysaccharide (APS) in combination with three-dimensional conformal radiotherapy (3D-CRT) for elderly patients with non-small cell lung cancer (NSCLC). Methods: A total of 80 elderly patients with I~IV stage NSCLC were randomly divided into two groups. The 40 patients in the therapeutic group received radiotherapy in combination with APS. The 40 patients in the control group received radiotherapy alone. Both groups received 3D-CRT with a total dose of 50~ 70Gy, 2.0Gy/fraction, once a day, 5 times per week. The patients in the therapeutic group were treated with radiotherapy combined with injection of APS (250mg in 5% glucose) or normal saline (500 mL) intravenously once a day until the end of radiotherapy. The short-term efficacy and patients' quality of life were evaluated. The T-lymphocyte subpopulation and peripheral blood leukocyte count were also measured after treatment. Results: The short-term effective rates of the therapeutic group and the control group were 87.5% and 72.5%, respectively, without significant difference between the two groups (P>0.05). The decrease of peripheral blood leukocyte count after treatment in the control group was significant (P<0.05). In the therapeutic group, the T-lymphocyte subsets CD8 and CD4/CD8 were improved after treatment, with a significant difference (P< 0.05). But in the control group, no significant changes in T-lymphocyte subsets CD8 and CD4/CD8 were observed after treatment (P>0.05). Patients' quality of life in the therapeutic group was superior to that in the control group, with a significant difference (P<0.05). Conclusion: APS in combination with 3D-CRT can reduce the side effects of radiation and improve the quality of life of elderly patients with NSCLC.
8.Vaginal paravaginal repair plus vaginal bridge repair in treatment of female pelvic organ prolapse

Objective To explore the effectiveness and safety of vaginal paravaginal repair(VPVR) plus vaginal bridge repair in the treatment of female pelvic organ prolapse (POP). Methods Sixty-five patients with different defects of pelvic floor underwent VPVR or plus vaginal bridge repair for posterior vaginal wall. Patients were followed up after operation. The cure rate was estimated subjectively and objectively. The patients' quality of life was evaluated by the pelvic floor distress inventory short form 20 (PFDI-20). Results All 65 cases were treated by vaginal hysterectomy and anterior vaginal repair, in which there were 33 cases underwent VPVR while 32 cases underwent VPVR plus middle area repair. Forty concomitant procedures for vaginal bridge repair were also performed. The average operative time was (110.00±20.12) min and blood loss was (119.52±45.33) ml. The symptom of stress urinary incontinence of 25 cases significantly released after operation. Four incision recovery delayed and there were no other complicatious occurred. Patients were followed up for 6-29 months,the objective cure rate was 100.00% (65/65) and subjective cure rate was 92.31%(60/65), and 58 cases (89.23%)improved significantly with the quality of life comparing with that of pre-operation by completing PFDI-20 (P<0.01). Conclusions It is an effective and safe procedure for VPVR plus vaginal bridge repair to correct median to severe anterior vaginal prolapse and posterior vaginal wall prolapse. More clinical trials are needed to evaluate their long-term outcome.
