3.Preoperative CT prediction for Masaoka staging of thymic epithelial tumor

Objective To discuss the value of CT prognosis on the Masaoka staging system of thymic epithelial tumors(TET) before surgical resection.Methods The CT images of 102 patients with TET proved by surgery and pathology were reviewed retrospectively.The TET were reclassified according to Masaoka stage system.The size,homogeneity,sharp,contour,infiltration of surrounding tissue,and metastasis on CT were analyzed with Logistic analysis.The diagnostic value was also evaluated with a ROC curve.Results Masaoka pathologic stages were stage Ⅰ for 36 (35.3 %),stage Ⅱ for 27 (26.5 %),stage Ⅲ for 30 (29.4 %),and stage Ⅳ for 9 (8.8 %).A multivariable Logistic regression model showed that TET with larger size of tumor (20/35,P =0.0371,OR =4.539),irregular or lobulated tumor contour (26/42,P =0.0230,OR =4.870),heterogeneous (21/33,P =0.0154,OR =6.020),infiltration of surrounding fat (25/32,P =0.0019,OR =14.005),and pleural seeding (11/11,P =0.0032,OR =36.153)were more likely to have stage Ⅲ or Ⅳ disease.The area under ROC curve was 0.940.Conclusions The tumor CT imaging features can differentiate between stage Ⅰ,Ⅱ and stage Ⅲ,Ⅳ disease.This helps identified patients more likely to benefit from neoadjuvant therapy.
4.Research in using cycle of Lu Wofu lubricant preservation

Objective To explore the effective using cycle of Lu Wofu lubricant preservation with different cleaning methods,and provide basis for change time of lubricant preservation.Methods 20 000 bricant was ensured under sterile conditions.400 pieces of the same equipment were extracted from surgical instruments every day,and they were randomly divided into the manual cleaning group and the mechanical cleaning group with 200 pieces in each group,each group adopted manual cleaning and mechanical cleaning.The Lu Wofu lubricant was reused and preserved for one week.Lubricant preservation was extracted and sent to do bacterial culture with fixed people,time and quantity.Results of two groups were compared.Results The qualified rate of the two groups at 24-hour was 100%,at 48-hour the manual cleaning group was 92%,the mechanical cleaning group was 77%,at 72-hour the manual cleaning group was 81%,the mechanical cleaning group was 19%,above 72 hours; both the manual cleaning group and the mechanical group was zero.Over 48 hours in the mechanical group and above 72 hours in the manual cleaning group appeared more than 10 million bacterial colonies.There was significant difference between the two groups.Conclusions The effective using cycle of Lu Wofu lubricant preservation was 72 hours for manual cleaning and 48 hours for mechanical cleaning,the manual cleaning delays one day over mechanical cleaning to change lubricant,which can reduce the waste of resources and cost savings.
5.Clinical characteristics and treatment of ceftriaxone-associated postrenal acute renal insufficiency

Objective To investigate the clinical characteristics and treatment of acute postrenal acute renal dysfunction associated with ceftriaxone.Methods Twenty-five cases of the ceftriaxione-associated acute postrenal renal insufficiency were reviewed.There were 16 males and 9 females,mean age 28years.The serum contents of BUN and Cr were ( 18.6 ± 7.0) mmol/L and (635.5 ± 248.7 ) μmol/L,respectively.All patients were divided into two groups depending on the therapy:11 patients accepted the drug therapy (alkalinization of the urine,antispasmodic,etc) and 14 patients accepted the intraureteral cannula.The clinical characteristics and the treatment effect were compared between the 2 groups.Results The patients of the intraureteral cannula group ( 1.4 ± O.7 d) went to hospital earlier than the drug therapy group (3.0 ± 1.4 d) ( P =0.045 ) after the symptom of oliguria or anuria appeared.There were no significant differences in serum creatinine,urea nitrogen,and the age between the 2 groups ( P > 0.05 ).All the patients were cured after treatment.There were no significant differences in recovery time (2.9 ± 1.1 d and 3.2 ± 1.2 d,P =0.963) and hospitalization time (7.0 ±2.3 d and 5.9 +3.9 d,P =0.568) between the 2 groups.Conclusions The acute renal failure associated with ceftriaxone should have high attention.The prompt medical attention,including the intraureteral eannula and the drug therapy,can both achieve the satisfying curative effect.
6.Histomorphometric evaluation of ridge preservation after molar tooth extraction

Objective:To evaluate bone formation in human extraction sockets with absorbed surrounding walls augmented with Bio-Oss(R) and Bio-Gide(R) after a 6-month healing period by histologic and histomorphometric analyses.Methods:Six fresh molar tooth extraction sockets in 6 patients who required periodontally compromised moral tooth extraction were included in this study.The six fresh extraction sockets were grafted with Bio-Oss(R) particle covered with Bio-Gide(R).The 2.8 mm × 6.0 mm cylindric bone specimens were taken from the graft sites with aid of stent 6 months after the surgery.Histologic and histomorphometric analyses were performed.Results:The histological results showed Bio-0ss(R) particles were easily distinguished from the newly formed bone,small amounts of new bone were formed among the Bio-Oss(R) particles,large amounts of connective tissue were found.Intimate contact between the newly formed bone and the small part of Bio-Oss(R) particles was present.All the biopsy cylinders measurement demonstrated a high inter-individual variability in the percentage of the bone,connective tissues and BioOss(R) particles.The new bone occupied 11.54% (0-28.40%) of the total area;the connective tissues were 53.42% (34.08%-74.59%) and the Bio-Oss(R) particles were 35.04% (13.92%-50.87%).The percentage of the particles,which were in contact with bone tissues,amounted to 20.13% (0-48.50%).Conclusion:Sites grafted with Bio-Oss(R) particles covered with Bio-Gide(R) were comprised of connective tissues and small amounts of newly formed bone surrounding the graft particles.
7.Clinical manifestations and esophageal motility characteristics of 180 patients with gastroesophageal reflux disease and extra-esophageal symptoms

Objective:To analyze the clinical manifestations and esophageal motility characteristics of patients with gastroesophageal reflux disease (GERD) and extra-esophageal symptoms.Methods:From January 1 to October 30, 2018, at PLA Rocket Force Characteristic Medical Center, 180 hospitalized patients diagnosed with GERD and extra-esophageal symptoms were retrospectively analyzed. The patients were divided into laryngopharyngeal symptom group (65 cases), airway symptom group (58 cases) and mixed symptom group (57 cases). General data, clinical symptoms, gastroscopic manitestations, the results of high-resolution esophageal manometry and 24-hour multichannel intraluminal impedance and pH monitoring of each group were analyzed and compared. Chi-square test and one-way analysis of variance were used for statistical analysis.Results:The patients aged <40, 40 to 60 and >60 years accounted for 12.8% (23/180), 53.3% (96/180) and 33.9% (61/180), respectively, and the difference was statistically significant ( χ2=12.030, P=0.017). There were 18.9%(34/180) of patients without typical reflux symptoms. There were statistically significant differences in the incidence of ectopic esophagogastric mucosa or Barrett esophagus under gastroscopy between laryngopharyngeal symptom group, airway symptom group and mixed symptom group (21.5%, 14/65; 5.2%, 3/58 and 8.8%, 5/57, respectively) ( χ2=8.578, P=0.014). There were no statistically significant differences in the lower esophageal sphincter pressure (LESP), upper esophageal sphincter pressure or distal contractile integral between laryngopharyngeal symptom group, airway symptom group and mixed symptom group ((8.57±0.76), (8.87±0.79), and (10.51±0.97) mmHg (1 mmHg=0.133 kPa); (44.75±2.86), (42.81±4.06), and (39.14±3.20) mmHg; (506.13±64.30), (432.59±78.10), and (682.99±82.28) mmHg·s·cm)(all P>0.05). The DeMeester score of laryngopharyngeal symptom group , mixed symptom group and airway symptom group was (14.33±2.09), (21.94±5.30) and (30.47±5.85) points, respectively, and the difference was statistically significant ( F=3.226, P=0.043). The results of multi-channel impedance monitoring showed that acid reflux and weak acid reflux were the main reflux in the patients, which accounted for 55.5% (76/137) and 34.3% (47/137), respectively. Among 87.6% (120/137) of the patients, reflux mainly occurred in the upright position while 12.4% (17/137) of the patients had reflux in the supine position. Conclusions:The extra-esophageal symptoms of GERD is associated with age. Ectopic esophagogastric mucosa or Barrett esophagus are more common in GERD patients with laryngopharyngeal symptoms. There are more acid exposure and pathologic acid reflux in GERD mainly with airway symptoms. Weak acid reflux at upright position plays an important role in the reflux mechanism of GERD with extra-esophageal symptoms.
8.Radiographic evaluation of ridge preservation after molar tooth extraction:a con-trolled clinical trial

Objective:To compare the bone dimensional changes following tooth extraction alone with extraction plus ridge preservation ( using deproteinized boving bone mineral Bio-Oss? and bioresorbable collagen mambrane Bio-Gide?) in periodontal compromised extraction sockets .Methods: Eighteen molars of sixteen subjects requiring tooth extraction because of periodontal destruction were enrolled in this study .The subjects were assigned to the control group ( extraction alone , EXT) or to the test group ( ridge-preservation procedure with Bio-Oss? and Bio-Gide?, RP) .Parallel periapical X-rays and cone-beam computed tomography ( CBCT ) scans were taken immediately after tooth extraction alone or plus ridge-preservation ( baseline ) and 6 months later .The changes of horizontal ridge width and vertical ridge height were assessed .Results:At the central buccal aspect , the ridge height increased 2 .9 mm in RP group, and reduced 1.0 mm in EXT group.At the distal buccal aspect , the ridge height increased 1.45 mm in RP group, and reduced 1.45 mm in EXT group.The differences between the groups reached statistical significance (P<0.05).The mean ridge width increased at the 1 mm below the crest (the horizontal ridge width was measured with grafting material at three levels at 1 mm below the most coronal aspect of the crest,HW1), which amounted to 3.40 to 5.80 mm in RP group, and 1.45 to 2.90 mm in EXT group.The mean ridge increased at the 4 mm below the crest ( the horizontal ridge width was measured with grafting material at three levels at 4 mm below the most coronal aspect of the crest ,HW4 ) , which amounted to 0.40 to 3.50 mm in RP group, and reduced 0.10 to increased 0.15 mm in EXT group.The test group and the control group were not significantly different (P>0.05).Conclusion:The ridge-preservation approach using Bio-Oss? in combination with Bio-Gide? can significantly increase vertical ridge height and horizontal ridge width after tooth extraction compared with extraction alone in periodontal compromised molars .
9.Correlation Between TCM Syndromes of Patients with Gastric Carcinoma and Level of Serum sE-cad

Objective To explore the correlation between the syndromes of patients with gastric carcinoma and level of serum sE-cad. Methods Totally 190 cases of different syndromes of patients with gastric carcinoma were collected, and serum level of sE-cad was measured by ELISA. The correlation between the syndromes of gastric cancer and level of serum sE-cad was analyzed by SPSS16.0 software. Results The level of serum sE-cad of interior retention of blood stasis syndrome was significant different with that of deficiency syndrome of spleen and stomach, yin impairment syndrome due to stomach heat, deficiency syndrome of both qi and yin (P<0.05). Besides, this result was the same as the results of gender stratification analysis in men. The difference in serum level of sE-cad in the type of deficiency syndrome of spleen and stomach was significant different with that of deficiency syndrome of both qi and yin in women (P<0.05). Conclusion Level of serum sE-cad was correlated with syndrome type of patients with gastric carcinoma.
10. Autophagy: Divergent effects on disease

Autophagy, or cellular self-eating, is a cellular pathway involving the engulfment of intracellular organelles or pathogens, leading to protein and organelle degradation, elimination of pathogens or energy recycling. The two important steps of autophagy, the discovery of autophagosome formation and organelle degradation in lysosome, have won the 2016 and 1974 Nobel Prize in Physiology or Medicine, respectively, which indicates the great significance of autophagy. The primordial and vital function of autophagy is preserved in all eukaryotic organisms, from yeast to humans. Autophagic dysfunction can result in diverse pathologicd changes, and is closely associated with microbial infection, cancer, neurodegeneration and ischemic diseases. Moreover, autophagy plays complicated and even divergent roles in different types of diseases, cells, conditions and pathological processes.
