OBJECTIVETo observe the protective effect of Panax Ginseng (PG), Aconitum Carmichaeli (AC), and their combination (PG-AC) on myocardial ischemia/reperfusion injury in rats.
METHODSRat's ischemia reperfusion injury model was established by ligating left anterior descending coronary artery for 60 min followed by reperfusion for 240 min. Forty male rats were randomly divided into five groups, the sham operation group, the model group, the three treated groups. The three treated groups were treated with PG, AC and PG-AC respectively by given the drugs 10 min before ischemia reperfusion, and to the sham operation group and the model group, saline was given instead. The infarction area, pathologic changes of myocardial tissue (under light and electron microscopy), activity of creatine phosphokinase (CK) and lactate dehydrogenase (LDH) in serum, content of malondialdehyde (MDA) and superoxide dismutase (SOD) in myocardial tissue were observed to evaluate the protective effect of treatment.
RESULTSThe area of acute myocardial infarction was lesser, activity of LDH and CK were lower in the three treated groups than those in the model group. Content of SOD was significantly higher and that of MDA was markedly lower in the former three than those in the model group. Light and electron microscopic examination showed that the necrotic degeneration and pathologic changes of myocardiocytes in the treated groups were significantly milder than that of the model group. As comparing the effect between the three treated groups, PG-AC showed the best, and insignificant difference was shown between PG and AC.
CONCLUSIONBoth PG and AC, and their combination have obviously protective effects on myocardium against ischemia reperfusion injury, which of PG-AC is superior to that of PG or AC used singly.

In order to confirm the alteration and significance of cigarette smoke exposure on SP-A in rats, 20 Wistar rats were assigned randomly to two groups: an N group (n=10), and an S group (n=10). The ultra-structural change was observed by electron microscopy. The number of cells positive for SPA was by immunohistochemically measured. The mRNA expression in the lung tissues was determined by reverse transcription polymerase chain reaction (RT-PCR). The number of cells positive for SPA of the S group (0.52 +/- 0.05) was lower than that of the N group (0.72+/-0.06) (P<0.05). The levels of mRNA of SPA in the lung tissues of the S group (0.3522+/-0.0512) was significantly lower than that of the N group (0.4432+/-0.05628) (P<0.05). It is concluded that cigarette smoke alone decreased the level of SP-A and that might have an important effect on surfactant metabolism and the host defense functions of surfactant in the peripheral airways, which might play a crucial role in the development of chronic obstructive lung disease.

OBJECTIVETo explore the clinical effect of the lobulated medial sural artery perforator flap in repairing soft tissue defect in hand or foot.
METHODSSince March 2012 to September 2014, 6 cases with soft tissue defects in hands or feet were treated by lobulated medial sural artery flaps pedicled with 1st musculo-cutaneous perforator and 2st musculo-cutaneous perforator of the medial sural artery. The size of the flaps ranged from 4.5 cm x 10.0 cm to 6.0 cm x 17.0 cm.
RESULTS5 cases of lobulated flap survived smoothly, only 1 lobulated flap had venous articulo, but this flap also survived after the articulo was removed by vascular exploration. All flaps had desirable appearance and sensation and the two-point discrimination was 6 mm in mean with 4 to 12 months follow-up (average, 7 months). Linear scar was left in donor sites in 3 cases and skin scar in 3 cases. There was no malfunction in donor sites.
CONCLUSIONSLobulated medial sural artery perforator flap is feasible and ideal method for the treatment of soft tissue defect in hand or foot with satisfactory effect.

ObjectiveTo test the reproducibility of wave intensity (WI)analysis derived measurements of pulse wave velocity (PWV),and to compare it with the traditional method-applanation tonometry.MethodsOne hundred and ten outpatient volunteers were enrolled in the study. The R-W1 of right brachial artery and right posterior tibial artery were measured through WI in diagnostic ultrasound equipment (Aloka α10),and were used to calculate the right brachial artery PWV which were also measured by tonometry (VP-1000) simutaneously;thus 30 of these volunteers were randomly selected to repeat the above examinations in the same session.Differences between the two methods were investigated by means of a paired t-test,and their linear correlations were also analyzed.Bland-Altman method was used to analyze the agreement between the two methods and their intraobserver intrasession variabilities.ResultsMean baPWVs determined by WI and tonometry were (13.03 ± 1.93) m/s and ( 12.05 ± 2.02) m/s,respectively (P <0.001),and the mean of their difference was (0.98 ± 1.1)m/s giving 95 % limits of agreement of ( - 1.18 m/s,3.14 m/s).Values of PWV obtained by the two systems were highly correlated( r =0.85,P< 0.001 ),with their intraobserver intrasession variabilities being 8.2% and 7.0%,respectively.ConclusionsWI provides a new noninvasive and convenient method to measure PWV with good agreement and similar reproducibility to the standard tonometry system.
